Background/Purpose: The primary treatment goals for gouty arthritis (GA) are rapid relief of pain and inflammation during acute attacks, and long-term hyperuricemia management. A post-hoc analysis of 2 pivotal trials was performed to assess efficacy and safety of canakinumab (CAN), a fully human monoclonal anti-IL-1_ antibody, vs triamcinolone acetonide (TA) in GA patients unable to use NSAIDs and colchicine, and who were on stable urate lowering therapy (ULT) or unable to use ULT. Methods: In these 12-week, randomized, multicenter, double-blind, double-dummy, active-controlled studies (_-RELIEVED and _-RELIEVED II), patients had to have frequent attacks (_3 attacks in previous year) meeting preliminary GA ACR 1977 criteria, and were unresponsive, intolerant, or contraindicated to NSAIDs and/or colchicine, and if on ULT, ULT was stable. Patients were randomized during an acute attack to single dose CAN 150 mg s.c. or TA 40 mg i.m. and were redosed "on demand" for each new attack. Patients completing the core studies were enrolled into blinded 12-week extension studies to further investigate on-demand use of CAN vs TA for new attacks. The subpopulation selected for this post-hoc analysis was (a) unable to use NSAIDs and colchicine due to contraindication, intolerance or lack of efficacy for these drugs, and (b) currently on ULT, or contraindication or previous failure of ULT, as determined by investigators. Subpopulation comprised 101 patients (51 CAN; 50 TA) out of 454 total. Results: Several co-morbidities, including hypertension (56%), obesity (56%), diabetes (18%), and ischemic heart disease (13%) were reported in 90% of this subpopulation. Pain intensity (VAS 100 mm scale) was comparable between CAN and TA treatment groups at baseline (least-square [LS] mean 74.6 and 74.4 mm, respectively). A significantly lower pain score was reported with CAN vs TA at 72 hours post dose (1st co-primary endpoint on baseline flare; LS mean, 23.5 vs 33.6 mm; difference _10.2 mm; 95% CI, _19.9, _0.4; P_0.0208 [1-sided]). CAN significantly reduced risk for their first new attacks by 61% vs TA (HR 0.39; 95% CI, 0.17-0.91, P_0.0151 [1-sided]) for the first 12 weeks (2nd co-primary endpoint), and by 61% vs TA (HR 0.39; 95% CI, 0.19-0.79, P_0.0047 [1-sided]) over 24 weeks. Serum urate levels increased for CAN vs TA with mean change from baseline reaching a maximum of _0.7 _ 2.0 vs _0.1 _ 1.8 mg/dL at 8 weeks, and _0.3 _ 2.0 vs _0.2 _ 1.4 mg/dL at end of study (all had GA attack at baseline). Adverse Events (AEs) were reported in 33 (66%) CAN and 24 (47.1%) TA patients. Infections and infestations were the most common AEs, reported in 10 (20%) and 5 (10%) patients treated with CAN and TA respectively. Incidence of SAEs was comparable between CAN (gastritis, gastroenteritis, chronic renal failure) and TA (aortic valve incompetence, cardiomyopathy, aortic stenosis, diarrohea, nausea, vomiting, bicuspid aortic valve) groups (2 [4.0%] vs 2 [3.9%]). Conclusion: CAN provided superior pain relief and reduced risk of new attack in highly-comorbid GA patients unable to use NSAIDs and colchicine, and who were currently on stable ULT or unable to use ULT. The safety profile in this post-hoc subpopulation was consistent with the overall _-RELIEVED and _-RELIEVED II population.

