OBJECTIVE: To estimate the prevalence of excessive daytime sleepiness (EDS) and its associated factors in a mixed population of employed Australian workers. METHODS: Study participants (n = 707) were volunteers from various Melbourne workplaces, participating in a workplace physical activity program in 2008. Daytime sleepiness was assessed using the Epworth Sleepiness Scale (ESS), with EDS defined as ESS scores >10. RESULTS: In this population of adult employees (40.0% male; mean age 40.2 ± 10.4 years), prevalence of EDS was 16.0%. Characteristics associated with EDS and higher ESS scores were age, higher body mass index, markers of poorer diet, and markers of poorer mental health. CONCLUSIONS: Excessive daytime sleepiness is potentially an important contributor to lower productivity and poorer mental health in the workplace. Our finding suggests that workplace health programs aimed at improving diet and body weight may also help alleviate EDS.

OBJECTIVE: To assess the prevalence of risk factors and risk for cardiovascular disease and Type 2 diabetes in employees with sedentary occupations enrolled in a workplace health-promotion program. METHODS: Participants (n = 762) were recruited from ten Melbourne workplaces, participating in a physical activity program. Demographic, behavioral, biomedical, and physical measurements were collected. RESULTS: The majority of employees were not meeting recommended guidelines for physical activity (62%), fruit intake (70%), vegetable intake (86%), body mass index (58%), or waist circumference (53%). Most had intermediate (53%) or high (7%) risk of developing Type 2 diabetes. CONCLUSIONS: The majority of Australian adults in sedentary occupations were not meeting guidelines for a number of chronic disease risk factors and a substantial proportion were unaware of their increased risk. This study supports the potential of chronic disease risk factor detection and intervention programs in the workplace.

OBJECTIVES: This study examined the relationship between psychosocial working factors such as job control, job demands, job insecurity, supervisor support, and workplace bullying as risk factors for suicide ideation. METHODS: We used a logistic analytic approach to assess risk factors for thoughts of suicide in a cross-sectional sample of working Australians. Potential predictors included psychosocial job stressors (described above); we also controlled for age, gender, occupational skill level, and psychological distress. RESULTS: We found that workplace bullying or harassment was associated with 1.54 greater odds of suicide ideation (95% confidence interval 1.64 to 2.05) in the model including psychological distress. Results also suggest that higher job control and security were associated with lower odds of suicide ideation. CONCLUSIONS: These results suggest the need for organizational level intervention to address psychosocial job stressors, including bullying.

Between 1984 and 1997, six cases of urothelial cancer and 14 cases of renal cell cancer occurred in a group of 500 underground mining workers in the copper-mining industry of the former German Democratic Republic, with high exposures to explosives containing technical dinitrotoluene. Exposure durations ranged from 7 to 37 years, and latency periods ranged from 21 to 46 years. The incidences of both urothelial and renal cell tumors in this group were much higher than anticipated on the basis of the cancer registers of the German Democratic Republic by factors of 4.5 and 14.3, respectively. The cancer cases and a representative group of 183 formerly dinitrotoluene- exposed miners of this local industry were interviewed for their working history and grouped into four exposure categories. This categorization of the 14 renal cell tumor cases revealed no dose-dependency concerning explosives in any of the four exposure categories and was similar to that of the representative group of employees, whereas the urothelial tumor cases were predominantly confined to the high-exposure categories. Furthermore, all identified tumor patients were genotyped by polymerase chain reaction, using lymphocyte DNA, regarding their genetic status of the polymorphic xenobiotic metabolizing enzymes, including the N-acetyltransferase 2 and the glutathione-S-transferases M1 and T1. This genotyping revealed remarkable distributions only for the urothelial tumor cases, who were exclusively identified as 'slow acetylators.' This points to the possibility of human carcinogenicity of dinitrotoluene, with regard to the urothelium as the target tissue.

