Objectives: To investigate whether neuromuscular performance predicts lower limb bone strength in different lower limb sites in postmenopausal women with mild knee osteoarthritis (OA). Methods: Neuromuscular performance of 139 volunteer women aged 50-68 with mild knee OA was measured using maximal counter movement jump test, isometric knee flexion and extension force and figure-of-eight-running test. Femoral neck section modulus (Z, mm3) was determined by data obtained from dualenergy X-ray absorptiometry. Data obtained using peripheral quantitative computed tomography was used to asses distal tibia compressive (BSId, g2/cm4) and tibial mid-shaft bending (SSImaxmid, mm3) strength indices. Results: After adjustment for height, weight and age, counter movement jump peak power production was the strongest independent predictor for Z (β=0.44; p<0.001) and for BSId (β=0.32; p=0.003). This was also true in concentric net impulse for Z (β=0.37; p=0.001) and for BSId (β=0.40; p<0.001). Additionally, knee extension force (β=0.30; p<0.001) and figure-of-eight-running test (β= -0.32; p<0.001) were among strongest independent predictors for BSId after adjustments. For SSImaxmid, concentric net impulse (β=0.33; p=0.002) remained as the strongest independent predictor after adjustments. Conclusions: Neuromuscular performance in postmenopausal women with mild knee OA predicted lower limb bone strength in every measured skeletal site.

This study aimed to compare the torque, torque ratio (Hamstrings:Quadriceps - H:Q), electromyographic (EMG) activity and EMG ratio (knee flexors:knee extensors EMG) in soccer players (SG, N=10) and active subjects (AG, N=10). Subjects performed three maximal voluntary isometric knee extensions and flexions at 45° and 90° to determine the peak torque and EMG activity. Torque and EMG activity of the knee flexor (biceps femoris [BF] and semitendinosus [ST]) were divided by the torque and EMG activity of the knee extensor (vastuls lateralis [VL] and rectus femoris [RF]) to calculate torque ratios (H:Q) and EMG ratios (BF:VL, BF:RF, ST:VL, ST:RF). The flexion torque was significantly higher for SG (p<0.05) in 45° and 90°. EMG activity for SG was significantly higher in agonist contractions for VL, RF and ST, and significantly lower in antagonist contractions for RF and ST when compared to AG Torque and EMG ratios were similar between groups and there were good correlations between torque ratio and BF:VL ratio (r=0.71, p=0.02) and BF:RF ratio (r=0.81, p=0.004) at 45. The EMG results could overestimate the joint balance calculated using torque ratios. Differences in recruitment pattern between soccer players and non-athletes can be related to the training routines and the EMG ratios presents applicable in trained populations.

This study examined the effect of fast-velocity concentric isokinetic resistance training (FV) on the rate of force development (RFD) at early (<100 ms) and late phases (>100 ms) of rising muscle force. Nine men participated in a 6-week resistance training intervention for the lower body, and nine matched subjects participated as controls (CON). During concentric isokinetic (180°s-1) knee extension training, subjects were instructed to do each contraction 'as fast and forcefully as possible'. Maximal muscle strength (MVC) and RFD (0-10, 0-20, ..., 0-250 ms from the onset of contraction) were measured during maximal voluntary isometric contraction of the knee extensors (KE). There were no significant changes in MVC of KE in both groups after intervention (FV = 314·2 ± 101·1 versus 338·7 ± 88·0 N{bullet operator}m, P>0·05; CON = 293·3 ± 94·8 versus 280·0 ± 72·2 N{bullet operator}m, P>0·05). The RFD increased 39-71% at time intervals up to 90 ms from the onset of the contraction (P<0·05), whereas no change occurred at later time intervals. Similarly, relative RFD (i.e.%MVC{bullet operator}s-1) (RFDr) increased 33-56% at time intervals up to 70 ms from the onset of the contraction (P<0·05). It can be concluded that a short period of resistance training performed with concentric fast-velocity isokinetic muscle contractions is able to enhance RFD and RFDr obtained at the early phase of rising muscle force. © 2013 The Authors Clinical Physiology and Functional Imaging © 2013 Scandinavian Society of Clinical Physiology and Nuclear Medicine.