INTRODUCTION. Neurally Adjusted Ventilatory Assist (NAVA) [1] is a new spontaneousassisted ventilatory mode which uses the diaphragmatic electrical activity (Eadi) to pilot the ventilator. Eadi is used to initiate the ventilator's pressurization and cycling off. Delivered inspiratory assistance is proportional to Eadi. NAVA can improve patient-ventilator synchrony [2] compared to pressure support (PS), but little is known about its effect on minute ventilation and oxygenation. OBJECTIVES. To compare the effects of NAVA and PS on minute ventilation and oxygenation and to analyze potential determinant factors for oxygenation. METHODS. Comparison between two 20-min periods under NAVA and PS. NAVA gain (proportionality factor between Eadi and delivered pressure) set as to obtain the same peak pressure as in PS. FIO2 and positive end-expiratory pressure (PEEP) were the same in NAVA and PS. Blood gas analyses were performed at the end of both recording periods. Statistical analysis: groups were compared with paired t tests or non parametric Wilcoxon signed-rank tests. p\0.05 was considered significant. RESULTS. [Mean ± SD]: 22 patients (age 66 ± 12 year, 7 M/15F, BMI 23.4 ± 3.1 kg/m2), 8 patients with COPD. Initial settings: PS 13 ± 3 cmH2O, PEEP 7 ± 2 cmH2O, NAVA gain 2.2 ± 1.8. Minute ventilation and PaCO2 were the same with both modes (p = 0.296 and 0.848, respectively). Tidal volume was lower with NAVA (427 ± 102 vs. 477 ± 102 ml, p\0.001). In contrast respiratory rate was higher with NAVA (25.6 ± 9.5 vs. 22.3 ± 8.9 cycles/min). Arterial oxygenation was improved with NAVA (PaO2 85.1 ± 28.9 vs. 75.8 ± 11.9 mmHg, p = 0.017, PaO2/FIO2 210 ± 53 vs. 195 ± 58 mmHg, p = 0.019). Neural inspiratory time (Tin) was comparable between NAVA and PS (p = 0.566). Among potential determinant factors for oxygenation, mean airway pressure (Pmean) was lower with NAVA (10.6 ± 2.6 vs. 11.1 ± 2.4 cmH2O, p = 0.006), as was the pressure time product (PTP) (6.8 ± 3.0 vs. 9.2 ± 3.5 cmH2O 9 s, p = 0.004). There were less asynchrony events with NAVA (2.3 ± 2.0 vs. 4.4 ± 3.8, p = 0.009).Tidal volume variability was higher with NAVA (variation coefficient: 30 ± 19.5 vs. 13.5 ± 8.6, p\0.001). Inspiratory time in excess (Tiex) was lower with NAVA (56 ± 23 vs. 202 ± 200 ms, p = 0.001). CONCLUSION. Despite lower Pmean and PTP in NAVA, arterial oxygenation was improved compared to PS. As asynchronies may be associated with an increased work of breathing and a higher oxygen consumption, their decrease in number with NAVA could be an explanation for oxygenation improvement. Another explanation could be the increase in VT variability. Further studies should now be performed to confirm the potential of NAVA in improving arterial oxygenation and explore the underlying mechanisms.

The authenticity of vegetable oils consumed in Slovenia and Croatia was investigated by carbon isotope analysis of the individual fatty acids by the use of gas chromatography-combustion-isotope ratio mass spectrometry (GC/C/IRMS), and through carbon isotope analysis of the bulk oil. The fatty acids from samples of olive, pumpkin, sunflower, maize, rape, soybean, and sesame oils were separated by alkaline hydrolysis and derivatized to methyl esters for chemical characterization by capillary gas chromatography/mass spectrometry (GC/MS) prior to isotopic analysis. Enrichment in heavy carbon isotope (C-13) of th, bulk oil and of the individual fatty acids are related to (1) a thermally induced degradation during processing (deodorization, steam washing, or bleaching), (2) hydrolytic rancidity (lipolysis) and oxidative rancidity of the vegetable oils during storage, and (3) the potential blend with refined oil or other vegetable oils. The impurity or admixture of different oils may be assessed from the delta C-13(16:0) VS. delta C-13(18:1) covariations. The fatty acid compositions of Slovenian and Croatian olive oils are compared with those from the most important Mediterranean producer countries (Spain, Italy, Greece, and France).

