OBJETIVOS: Avaliar o efeito da facoemulsificação com incisão em córnea clara no meridiano mais curvo sobre a magnitude do astigmatismo ceratométrico pré-operatório. Mapear a magnitude do astigmatismo cirurgicamente induzido por incisões nas posições nasal (N), temporal (T), temporal superior (TS) e temporal inferior (TI). MÉTODOS: Foi realizado estudo ceratométrico prospectivo em 48 olhos de 48 pacientes, submetidos a facoemulsificação com incisão do meridiano mais curvo. As medidas ceratométricas foram realizadas no pré-operatório e um mês após a realização da cirurgia. O astigmatismo cirurgicamente induzido foi determinado pelo método das coordenadas retangulares em 10 passos, modificado. RESULTADOS: Foram operados 21 olhos direitos e 27 olhos esquerdos e, de acordo com a posição das incisões em córnea clara, foram divididos em: 16 olhos N, 4 olhos T, 22 olhos TS, 6 olhos TI. A média da ametropia cilíndrica pré-operatória foi de 1,06D ± 0,65 e a pós-operatória de 0,89D ± 0,80. Houve diminuição estatisticamente significativa no astigmatismo corneal preexistente (p=0,016). A média total de astigmatismo cirurgicamente induzido foi de 0,94D ± 0,56. em relação aos grupos o astigmatismo cirurgicamente induzido foi de 1,06D ± 0,66 em N, 0,87D ± 0,20 em T, 0,95D ± 0,55 em TS e 0,61D ± 0,25 em TI. Não houve diferença estatística com relação ao astigmatismo cirurgicamente induzido nos grupos N, T, TS e TI (p=0,426). CONCLUSÃO: A técnica se mostrou efetiva na redução do astigmatismo ceratométrico pré-operatório. A média do astigmatismo cirurgicamente induzido foi de 1,06D ± 0,66 em N, 0,95D ± 0,55 em TS, 0,61D ± 0,25 em T e 0,87D ± 0,20 em TI.

Avaliaram-se as células endoteliais, a espessura corneana e a pressão intraocular (PIO) de cães portadores de catarata madura, empregando-se viscoelástico à base de hialuronato de sódio 3% e sulfato de condroitina 4% e hidroxipropilmetilcelulose 2%, utilizando-se 20 cães, distribuídos entre os dois grupos dos viscoelásticos. A técnica cirúrgica adotada foi a da facoemulsificação bimanual. As avaliações tonométricas foram efetuadas antes e após o ato cirúrgico, aos 1, 7, 14, 21, 28 e 60 dias de pós-operatório, e a microscopia especular, antes e após 7, 28 e 60 dias. Não houve diferença estatística entre os grupos quanto à PIO, com exceção aos 14 dias, em que se observou maior PIO com o uso de hialuronato de sódio 3% e sulfato de condroitina 4%. Não houve diferença entre os grupos quanto aos parâmetros relacionados ao endotélio, com diminuição discreta da densidade celular endotelial e aumento da área celular com a utilização de hidroxipropilmetilcelulose 2%. A utilização de ambos os dispositivos viscoelásticos analisados é recomendada para o procedimento de facoemulsificação em cães.

Foram utilizados 20 cães de raças e idades variadas, machos e fêmeas, portadores de catarata e não diabéticos, os quais foram submetidos ao exame oftálmico. Posteriormente, realizaram-se mensurações oculares empregando-se um ecobiômetro ultra-sônico (ultra-sonografia modo-A) para o cálculo do poder dióptrico da lente intra-ocular por meio da fórmula SRK/T. O comprimento axial médio foi de 19,94±1,12mm. Todos os animais foram submetidos à facoemulsificação extracapsular. A lente calculada foi implantada no transoperatório da cirurgia de catarata, obtendo-se média de 37,33±3,05D. A avaliação pós-cirúrgica do erro refracional aos 60 dias de pós-operatório, pela retinoscopia, com a utilização da esquiascopia, foi de 5,57±1,59D. A fórmula SRK/T não ofereceu bons resultados.