This study aimed to evaluate the motor response time and ability to develop joint torque at the knee and ankle in older women with and without a history of falls, in addition to investigating the effect of aging on these capacities. We assessed 18 young females, 21 older female fallers and 22 older female non-fallers. The peak torque, rate of torque development, rate of electromyography (EMG) rise, reaction time, premotor time and motor time were obtained through a dynamometric assessment and simultaneous electromyography. Surface EMGs of the rectus femoris (RF), vastus lateralis (VL), biceps femoris (BF), gastrocnemius lateralis (GL) and tibialis anterior (TA) muscles were recorded. Knee extension and flexion peak torques were lower in older fallers than in non-fallers. Knee extension and flexion and ankle plantarflexion and dorsiflexion peak torques were lower in both older groups than in the younger group. The rate of EMG rise of the BF and the motor time of the TA were lower and higher, respectively, in older fallers than in the younger adults. The time to reach peak torque in knee extension/flexion and ankle plantarflexion/dorsiflexion and the motor times of the RF, VL, BF and GL were higher in both older groups than in the younger groups. The motor time of the TA during ankle dorsiflexion and the knee extension peak torque were the major predictors of falls in older women, accounting for approximately 28% of the number of falls. Thus, these results further reveal the biomechanical parameters that affect the risk of falls and provide initial findings to support the prescription of exercises in fall prevention programs. © 2013 Elsevier Ltd.

The objective of the present study was to compare the effects of a high speed isokinetic training (180°.s-1) and an isometric training (75°) on the maximum rate of force development (RFDmax) measured in the isokinetic and isometric modes. Twenty seven male non active subjects participated of this study (Mean + SD = body mass 78.6 + 14.1 kg; stature 175.1 + 8.9 cm; age 22.6 + 3.8 years). They were randomly divided into three groups: Control (GC); Isokinetic training (GISOC) and; Isometric training (GISOM). The subjects were submitted in different days to the following pre training protocols: 1) Familiarization to the isokinetic dynamometer tests; 2) Five maximum concentric isokinetic contractions of the knee extensors (180°.s-1) to access the maximum concentric torque (TMC) and the concentric RFDmax; 3) Two maximum isometric contractions of the knee extensors (75°) to access the maximum isometric torque (TMI) and the isometric RFDmax. The same tests were repeated after the training period, but without the familiarization session. Eighteen training sessions were performed (3 times per week). The GISOC performed the entire training whit concentric isokinetic contractions whit the speed of 180°.s-1. The GISOM performed the entire training whit isometric contractions whit the angle between the thigh end the leg being 75° (0° = full knee extension). TMI, TMC, concentric RFDmax, isometric RFDmax values of the GC was not different between pre and post training. GISOM increased only the TMI and the GSIOC increased the TMC, concentric RFDmax and isometric RFDmax. Furthermore, the GISOC had a higher percentage increase of the isometric RFDmax than the isokinetic RFDmax. Based on these results, it is possible to conclude that the increase in maximum strength corresponded to the training specificity theory, unlike to the RFDmax. OBJECTIVE: To determine differences between hypermobile subjects and controls in terms of maximum strength, rate of force development, and balance. METHODS: We recruited 13 subjects with hypermobility and 18 controls. Rate of force development and maximal voluntary contraction (MVC) during single leg knee extension of the right knee were measured isometrically for each subject. Balance was tested twice on a force plate with 15-second single-leg stands on the right leg. Rate of force development (N/second) and MVC (N) were extracted from the force-time curve as maximal rate of force development (= limit Deltaforce/Deltatime) and the absolute maximal value, respectively. RESULTS: The hypermobile subjects showed a significantly higher value for rate of force development (15.2% higher; P = 0.038, P = 0.453, epsilon = 0.693) and rate of force development related to body weight (16.4% higher; P = 0.018, P = 0.601, epsilon = 0.834) than the controls. The groups did not differ significantly in MVC (P = 0.767, P = 0.136, epsilon = 0.065), and MVC related to body weight varied randomly between the groups (P = 0.921, P = 0.050, epsilon = 0.000). In balance testing, the mediolateral sway of the hypermobile subjects showed significantly higher values (11.6% higher; P = 0.034, P = 0.050, epsilon = 0.000) than that of controls, but there was no significant difference (4.9% difference; P = 0.953, P = 0.050, epsilon = 0.000) in anteroposterior sway between the 2 groups. CONCLUSION: Hypermobile women without acute symptoms or limitations in activities of daily life have a higher rate of force development in the knee extensors and a higher mediolateral sway than controls with normal joint mobility.