Objetivos: La preocupación por la crisis económica que azota España y por lo tanto, a la comunidad autónoma de Navarra, sumado a la evidencia científica de las consecuencias que sufre el bebé al ser separado de su madre tras el nacimiento, siendo más notable por debajo de las 37 semanas de gestación, y al elevado estrés y sufrimiento de los padres cuando su hijo está encerrado en una incubadora, llevó al planteamiento de querer demostrar los posibles beneficios fisiológicos y psicológicos del método madre canguro (MMC) en bebés prematuros y sus madres. Metodología: Será un estudio experimental, longitudinal y prospectivo dirigido a 40 mujeres y a sus bebés prematuros, que den a luz en el hospital Virgen del Camino. El grado de satisfacción se determinará con preguntas extraídas de la escala validada Caring Assement Instrument (CARE- Q) y con otro cuestionario no validado. Los parámetros fisiológicos se medirán con un termómetro de mercurio y una cinta métrica. El programa informático SPSS® vs 21.0. será el elegido para el análisis estadístico. Limitaciones del estudio: La larga duración de la participación de la unidad madre-hijo, la tercera etapa domiciliaria del MMC, las restricciones de la UCIN del hospital Virgen del Camino junto al uso de una escala no validada, podría conllevar a la aparición de obstáculos a la hora, tanto de poder llevar a cabo el proyecto, como de la obtención de sus resultados.

PURPOSE: We investigated the changes in physiological and performance parameters after a Live High-Train Low (LHTL) altitude camp in normobaric (NH) or hypobaric hypoxia (HH) to reproduce the actual training practices of endurance athletes using a crossover-designed study. METHODS: Well-trained triathletes (n = 16) were split into two groups and completed two 18-day LTHL camps during which they trained at 1100-1200 m and lived at 2250 m (P i O2 = 111.9 ± 0.6 vs. 111.6 ± 0.6 mmHg) under NH (hypoxic chamber; FiO2 18.05 ± 0.03%) or HH (real altitude; barometric pressure 580.2 ± 2.9 mmHg) conditions. The subjects completed the NH and HH camps with a 1-year washout period. Measurements and protocol were identical for both phases of the crossover study. Oxygen saturation (S p O2) was constantly recorded nightly. P i O2 and training loads were matched daily. Blood samples and VO2max were measured before (Pre-) and 1 day after (Post-1) LHTL. A 3-km running-test was performed near sea level before and 1, 7, and 21 days after training camps. RESULTS: Total hypoxic exposure was lower for NH than for HH during LHTL (230 vs. 310 h; P < 0.001). Nocturnal S p O2 was higher in NH than in HH (92.4 ± 1.2 vs. 91.3 ± 1.0%, P < 0.001). VO2max increased to the same extent for NH and HH (4.9 ± 5.6 vs. 3.2 ± 5.1%). No difference was found in hematological parameters. The 3-km run time was significantly faster in both conditions 21 days after LHTL (4.5 ± 5.0 vs. 6.2 ± 6.4% for NH and HH), and no difference between conditions was found at any time. CONCLUSION: Increases in VO2max and performance enhancement were similar between NH and HH conditions.

