A agressividade na infância tem sido apresentada como queixa recorrente por pais e educadores, o que instaura um cenário preocupante na medida em que a identificação da criança e sua família como principais responsáveis ainda é acentuada. Neste estudo apresenta-se inicialmente uma compreensão da agressividade na infância a partir da Abordagem Centrada na Pessoa, assim como a proposta de educação neste referencial articulando com alguns princípios da teoria da complexidade. Considerando que a criança reconhecida como agressiva vem se constituído num processo de subjetivação no qual as pessoas socialmente significativas a ela estão implicadas, esta pesquisa por meio de uma investigação fenomenológica teve como objetivo verificar a configuração deste reconhecimento a partir da análise dos depoimentos dos participantes: a criança identificada como agressiva, um colega, a mãe e a professora. O estudo foi realizado numa escola selecionada a partir do mapeamento feito pelo Observatório de Violência nas Escolas Núcleo-Pa. Os resultados encontrados apontam para: uma visão de subjetividade linear subsidiando as forma de relacionar; o distanciamento docente utilizado como recurso para evitar o conflito; a agressividade manifestada denunciando as histórias pregressas do aluno e a vivência atual; uma relação entre a condição da criança reagir ao rótulo e a história familiar; as repercussões da forma como é reconhecida na escola em seu processo de aprendizagem. Os sentimentos vivenciados pelos participantes permitem alertar para a condição de implicabilidade que os envolve, reafirmando a necessidade de se buscar caminhos que promovam mudanças na forma de ver o aluno e a própria escola. Tais mudanças precisam ser instauradas a partir de uma visão de subjetividade humana interativa complexa, que possibilite entender a agressividade a partir de um cenário intersubjetivo que pode revelar múltiplos significados.

Este estudo avalia a implantação e desenvolvimento do Serviço de Plantão Psicológico em um Centro de Terapia Intensiva - CTI de um hospital universitário vinculado a rede pública de saúde, na capital paraense. O serviço foi disponibilizado aos familiares de pacientes internados e demais membros da equipe de saúde intensivista, funcionando na antessala do referido setor, duas vezes por semana, durante quatro meses. Alicerçado sob os pilares da Abordagem Centrada na Pessoa (ACP) buscou-se compreender os pressupostos teórico-metodológicos que fundamentam essa modalidade de cuidado, as especificidades do setting no que se referem aos objetivos, ações e funções do plantonista, assim como, as urgências reveladas neste contexto. Para tanto, elegeu-se como método do estudo a pesquisa qualitativa de base fenomenológica, sendo avaliadas as trajetórias do semear e germinar do Plantão Psicológico. São analisados seis casos clínicos, os quais lançam luz sobre essa modalidade de atenção psicológica no CTI. Quanto ao perfil da clientela atendida se observou que essa foi composta predominantemente por familiares, mulheres entre 20 a 75 anos, em média com o Ensino Fundamental e renda de um salário mínimo mensal. Os resultados indicam a necessidade e viabilidade da oferta do Plantão Psicológico no CTI, as demandas urgentes por auxílio psicológico, desveladas nos sentidos que os clientes atribuíram as suas experiências, tais como, medo de que o familiar faleça, sensação de abandono do familiar, culpa por não poder permanecer ao seu lado, tristeza intensa em razão do estado de saúde ou quando do óbito, entre outros. Dois tipos de atendimentos foram, naturalmente, criados: o individual e o grupal, sendo consideradas as especificidades das demandas. Ressalta-se também quanto a esta modalidade a disponibilização do pronto atendimento as urgências, acolhimento e estímulo a comunicação. Portanto, considera-se que a oferta do Plantão Psicológico no CTI revelou-se necessária como um espaço de cuidado psíquico aceito, utilizado e legitimado pelos clientes, além de se configurar em dois momentos distintos, antes e após ás visitas, sendo que no primeiro destes, destacam-se as intervenções voltadas ao acolhimento e fortalecimento da organização do self, enquanto no segundo, aquelas voltadas a ajudar os clientes na ressignificação de suas experiências ameaçadoras e a reorganização do self.