A cohort of 161 underground miners who had been highly exposed to dinitrotoluene (DNT) in the copper-mining industry of the former German Democratic Republic was reinvestigated for signs of subclinical renal damage. The study included a screening of urinary proteins excreted by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE), and quantitations of the specific urinary proteins α 1-microglobulin and glutathione-S-transferase α (GST α) as biomarkers for damage of the proximal tubule and glutathione-S-transferase π (GST π) for damage of the distal tubule. The exposures were categorized semiquantitatively (low, medium, high, and very high), according to the type and duration of professional contact with DNT. A straight dose-dependence of pathological protein excretion patterns with the semiquantitative ranking of DNT exposure was seen. Most of the previously reported cancer cases of the urinary tract, especially those in the higher exposed groups, were confined to pathological urinary protein excretion patterns. The damage from DNT was directed toward the tubular system. In many cases, the appearance of Tamm-Horsfall protein, a 105-kD protein marker, was noted. Data on the biomarkers α 1-microglobulin, GST α, and GST π consistently demonstrated a dose-dependent increase in tubular damage, which confirmed the results of screening by SDS-PAGE and clearly indicated a nephrotoxic effect of DNT under the given conditions of exposure. Within the cluster of cancer patients observed among the DNT-exposed workers, only in exceptional cases were normal biomarker excretions found.

Objectives Demonstrate the application of decision trees – classification and regression trees (CARTs), and their cousins, boosted regression trees (BRTs) – to understand structure in missing data. Setting Data taken from employees at three different industry sites in Australia. Participants 7915 observations were included. Materials and Methods The approach was evaluated using an occupational health dataset comprising results of questionnaires, medical tests, and environmental monitoring. Statistical methods included standard statistical tests and the ‘rpart’ and ‘gbm’ packages for CART and BRT analyses, respectively, from the statistical software ‘R’. A simulation study was conducted to explore the capability of decision tree models in describing data with missingness artificially introduced. Results CART and BRT models were effective in highlighting a missingness structure in the data, related to the Type of data (medical or environmental), the site in which it was collected, the number of visits and the presence of extreme values. The simulation study revealed that CART models were able to identify variables and values responsible for inducing missingness. There was greater variation in variable importance for unstructured compared to structured missingness. Discussion Both CART and BRT models were effective in describing structural missingness in data. CART models may be preferred over BRT models for exploratory analysis of missing data, and selecting variables important for predicting missingness. BRT models can show how values of other variables influence missingness, which may prove useful for researchers. Conclusion Researchers are encouraged to use CART and BRT models to explore and understand missing data.

Objective: To examine whether positive mental health (PMH)—a positively focused well-being construct—moderates the job stress–distress relationship. Methods: Longitudinal regression was used to test two waves of matched, population-level data from a sample of older, working Australian adults (n = 3291) to see whether PMH modified the relationship between work stress and later psychological distress. Results: Time 1 work stress was positively associated with distress at both time points. Positive mental health was negatively associated with work stress at both time points. Positive mental health modified the impact of work stress on psychological distress. This effect only occurred for those with the highest levels of PMH. Conclusions: Positive mental health may help protect workers from the effect of workplace stress but only in a small proportion of the population. Therefore, to improve workplace mental health, workplaces need to both prevent stress and promote PMH.

El objetivo de este estudio fue realizar una prueba de validez diagnostica del test neural 1 para el diagnóstico del Síndrome de Túnel del Carpo (STC) utilizando como prueba de referencia o de oro el test de conducción nerviosa. En este estudio participaron 115 sujetos, 230 manos con sospecha clínica de STC quienes fueron evaluados con el test de conducción nerviosa y el test neural 1. Se encontró una sensibilidad del 93.0% (IC 95%:88,21-96,79) y una especificidad del 6,67% (IC 95%:0,0-33,59), razón de verosimilitud positiva fue de 1,00 y razón de verosimilitud negativa de 1,05. Valor predictivo positivo de 86,9% y un valor predictivo negativo de 12,5%. Se concluye que el test neural 1 es una prueba clínica de alta sensibilidad y baja especificidad de gran utilidad para el monitoreo e identificación del STC. Es un procedimiento para el diagnóstico clínico de bajo costo que puede incluirse en los exámenes de rutina de los trabajadores como complemento a las pruebas clínicas sugeridas por las Gatiso para dar mayor precisión a la identificación temprana del STC. Se sugiere combinarla con otros test de mayor especificidad para ser aplicada en trabajadores en condiciones de riesgo o que presenten síntomas en miembros superiores y realizar otros estudios en donde participen sujetos sin diagnóstico clínico del STC.

