Taking the various values ascribed to biodiversity as its point of departure rather many years ago, the present study aims at deriving a conservation strategy for Uttara Kannada. This hilly district, with the highest proportion of its area under forests in South India, is divided into five ecological zones: coastal, northern evergreen, southern evergreen, moist deciduous, and dry deciduous. The heavily-populated coastal zone includes mangrove forests and estuarine wetlands. The evergreen forests are particularly rich in the diversity of plant species which they support - including wild relatives of a number of cultivated plants. They also serve a vital function in watershed conservation. The moist deciduous forests are rich in bird species; both moist and dry deciduous forests include a number of freshwater ponds and lakes that support a high diversity of aquatic birds.Reviewing the overall distribution of biodiversity, we identify specific localities - including estuaries, evergreen forests, and moist deciduous forests - which should be set aside as Nature reserves. These larger reserves must be complemented by a network of traditionally-protected sacred groves and sacred trees that are distributed throughout the district and that protect today, for instance, the finest surviving stand of dipterocarp trees.We also spell out the necessary policy-changes in overall development strategy that should stem the ongoing decimation of biodiversity. These include (1) revitalizing community-based systems of sustainable management of village forests and protection of sacred groves and trees; (2) reorienting the usage-pattern of reserve forests from production of a limited variety of timber and softwood species for industrial consumers, to production of a larger diversity of non-wood forest produce of commercial value to support the rural economy; (3) utilizing marginal lands under private ownership for generating industrial wood supplies; and (4) provision of incentives for in situ maintenance of land-races of cultivated plants - especially evergreen, fruit-yielding trees - by the local people.It is proposed that this broad framework be now taken to the local communities, and that an action-plan be developed on the basis of inputs provided - and initiatives taken - by them.

Present work shows the feasibility of decentralized energy options for the Tumkur district in India. Decentralized energy planning (DEP) involves scaling down energy planning to subnational or regional scales. The important aspect of the energy planning at decentralized level would be to prepare an area-based DEP to meet energy needs and development of alternate energy sources at least-cost to the economy and environment. The geographical coverage and scale reflects the level at which the analysis takes place, which is an important factor in determining the structure of models. In the present work, DEP modeling under different scenarios has been carried out for Tumkur district of India for the year 2020. DEP model is suitably scaled for obtaining the optimal mix of energy resources and technologies using a computer-based goal programming technique. The rural areas of the Tumkur district have different energy needs. Results show that electricity needs can be met by biomass gasifier technology, using biomass feedstock produced by allocating only 12% of the wasteland in the district at 8 t/ha/yr of biomass productivity. Surplus electricity can be produced by adopting the option of biomass power generation from energy plantations. The surplus electricity generated can be supplied to the grid. The sustainable development scenario is a least cost scenario apart from promoting self-reliance, local employment, and environmental benefits. Certain parts of the State of Nagaland situated in the northeastern region of India have been experiencing rainfall deficit over the past few years leading to severe drought-like conditions, which is likely to be aggravated under a climate change scenario. The state has already incurred considerable losses in the agricultural sector. Regional vulnerability assessments need to be carried out in order to help policy makers and planners formulate and implement effective drought management strategies. The present study uses an 'index-based approach' to quantify the climate variability-induced vulnerability of farmers in five villages of Dimapur district, Nagaland. Indicators, which are reflective of the exposure, sensitivity and adaptive capacity of the farmers to drought, were quantified on the basis of primary data generated through household surveys and participatory rural appraisal supplemented by secondary data in order to calculate a composite vulnerability index. The composite vulnerability index of village New Showba was found to be the least, while Zutovi, the highest. The overall results reveal that biophysical characteristics contribute the most to overall vulnerability. Some potential adaptation strategies were also identified based on observations and discussions with the villagers.