Developing career-choice readiness is an important task in adolescence, but current theory and research has provided a rather static view of the phenomenon. The present study investigated the development of career-choice readiness among a group of 325 Swiss students assessed four times every 5 months from seventh through eighth grade. A variable-centered approach applying latent curve modeling showed not only a linear increase of readiness over time but also significant inter-individual differences in the level and development of readiness. Higher levels were predicted by more self-esteem and generalized self-efficacy and fewer perceived barriers while increase in readiness was predicted by increase in occupational information. A person-centered approach applying latent class-growth analysis identified four distinct developmental trajectories: high-increasing (42%), high-decreasing (5%), moderate-increasing (42%), and constantly low (11%). Students with different trajectories showed significant differences in core self-evaluations, occupational knowledge, and barriers. The results suggest that environmental demands promote a developmental trend in readiness development that overrules individual differences for the majority of students. Individual differences affect the level of readiness to a greater extent than the process of its development. Career information seems pivotal for readiness increase.

Research on career adaptability predominantly uses variable-centered approaches that focus on the average effects in terms of the predictors and outcomes within a given sample. Extending this research, the present paper used a person-centered approach to determine whether subgroups with distinct adaptability profiles in terms of concern, control, curiosity and confidence can be identified. We also explored the relationship between the various adaptability profiles and adapting (career planning, career decision-making difficulties, career exploration, and occupational self-efficacy beliefs) and adaptivity (core self-evaluations and proactivity). Using latent profile analysis, we found distinct adaptability profiles among 350 German university students. Students with different profiles differed significantly in their levels of adapting. This finding was confirmed in a second study of 1226 students selected from the same population. In both samples, the adaptability profiles differed mainly in terms of their adaptability levels but not their shape. Moreover, in both samples, the students whose profiles indicated generally higher adaptability showed more adapting compared with the students whose profiles indicated generally lower adaptability. Study 2 also showed that students with higher-adaptability profiles showed significantly higher adaptivity. The results suggest that level effects dominate adaptability profiles, implying the existence of a general adaptability factor within university students that is meaningfully related to adapting and adaptivity.

Motivation plays a key role in successful entry into working life. Based on a cross-sectional and a one-year longitudinal study, we used a person-centered approach to explore work-related motivation (i.e., autonomous goals, positive affect, and occupational self-efficacy) among 577 students in 8th grade (Study 1) and 949 adolescents in vocational training (Study 2). Based on latent profile analysis, in both studies we identified four groups that were characterized by different levels of overall motivation and one group characterized by low positive affect and mean levels in autonomous goals and self-efficacy. Profiles characterized by high levels of motivation showed the highest levels of positive work expectations and goal engagement and the lowest levels of negative work expectations in Study 1 and the highest levels of person-job fit, work engagement, and job satisfaction in Study 2. Moreover, latent difference score analysis showed that motivational profiles predicted changes in person-job fit and work engagement across one year but not in job satisfaction. The results imply that career counselors should be aware of characteristic motivational patterns of clients that may require specific counseling approaches.

Research has shown that chance events affect careers but has not established the nature of their effects. Moreover, the relationship between chance and career decidedness is not well understood. The present study used a person-centered approach with latent profile analysis to examine 312 Swiss adolescents in their first year of vocational training. We identified five qualitatively differing profiles according to levels of perceived chance events and career decidedness: balanced scorers, undecided with mean chance, undecided with high chance, decided with chance, and decided without chance. The groups differed significantly in work motivation (i.e., occupational self-efficacy beliefs, perceived person-job fit, and work engagement). Decided adolescents reported more favorable work motivation regardless of their level of perceived chance events. The results imply that promoting decidedness remains a valuable goal in career counseling despite the occurrence of unpredicted events.