Avaliou-se, retrospectivamente, a evolução pós-operatória das varizes esofagogástricas em 40 pacientes submetidos a um dos seguintes procedimentos cirúrgicos: a (n=27) derivação esplenorrenal distal (ERD) e B (n=13) derivação esplenorrenal proximal (ERP). Todos os pacientes tinham hipertensão porta esquistossomótica com diagnóstico prévio de varizes do esôfago, presentes ou não no estômago, com um ou mais episódios de sangramento. Os pacientes foram submetidos a um dos procedimentos cirúrgicos de acordo com a preferência do cirurgião assistente. Foram realizadas, nesses pacientes, endoscopias no período pré-operatório e aos seis, 12 e 18 meses no pós-operatório. Os dados de cada endoscopia foram coletados e comparados entre os grupos, verificando-se a presença de varizes do esôfago e estômago nos diferentes períodos, comparando esses achados através do teste do qui-quadrado, com significância para p<0,05. Os resultados obtidos não mostraram casos de ressangramento até o 18º mês pós-operatório, nem casos de encefalopatia. Foram diagnosticadas varizes esofágicas, no pré-operatório, em 100% dos pacientes nos dois grupos. No período pós-operatório, houve redução significativa das varizes do esôfago, quando estudados os dois grupos conjuntamente, para 40% no sexto mês (p = 0,0002), 30% no 12º mês (p = 0,003) e 27,5% no 18º mês (p = 0,003). No sexto mês pós-operatório, a incidência de varizes do esôfago foi maior nos pacientes com ERD quando comparados àqueles com ERP (51,9% vs. 15,4%, p = 0,03). Quando estudadas as varizes aos 12 e 18 meses não foi observada diferença significativa entre pacientes submetidos a ERD ou ERP (12º mês, 37% vs. 15,4%; 18º mês, 25,9% vs. 30,8%). Foram vistas varizes gástricas em 37,5% dos pacientes, com redução significativa no sexto mês (2,5%, p = 0,005). Entretanto, quando comparada com a freqüência do sexto mês, houve aumento significativo no 12º mês (5%, p = 0,00001) e 18º mês (7,5%, p = 0,02). Quando comparados os grupos, no período pré-operatório, estas varizes estiveram presentes mais freqüentemente no grupo submetido a ERP (69,2% vs. 26%, p = 0,0005), sem diferença significativa no período pós-operatório (6º mês, 16,6% vs. 0%; 12º mês, 33,3% vs. 0%; 18º mês, 33,3% vs. 11,1%). Este trabalho demonstrou que os dois tipos de cirurgia têm resultado semelhante em relação à resolução das varizes do esôfago e estômago no 18º mês, mas os resultados indicam que a redução na incidência das varizes do esôfago se acompanha de aumento das varizes gástricas, provavelmente devido à abertura de novas vias colaterais de drenagem em casos de persistência de uma pressão porta aumentada ou mau funcionamento da derivação esplenorrenal.

Nykyaikaisen sotilaan on kyettävä säilyttämään toimintakykynsä nopeasti muuttuvissa tilanteissa. Viime aikoina käydyt sodat ja tutkimukset ovat osoittaneet, että sotilailta edel-lytetään edelleen hyvää fyysistä suorituskykyä (David 1995). On myös esitetty väittämiä sotilaiden suorituskykyvaatimuksien lisääntymisestä nykyisissä operaatioissa (Maavoi-maesikunta 2004, 276). Sotilastehtävien kuormittavuudesta saadaan tietoa mittaamalla koehenkilöiden aktiivisuutta ja fysiologisia vasteita. Fyysiseen kuormittavuuteen kohdis-tuvia tutkimuksia voidaan käyttää arvioitaessa sotilaiden suorituskyvyn vaatimuksia tai kehitettäessä tehtäväkohtaista taktiikkaa ja koulutusta. Tämän tutkimuksen tarkoituksena oli selvittää kolme vuorokautta kestävän partiotiedusteluharjoituksen aiheuttamia fysiolo-gisia vasteita ja mielialan muutoksia, harjoituksen aikaista tiedustelijan energia- ja neste-tasapainoa sekä lepo- ja palautumisaikaa. Tutkimus toteutettiin Reserviupseerikoulun yhteistoimintaharjoituksessa, jossa tutkimus-otos harjoitteli