A network was established to acquire basic knowledge of Cryptococcus neoformans in IberoAmerican countries. To this effect, 340 clinical, veterinary, and environmental isolates from Argentina, Brazil, Chile, Colombia, Mexico, Peru, Venezuela, Guatemala, and Spain were typed by using M13 polymerase chain reaction-fingerprinting and orotidine monophosphate pyrophosphorylase (URA5) gene restriction fragment length polymorphsm analysis with Hhal and Sau961 in a double digest. Both techniques grouped all isolates into eight previously established molecular types. The majority of the isolates, 68.2% (n=232), were VNI (var. grubii, serotype A), which accords with the fact that this variety causes most human cryptococcal infections worldwide. A smaller proportion, 5.6% (n=19), were VNII (var. grubii, serotype A); 4.1% (n=14), VNIII (AD hybrid), with 9 isolates having a polymorphism in the URA5 gene; 1.8% (n=6), VNIV (var. neoformans, serotype D); 3.5% (n=12), VGI; 6.2% (n=21), VGII; 9.1% (n=31), VGIII, and 1.5% (n=5) VGIV, with all four VG types containing var. gattii serotypes B and C isolates.

Cryptococcus neoformans and Cryptococcus gattii are important agents of meningoencephalitis in humans in the city of Belém. This clinical data suggests that the region may be a highly endemic area for the pathogenic Cryptococcus species within the state of Pará (PA), Northern Brazil. Preliminary analysis of 11 environmental samples from the city of Belém showed two positive locations, including a hollow of a kassod tree (Senna siamea) colonized simultaneously by C. gattii molecular type VGII and C. neoformans molecular type VNI, and a birdcage in a commercial aviary positive for C. neoformans, molecular type VNI. This is the first evidence of an environmental occurrence of molecular types VNI and VGII in PA.

This study involved a total of 116 samples, 79 taken from pigeon droppings and 37 of atmospheric air taken close to accumulations of excrement. Cryptococcus neoformans var. grubii was isolated from 11 (13.9%) of these samples. Other species of Cryptococcus were also isolated from these samples, such as C. albidus (12.6%) and C. laurentii (8.9%). C. neoformans was not isolated from the air samples, though C. albidus (5.4%) was. All the strains of C. neoformans were found to belong to the A serotype (C. neoformans var. grubii). In regard to the studies with the antifungal agents 5-fluorocytosine, fluconazole, itraconazole, amphotericin B and voriconazole, by means of the microdilution method (EUCAST), we point out that one sample demonstrated resistance to fluconazole, this being especially significant because this is an environmental strain.

Cryptococcus neoformans is an opportunistic fungal pathogen that causes meningoencephalitis as the most frequent clinical presentation in immunocompromised patients, mainly in people infected by HIV. This fungus is an environmental encapsulated yeast, commonly found in soil enriched with avian droppings and plant material. A total of 290 samples of pigeon and the other avian droppings, soil, ornamental trees and vegetable material associated with Eucalyptus trees were collected to study environmental sources of Cryptococcus species in Goiânia, Goiás State. The determination of varieties, serotypes and the susceptibility in vitro to fluconazole, itraconazole and amphotericin B of C. neoformans isolates were performed. C. neoformans var. grubii (serotype A) was found in 20.3% (36/177) of pigeon dropping samples and in 14.3% (5/35) of samples of Eucalyptus. None of the environmental isolates of C. neoformans showed in vitro resistance to three antifungal agents. The knowledge of major route for human cryptococcal infection (inhalation of infectious particles from saprophytic sources) and a total of 60 C. neoformans isolates obtained from AIDS patients with cryptococcal meningitis between October 2001 and April 2002 justify the study of the habitats of these yeasts as probable sources of cryptococcosis in this city.

Cryptococcosis has been a significant cause of morbidity and mortality in patients with Aids. Many reservoirs of the agent Cryptococcus neoformans have been reported, but the ecology of this yeast must be elucidated in order to establish surveillance programs and to prevent infections. The objective of this study was to evaluate the presence of C. neoformans in Rio de Janeiro City, RJ, Brazil. Ten churches were selected for sampling and detection of the yeast collecting pigeon dropping, air samples from church towers and neighboring areas during one year. The data demonstrated that C. neoformans has been present in every church selected and was present in 37.8% of 219 pigeon dropping samples. As well as, the yeast was isolated from soil, insects, eggs, pigeon nests and feathers. Fifteen air samples (4.9%) were positive. The growth on C.G.B. medium showed that all strains belonged to C. neoformans var. neoformans, with 98.8% of the strains belonging to serotype A.