This study evaluated inter- and intra-individual changes in acculturation, acculturative stress, and adaptation experiences, as well as their associations with adjustment outcomes among a group of Latino adolescents in South Florida. Specifically, the current study investigated the incidence, changes, and effects of stressors that arise from acculturation experiences (e.g., related to culture, discrimination, language difficulties) among Latino youth by employing a person-centered approach and a longitudinal research design. Four separate groups of analyses were conducted to investigate (a) within-group differences in levels of reported acculturative stress, (b) patterns of continuity and discontinuity in levels of acculturative stress across time, (c) adjustment outcomes associated with distinct patterns of acculturative stress within each measurement occasion, and (d) predictive relations between longitudinal acculturative stress trajectories in early adolescence and psychosocial adjustment outcomes in young adulthood. ^ Results from the multivariate analyses indicated great within group heterogeneity in acculturative stress among Latino youth during early adolescence, as well as significant continuity and discontinuity in the patterns of shifts among acculturative stress profiles between contiguous measurement occasions. Within each developmental period, membership in acculturative stress clusters was significantly and differentially associated with multiple adjustment outcomes, suggesting that maladaptive outcomes are more likely to occur among Latino adolescents experiencing high levels of psychological distress across multiple acculturative domains. In general, Latino youth acculturation is best understood as multi-dimensional, to be variable across time, and to be fluid and responsive to multiple factors and influences. Implications for preventive strategies are discussed with regard to acculturation and developmental psychology research literatures. ^

Bakgrund: HIV är en infektionssjukdom som kan överföras vid bland annat oskyddade samlag, via blod och transplanterad vävnad. Då det idag finns antivirala läkemedel ses HIV inte längre som en dödlig utan en kronisk sjukdom. Personer med HIV/AIDS upplever att sjukdomen påverkar den fysiska och psykiska hälsan negativt. Sjuksköterskor har bristande kunskaper om sjukdomen samt är ovilliga att ge omvårdnad till dessa personer. Syfte: Syftet med litteraturöversikten var att beskriva personers upplevelser av att leva med HIV/AIDS. Metod: Litteraturöversikten baserades på 13 artiklar med kvalitativ design som analyserades enligt Fribergs femstegsmodell. En induktiv ansats användes. Resultat: Huvudtemat stigmatisering identifierades tillsammans med temat transition med tillhörande fem underteman samt temat rädsla med tillhörande tre underteman. Slutsats: Personerna med HIV/AIDS upplevde sig stigmatiserade av närstående, i samhället och i kontakt med hälso- och sjukvården. För att stigmatiseringen ska upphöra behöver sjuksköterskor mer kunskap om HIV/AIDS. För att nå ett personcentrerat förhållningssätt behöver sjuksköterskor se personerna bakom sjukdomen samt medvetandegöra sina attityder och förutfattade meningar mot personerna som lever med HIV/AIDS.

As infeções sexualmente transmissíveis (IST) constituem um problema persistente de saúde pública, sendo os adolescentes e adultos jovens os que apresentam as taxas de prevalência mais elevadas para algumas IST. As IST não víricas nos países desenvolvidos incluem a Chlamydia Trachomatis, a Neisseria gonorrhoeae, o Treponema pallidume a Trichomonas vaginalis. A deteção precoce das IST não víricas tem impacto positivo a nível individual e na saúde pública: permite instituição atem- pada de tratamento adequado, a redução de transmissão entre parceiros, bem como reduzir as complicações a longo prazo, nomeadamente doença inflamatória pélvica, dor pélvica crónica, gravidez ectópica e infertilidade. Várias sociedades médicas internacionais publicaram recomendações para o rastreio de algumas IST não víricas em de- terminados grupos. Em Portugal, a Direção Geral de Saúde (DGS) atualizou em 2014 a norma sobre a notificação obrigatória de doenças transmissíveis, que inclui a gonorreia, a sífilis e a infeção por Chlamydia Trachomatis. Não obstante, os estudos sobre a epidemiologia de IST são parcos em Portugal e apenas recentemente foi contemplado o rastreio oportunístico de infeção genital por Chlamydia Trachomatis no Plano Nacional de Saúde 2011-2016. O médico de família através da sua abordagem holística centrada na pessoa, no seu contexto familiar e social (focando antecedentes pessoais / comportamentos de risco) tem necessariamente um papel determinante na prevenção primária e no rastreio das IST.