pataljoonan hyökkäyksen tiedustelua jalan partioimalla. Koehenkilöt valit-tiin arpomalla ja heistä muodostettiin yksi tiedusteluryhmä (n=10). Koehenkilöiden fyysi-nen kunto määritettiin epäsuoralla polkupyöräergometritestillä (Milfit), jonka mukaan joukko todettiin hyväkuntoiseksi (VO2max = 55.9 ± 3.8 ml/kg-1/min-1). Harjoitusta edeltä-vänä vuorokautena ja harjoituksen aikana sotilaiden fysiologisia vasteita mitattiin ottamal-la veri- ja hormoninäytteitä, seuraamalla sydämen sykevaihtelua ja sykemuutoksia sekä mittaamalla kehon painoa. Sotilaiden toimintaa seurattiin liikkumisen osalta satelliittipai-kannuksella sekä energian- ja nesteensaannin osalta päiväkirjalla ja kyselyllä. Mielialan muutoksia seurattiin päivittäisellä POMS kyselyllä. Tutkimuksen mukaan partiotiedustelu tapahtui hengitys- ja verenkiertoelimistön osalta kohtuullisen matalalla intensiteetillä (HR = 44 ± 4 % maksimista; VO2 = 16 ± 3 % maksi-mista). Kuormitus oli merkitsevästi suurempi kuin kasarmipalveluksessa (p<0.01). Mitta-uksien perusteella tiedustelijat kärsivät harjoituksessa energia- ja nestevajeesta sekä unen-puutteesta. Harjoituksen aikaisen keskimääräisen energiankulutuksen (4646 ± 674 kcal/vrk) ja energiansaannin (2200 ± 326 kcal/vrk) perusteella sotilaiden energiavaje oli 2405 ± 890 kcal/vrk. Energia- ja nestevajeen seurauksena sotilaiden paino laski 2.2 ± 0.8 kg eli 2.8 %. Punasolujen tilavuus laski (-4.2 %, p<0.01) ja plasmatilavuus samoin (-4.3 %, p<0.01). Tämä oli luultavasti seurausta negatiivisesta nestetasapainosta, koska sotilaat joivat keskimäärin vain 2.9 ± 0.8 l/vrk. Energiankulutuksen mukaan laskettuna heidän olisi tullut nauttia nestettä vähintään 4.6 l/vrk. Sykevälivaihteluanalyysi osoitti, että fyysi-sesti raskaimpana vuorokautena (0. vs. 2.vrk) vagaalinen aktiivisuus väheni RMSSD:llä mitattuna (54 vs. 39 ms, p<0.05), joka saattaa olla merkki muutoksesta sympaattis- vagaa-lisessa tasapainossa (Otzenberger ym. 1998). Verinäytteistä saatujen kuormitusta kuvaavi-en muuttujien vaihtelut tukivat aiempia havaintoja sotilastehtävien kuormittavuudesta. Merkittävimmät muutokset alku ja loppumittauksen välillä olivat testosteronin 34 % (p<0.01) ja vapaan testosteronin 31 % (p<0.05) lasku sekä kreatiinikinaasin nousu 2.2 -kertaiseksi (p<0.001). Kortisolin ja kasvuhormonin osalta muutoksien suunta vaihteli eri harjoituspäivinä. Tyroksiinin pitoisuus oli lievässä nousussa koko harjoituksen ajan. Kolme vuorokautta kestäneen partiotiedusteluharjoituksen kuormittavuus hyväkuntoisilla sotilailla muodostui matalaintensiteettisestä pitkäkestoisesta rasituksesta. Harjoituksen aikainen RMSSD:n lasku osoitti sympaattis- vagaalisen säätelyn aktiivisuuden muutoksia laskien parasympaattista aktiivisuutta. Sotilaiden fysiologiset vasteet heijastivat harjoituk-sen aiheuttamaa sotilaallista fyysistä stressiä, joka muodostui huomattavan negatiivisesta energia- ja nestetasapainosta sekä univajeesta. Tästä huolimatta sotilaat säilyttivät korkean aktiivisuuden tason. Huomattavalla energiavajeella ei näyttänyt olevan suurta merkitystä hyväkuntoisien sotilaiden tehtävistä suoriutumiseen lyhyessä alle 4 vrk kestävässä harjoi-tuksessa. Sotilasjohtajien koulutuksessa ja koulutuksen suunnittelussa on korostettava energiatasapainon palauttamisen merkitystä sekä opetettava tehtävien tehokasta organi-sointia fyysisen palautumisen mahdollistamiseksi ja toimintakyvyn säilyttämiseksi.