Actualmente se considera que las organizaciones con resultados positivos y rendimientos crecientes tienen aspectos y características particulares que las diferencian de aquellas organizaciones que no obtienen los mejores resultados en el mercado. Para que las organizaciones sean saludables deben garantizar el bienestar del empleado y comprometerse con el mejoramiento continuo del mismo. Es así como surgen las escalas de medidas de bienestar, las cuales contribuyen directamente al bienestar del empleado y sus resultados positivos dentro de la organización. Este estudio hace referencia a las medidas de bienestar más utilizadas en un periodo de diez años (2002-2012), con el propósito de establecer la relación entre las medidas de bienestar, el bienestar de los empleados y las organizaciones saludables. Para determinar dicha relación, se llevó a cabo un análisis detallado de los estudios realizados sobre las escalas de medidas de bienestar utilizadas durante el periodo de tiempo 2002 y 2012. Los resultados arrojados señalan que las medidas de bienestar más utilizadas durante este periodo son: Satisfacción laboral, Clima organizacional, Engagement y Calidad de vida laboral, mientras que las cuatro medidas de bienestar menos utilizadas son: Remuneración y Bienestar subjetivo.

Los bomberos aeronáuticos son los encargados de atender todas las emergencias en los aeropuertos y sus cercanías. Estas emergencias incluyen emergencias aéreas, en tierra, eventos con materiales peligros e incendios, entre otros. Su trabajo tiene como características la realización de actividades durante periodos largos de baja intensidad y periodos cortos de alta intensidad. De acuerdo con estas características, es necesario que los bomberos aeronáuticos tengan una buena condición física. El consumo máximo de oxígeno (VO2 máx) como indicador de capacidad aeróbica resulta indispensable para conocer el desempeño de los bomberos en su trabajo. El objetivo de este estudio es determinar la capacidad aeróbica de los bomberos aeronáuticos y sus factores determinantes. Por tanto se desarrolló un estudio transversal de tipo descriptivo en una muestra de 23 hombres bomberos aeronáuticos. Se obtuvo información acerca de sus variables socio-demográficas, se determinó el VO2 máx y umbral ventilatorio mediante análisis de gases espirados durante un protocolo de ejercicio máximo sobre tapiz rodante, se evaluó la composición corporal mediante adipometría y se determinó el nivel de actividad física mediante el cuestionario internacional de actividad física IPAQ. Se encontró que la muestra tenia una edad de 32,6 ± 4,8 años, peso de 78,4 ± 9,8 kg, porcentaje de grasa de 14,8 ± 3,8 %, índice de masa corporal de 25,7 ± 2,7 y VO2máx de 44,6 ± 6. No se encontraron cambios significativos del VO2máx con la edad, pero si con la actividad física, porcentaje de grasa e índice de masa corporal. Se sugiere que el entrenamiento de los bomberos aeronáuticos durante su jornada laboral sea de intervalos de alta intensidad y que se monitorice su nivel de actividad física y composición corporal.

Las formas de adquirir una enfermedad son diversas. Los seres humanos están expuestos a diferentes factores de riesgo ambientales, físicos y ocupacionales entre otros, estos últimos se presentan con mayor o menor frecuencia dependiendo de la actividad laboral que realiza un trabajador, del tiempo que emplea para cumplir la tarea o la manera en que ejecuta ésta. Estos factores se ven asociados con aspectos propios del individuo que de alguna manera los aumentan o los disminuyen. Este proyecto investigativo pretende realizar una aproximación a la evidencia epidemiológica que se contempla en el estudio de las condiciones de riesgo laboral asociadas a la presentación del Síndrome del Túnel del Carpo (STC), con el fin de dejar elementos que contribuyan a la elaboración de diferentes instrumentos de evaluación de las condiciones de riesgo, que pueden estar vinculadas a la presentación de este problema.

El movimiento en el complejo articular de hombro, al igual que en las demás partes del cuerpo se realiza gracias a los sistemas óseo, articular y muscular. Cuando se presentan movimientos repetitivos, posturas y fuerzas en el lugar de trabajo al desarrollar las tareas, se pueden originar una serie de patologías profesionales en los trabajadores ocupacionalmente expuestos. Estas patologías no solamente se asocian a estos factores de riesgo, sino también, a una concepción inadecuada del diseño del puesto de trabajo en dónde los trabajadores desarrollan su actividad laboral. se pretende a través de este proyecto de investigación dar a conocer una herramienta de observación, detección de éstos, y medición tridimensional de los movimientos que puedan llegar a representar un riesgo, facilitando la recolección de datos de forma precisa y rápida, así como la apreciación de las condiciones de trabajo, por parte de los profesionales interesados en Salud Ocupacional.