This paper primarily intends to develop a GIS (geographical information system)-based data mining approach for optimally selecting the locations and determining installed capacities for setting up distributed biomass power generation systems in the context of decentralized energy planning for rural regions. The optimal locations within a cluster of villages are obtained by matching the installed capacity needed with the demand for power, minimizing the cost of transportation of biomass from dispersed sources to power generation system, and cost of distribution of electricity from the power generation system to demand centers or villages. The methodology was validated by using it for developing an optimal plan for implementing distributed biomass-based power systems for meeting the rural electricity needs of Tumkur district in India consisting of 2700 villages. The approach uses a k-medoid clustering algorithm to divide the total region into clusters of villages and locate biomass power generation systems at the medoids. The optimal value of k is determined iteratively by running the algorithm for the entire search space for different values of k along with demand-supply matching constraints. The optimal value of the k is chosen such that it minimizes the total cost of system installation, costs of transportation of biomass, and transmission and distribution. A smaller region, consisting of 293 villages was selected to study the sensitivity of the results to varying demand and supply parameters. The results of clustering are represented on a GIS map for the region.

Thai pangas, Pangasius hypophthalmus is one of the important aquaculture species in Bangladesh. Over the last few years spectacular development has been taking place in Thai pangas farming in Mymensingh district. Due to availability of easy breeding and culture techniques as well as quick return, more and more people are converting their rice fields into pangas farms overnight. The present study was carried out to examine health and disease status of Thai pangas mainly through clinical, histopathological and bacteriological techniques. In addition, for collecting primary data on disease and health status of Thai pangas and the resultant socioeconomic impacts on rural households, questionnaire interview and participatory rural appraisal tools were used with selected farming households in three upazilas of Mymensingh district. The most prevalent diseases as reported by the farmers were red spot, followed by anal protrusion, tail and fin rot, pop eye, dropsy and gill rot. Other conditions like cotton wool type lesion, ulceration and white spot were reported but with lower incidence. Four isolates of Aeromonas hydrophila were recovered from kidney and lesion of diseased fish. Hemorrhage over the body especially near mouth and caudal region was noticed in the fishes associated with aeromonad infection. Internally, kidney, liver and spleen became swollen and enlarged. The isolates varied with their pathogenicity. All the four isolates were sensitive to Nitrofurantoin, Cotrimoxazole and Tetracycline but were resistant to Amoxycilline. An attempt was made to treat diseased fish with extracts from neem leaf, garlic and turmeric. Recovery of infection was monitored through mortality and histopathology. General histopathological changes of different organs were also studied. Extract from neem (Azadirachta indica) leaf gave better result. Telangiectasis, lamellar hypertrophy and hyperplasia hemorrhage, lamellar fusion, necrosis of lamellar epithelial cells, presence of parasites and their cysts were the major pathology of gills. Hemorrhagic lesion, pyknotic nuclei and melanomacrophage centers (MMC) were found in the liver of fish. Major pathologies in kidney of fish included presence of MMC, necrotic and ruptured kidney tubules, severe haemopoietic necrosis, and hemorrhage. The economic loss due to disease in Thai pangas farming was estimated from the difference between expected production and actual production. On an average, Thai pangas farmers of Mymensingh incur a loss of Tk. 23,104/ha/cycle due to fish disease (3.6% of expected total production). The loss, however, varied with location and size of farms, type of farmers and management practices. The study also highlighted fish health management related problems and recommended further work for the development of user-friendly farmer-oriented fish health management packages.