The pentavalent antimonial (Sb5+) meglumine is the drug of choice for the treatment of cutaneous leishmaniasis (CL) in Brazil. Although the cardiotoxicity of high-dose, long-term Sb5+ therapy is well known, the use of low-dose, short-term meglumine has been considered to be safe and relatively free from significant cardiac effects. In order to investigate the cardiotoxicity of low-dose, short-term therapy with meglumine in cutaneous leishmaniasis, 62 CL patients treated with meglumine were studied. A standard ECG was obtained before and immediately after the first cycle of treatment (15 mg Sb5+ kg-1 day-1). The electrocardiographic interpretation was carried out blindly by two investigators using the Minnesota Code. There were no significant differences in qualitative ECG variables before and after meglumine treatment. However, the corrected QT interval was clearly prolonged after antimonial therapy (420.0 vs 429.3 ms, P<10-6). QTc augmentation exceeded 40 ms in 12 patients, 7 of whom developed marked QTc interval enlargement (500 ms) after meglumine therapy. This previously unrecognized cardiac toxicity induced by short-term, low-dose antimonial therapy has potentially important clinical implications. Since sudden death has been related to QTc prolongation over 500 ms induced by high-dose antimonial therapy, routine electrocardiographic monitoring is probably indicated even in CL patients treated with short-term, low-dose meglumine schedules. Until further studies are conducted to establish the interactions between pentavalent antimonials and other drugs, special care is recommended when using meglumine in combination with other medications, in particular with drugs that also increase the QTc interval.

Streptozotocin (STZ)-induced diabetes in rats is characterized by cardiovascular dysfunction beginning 5 days after STZ injection, which may reflect functional or structural autonomic nervous system damage. We investigated cardiovascular and autonomic function, in rats weighing 166 ± 4 g, 5-7, 14, 30, 45, and 90 days after STZ injection (N = 24, 33, 27, 14, and 13, respectively). Arterial pressure (AP), mean AP (MAP) variability (standard deviation of the mean of MAP, SDMMAP), heart rate (HR), HR variability (standard deviation of the normal pulse intervals, SDNN), and root mean square of successive difference of pulse intervals (RMSSD) were measured. STZ induced increased glycemia in diabetic rats vs control rats. Diabetes reduced resting HR from 363 ± 12 to 332 ± 5 bpm (P < 0.05) 5 to 7 days after STZ and reduced MAP from 121 ± 2 to 104 ± 5 mmHg (P = 0.007) 14 days after STZ. HR and MAP variability were lower in diabetic vs control rats 30-45 days after STZ injection (RMSSD decreased from 5.6 ± 0.9 to 3.4 ± 0.4 ms, P = 0.04 and SDMMAP from 6.6 ± 0.6 to 4.2 ± 0.6 mmHg, P = 0.005). Glycemia was negatively correlated with resting AP and HR (r = -0.41 and -0.40, P < 0.001) and with SDNN and SDMMAP indices (r = -0.34 and -0.49, P < 0.01). Even though STZ-diabetic rats presented bradycardia and hypotension early in the course of diabetes, their autonomic function was reduced only 30-45 days after STZ injection and these changes were negatively correlated with plasma glucose, suggesting a metabolic origin.

To examine the possible age-related blood pressure (BP) deregulation in response to central hypervolemia, we measured spontaneous baroreflex sensitivity (SBRS), carotid arterial compliance (CC), and R-R interval coefficient of variation (RRICV) during basal and thermoneutral resting head-out-of-water immersion (HOWI) in 7 young (YG = 24.0 ± 0.8 years) and 6 middle-aged/older (OL = 59.3 ± 1.3 years) healthy men. Compared with basal conditions (YG = 19.6 ± 4.0 vs OL = 6.1 ± 1.5 ms/mmHg, P < 0.05), SBRS remained higher in YG than OL during rest HOWI (YG = 23.6 ± 6.6 vs OL = 9.3 ± 2.1 ms/mmHg, P < 0.05). The RRICV was significantly different between groups (YG = 6.5 ± 1.4 vs OL = 2.8 ± 0.4%, P < 0.05) under HOWI. The OL group had no increase in CC, but a significant increase in systolic BP (basal = 115.3 ± 4.4 vs water = 129.3 ± 5.3 mmHg, P < 0.05) under HOWI. In contrast, the YG group had a significant increase in CC (basal = 0.16 ± 0.01 vs water = 0.17 ± 0.02 mm²/mmHg, P < 0.05) with no changes in systolic BP. SBRS was positively related to CC (r = 0.58, P < 0.05 for basal vs r = 0.62, P < 0.05 for water). Our data suggest that age-related vagal dysfunction and reduced CC may be associated with SBRS differences between YG and OL groups, and with BP elevation during HOWI in healthy older men.