Purpose of the Study: A framework aids choice of interventions to manage wandering and prevent elopement in consideration of associated risks and mobility needs of wanderers. ---------- Design and Methods: A literature review, together with research results, published wandering tools, clinical reports, author clinical experience, and consensus-based judgments was used to build a decision-making framework. Results: Referencing a published definition of wandering and originating a clinical description of problematic wandering, authors introduce a framework comprising (1) wandering and related behaviors; (2) goals of wandering-specific care, (3) interpersonally, technologically, and policy-mediated wandering interventions, and (4) estimates of relative frequencies of wandering behaviors, magnitudes of elopement risk, and restrictiveness of strategies. ---------- Implications: Safeguarding wanderers from elopement risk is rendered person-centered and humane when goals of care guide intervention choice. Despite limitations, a reasoned, systematized approach to wandering management provides a basis for tailoring a specialized program of care. The need for framework refinement and related research is emphasized.

Objective The main objective of the project was to explore the barriers and obstacles impeding a person-centred approach to planning and private housing for people with disability. Method Methodologically, the project involved explanation building using a multiple case study approach supported by a contextual study. It focussed initially on three organisations and their attempts to integrate innovative and what they regarded as person-centred models of housing into the private housing market for people with disability. It also included a fourth case highlighting the experiences of individuals with disability in accessing suitable and affordable housing. Results Using an ecological framework, the project found that: • Challenges exist within systems (such as the macro cultural, economic, regulatory systems through to local community, family and intra personal systems) as well as with interaction between systems • Reaching across systems is a key role for organisations and individuals but is very challenging with distance from the individual as well as from the policy/funding/service systems being a key aspect of the nature and extent by which they are challenged • In the case of housing for people with disability a ‘disability space’ is assumed and maintained disparately within each system and is separate from the ‘mainstream space’ with the established policy, legal, funding structures making it difficult to move between the two spaces. Conclusions Based on these findings, the project makes recommendations for government, community organisations, the housing industry, people with disability and their families and support networks, as well as for future research. An overarching recommendation is the need to address housing stock availability and suitability by adopting a mainstream approach rather than a disability-first/disability-specific approach.

O consumo de cocaína e crack gera importantes repercussões para saúde. Em relação aos usuários destas drogas, há predomínio dos homens sobre as mulheres. Em virtude das mulheres serem o grupo minoritário, o cuidado de saúde mental nem sempre observa as especificidades do gênero feminino e suas vulnerabilidades no processo saúde-doença. Para investigar esta problemática, foi proposto o objeto de estudo "As singularidades do gênero feminino no cuidado psicossocial às usuárias de cocaína e crack". Delimitaram-se os seguintes objetivos: Analisar o cuidado psicossocial às mulheres usuárias de Crack e Cocaína e Discutir a abordagem das singularidades do gênero feminino neste cuidado. Adotou-se como referencial teórico da pesquisa a categoria Gênero. Para alcançar estes objetivos, optou-se por pesquisa qualitativa, que foi desenvolvida no único CAPS ad do município de Duque de Caxias, localizado na Baixada Fluminense do Estado do Rio de Janeiro. Os participantes da pesquisa foram profissionais de saúde que exercem o cuidado das mulheres usuárias de cocaína e crack. Para coleta de dados, utilizou-se a triangulação de técnicas: a) observação sistemática nos espaços de cuidado coletivo; b) entrevistas semiestruturadas com os profissionais de saúde e c) análise documental dos prontuários das mulheres. A análise dos dados empíricos foi orientada pela Hermenêutica-Dialética. Foram analisados 113 prontuários das mulheres assistidas no CAPS ad. A maioria das mulheres estava na faixa etária de 20 a 34 anos, solteiras, mães com prole menor de idade, que viviam com os familiares, não tinham fonte de renda própria e envolvimento com a justiça. Quase a totalidade utilizava também outras drogas, como tabaco, maconha e álcool. Foram entrevistados 17 profissionais de saúde. As categorias da pesquisa foram: Concepções dos profissionais sobre o cuidado psicossocial: centrado na pessoa e centrado na doença; as questões do gênero feminino e as usuárias de crack e cocaína; a condição feminina e suas influências no cuidado psicossocial. As singularidades de gênero no cuidado psicossocial foram reveladas no comportamento e enfrentamento das mulheres frente ao uso de cocaína e crack, mas também nas estratégias de cuidado adotadas pelos profissionais. O cuidado psicossocial por vezes reforça os estereótipos de gênero e, por outra, estimula o exercício da autonomia feminina. Os profissionais apresentaram percepções determinadas pelas questões de gênero, atribuindo às mulheres características distintivas, como a "fragilidade" e a dependência emocional, que interferem nas vivências femininas acerca do uso de cocaína e crack. A prostituição surgiu como uma consequência da vulnerabilidade do gênero feminino no contexto de consumo de drogas. Recomenda-se a implementação de ações programáticas direcionadas para as singularidades da clientela feminina e a discussão das iniquidades de gênero no âmbito da formação profissional, da assistência e da pesquisa para superar a práxis reducionista e a naturalização das diferenças e da subalternidade feminina nestes espaços de produção de saúde. Como integrante da equipe de saúde, enfermeiros e auxiliares de enfermagem necessitam estar sensibilizados para as questões de gênero e terem uma maior participação no cuidado individual e coletivo desta clientela.

