The implants are usually used after removal of the eye or its internal content, in order to maintain the orbital volume or, as in medicine, provide also the subsequent placement of ocular prosthesis. Considering prosthetic surgery procedures of high relevance in human and veterinary ophthalmology, this paper seek to review the main types of implants described in national and international literature, as well as describe the main complications when they are used in humans and veterinary patients.

Background: Tooh extraction was demonstrated to increase sympathetic modulation of the heart, however, it is not understood the effects of endodontic treatment on cardiac autonomic regulation. We evaluated heart rate variability (HRV) during endodontic treatment. Method: 50 male and female patients aged between 18 and 40 years old and diagnosed with irreversible pulpitis or pulp necrosis of the lower molars with indication of gender endodontic treatment were analyzed. HRV was analyzed in the time (SDNN, RMSSD, pNN50) and frequency (LF, HF and LF/HF ratio) domains recorded in the first session of root canal treatment. The indices were analyzed in the following periods: T1) ten minutes before the endodontic treatment, T2) ten minutes after the administration of anesthesia before endodontic treatment, T3) during the entire period of endodontic treatment and T4) thirty minutes after the end of the endodontic treatment. Results: The SDNN, RMSSD and pNN50 indices increased at T2 compared to T1, the pNN50 and RMSSD indices increased at T3 and T4 compared to T2. The LF in normalized units increased at T4 compared to T2 and in absolute units it was increased at T4 compared to T1. The HF in normalized units was reduced at T4 compared to T2 and in absolute units was reduced at T1, T3 and T4 compared to T2. The LF/HF ratio was higher at T4 compared to T2. Conclusion: The global modulation of heart increases after local anesthesia and vagal tone reduce during surgery procedures and after the surgical procedures.

Photodynamic Therapy (PDT) is a therapeutic method which employs a photosensitizer and light to cause cellular death. The chemical compounds have low or none toxicity for hosts cells. Under the incidence of light, in an appropriated wavelength, these chemical compounds produce reactive oxygen which affects the biomolecules of the target-cells. The specific illumination of the affected area increases the selectivity of the therapy, since the photodynamic process occurs only in the irradiated area. Pythiosis, for instance, is a life-threatening emerging disease caused by a fungus-like organism called Pythium insidiosum. The disease occurs in man and other animals, being mostly observed in horses. Human pythiosis may present as ophthalmic, cutaneous-subcutaneous and systemic forms of lesions. Due to the fact that P. insidiosum is not a true fungus, it is refractory to most antifungal drugs and the treatment of the disease is difficult. Extensive surgery procedures, such as limb amputation, are the treatment of choice, however relapses may occur frequently. Although not totally effective, the use of immunotherapy associated to surgery have shown some results. Considering that pythiosis is an emerging disease few explored in its etiological and therapeutic aspects, which are limited and few effective, it is of great importance to encourage the development of researches for new strategies of treatment. In this sense, it was evaluated the effect of PDT on in vitro growth of the pathogen employing two chemical compounds as photosensitizer, porphyrin and chlorine, at different concentrations in combination with several energetic dosages. Porphyrin showed inhibition of growth at 25mg/mL with 100J/cm2 of energetic dosage and chlorine showed similar results employing low concentrations (0,7, 1,0 and 1,3mg/mL) with 70J/cm2 of energetic dosage... Pythiosis is caused by the oomycetous Pythium insidiosum and affect domestic and wild animals and man. The presence of water and vegetal material is fundamental for its life cycle in nature. The biflagellate zoospore are the infective form of this pathogen. The lesions are generally of granulomatous aspect, which frequently may be contaminated by secondary bacterial infection in skin and subcutaneous tissue. Dissemination to systemic tissues may also occur and it may be due to the spread of the pathogen from cutaneous lesions, as well as a primary source of infection. Clinical signs depend on the affected tissue. Diagnosis of pythiosis is based on the clinical manifestations, histopathological sections and culture of the pathogen. Serological tests may also be employed and more recently molecular biology has been introduced as a sensitive, specific and a rapid method for conclusive diagnosis. Treatment is often difficult and extensive surgery procedures are required, however, depending on the anatomic region and size of the lesion, such procedure is unfeasible and relapses are frequent. Due to the climate changes, which has contributed to increase the incidence of pythiosis, it is necessary the search for new therapeutic protocols

