O presente estudo avaliou a velocidade da movimentação ortodôntica e as alterações dentárias e esqueléticas na distalização de molares superiores sob influência do laser de baixa intensidade por meio de telerradiografias laterais. A amostra foi constituída por 18 pacientes, portadores de maloclusão Classe II, com idade média inicial de 14,4 anos e divididos em dois grupos: Grupo I - laser e Grupo II - controle. Os pacientes foram tratados com um dispositivo ortodôntico denominado Pêndulo de Hilgers modificado, concomitantemente foi realizado a aplicação do laser de baixa intensidade na região radicular dos primeiros e segundos molares superiores, dos lados direito e esquerdo, nos pacientes do grupo I, sob o seguinte protocolo: 780nm de comprimento de onda, 40mW de potência, 10J/cm² de densidade de energia, 0,4J de energia por ponto durante 10 segundos cada e resultando em energia total de 20,8J. Foram distribuídos 52 pontos por vestibular e palatina dos primeiros e segundos molares superiores. A aplicação do laser foi realizada no dia da ativação da mola de TMA e repetida a cada mês até a sobrecorreção da relação molar. As telerradiografias laterais foram realizadas no início do tratamento (T1), ao final do terceiro mês (T2) e ao final da distalização (T3) com a relação molar sobrecorrigida em aproximadamente 1mm. Após análise cefalométrica foram realizadas comparações das alterações obtidas entre as fases Inicial (T1) e Final (T3) e Inicial (T1) e terceiro mês (T2) e comparadas entre os grupos. Os resultados mostraram inclinação para distal dos primeiros e segundos molares superiores, inclinação vestibular dos incisivos centrais superiores acentuada pela presença dos segundos molares e ausência de alterações dentárias verticais significativas, rotação do plano mandibular no sentido horário e o consequente aumento da altura facial anterior inferior. Concluiu-se que os efeitos do Pêndulo de Hilgers modificado sob a metodologia realizada provocaram alterações predominantemente dentoalveolares e que a movimentação dos primeiros e segundos molares superiores sob efeito da laserterapia, não apresentou diferença estatisticamente significante na velocidade de movimentação ortodôntica e em nenhuma das variáveis estudadas.

Orthodontic space closure is a treatment alternative when a maxillary central incisor is missing. The objective of this report was to present an unusual treatment in which a right maxillary central incisor was moved through the midpalatal suture to replace the absent contralateral tooth. The biologic aspects and clinical appearance of the recontoured lateral and central incisors were analyzed. The position of the examined teeth and the appearance of the surrounding soft tissues were satisfactory; however, the upper midline frenulum deviated to the left. The incisor was successfully moved with no obvious detrimental effects as observed on the final radiographs. In the radiographic and tomographic examinations, the midline suture seemed to have followed the tooth movement. The patient expressed satisfaction with the results. It was concluded that orthodontic movement of the central incisor to replace a missing contralateral tooth is a valid treatment option, and the achievement of an esthetic result requires an interdisciplinary approach, including restorative dentistry and periodontics. This study evaluated the effect of heat treatment on CrNi stainless steel orthodontic archwires. Half of forty archwires of each thickness - 0.014 (0.35 mm), 0.016 (0.40 mm), 0.018 (0.45 mm) and 0.020 (0.50 mm) (totalling 160 archwires) - were subjected to heat treatment while the remainder were not. All of the archwires had their individual thickness measured in the anterior and posterior regions using AutoCad 2000 software before and after compressive and tensile strength testing. The data was statistically analysed utilising multivariance ANOVA at a 5% significance level. All archwires without heat treatment that were subjected to tensile strength testing presented with anterior opening, which was more accentuated in the 0.020 archwires. In the posterior region, the opening produced by the tensile force was more accentuated in the archwires without heat treatment. There was greater stability in the thermally treated archwires, especially those subjected to tensile strength testing, which indicates that the heat treatment of orthodontic archwires establishes a favourable and indispensable condition to preserve the intercanine width.

Restoring a misaligned tooth with an inadequate contact point is a challenge to the practitioner. In some instances, teeth that could be repositioned and adequately restored are extracted. Thus, the aim of this article was to describe a treatment using orthodontic and prosthetic techniques to restore esthetics and function in a patient with a distally drifted maxillary lateral incisor. The patient's functional and esthetic expectations were successfully met with the outlined treatment.

Information about orthodontic movement of teeth with hypercementosis is scarce. As cementum deposition continues to occur, cementum is expected to change the shape of the root and apex over time, but this has not yet been demonstrated. Nor has it ever been established whether it increases or decreases the prevalence of root resorption during orthodontic treatment. The unique biological function of the interconnected network of cementocytes may play a role in orthodontic movement and its associated root resorptions, but no research has ever been conducted on the topic. Unlike cementum thickness and hypercementosis, root and apex shape has not yet been related to patient age. A study of the precise difference between increased cementum thickness and hypercementosis is warranted. Hypercementosis refers to excessive cementum formation above and beyond the extent necessary to fulfill its normal functions, resulting in abnormal thickening with macroscopic changes in the tooth root, which may require the delivery of forces that are different from conventional mechanics in their intensity, direction and distribution. What are the unique features and specificities involved in moving teeth that present with hypercementosis? Bodily movements would be expected to occur, since inclination might prove difficult to achieve, but would the root resorption index be higher or lower?