The aim of the present study was to evaluate the effect of amiodarone on mean arterial pressure (MAP), heart rate (HR), baroreflex, Bezold-Jarisch, and peripheral chemoreflex in normotensive and chronic one-kidney, one-clip (1K1C) hypertensive rats (N = 9 to 11 rats in each group). Amiodarone (50 mg/kg, iv) elicited hypotension and bradycardia in normotensive (-10 ± 1 mmHg, -57 ± 6 bpm) and hypertensive rats (-37 ± 7 mmHg, -39 ± 19 bpm). The baroreflex index (deltaHR/deltaMAP) was significantly attenuated by amiodarone in both normotensive (-0.61 ± 0.12 vs -1.47 ± 0.14 bpm/mmHg for reflex bradycardia and -1.15 ± 0.19 vs -2.63 ± 0.26 bpm/mmHg for reflex tachycardia) and hypertensive rats (-0.26 ± 0.05 vs -0.72 ± 0.16 bpm/mmHg for reflex bradycardia and -0.92 ± 0.19 vs -1.51 ± 0.19 bpm/mmHg for reflex tachycardia). The slope of linear regression from deltapulse interval/deltaMAP was attenuated for both reflex bradycardia and tachycardia in normotensive rats (-0.47 ± 0.13 vs -0.94 ± 0.19 ms/mmHg and -0.80 ± 0.13 vs -1.11 ± 0.13 ms/mmHg), but only for reflex bradycardia in hypertensive rats (-0.15 ± 0.02 vs -0.23 ± 0.3 ms/mmHg). In addition, the MAP and HR responses to the Bezold-Jarisch reflex were 20-30% smaller in amiodarone-treated normotensive or hypertensive rats. The bradycardic response to peripheral chemoreflex activation with intravenous potassium cyanide was also attenuated by amiodarone in both normotensive (-30 ± 6 vs -49 ± 8 bpm) and hypertensive rats (-34 ± 13 vs -42 ± 10 bpm). On the basis of the well-known electrophysiological effects of amiodarone, the sinus node might be the responsible for the attenuation of the cardiovascular reflexes found in the present study.

We examined the effect of exercise training (Ex) without (Ex 0%) or with a 3% workload (Ex 3%) on different cardiac and renal parameters in renovascular hypertensive (2K1C) male Fisher rats weighing 150-200 g. Ex was performed for 5 weeks, 1 h/day, 5 days/week. Ex 0% or Ex 3% induced similar attenuation of baseline mean arterial pressure (MAP, 119 ± 5 mmHg in 2K1C Ex 0%, N = 6, and 118 ± 5 mmHg in 2K1C Ex 3%, N = 11, vs 99 ± 4 mmHg in sham sedentary (Sham Sed) controls, N = 10) and heart rate (HR, bpm) (383 ± 13 in 2K1C Ex 0%, N = 6, and 390 ± 14 in 2K1C Ex 3%, N = 11 vs 371 ± 11 in Sham Sed, N = 10,). Ex 0%, but not Ex 3%, improved baroreflex bradycardia (0.26 ± 0.06 ms/mmHg, N = 6, vs 0.09 ± 0.03 ms/mmHg in 2K1C Sed, N = 11). Morphometric evaluation suggested concentric left ventricle hypertrophy in sedentary 2K1C rats. Ex 0% prevented concentric cardiac hypertrophy, increased cardiomyocyte diameter and decreased cardiac vasculature thickness in 2K1C rats. In contrast, in 2K1C, Ex 3% reduced the concentric remodeling and prevented the increase in cardiac vasculature wall thickness, decreased the cardiomyocyte diameter and increased collagen deposition. Renal morphometric analysis showed that Ex 3% induced an increase in vasculature wall thickness and collagen deposition in the left kidney of 2K1C rats. These data suggest that Ex 0% has more beneficial effects than Ex 3% in renovascular hypertensive rats.