BACKGROUND: The proportion of births attended by skilled health personnel is one of two indicators used to measure progress towards Millennium Development Goal 5, which aims for a 75% reduction in global maternal mortality ratios by 2015. Rwanda has one of the highest maternal mortality ratios in the world, estimated between 249-584 maternal deaths per 100,000 live births. The objectives of this study were to quantify secular trends in health facility delivery and to identify factors that affect the uptake of intrapartum healthcare services among women living in rural villages in Bugesera District, Eastern Province, Rwanda. METHODS: Using census data and probability proportional to size cluster sampling methodology, 30 villages were selected for community-based, cross-sectional surveys of women aged 18-50 who had given birth in the previous three years. Complete obstetric histories and detailed demographic data were elicited from respondents using iPad technology. Geospatial coordinates were used to calculate the path distances between each village and its designated health center and district hospital. Bivariate and multivariate logistic regressions were used to identify factors associated with delivery in health facilities. RESULTS: Analysis of 3106 lifetime deliveries from 859 respondents shows a sharp increase in the percentage of health facility deliveries in recent years. Delivering a penultimate baby at a health facility (OR = 4.681 [3.204 - 6.839]), possessing health insurance (OR = 3.812 [1.795 - 8.097]), managing household finances (OR = 1.897 [1.046 - 3.439]), attending more antenatal care visits (OR = 1.567 [1.163 - 2.112]), delivering more recently (OR = 1.438 [1.120 - 1.847] annually), and living closer to a health center (OR = 0.909 [0.846 - 0.976] per km) were independently associated with facility delivery. CONCLUSIONS: The strongest correlates of facility-based delivery in Bugesera District include previous delivery at a health facility, possession of health insurance, greater financial autonomy, more recent interactions with the health system, and proximity to a health center. Recent structural interventions in Rwanda, including the rapid scale-up of community-financed health insurance, likely contributed to the dramatic improvement in the health facility delivery rate observed in our study.

While policies often target malaria prevention and treatment - proximal causes of malaria and related health outcomes - too little attention has been given to the role of household- and individual-level socio-economic status (SES) as a fundamental cause of disease risk in developing countries. This paper presents a conceptual model outlining ways in which SES may influence malaria-related outcomes. Building on this conceptual model, we use household data from rural Mvomero, Tanzania, to examine empirical relationships among multiple measures of household and individual SES and demographics, on the one hand, and malaria prevention, illness, and diagnosis and treatment behaviours, on the other. We find that access to prevention and treatment is significantly associated with indicators of households' wealth; education-based disparities do not emerge in this context. Meanwhile, reported malaria illness shows a stronger association with demographic variables than with SES (controlling for prevention). Greater understanding of the mechanisms through which SES and malaria policies interact to influence disease risk can help to reduce health disparities and reduce the malaria burden in an equitable manner.

PURPOSE. To assess the prevalence of age-related macular degeneration (AMD) in a rural population in Northern India. METHODS. In a pilot feasibility study, 1443 people (median age, 60 years; 52% women), were identified from enumeration of the 50+ age group in 11 randomly sampled villages from a rural, periurban district of Haryana, Northern India. Of those identified, 87% attended an eye examination that included digital fundus photography. Fundus images were graded at a single reading center using definitions from the Wisconsin Age-Related Maculopathy Grading System. RESULTS. Fundus photographs were available for 1101 participants. Overall, 28.8% of participants had ungradable fundus images due to cataract. Including all with ungradable images in the denominator, the prevalence of soft drusen was 34.0% (95% confidence interval [CI] 26.1–42.9); of soft indistinct drusen, 2.2% (95% CI, 1.1–4.4); and of pigmentary irregularities, 10.8% (95% CI, 7.1–16.1). There were 15 (1.4%) cases of late-stage AMD (95% CI, 0.8–2.3) with the prevalence rising from 0.4% in the 50- to 59-year age range to 4.6% in those aged 70 years or older. CONCLUSIONS. Drusen and pigmentary irregularities are common among the rural northern Indian population. The prevalence of late AMD is similar to that encountered in Western settings and is likely to contribute significantly to the burden of vision loss in older people in the developing world.