Activity of the vasti has been argued to vary through knee range of movement due to changes in passive support of the patellofemoral joint and the relative contribution of these muscles to knee extension. Efficient function of the knee is dependent on optimal control of the patellofemoral joint, largely through coordinated activity of the medial and lateral quadriceps. Motor unit synchronization may provide a mechanism to coordinate the activity of vastus medialis (VMO) and vastus lateralis (VL), and may be more critical in positions of reduced passive support for the patellofemoral joint (i.e., full extension). Therefore, the aim of this study was to determine whether the degree of motor unit synchronization between the vasti muscles is dependent on joint angle. Electromyographic (EMG) recordings of single motor unit action potentials (MUAPs) were made from VMO and multiunit recordings from VL during isometric contractions of the quadriceps at 0 degrees, 30 degrees, and 60 degrees of knee flexion. The degree of synchronization between motor unit firing was evaluated by identification of peaks in the rectified EMG averages of VL, triggered from MUA-Ps in VMO. The proportion of cases in which there was a significant peak in the triggered averages was calculated. There was no significant difference in the degree of synchronization between the vasti at different knee angles (p = 0.57). These data suggest that this basic coordinative mechanism between the vasti muscles is controlled consistently throughout knee range of motion, and is not augmented at specific angles where the requirement for dynamic control of stability is increased. Aging is associated with loss of endurance; however, aging is also associated with decreased fatigue during maximal isometric contractions. The aims of this study were to examine the relationship between age and walking endurance (WE) and maximal isometric fatigue (MIF) and to determine which metabolic/fitness components explain the expected age effects on WE and MIF. Subjects were 96 pre-menopausal women. Oxygen uptake (walking economy) was assessed during a 3-mph walk; aerobic capacity and WE by progressive treadmill test; knee extension strength by isometric contractions, MIF during a 90-s isometric plantar flexion (muscle metabolism measured by 31P MRS). Age was related to increased walking economy (low VO2, r = −0.19, P < 0.03) and muscle metabolic economy (force/ATP, 0.34, P = 0.01), and reduced MIF (−0.26, P < 0.03). However, age was associated with reduced WE (−0.28, P < 0.01). Multiple regression showed that muscle metabolic economy explained the age-related decrease in MIF (partial r for MIF and age −0.13, P = 0.35) whereas walking economy did not explain the age-related decrease in WE (partial r for WE and age −0.25, P < 0.02). Inclusion of VO2max and knee endurance strength accounted for the age-related decreased WE (partial r for WE and age = 0.03, P > 0.80). In premenopausal women, age is related to WE and MIF. In addition, these results support the hypothesis that age-related increases in metabolic economy may decrease MIF. However, decreased muscle strength and oxidative capacity are related to WE.