Cryptococcus neoformans is an encapsulated yeast, etiological agent of cryptococcosis. The species is commonly associated with pigeon droppings and plant materials. The aim of the present work was to verify the presence of the yeast in pigeon droppings, and to identify the isolates obtained in serotypes and mating types (MAT). Ten samples of pigeon droppings were collected in the rural area of the city of Alfenas, Brazil. Samples were inoculated in agar Niger medium for fungal isolation and 22 isolates with characteristics of C. neoformans were obtained. The serotypes and MAT were determined by multiplex PCR using specific primers. Serotypes were also determined by using the Kit Crypto Check. Among the 22 samples evaluated, eight were identified as C. neoformans by classic identification tests. These samples were characterized as serotype A by the Kit Crypto check and as serotype A MAT alpha by the multiplex PCR. The present study reinforces the evidence that pigeon droppings are a reservoir for C. neoformans and confirms the prevalence of C. neoformans var. grubii (Aalpha) among environmental isolates. It also demonstrates that multiplex PCR is an acceptable alternative for serotype analysis because it reduces the costs for each reaction and analyses serotype and MAT simultaneously.

Cryptococcus neoformans is the major cause of fungal meningitis, a potentially lethal mycosis. Bird excreta can be considered a significant environmental reservoir of this species in urban areas, thirty-three samples of pigeon excreta were collected within the city of Vitoria, Brazil. Cryptococcus neoformans was isolated and identified using standard biochemical assays in ten samples. PCR amplification with primer M13 and orotidine monophosphate pyrophosphorylase (URA5) gene-restriction fragment length polymorphism (RFLP) analysis discerned serotypes and genotypes within this species. All isolates were serotype A (C. neoformans var. grubii) and genotype VNI. The two alternative alleles a and α at the mating type locus were determined by PCR amplification and mating assays performed on V8 medium. All isolates were MAT α mating type but only 50% were able to mate in vitro with the opposite mating type MAT a tester strains (JEC20, KN99a and Bt63). This study adds information on the ecology and molecular characterization of C. neoformans in the Southeast region of Brazil.

Cryptococcus neoformans é a levedura capsulada causadora de criptococose em humanos e animais. A variedade neoformans, encontrada em diversas fontes ambientais, inclusive habitats de aves, é importante causa de mortalidade em indivíduos com AIDS em todo o Mundo. Contudo, ainda não há estudos sobre a sua ecologia na região Centro Oeste brasileira, onde há registro da ocorrência de casos humanos da micose. Para estudar fontes saprofíticas de C. neoformans, na cidade de Campo Grande, foram coletadas 20 amostras de excretas de aves em distintos ambientes. Suspensão das amostras em salina estéril foram semeadas em placas com meio ágar níger. Após 5 dias, colônias mucóides marrom-escuro foram subcultivadas para identificação através de provas morfofisiológicas, determinação da variedade e sorotipagem. C. neoformans var. neoformans sorotipo A foi isolado de 10 (50%) das amostras, comprovando a ocorrência saprofítica de C. neoformans na cidade de Campo Grande, relacionada a habitat de aves em cativeiro.

Estudo prospectivo foi realizado no Hospital de Clínicas da Universidade Federal de Uberlândia, entre março de 1998 e novembro de 2003, em 96 pacientes com diagnóstico clínico e laboratorial de criptococose, sendo 81,3% portadores de Aids. Cepas de Cryptococcus neoformans foram obtidas de diferentes amostras, sendo 77% em líquido cefalorraquidiano. A var neoformans foi isolada em 89 casos e a var gattii em 7. A meningoencefalite criptocócica (56,3% dos casos), foi a manifestação clínica mais descrita, seguida da fungemia (13,5%). Entre os fatores de risco, a AIDS (81,3%) foi o mais comumente associado à micose. A pesquisa direta do fungo realizada em 121 amostras demonstrou o microrganismo em 98,3%, com cultura (+) em todas. Dos pacientes, 59,4% foram tratados com anfotericina B ou derivados triazólicos, sendo que 72,9% evoluíram para óbito, em particular os portadores de AIDS (62,5%). Atualmente, a criptococose tem sido diagnosticada com muita freqüência em nosso meio e constitui uma das doenças oportunísticas de maior morbidade e mortalidade nos pacientes com AIDS.