Esta pesquisa objetivou caracterizar as vítimas de ocorrência de trânsito submetidas a procedimentos anestésico-cirúrgicos, segundo dados demográficos e clínicos, e identificar os preditores de intercorrências no período transoperatório (choque hemorrágico ou óbito). Estudo de corte transversal desenvolvido a partir de consulta aos prontuários dos pacientes submetidos à cirurgia, no Instituto Central do Hospital das Clínicas da FMUSP. Nos 69 pacientes, predominaram os jovens, do sexo masculino, vítimas de acidentes motociclísticos/ciclísticos e que receberam atendimento pré-hospitalar. As variáveis: abdome como região mais gravemente lesada pelo Injury Severity Score e as cirurgias geral e ortopédica, apresentaram associação estatística significativa com choque e óbito. A idade associou-se apenas com choque. No modelo final, o Injury Severity Score foi preditor para choque e óbito e a cirurgia geral, apenas para choque. A cirurgia ortopédica foi fator de proteção para óbito. Esses achados subsidiam a equipe cirúrgica no planejamento de estratégicas que visem à redução de desfechos indesejados.

Objetivos: Estudar o efeito da Oxigenoterapia Hiperbárica sobre as lesões causadas pela radiação ionizante na veia cava e aorta. Método: Foram utilizados 32 ratos adultos distribuídos aleatoriamente em 4 grupos: 3 no grupo controle; 10 no grupo oxigenoterapia hiperbárica (5 sessões e 3 ATA e após 3 dias, operados), 9 no grupo radioterapia (8 sessões de 5Gy, atingindo 54Gy, sendo operados após 7 dias), 10 no grupo radioterapia e oxigenoterapia hiperbárica (procedimento radioterapia, observação por 7 dias, submetidos a 5 sessões de oxigenoterapia hiperbárica e após 3 dias, são operados. Ressecamos a veia cava e aorta infrarenal para estudo morfológico (integridade endotelial, infiltrado inflamatório e integridade da vasa-vasorum), avaliação do ciclo celular pelo método do AgNOR e índice de apoptose nas distintas camadas dos vasos, através da Caspase-3 (imuno-histoquímica). Resultados: O grupo radioterapia – oxigenoterapia hiperbárica apresentou diminuição estatisticamente significativa das alterações morfológicas em vasa-vasorum, aumento da atividade celular na camada média e também diminuição do índice de apoptose nas camadas íntima e média da aorta infra-renal quando comparados ao grupo apenas submetidos à radiação ionizante. A lesões rádio induzidas na veia cava não apresentaram melhora significativa com a oxigenoterapia hiperbárica Conclusão: A oxigenoterapia hiperbárica é capaz de minimizar as lesões radio-induzidas em fase aguda em aorta.

INTRODUCTION: Surgical site infections (SSI) are the most common hospital-acquired infections among surgical patients, with significant impact on patient morbidity and health care costs. The Basel SSI Cohort Study was performed to evaluate risk factors and validate current preventive measures for SSI. The objective of the present article was to review the main results of this study and its implications for clinical practice and future research. SUMMARY OF METHODS OF THE BASEL SSI COHORT STUDY: The prospective observational cohort study included 6,283 consecutive general surgery procedures closely monitored for evidence of SSI up to 1 year after surgery. The dataset was analysed for the influence of various potential SSI risk factors, including timing of surgical antimicrobial prophylaxis (SAP), glove perforation, anaemia, transfusion and tutorial assistance, using multiple logistic regression analyses. In addition, post hoc analyses were performed to assess the economic burden of SSI, the efficiency of the clinical SSI surveillance system, and the spectrum of SSI-causing pathogens. REVIEW OF MAIN RESULTS OF THE BASEL SSI COHORT STUDY: The overall SSI rate was 4.7% (293/6,283). While SAP was administered in most patients between 44 and 0 minutes before surgical incision, the lowest risk of SSI was recorded when the antibiotics were administered between 74 and 30 minutes before surgery. Glove perforation in the absence of SAP increased the risk of SSI (OR 2.0; CI 1.4-2.8; p <0.001). No significant association was found for anaemia, transfusion and tutorial assistance with the risk of SSI. The mean additional hospital cost in the event of SSI was CHF 19,638 (95% CI, 8,492-30,784). The surgical staff documented only 49% of in-hospital SSI; the infection control team registered the remaining 51%. Staphylococcus aureus was the most common SSI-causing pathogen (29% of all SSI with documented microbiology). No case of an antimicrobial-resistant pathogen was identified in this series. CONCLUSIONS: The Basel SSI Cohort Study suggested that SAP should be administered between 74 and 30 minutes before surgery. Due to the observational nature of these data, corroboration is planned in a randomized controlled trial, which is supported by the Swiss National Science Foundation. Routine change of gloves or double gloving is recommended in the absence of SAP. Anaemia, transfusion and tutorial assistance do not increase the risk of SSI. The substantial economic burden of in-hospital SSI has been confirmed. SSI surveillance by the surgical staff detected only half of all in-hospital SSI, which prompted the introduction of an electronic SSI surveillance system at the University Hospital of Basel and the Cantonal Hospital of Aarau. Due to the absence of multiresistant SSI-causing pathogens, the continuous use of single-shot single-drug SAP with cefuroxime (plus metronidazole in colorectal surgery) has been validated.