Purpose: To compare a single intraoperative sub-Tenon's capsule triamcinolone acetonicle injection with steroid drops in the treatment of ocular inflammation after cataract surgery.Design: Randomized, double-masked controlled trial.Participants: A total of 100 patients were randomized prospectively into 2 groups: 50 patients treated with 1% prednisolone eyedrops (control group A) and 50 patients treated with sub-Tenon's capsule triamcinolone (treatment group B).Methods: All patients underwent phacoemulsification and intraocular posterior lens implantation. After surgery, patients were randomized to receive either (group B) an intraoperative 40 mg triamcinolone acetonicle sub-Tenon's capsule injection or (group A) 1% prednisolone acetate eyedrops, according to the following schedule: 1 drop 4 times daily (week 1), 3 times daily (week 2), 2 times daily (week 3), once daily (week 4). To mask the study, group B received vehicle drops administered on a similar schedule, and group A received an intraoperative sub-Tenon's capsule injection of a 1 ml balanced salt solution.Main Outcome Measures: the main outcome measures included inflammation (cell, flare, ciliary flush), intraocular pressure, and lack of response.Results: Triamcinolone was shown to have anti-inflammatory efficacy clinically equivalent to conventional 1% prednisolone eyedrops in reducing intraocular inflammation, as measured by clinical methods. Triamcinolone was found to be as safe as the prednisolone in terms of adverse effects, changes in visual acuity, intraocular pressure, and biomicroscopic and ophthalmoscopic variables. on the third, seventh, fourteenth, and twenty-eighth postoperative days, a significantly lower intraocular pressure (P<0.01) was noted in the triamcinolone group than in the prednisolone group.Conclusions: A single intraoperative 40-mg triamcinolone acetonide sub-Tenon's capsule injection demonstrated a clinically equivalent therapeutic response and ocular tolerance compared with 1% prednisolone drops in controlling postoperative inflammation after uncomplicated cataract surgery and merits further investigation. BACKGROUND AND OBJECTIVES: The objective of the present study was to evaluate the degree of sedation, intraocular pressure, and hemodynamic changes with premedication with low doses of oral clonidine, 100 μg and 200 μg, in outpatient cataract surgeries. METHODS: This is a randomized, double-blind, clinical study undertaken at the Universidade Federal de São Paulo with 60 patients of both genders, physical status ASA 1 and 2, ages 18 to 80 years. Patients were separated into three groups: placebo, clonidine 100 μg, and clonidine 200 μg. Intraocular pressure, heart rate, and blood pressure besides assessment of sedation were measured before and 90 minutes after the administration of clonidine. Sedation levels were classified according to the Ramsay sedation scale. RESULTS: Patients who received placebo and 100 μg of clonidine did not show reduction in heart rate, while a reduction in heart rate was observed in patients who received 200 μg of clonidine, and this difference was statistically significant. Patients who received 200 μg of clonidine also had a reduction in systolic and diastolic blood pressure (p < 0.05). One patient who received 200 μg of clonidine developed severe hypotension, with systolic pressure < 80 mmHg. Patients treated with clonidine had a reduction in intraocular pressure (p < 0.05). Ninety minutes after the oral administration of placebo and 100 μg and 200 μg of clonidine, 25%, 60%, and 80% of the patients respectively were classified as Ramsay 3 or 4. CONCLUSIONS: Clonidine 100 μg can be indicated as premedication for fasciectomies, being effective in sedation and reduction of intraocular pressure, without adverse effects on blood pressure and heart rate.

This study aimed to describe and compare the ultrasonographic alterations in dogs' eyes submitted to facectomy with or without intraocular lens implant (IOL), to assist in the diagnosis of possible alterations related to the surgical procedure and IOL implantation. Nineteen dogs with cataract (21 eyes) were submitted to phacoemulsification and late postoperative evaluation (at five years). The animals were initially submitted to complete ophthalmological exams which preceded the sonogram. Dogs were divided in three groups: (CA) aphakic dogs (n= 11); (PP) pseudophakic dogs with implantation of two IOLs in piggyback (n=5) and (PL) pseudophakic dogs with implantation of a veterinary IOL (n = 5). The ultrasound was carried under the administration of a topic local anesthetic, with a multi frequency linear transducer of 10 MHz. Biometric eye measurements were also performed. The clinical alterations observed were IOL dislocation, retinal detachment, asteroid hyalosis, and vitreous degeneration. Ultrasound examination was an excellent diagnostic tool, as it was possible to confirm and classify these changes. The piggyback implant reduced the measurements between the ciliary body and the vitreous chamber obtained from the ocular biometry when compared to other usual procedures, with no difference between the axial length and the anterior chamber.

This study aims, by a literature review, to evaluate the effects of the phacoemulsification surgery into the endothelial cells of the cornea, especially on dogs. The phacoemulsification is a surgical technique of cataract surgery, and presents a series of factors pre, trans and post operation that can affect the cornea endothelium. Knowing that the cornea transparency is essential to the preservation of the visual acuity, and that the endothelium cells do not demonstrate an adequate regeneration, it is important to the surgeon to be aware of these factors in order to avoid or to minimize them. Among then can be mentioned: previous diseases, surgical and chemical trauma, intra-ocular lens, uveitis etc. As consequence, a reduction on the number of endothelium cells and edema can be observed, which can lead to a loss of vision by definitive cornea opacity or to late visual recuperation

Cataract is the leading cause of blindness in dogs. It is defined as an opacity in the lens and is independent of resulting visual deficit. Although there are a growing number of studies related to drug therapy of cataract, the treatment is still considered exclusively surgical. Among the techniques used to remove cataract, phacoemulsification is the best performer. Anesthesia in intraocular surgery has several peculiarities, which are essential to successfully conduct phacoemulsification in dogs. In these patients, a safe and effective anesthesia requires the maintenance of intraocular pressure close to normal, preventing the activation of the oculo-cardiac reflex and complete immobilization of the eye. The appropriate conditions for such surgical procedures can be established through the use of drugs from different pharmacological groups for premedication, induction and maintenance of anesthesia, making a balanced anesthesia. This work was compiled from a careful review of the literature on anesthesia in cataract surgery. Considerations on the pre-medication, induction and maintenance of anesthesia are discussed, aiming to contribute to the phacoemulsification becomes more safe and suitable for the anesthesiologist and the surgeon as well as comfortable for dogs undergoing surgery