Os combustíveis fósseis, como o carvão, o petróleo e o gás, constituem fontes de energia que em breve se esgotarão e que são demasiado caras para serem desperdiçadas pelas centrais elétricas na produção de electricidade. Para além desse facto, existem outros argumentos (sobretudo económicos) que inviabilizam a utilização destas fontes de energia em algumas regiões, abrindo caminho a fontes de energia alternativas (e.g. solar, eólica, biomassa, mini-hídricas, geotérmicas, etc) e preferencialmente com contornos locais. No caso particular de Moçambique, tem-se verificado um interesse crescente por parte do governo e de várias ONGs na promoção do uso de energias alternativas para as zonas onde a energia convencional não chega e não chegará, devido aos custos muito elevados que esse processo acarretaria. Esta dissertação apresenta um estudo aprofundado do dimensionamento dum sistema híbrido de geração de energia elétrica envolvendo gerador FV e grupo eletrogéneo de emergência para a Escola Rural da Nangade, situada no Distrito de Nangade, na Província do Cabo Delgado. São também descritos os diversos componentes e as tecnologias associadas a um sistema deste género, com a inclusão de sistemas inteligentes de controlo de energia com a utilização de inversores bidireccionais (inversores de bateria e carregadores) para sistemas isolados. Os resultados são apresentados de forma a facilitar a aplicação e montagem deste tipo de sistemas in loco. Espera-se que esta dissertação possa servir de base no futuro próximo, para a implementação deste tipo de sistemas para permitir a melhoria da qualidade de ensino através de melhores infraestruturas, democratizando desta forma o acesso à educação para as crianças das zonas rurais das várias províncias de Moçambique. Como as energias renováveis são parte integrante do Sistema Elétrico Nacional, apresenta-se resumidamente, no anexo 17, o “Plano de Desenvolvimento na Área de Energia de Moçambique”.

OBJECTIVE: The study tests the hypothesis that a low daily fat intake may induce a negative fat balance and impair catch-up growth in stunted children between 3 and 9y of age. DESIGN: Randomized case-control study. SETTING: Three rural villages of the West Kiang District, The Gambia. SUBJECTS: Three groups of 30 stunted but not wasted children (height for age z-score < or = -2.0, weight for height z-score > or = -2.0) 3-9 y of age were selected by anthropometric survey. Groups were matched for age, sex, village, degree of stunting and season. INTERVENTION: Two groups were randomly assigned to be supplemented five days a week for one year with either a high fat (n = 29) or a high carbohydrate biscuit (n = 30) each containing approximately 1600 kJ. The third group was a non supplemented control group (n = 29). Growth, nutritional status, dietary intake, resting energy expenditure and morbidity were compared. RESULTS: Neither the high fat nor the high carbohydrate supplement had an effect on weight or height gain. The high fat supplement did slightly increase adipose tissue mass. There was no effect of supplementation on resting energy expenditure or morbidity. In addition, the annual growth rate was not associated with a morbidity score. CONCLUSIONS: Results show that neither a high fat nor a high carbohydrate supplement given during 12 months to stunted Gambian children induced catch-up growth. The authors suggest that an adverse effect of the environment on catch-up growth persists despite the nutritional interventions.

Contexte : En République de Guinée, depuis 1984, l’ensemble des structures de soins ont intégré le programme de Soins de santé primaires et Médicaments Essentiels (PEV/SSP/ME). Pour la réalisation de ce programme, d’importants efforts et des sommes des millions de dollars ont été investis, mais les indicateurs de santé du pays sont toujours des plus alarmants du monde (EDS- 2005). Objectif : Evaluer la performance des structures de soins de santé primaires (SSP) d’un district sanitaire guinéen à partir des documents administratifs suivi d’une enquête sur la satisfaction des prestataires et des bénéficiaires et des parties prenantes du district. Méthodologie : Il s’agit d’une étude descriptive de cas touchant 10 des 18 structures de soins de santé primaires du district sanitaire de Labé. Elle porte sur une analyse quantitative de résultats de 10 contrôles semestriels (2004-2009) et sur une analyse qualitative composée d’entretiens menés auprès de 308 bénéficiaires et de quelques membres des Comités de gestion des structures pour apprécier le niveau de performance des structures ciblées. Résultats : Toutes les structures publiques du district sanitaire sous étude étaient intégrées1. Malgré cela, la tendance moyenne des consultations affiche une allure sinusoïdale (fluctuante). Bien que la disponibilité, l’accessibilité, l’utilisation et la couverture adéquate et effective des services de Consultation Primaire Curative (CPC) et de Planification Familiale (PF) n’ont pas connu d’amélioration durant la période de 2004 à 2009. La tendance moyenne de la Consultation prénatale (CPN) et celle de la Vaccination (VA) se sont améliorées au cours de la période d’étude. Les prestataires de services SSP déclarent être assez satisfaits de leur formation mais ne le sont pas pour leur condition de travail surtout ceux du milieu rural (faible rémunération, environnement difficile), qualité moindre de la supervision et ruptures fréquentes de stock en médicaments essentiels. Pour les bénéficiaires, leur satisfaction se limite au respect de leurs valeurs culturelles et de leur interaction avec les prestataires de soins. Cependant, ils déplorent le long temps d’attente, la mauvaise qualité de l’accueil, les coûts élevés des prestations et le manque d’équité qui sont des facteurs qualifiés comme des éléments de contreperformance des structures. Pour les autorités et des parties prenantes, la rupture des stocks en médicaments essentiels, le manque d’équipements et la faible motivation des prestataires sont les facteurs majeurs qui entravent la performance des structures sanitaires, surtout en milieu rural. Conclusion : Malgré l’intégration du programme des SSP dans les structures de SSP du district sanitaire de Labé, on note encore une insuffisance de leur utilisation, la faiblesse de la couverture, le manque de suivi et supervision des structures Une étude actualisée et plus étendue pourrait mieux cerner le sujet.