Position du problème: La mise en place de la tarification à l'activité pour les hôpitaux de court séjour pourrait entraîner une diminution des durées de séjour pour raisons financières. L'impact potentiel de ce phénomène sur la qualité des soins n'est pas connu. Les réadmissions identifiées à l'aide des données administratives hospitalières sont, pour certaines situations cliniques, des indicateurs de qualité des soins valides. Méthode: Étude rétrospective du lien entre la durée de séjour et la survenue de réadmissions imprévues liées au séjour initial, pour les cholécystectomies simples et les accouchements par voie basse sans complication, à partir des données du programme de médicalisation des systèmes d'information de l'Assistance publique-Hôpitaux de Paris des années 2002 à 2005. Résultats: Pour les deux procédures, la probabilité de réadmission suit une courbe en " J ". Après ajustement sur l'âge, le sexe, les comorbidités associées, l'hôpital et l'année d'admission, la probabilité de réadmission est plus élevée pour les durées de séjour les plus courtes : pour les cholécystectomies, odds ratio : 6,03 [IC95 % : 2,67-13,59] pour les hospitalisations d'un jour versus trois jours ; pour les accouchements, odds ratio : 1,74 [IC95 % : 1,05-2,91] pour les hospitalisations de deux jours versus trois jours. Conclusion: Pour deux pathologies communes, les durées de séjour les plus courtes sont associées à des probabilités de réadmission plus élevées. L'utilisation routinière des données du programme de médicalisation des systèmes d'information peut permettre d'assurer le suivi de la relation entre la réduction de la durée de séjour et les réadmissions. The prospective payment system for the French short-stay hospitals creates a financial incentive to reduce length of stay. The potential impact of the resulting decrease in length of stay on the quality of healthcare is unknown. Readmission rates are valid outcome indicators for some clinical procedures. Methods: Retrospective study of the association between length of stay and unplanned readmissions related to the initial stay, for two procedures: cholecystectomy and vaginal delivery. Data: Administrative diagnosis-related groups database of "Assistance publique-Hopitaux de Paris", a large teaching hospital, for years 2002 to 2005. Results: The risk of readmission according to length of stay, taking age, sex, comorbidity, hospital and year of admission into account, followed a J-shaped curve for both procedures. The probability of readmission was higher for very short stays, with odds ratios and 95% confidence intervals of 6.03 [2.67-13.59] for cholecystectomies (1- versus 3-night stays), and of 1.74 [1.05-2.91] for vaginal deliveries (2- versus 3-night stays). Conclusion: For both procedures, the shortest lengths of stay are associated with a higher readmission probability. Suitable indicators derived from administrative databases would enable monitoring of the association between length of stay and readmissions.

For severe obesity (BMI > 35 kg/m2), bariatric surgery is not only the best, but often the only means of obtaining sufficient and durable weight loss. This article aims to review the available bariatric procedures. Gastric bypass remains the reference when it comes to the risk/benefit ratio. Gastric banding is declining rapidly due to the high prevalence of long-term complications. Primary malabsorptive procedures remain largely unpopular because of their potential nutritional complications. Sleeve gastrectomy, although it is not reversible as it includes a significant gastric resection, increases currently in popularity because of its apparent simplicity and the fact that early results regarding weight loss mimic those obtained with gastric bypass.

Recently, a new combination of a self-adhering mesh together with a liquid 2-octyl cyanoacrylate adhesive (Prineo™; Ethicon, Inc., Somerville, NJ, USA) has been introduced to replace intracutaneous running suture.

This study involved a retrospective evaluation of patients subjected to surgery for dentofacial deformities treated without induced controlled hypotension (group I, n=50) and a prospective evaluation of patients who were subjected to surgery under hypotensive general anaesthesia (group II, n=50). No statistical differences were found between the study groups with regard to the duration of surgery. However, there were statistically significant differences in the need for blood transfusion and the occurrence of bradycardia during the maxillary down-fracture. Hypotensive anaesthesia decreased the need for a blood transfusion and the occurrence of bradycardia, and is therefore considered highly beneficial for patients undergoing orthognathic surgery.