Rationale, aims and objectives: Intermediate care (IC) describes a range of services targeted at older people, aimed at preventing unnecessary hospitalisation, promoting faster recovery and maximising independence. The introduction of IC has created a new interface between primary and secondary care. Older people are known to be at an increased risk of medication-related problems when transferring between healthcare settings and pharmacists are often not included as part of IC multidisciplinary teams. This study aimed to explore community pharmacists’ (CPs) awareness of IC services and to investigate their views of and attitudes towards the medicines management aspects of such services, including the transfer of medication information.

Method: Semi-structured interviews were conducted, recorded, transcribed verbatim and analysed using a constant comparative approach with CPs practising in the vicinity of IC facilities in Northern Ireland, UK.

Results: Interviews were conducted with 16 CPs. Three themes were identified and named ‘left out of the loop’, ‘chasing things up’ and ‘closing the loop’. CPs felt that they were often ‘left out of the loop’ with regards to both their involvement with local IC services and communication across the healthcare interfaces. As a result, CPs resorted to ‘chasing things up’ as they had to proactively try to obtain information relating to patients’ medications. CPs viewed themselves as ideally placed to facilitate medicines management across the healthcare interfaces (i.e., ‘closing the loop’), but several barriers to potential services were identified.

Conclusion: CPs have limited involvement with IC services. There is a need for improvement of effective communication of patients’ medication information between secondary care, IC and community pharmacy. Increasing CP involvement may contribute to improving continuity of care across such healthcare interfaces, thereby increasing the person-centeredness of service provision.

Autistic adults with limited speech and additional learning disabilities are people whose perceptions and interactions with their environment are unique, but whose experiences are under-explored in design research. This PhD by Practice investigates how people with autism experience their home environment through a collaboration with the autism charity Kingwood Trust, which gave the designer extensive access to a community of autistic adults that it supports. The PhD reflects upon a neurotypical designer’s approach to working with autistic adults to investigate their relationship with the environment. It identifies and develops collaborative design tools for autistic adults, their support staff and family members to be involved. The PhD presents three design studies that explore a person’s interaction with three environmental contexts of the home i.e. garden, everyday objects and interiors. A strengths-based rather than a deficit-based approach is adopted which draws upon an autistic person’s sensory preferences, special interests and action capabilities, to unravel what discomfort and delight might mean for an autistic person; this approach is translated into three design solutions to enhance their experience at home. By working beyond the boundaries of a neurotypical culture, the PhD bridges the autistic and neurotypical worlds of experience and draws upon what the mainstream design field can learn from designing with autistic people with additional learning disabilities. It also provides insights into the subjective experiences of people who have very different ways of seeing, doing and being in the environment

Personal Support Workers (PSWs) spend a large amount of time with long-term care (LTC) home residents providing assistance with their activities of daily living. The s limited research on their perceptions of cultural competence presents the need to bridge this knowledge gap. The researcher conducted a qualitative case study at a LTC home in Ontario. Data were collected by conducting a policy document analysis, a key informant interview with the Director of Care (DOC), and two focus groups with PSWs. The five major overarching themes were: The Culture of the LTC Home, Provision of a Supportive Environment, Collaborative Team Approach to Care, Building a Relationship with the Residents, and Maintenance of Staff Morale. The findings illuminated the broad nature of culture, connections to person centered care, and the factors that facilitate or hinder PSWs’ culturally competent care. The ambiguous perception of cultural competence among PSWs suggests further research and education on cultural competence in LTC home settings.