Introduction: Hamstring strain injuries (HSI) are the predominant non-contact injury in many sports. Eccentric hamstring muscle weakness following intermittent running has been implicated within the aetiology of HSI. This weakness following intermittent running is sometimes greater eccentrically than concentrically, however the cause of this unique, contraction mode specific phenomenon is unknown. The purpose of this research was to determine whether declines in knee flexor strength following overground repeat sprints are caused by declines in voluntary activation of the hamstring muscles. Methods: Seventeen recreationally active males completed 3 sets of 6 by 20m overground sprints. Maximal isokinetic concentric and eccentric knee flexor and concentric knee extensor strength was determined at ±1800.s-1 and ±600.s-1 while hamstring muscle activation was assessed using surface electromyography, before and 15 minutes after the running protocol. Results: Overground repeat sprint running resulted in a significant decline in eccentric knee flexor strength (31.1 Nm; 95% CI = 21.8 to 40.3 Nm; p < 0.001). However, concentric knee flexor strength was not significantly altered (11.1 Nm; 95% CI= -2.8 to 24.9; p=0.2294). Biceps femoris voluntary activation levels displayed a significant decline eccentrically (0.067; 95% CI=0.002 to 0.063; p=0.0325). However, there was no significant decline concentrically (0.025; 95% CI=-0.018 to 0.043; p=0.4243) following sprinting. Furthermore, declines in average peak torque at -1800.s-1 could be explained by changes in hamstring activation (R2 = 0.70). Moreover, it was change in the lateral hamstring muscle activity that was related to the decrease in knee flexor torque (p = 0.0144). In comparison, medial hamstring voluntary activation showed no change for either eccentric (0.06; 95% CI = -0.033 to 0.102; p=0.298) or concentric (0.09; 95% CI = -0.03 to 0.16; p=0.298) muscle actions following repeat sprinting. Discussion: Eccentric hamstring strength is decreased significantly following overground repeat sprinting. Voluntary activation deficits in the biceps femoris muscle explain a large portion of this weakness. The implications of these findings are significant as the biceps femoris muscle is the most frequently strained of the knee flexors and fatigue is implicated in the aetiology of this injury.

Objective To analyze the ability to discriminate between healthy individuals and individuals with chronic nonspecific low back pain (CNLBP) by measuring the relation between patient-reported outcomes and objective clinical outcome measures of the erector spinae (ES) muscles using an ultrasound during maximal isometric lumbar extension. Design Cross-sectional study with screening and diagnostic tests with no blinded comparison. Setting University laboratory. Participants Healthy individuals (n=33) and individuals with CNLBP (n=33). Interventions Each subject performed an isometric lumbar extension. With the variables measured, a discriminate analysis was performed using a value ≥6 in the Roland and Morris disability questionnaire (RMDQ) as the grouping variable. Then, a logistic regression with the functional and architectural variables was performed. A new index was obtained from each subject value input in the discriminate multivariate analysis. Main Outcome Measures Morphologic muscle variables of the ES muscle were measured through ultrasound images. The reliability of the measures was calculated through intraclass correlation coefficients (ICCs). The relation between patient-reported outcomes and objective clinical outcome measures was analyzed using a discriminate function from standardized values of the variables and an analysis of the reliability of the ultrasound measurement. Results The reliability tests show an ICC value >.95 for morphologic and functional variables. The independent variables included in the analysis explained 42% (P=.003) of the dependent variable variance. Conclusions The relation between objective variables (electromyography, thickness, pennation angle) and a subjective variable (RMDQ ≥6) and the capacity of this relation to identify CNLBP within a group of healthy subjects is moderate. These results should be considered by clinicians when treating this type of patient in clinical practice.

Hamstring strain injuries are the predominant injury in many sports, costing athletes and clubs a significant financial and performance burden. Therefore the ability to identify and intervene with individuals who are considered at a high risk of injury is important. One measure which has grown in popularity as an outcome variable following hamstring intervention/prevention studies and rehabilitation is the angle of peak knee flexor torque. This current opinion article will firstly introduce the measure and the processes behind it. Secondly, this article will summarise how the angle of peak knee flexor torque has been suggested to measure hamstring strain injury risk. Finally various limitations will be presented and outlined as to how they may influence the measure. These include the lack of muscle specificity, the common concentric contraction mode of assessment, reliability of the measure, various neural contributions (such as rate of force development and neuromuscular inhibition) as well as the lack of prospective data showing any predictive value in the measure.