Avaliou-se a atividade de fungicidas azólicos de uso agronômico (epoxiconazol, difenoconazol e ciproconazol) em comparação ao antifúngico de uso terapêutico fluconazol sobre 23 amostras ambientais de Cryptococcus neoformans var neoformans isoladas de fezes de pombos, as quais foram coletadas em fazendas com práticas agrícolas empregando compostos azólicos e 11 amostras clínicas isoladas de pacientes portadores de criptococose. Os testes de sensibilidade foram realizados pela técnica de diluição em agar. A concentração inibitória mínima capaz de inibir 50% dos isolados ambientais (CIM 50) foi de 6,0µg/mL para epoxiconazol, 1,0µg/mL para difenoconazol, 2,0µg/mL para ciproconazol e 64,0µg/mL para fluconazol. Entre os isolados clínicos os valores de CIM 50 foram 2,0µg/mL, 0,38µg/mL, 1,0µg/mL e 16,0µg/mL para epoxiconazol, difenoconazol, ciproconazol e fluconazol, respectivamente. Os valores de CIM 50 em relação aos isolados de origem ambiental foram maiores do que os valores para os isolados de origem clínica. Em nosso estudo, frente ao mesmo antifúngico, as amostras ambientais apresentaram comportamento significativamente diferente em relação às amostras clínicas (p < 0,05). Diferenças (p<0,05) também foram observadas entre os valores de concentração inibitória apresentados pelo fluconazol e os outros antifúngicos de uso agronômico tanto no grupo dos isolados ambientais quanto clínicos.

Introduction In this study, the clinical features, underlying diseases and clinical outcomes of patients with cryptococcosis were investigated. In addition, a molecular analysis of the Cryptococcus neoformans species complex isolated from these patients was performed. Methods A prospective study of 62 cases of patients with cryptococcal infection was conducted at the Hospital de Doenças Tropicais de Goiás Dr. Anuar Auad from 2009-2010. Cryptococcal meningitis cases were diagnosed by direct examination and cerebrospinal fluid (CSF) sample culture. The profiling of these patients was assessed. The CSF samples were submitted to India ink preparation and cultured on Sabouraud dextrose agar, and C. neoformans was identified by the production of urease, a positive phenoloxidase test and assimilation of carbohydrates. C. neoformans and C. gattii isolates were distinguished by growth on L-canavanine-glycine-bromothymol blue medium, and molecular analysis was conducted via PCR fingerprinting reactions using M13 and (GACA)4 primers. Results From the 62 patients with cryptococcosis, 71 isolates of CSF were obtained; 67 (94.4%) isolates were identified as C. neoformans var. grubii/VNI, and 4 (5.6%) were identified as C. gattii/VGII. Of these patients, 53 had an HIV diagnosis. The incidence of cryptococcosis was higher among patients 20-40 years of age, with 74.2% of the cases reported in males. Cryptococcus-related mortality was noted in 48.4% of the patients, and the symptoms were altered sensorium, headache, fever and stiff neck. Conclusions The high morbidity and mortality observed among patients with cryptococcosis demonstrate the importance of obtaining information regarding the epidemiological profile and clinical course of the disease in the State of Goiás, Brazil.

Cryptococcosis is reported in adults and is often acquired immune deficiency syndrome (AIDS)-associated; however, its frequency in children is low. Based on the National Survey on Cryptococcosis conducted in Colombia, an epidemiological and clinical analysis was performed on cases of the disease observed in children less than 16 years old between 1993-2010. We found 41 affected children (2.6% prevalence) from the 1,578 surveys received. The country mean annual incidence rate was 0.017 cases/100,000 children under 16 years, while in Norte de Santander the incidence rate was 0.122 cases/100,000 (p < 0.0001). The average age of infected children was 8.4 and 58.5% were male. In 46.3% of cases, a risk factor was not identified, while 24.4% had AIDS. The most frequent clinical manifestations were headache (78.1%), fever (68.8%), nausea and vomiting (65.6%), confusion (50%) and meningeal signs (37.5%). Meningitis was the most frequent clinical presentation (87.8%). Amphotericin B was given to 93.5% of patients as an initial treatment. Positive microbiological identification was accomplished by India ink (94.7%), latex in cerebrospinal fluid (100%) and culture (89.5%). Out of 34 isolates studied, Cryptococcus neoformans var. grubii (VNI 85.3%, VNII 8.8%) was isolated in 94.1% of cases and Cryptococcus gattii (VGII) was isolated in 5.9% of cases. These data are complemented by a literature review, which overall suggests that cryptococcosis in children is an unusual event worldwide.