L’accouchement et ses conséquences demeurent une des principales causes d’incapacité et de décès pour les femmes dans les pays en développement et comprendre l’utilisation tardive des soins obstétricaux d’urgence au moment d’une complication obstétricale constitue un véritable défi en santé publique. La présente étude qualitative relate l’expérience d’accouchement difficile au Mali, en milieu rural. Dans un contexte de pluralité de systèmes de soins, l’objet de cette étude consiste à déterminer les raisons de l’arrivée tardive des femmes au centre de santé de 1ier ligne, à saisir la compréhension qu’elles ont de l’utilisation des services sanitaires, enfin à reconstruire les processus de prise de décisions de recourir aux soins modernes. Cette étude s’inscrit dans une démarche de type ethnographique. Des entretiens semi dirigés et l’observation des interactions entre les femmes et les professionnels de santé ont constitué le corpus de données. Nous retenons qu’une série de facteurs entrent en jeu pour comprendre le problème de l’utilisation tardive des soins obstétricaux d’urgence. Des contraintes exogènes liées à la distance et l’immédiateté de l’urgence obstétricale c'est-à-dire la mobilisation des ressources et des moyens de transport, accompagnées de contraintes endogènes telles que la subordination des femmes au consentement familial, la perception de la qualité des soins et de la compétence des soignants, influencent systématiquement le choix de recourir aux soins modernes. Le phénomène de gestion collective de la complication obstétricale s’inscrit dans un contexte d’intéractions complexes où l’opinion des femmes est totalement minimisée.

The present study made an attempt to analyze the socio-economic background and the consumption pattern of scheduled caste households in Idukki district. The objectives of the study are to examine consumption pattern among the scheduled cast population, differences in the average consumption expenditure of different decile groups, consumption expenditure elasticity of items, variations in expenditure of SC households on food, non-food and total expenditure and to examine the association between consumption expenditure and variables such as income, education, occupation and area of residence. The study reveals that the Monthly Per Capita Expenditure of scheduled castes population in rural Kerala is lower than that of the general population. Average household size is higher in rural sector for Scheduled Caste in Kerala as well as all-India. The per capita expenditure of Scheduled Castes of rural Kerala is found to be much lower than that of general population. The study has found that the levels of livings of the Scheduled Castes are far the below the expectations. Large percentage of the Scheduled Caste belongs to the lower income groups. This is due their very low economic status and the consequent employment prospects in low paying occupations. The consumption standards of the majority of Scheduled Castes are found much below that of General population. Effective implementation of the Schemes for their economic upliftment is needed for improving their consumption standards, Minimum Wage Act in the case of agricultural labourers etc. are some of recommendations on the basis of this study.