Estimulação transcraniana por corrente contínua (ETCC) sobre áreas corticais pré-selecionadas, tem aumentado o desempenho físico de diferentes populações. Porém, lacunas persistem no tocante aos mecanismos subjacentes à estes efeitos. Assim, a presente tese objetivou: a) investigar os efeitos da ETCC anódica (aETCC) e placebo (Sham) no córtex motor (CM) de indivíduos saudáveis sobre o desempenho de força máxima; b) comparar os efeitos da ETCC sobre a produção de força máxima e estabilidadade da força durante exercícios máximo e submáximo em sujeitos hemiparéticos e saudáveis; c) investigar o efeito da ETCC sobre a conectividade funcional inter-hemisférica (coerência eletroencefalográfica cEEG) do córtex pré-frontal (CPF), desempenho aeróbio e dispêndio energético (EE) durante e após exercício máximo e submáximo. No 1 estudo, 14 adultos saudáveis executaram 2 sessões de exercício máximo de força (EMF) dos músculos flexores e extensores do joelho dominante (3 séries de 10 rep máximas), precedidos por aETCC ou Sham (2mA; 20 mim). aETCC não foi capaz de aumentar o trabalho total e pico de torque (PT), resistência à fadiga ou atividade eletromiográfica durante o EMF. No 2 estudo, 10 hemiparéticos e 9 sujeitos saudáveis receberam aETCC e Sham no CM. O PT e a estabilidade da força (coeficiente de variação - CV) foram avaliados durante protocolo máximo e submáximo de extensão e flexão unilateral do joelho (1 série de 3 reps a 100% do PT e 2 séries de 10 reps a 50% do PT). Nenhuma diferença no PT foi observada nos dois grupos. Diminuições no CV foram obervadas durante a extensão (~25-35%, P<0.001) e flexão de joelho (~22-33%, P<0.001) após a aETCC comparada com Sham nos hemiparéticos, entretanto, somente o CV na extensão de joelhos diminuiu (~13-27%, P<0.001) nos saudáveis, o que sugere que aETCC pode melhorar o CV, mas não o PT em sujeitos hemiparéticos. No 3 estudo, 9 adultos saudáveis realizaram 2 testes incrementais máximos precedidos por aETCC ou Sham sobre o CPF com as respostas cardiorrespiratórias, percepção de esforço (PSE) e cEEG do CPF sendo monitoradas. O VO2 de pico (42.64.2 vs. 38.23.3 mL.kg.min-1; P=0,02), potência total (252.776.5 vs. 23773.3 W; P=0,05) e tempo de exaustão (531.1140 vs. 486.7115.3 seg; P=0,04) foram maiores após aETCC do que a Sham. Nenhuma diferença foi encontrada para FC e PSE em função da carga de trabalho (P>0,05). A cEEG do CPF aumentou após aETCC vs. repouso (0.700.40 vs. 0.380.05; P=0,001), mas não após Sham vs. repouso (0.360.49 vs. 0.330.50; P=0,06), sugerindo que a aETCC pode retardar a fadiga aumentando a conectividade funcional entre os hemisférios do CPF e desempenho aeróbio durante exercício exaustivo. No 4 estudo, o VO2 e EE foram avaliados em 11 adultos saudáveis antes, durante a aETCC ou Sham no CPF e 30 min após exercício aeróbio submáximo isocalórico (~200kcal). Diferenças não foram observadas no VO2 vs. repouso durante aETCC e Sham (P=0.95 e P=0.85). Porém, a associação entre exercício e aETCC aumentou em ~19% o EE após ao menos, 30 min de recuperação após exercício quando comparada a Sham (P<0,05).

An isometric torque-production task was used to investigate interference and retention in adaptation to multiple visuomotor environments. Subjects produced isometric flexion-extension and pronation-supination elbow torques to move a cursor to acquire targets as quickly as possible. Adaptation to a 30 degrees counter-clockwise (CCW) rotation (task A), was followed by a period of rest (control), trials with no rotation (task B0), or trials with a 60 degrees clockwise (CW) rotation (task B60). For all groups, retention of task A was assessed 5 h later. With initial training, all groups reduced the angular deviation of cursor paths early in the movements, indicating feedforward adaptation. For the control group, performance at commencement of the retest was significantly better than that at the beginning of the initial learning. For the B0 group, performance in the retest of task A was not dissimilar to that at the start of the initial learning, while for the B60 group retest performance in task A was markedly worse than initially observed. Our results indicate that close juxtaposition of two visuomotor environments precludes improved retest performance in the initial environment. Data for the B60 group, specifically larger angular errors upon retest compared with initial exposures, are consistent with the presence of anterograde interference. Furthermore, full interference occurred even when the visuomotor environment encountered in the second task was not rotated (B0). This latter novel result differs from those obtained for force field learning, where interference does not occur when task B does not impose perturbing forces, i.e., when B consists of a null field (Brashers-Krug et al., Nature 382:252-255, 1996). The results are consistent with recent proposals suggesting different interference mechanisms for visuomotor (kinematic) compared to force field (dynamic) adaptations, and have implications for the use of washout trials when studying interference between multiple visuomotor environments.