Cryptococcosis is an infection that affects humans and animals, the etiology is attributed to Cryptococcus neoformans variety neoformans, C. neoformans var. grubii and Cryptococcus gattii. The infection is common in dogs and cats, causing respiratory, neurological, cutaneous and ocular infections. Aiming to better understand the epidemiology of cryptococcosis in animals in the region, this paper describe the occurrence and characterization of the Cryptococcus species involved in this illness in pet animals at Mato Grosso State, Brazil. Clinical samples of four cases, two in cats and two dogs, were submitted for pathological, microbiological and molecular analysis. Microscopically, in three cases, tissue sections stained with hematoxylin and eosin had absence to severe granulomatous reaction composed by histiocytes, multinucleated cells and lymphocytes infiltration. In one case, citological imprint analysis showed similar inflammatory mainly mononuclear and lymphocyte cells infiltration. All cases had variable amounts of intracellular and extracellular fungal structures compatible with Cryptococcus sp. on Periodic Acid-Schiff (PAS) stain. All clinical samples were positive for culture on Sabouraud Dextrose Agar (SDA) and morphologically classified as Cryptococcus sp. The isolates were PCR positive for C. gatti, being confirmed by sequencing technique. The findings characterize the molecular species involved in animal infections in the region, and may contribute to future studies of the epidemiology of C. gattii.

Cryptococcus neoformans var. grubii est responsable de la majeure partie des infections au Cryptococcus chez les individus infectés au VIH-1. Cryptococcus gattii infecte généralement les personnes immunocompétentes. Afin de comprendre les mécanismes responsables de la susceptibilité différentielle de ces espèces lors de l’infection au VIH-1, nous avons établi et caractérisé un modèle novateur de la cryptococcose chez des souris transgéniques (Tg) CD4C/HIVMutA exprimant des gènes du VIH-1, et qui développent une maladie similaire au SIDA. Les objectifs sont de démontrer une différence significative au niveau de la survie, de la réponse inflammatoire et du recrutement cellulaire pulmonaire en fonction de la présence du transgène et de l’espèce de Cryptococcus inoculée. Des analyses de survie, d’histopathologie et de cytométrie en flux sur les populations cellulaires pulmonaires ont été effectuées. Les souris Tg infectées avec C. neoformans H99 ou C23 ont démontré une survie réduite et une augmentation de la dissémination comparativement aux souris non-Tg, contrairement aux souris Tg infectées au C. gattii R265 ou R272. L’examen histopathologique des poumons de souris Tg infectées au H99 a montré une faible réponse inflammatoire, contrairement aux souris non-Tg. Pour la souche R265, il y avait une très faible réponse inflammatoire chez les deux types de souris. Enfin, l’étude des populations cellulaires du poumon a révélé chez les souris Tg une augmentation des pourcentages de macrophages interstitiels et de cellules polymorphonucléaires, ainsi qu’une diminution des lymphocytes T CD4+ et CD8+, indépendamment de l’infection au Cryptococcus. Ce modèle novateur représente donc un outil très pertinent pour l’étude de l’immunopathogenèse de la cryptococcose dans le contexte du VIH.

La cryptococcose chez les patients atteints du VIH-1 est principalement causée par Cryptococcus neoformans var. grubii tandis que Cryptococcus gattii infecte surtout les personnes immunocompétentes. Afin d’élucider les mécanismes causant la susceptibilité différentielle à l’égard de ces deux espèces de Cryptococcus dans le contexte de l’infection au VIH-1, nous avons utilisé un modèle novateur de la cryptococcose chez la souris transgénique CD4C/HIVMutA, qui exprime les gènes nef, env et rev du VIH-1. L’expression du transgène VIH-1 a augmenté le recrutement pulmonaire des macrophages alvéolaires mais a diminué celui des lymphocytes T CD4+ et CD8+ en réponse à l’infection par le C. neoformans ou le C. gattii. La production pulmonaire des chimiokines MCP-1 (CCL2) et RANTES (CCL5) était également réduite chez les souris transgéniques infectées par l’une ou l’autre de ces espèces de Cryptococcus. La production pulmonaire de MIP-1α, MIP-1β, TNF-α, TGF-β, IL-2, IL-4 et IL-13 était augmentée chez la souris infectée au C. neoformans comparativement à C. gattii. In vitro, les macrophages alvéolaires prélevés chez la souris Tg et stimulés par des agonistes ont produit davantage de MIP-1β, alors que les chimiokines MCP-1 et RANTES n’ont pas été détectées.