Introduction: The force delivered during rapid maxillary expansion (RME) produces areas of compression on the periodontal ligament of the supporting teeth. The resulting alveolar bone resorption can lead to unwanted tooth movement in the same direction. The purpose of this study was to evaluate periodontal changes by means of computed tomography after RME with tooth-tissue-borne and tooth-borne expanders. Methods: The sample comprised 8 girls, 11 to 14 years old, with Class I or II malocclusions with unilateral or bilateral posterior crossbites Four girls were treated with tooth-tissue-borne Haas-type expanders, and 4 were treated with tooth-borne Hyrax expanders. The appliances were activated up to the full 7-mm capacity of the expansion screw. Spiral CT scans were taken before expansion and after the 3-month retention period when the expander was removed. One-millimeter thick axial sections were exposed parallel to the palatal plane, comprising the dentoalveolar area and the base of the maxilla up to the inferior third of the nasal cavity. Multiplanar reconstruction was used to measure buccal and lingual bone plate thickness and buccal alveolar bone crest level by means of the computerized method. Results and Conclusions: RME reduced the buccal bone plate thickness of supporting teeth 0.6 to 0.9 mm and increased the lingual bone plate thickness 0.8 to 1.3 mm. The increase in lingual bone plate thickness of the maxillary posterior teeth was greater in the tooth-borne expansion group than in the tooth-tissue-borne group. RME induced bone dehiscences on the anchorage teeth's buccal aspect (7.1 ± 4.6 mm at the first premolars and 3.8 ± 4.4 mm at the mesiobuccal area of the first molars), especially in subjects with thinner buccal bone plates. The tooth-borne expander produced greater reduction of first premolar buccal alveolar bone crest level than did the tooth-tissue-borne expander. © 2006 American Association of Orthodontists.

The purpose of this retrospective study was to associate the amount of keratinized gingiva present in adolescents prior to orthodontic treatment to the development of gingival recessions after the end of treatment. The sample consisted of the intra-oral photographs and orthodontic study models from 209 Caucasian patients with a mean age of 11.20 ± 1.83 years on their initial records and 14.7 ± 1.8 years on their final records. Patients were either Angle Class I or II and were submitted to non-extraction orthodontic treatment. Gingival recession was evaluated by visual inspection of the lower incisors and canines as seen in the initial and final study models and intra-oral photographs. The amount of recession was quantified using a digital caliper and the observed post-treatment gingival margin alterations were classified as unaltered, coronal migration of the gingival margin or apical migration of the gingival margin. The width of the keratinized gingiva was measured from the mucogingival line to the gingival margin on the pre-treatment photographs. The teeth that developed gingival recession and those that did not have their gingival margin position changed did not differ in relation to the initial amount of keratinized gingiva (3.00 ± 0.61 and 3.5 ± 0.86 mm, respectively). Paradoxically, teeth that presented a coronal migration of the gingival margin had a smaller initial amount of keratinized gingiva (2.26 ± 0.31 mm). The mean amount of initial keratinized gingiva did not predispose lower incisors and canines to gingival recession.

To assess the occurrence, extension, and severity of gingival margin alterations in a sample of youth after orthodontic treatment. Records from 209 Caucasian adolescents (118 female and 91 male) before and after orthodontic treatment were selected. Patients presented a mean age at the beginning of orthodontic treatment of 11.20 ± 1.86 years and a final mean age of 14.72 ± 1.83 years. Class I and II patients with mandibular incisors and canines that were completely erupted and with spacing or crowding not exceeding 4 mm were evaluated. The presence of gingival recession on the labial surface of the mandibular anterior teeth was evaluated in intraoral photographs and casts made before and after treatment. The proportion of patients with gingival recession after treatment was statistically higher than at the beginning (P<.001). After orthodontic treatment, gingival recession was not present in any of the teeth for 63.6% of the patients; in 29.2% of the patients, recession was present in at least 1 tooth. In terms of severity, the majority of affected teeth (47%) presented gingival recession less than 2 mm and in 2% more than 4 mm. It may be concluded that alterations in the gingival margin, especially gingival recession, occur in patients after orthodontic therapy, but the extent and severity of this finding are low. Paresthesia of the lower lip is uncommon during orthodontic treatment. In the present case, paresthesia occurred during orthodontic leveling of an extruded mandibular left second molar. It was decided to remove this tooth from the appliance and allow it to relapse. A reanatomization was then performed by grinding. The causes and treatment options of this rare disorder are reviewed and discussed. The main cause of paresthesia during orthodontic treatment may be associated with contact between the dental roots and inferior alveolar nerve, which may be well observed on tomography scans. Treatment usually involves tooth movement in the opposite direction of the cause of the disorder.