Introdução: A aplicação das técnicas de Contrair-Relaxar com Contracção do Antagonista (CRCA) e de Músculo Energia (TME) promovem um aumento da flexibilidade muscular, contudo poucos estudos comparam a eficácia de ambas. Apresentam aspectos comuns como a contracção prévia do músculo a alongar sendo esta máxima na CRCA e uma percentagem da máxima na TME. Contudo, alguma evidência sugere que não existe correspondência entre a força produzida e a desejada pelo que este aspecto da TME carece de explicação. Objectivos: Confirmar se a técnica CRCA e a TME são efectivas no alongamento muscular dos isquiotibiais a curto prazo, caso sejam determinar qual a mais efectiva. Pretende-se ainda avaliar se a percepção ao esforço durante a aplicação da TME corresponde à força efectivamente realizada. Métodos: Efectuou-se um estudo experimental com 45 voluntários distribuídos aleatoriamente pelos grupos CRCA, TME e Controlo. Avaliou-se a amplitude articular passiva de extensão do joelho antes e depois de aplicar as técnicas, utilizando um goniómetro. Nos participantes submetidos à TME avaliou-se a percepção ao esforço, solicitando uma contracção submáxima isométrica de 40% medida através do dinamómetro de mão. Resultados: Verificou-se um efeito das técnicas entre as avaliações (Teste ANOVA medidas repetidas factor tempo: p<0,001) e entre os grupos (tempo*grupo: p<0,001). Comparando os grupos dois a dois, verificaram-se diferenças entre o grupo CRCA e o grupo Controlo (Teste Post Hoc Games-Howell: p=0,001) e entre o grupo TME e o grupo Controlo (p=0,009), não existindo diferenças entre os grupos CRCA e TME (p=0,376). Os grupos CRCA e TME obtiveram um ganho de 10,7º e de 11,4º respectivamente, não havendo diferenças significativas entre os ganhos (Teste T-Student Independente: p=0,599). Existiram diferenças significativas entre os 40% CMVI produzida e desejada (Teste Wilcoxon: p=0,018). Conclusão: Ambas foram efectivas no aumento da flexibilidade muscular dos isquiotibiais a curto prazo. Os efeitos foram comparáveis, mas dada a menor complexidade e menor solicitação a TME foi considerada mais eficiente. A percepção ao esforço durante a aplicação da TME não correspondeu ao esforço desejado, existindo uma tendência para a produção de intensidades de contracções maiores.

The Active Isolated Stretching (AIS) technique proposes that by contracting a muscle (agonist) the opposite muscle (antagonist) will relax through reciprocal inhibition and lengthen without increasing muscle tension (Mattes, 2000). The clinical effectiveness of AIS has been reported but its mechanism of action has not been investigated at the tissue level. Proposed mechanisms for increased range of motion (ROM) include mechanical or neural changes, or an increased stretch tolerance. The purpose of the study was to investigate changes in mechanical properties, i.e. stiffness, of skeletal muscle in response to acute and long-term AIS stretching for the hamstring muscle group. Recreationally active university-aged students (female n=8, male n=2) classified as having tight hamstrings, by a knee extension test, volunteered for the study. All stretch procedures were performed on the right leg, with the left leg serving as a control. Each subject was assessed twice: at an initial session and after completing a 6-week AIS hamstring stretch training program. For both test sessions active knee extension (ROM) to a position of "light irritation", passive resisted torque and stiffness were determined before and after completion of the AIS technique (2x10 reps). Data were collected using a Biodex System 3 Pro (Biodex Medical Systems, NY, USA) isokinetic dynamometer. Surface electromyography (EMG) was used to monitor vastus lateralis (VL) and hamstring muscle activity during the stretching movements. Between test sessions, 2x10 reps of the AIS bent knee hamstring stretch were performed daily for 6-weeks.