Most research on the effects of endurance training has focused on endurance training's health-related benefits and metabolic effects in both children and adults. The purpose of this study was to examine the neuromuscular effects of endurance training and to investigate whether they differ in children (9.0-12.9 years) and adults (18.4-35.6 years). Maximal isometric torque, rate of torque development (RTD), rate of muscle activation (Q30), electromechanical delay (EMD), and time to peak torque and peak RTD were determined by isokinetic dynamometry and surface electromyography (EMG) in elbow and knee flexion and extension. The subjects were 12 endurance-trained and 16 untrained boys, and 15 endurance-trained and 20 untrained men. The adults displayed consistently higher peak torque, RTD, and Q30, in both absolute and normalized values, whereas the boys had longer EMD (64.7+/-17.1 vs. 56.6+/-15.4 ms) and time to peak RTD (98.5+/-32.1 vs. 80.4+/-15.0 ms for boys and men, respectively). Q30, normalized for peak EMG amplitude, was the only observed training effect (1.95+/-1.16 vs. 1.10+/-0.67 ms for trained and untrained men, respectively). This effect could not be shown in the boys. The findings show normalized muscle strength and rate of activation to be lower in children compared with adults, regardless of training status. Because the observed higher Q30 values were not matched by corresponding higher performance measures in the trained men, the functional and discriminatory significance of Q30 remains unclear. Endurance training does not appear to affect muscle strength or rate of force development in either men or boys.

L’ablation de fibrillation auriculaire (FA) persistante est associée à des temps de procédure plus longs et à un moindre succès par rapport à l’ablation de FA paroxystique. Nous avons posé l’hypothèse que la restauration et le maintien du rythme sinusal ≥ 1 mois pré-procédure faciliteraient la terminaison de la FA et amélioreraient le succès de la procédure. Méthodes: Nous avons conduit une étude rétrospective comparant deux cohortes de patients consécutifs se présentant en FA persistante pour première ablation de FA : le groupe rythme sinusal (RS) avec restauration et maintien du rythme RS ≥ 1 mois pré-procédure (Groupe RS; N=40) et un groupe contrôle de patients en FA pré ablation (Groupe contrôle; N=40), apparié selon le sexe, l’âge, la longueur maximale de durée de FA. Une ablation de type en paliers (“stepwise”) a été réalisée dans les deux groupes (avec FA induite en début de procédure dans le groupe RS). Le succès a été défini par l’absence de récidive de FA ou de tachycardie atriale sans anti arythmique après un suivi minimal de 12 mois post procédure. Résultats: Durant la procédure de index d’ablation de FA, le cycle de FA était plus long dans le groupe RS par rapport au groupe contrôle (183±32 vs 166±20 ms, P=0.06) suggérant un remodelage inverse. Dans le groupe RS, la FA a été terminée plus fréquemment par l’ablation (95.0% vs 77.5%, P<0.05) et a demandé une ablation moins extensive avec moins d’ablation des électrocardiogrammes fragmentés (40.0% vs 87.5%, p<0.001) et moins de lésions linéaires (42.5% vs 82.5%, p<0.001). Les durées moyennes de procédure (199.8±69.8 vs 283.5±72.3 minutes, P<0.001), de fluoroscopie (51.0±24.9 vs 96.3±32.1 minutes, P<0.001), et de radiofréquence (47.5±18.9 vs 97.0±30.6 minutes, P<0.001) ont été plus courtes dans le groupe RS. Les succès cliniques ont été comparables dans les deux groupes après la première (55.0% vs 45.0%, P=0.28) et la dernière procédure (80.0% vs 70.0%, P=0.28), après une durée moyenne de suivi comparable (21.1±9.7 mois).

Este estudio de caso se enfoca en identificar cuales fueron los factores que motivaron la participación de los partidos kurdos PDK y UPK en la invasión a Irak 2003 – 2009. Con base en los supuestos teóricos de autores realistas K.Waltz, M. Ayoob y liberales y R. Keohane y J. Nye, se estudian las dinámicas y las interacciones que permitieron establecer un acercamiento con los Estados Unidos con un énfasis específico en los intereses políticos de los partidos kurdos. El kurdistán iraquí llega a ser el área clave para el acercamiento de la potencia a la zona y los intereses políticos permitieron establecer un escenario de cooperación e interdependencia, incrementando tanto la participación como la autonomía de los kurdos en las dinámicas del país y de la región.
