El estudio de células del sistema inmune innato en infecciones virales se ha centrado principalmente en las células dendríticas y las células NK. Los objetivos de este proyecto de investigación son evaluar el nivel de apoptosis de neutrófilos y las vías de activación de muerte celular en la infección por Virus de la Inmunodeficiencia Humana (VIH) y caracterizar la respuesta inmune inducida en monocitos/macrófagos infectados in vitro con el Encefalitis San Luis virus (ESLV). En individuos con infección por VIH se evaluará la apoptosis (anexina y citomorfología), la expresión de moléculas de adhesión y de receptor Toll-like (TLR) en neutrófilos. Además, se determinará la concentración sérica de citocinas pro-inflamatorias y anti-inflamatorias y de moléculas pro-apoptóticas y anti-apoptóticas. La infección por ESLV será realizada en una línea celular mieloide en la que se detectaran los antígenos virales a distintos días post-infección viral (inmunofluorescencia y citometría de flujo). Además, se determinará la expresión de antígenos de superficie y TLR. En sobrenadantes de cultivos de monocitos infectados con diferentes genotipos de ESLV a distintos días post-infección serán cuantificada la concentración de citocinas y determinada la apoptosis de monocito/macrófagos infectados (anexina y citomorfología). Este proyecto posibilitará una mayor comprensión de la regulación de la apoptosis en la infección por VIH, conocimiento que podrá ser de utilidad para avanzar en investigaciones futuras para el entendimiento de los mecanismos y la regulación de la apoptosis de componentes del sistema inmune innato. En la infección por ESLV el conocimiento de las características de la activación del macrófago cuando es infectado por este virus, los inmunomoduladores liberados y el impacto de la infección sobre la apoptosis de ésta célula podrían orientar hacia posibles blancos para el diseño futuro de estrategias terapéuticas o profilácticas contra esta infección.

A selective and differencial medium was developed for the isolation of Acinetobacter genus bacteria. This Acinobacter Agar Medium (p.H + 7.4) contains in grams per litre: thiotone, 10; yeast extract, 3; naC1, 5; saccharose, 10; mannitol, 10; sodium citrate, 0.5; sodium desoxycholate, 0.1; crystal violet, 0.00025; phenol red, 0.04 and agar-agar 15. This medium has the advantage of inhibiting the growth of cocci and Gram-positive bacilli, by the use of sodium citrate and sodium desoxycholate associated with the crystal violet; and of differentiating the Gram-negative bacilli from the Enterobacteriaceae, through the fermentative activity upon the saccharose and/or mannitol, contrasting with the complete inactivity of the Acinetobacter genus bacteria over those substances.

A burn patient was infected with Acinetobacter baumannii on transfer to the hospital after a terrorist attack. Two patients experienced cross-infection. Environmental swab samples were negative for A. baumannii. Six months later, the bacteria reemerged in 6 patients. Environmental swab samples obtained at this time were inoculated into a minimal mineral broth, and culture results showed widespread contamination. No case of infection occurred after closure of the unit for disinfection.

Després de la introducció de la teràpia antirretroviral d’alta eficàcia, s’ha objectivat a l’hospital vall d’hebron, una disminució progressiva de la incidencia de les principals infeccions oportunistes del sistema nerviós central (toxoplasmosis, meningitis tuberculosa, meningitis criptococócica i de la leucoencefalopatia multifocal progresiva-LMP). La supervivencia global d’aquestes infeccions va ser de 2 mesos, sent la de pitjor pronòstic la LMP, l’estimació de supervivencia de la qual als 36 mesos va ser del 36.4%. L’aparició de la síndrome Inflamatòria de reconstitució immunològica es va asociar a una recuperació immunològica més rápida, sense asociar-se a major mortalitat.

L'objectiu d'aquest treball és descriure els tres primers anys d'experiència en teràpia ambulatoria domiciliària endovenosa (TADE) en infeccions per microorganismes multiresistents a la nostra Unitat d'Hospitalització a Domicili alhora que avaluar la seva seguretat i eficacia. El grup control són la resta dels pacients amb TADE ingressats a la Unitat. En el grup estudi hi ha hagut més tornades inesperades i reingressos, probablement en relació a la major comorbilitat i dependència fisica dels pacients. La taxa de reaccions adverses és similar i s'ha reduit significativament el nombre d'hospitalitzacions comparant els tres mesos previs a l'ingrés i els tres posteriors. La rotació cíclica d'antibiòtics (RCA) consisteix a restringir de forma determinada i establerta un antibiòtic o classe d'antibiòtics durant un determinat període de temps, per a tornar a reintroduir-lo posteriorment. D'aquesta manera es pretén evitar l'aparició de resistències bacterianes derivades d'un ús continuat del mateix. En aquest treball, proposem la RCA com una estratègia per al control d'infeccions per gèrmens multirresistents i veure la seva influència en els gèrmens més freqüents que intervenen en les pneumònies nosocomials (NN). A través del registre ENVIN es van recollir les dades de tots els pacients ingressats a la UCI de l'Hospital Universitari la Fe durant dotze mesos consecutius. Es van dividir en 4 cicles de 3 mesos de durada cadascun d'ells. En el primer cicle es va restringir ampicilina/sulbactam, amikacina, cefalosporines i vancomicina; en el segon cicle carbapenems, amikacina i linezolid; en el tercer cicle tigeciclina, quinolones, tobramicina i linezolid i al quart i últim cicle, piperacilina/tazobactam, tobramicina i teicoplanina. Es va comparar amb els tres mesos previs al inici del treball, al qual l'ús d'antibiòtics va ser lliure. El temps global de l'estudi va ser de 15 mesos. El percentatge d'aïllaments d´Acinetobacter spp en el període basal va ser del 46,15% (n=6), de Pseudomonas aeruginosa 15,38% (n=2) i d'Escherichia coli 7, 69% (n=1). Al final de l'estudi els gèrmens aïllats van ser en un 8,57% (n=3) Acinetobacter spp i en un 37,14% (n=13) Pseudomonas aeruginosa.

Acinetobacter baumannii is a strictly aerobic bacterium which causes severe infections, however its pathogenic characteristics are not well defined. Thirteen A. baumannii strains isolated from urine of hospitalized and nonhospitalized patients with different ages were investigated for the presence of virulence factors. The isolates belonged to biotypes 2, 6, and 9 and were sensitive to imipenem. The majority of them showed resistance to amikacin, ceftazidime, ceftriaxone, ciprofloxacin, gentamicin, norfloxacin, and trimethoprim-sulfamethoxazole. None of A. baumannii strains presented genes codifying for 17 different virulence factors previously described in uropathogenic Escherichia coli, when tested by polymerase chain reaction (PCR). Nine isolates agglutinated human group AB erythrocytes, in presence of mannose, but none of them agglutinated group O erythrocytes. Adherence to polystyrene was observed in 7 isolates, and this result did not correlate with that obtained in hemagglutination assay. All the isolates were able to grow in iron-limiting conditions, showing that A. baumannii produces some type of siderophore. However, the genes iutA and fyuA, from iron uptake system of E. coli and Yersinia sp., respectively, were not present in the isolates, suggesting the presence of a different type of siderophore. The fimbriae of A. baumannii strains that mediates the adherence are possibly mannose-resistant, eventhough the mechanism of adherence to human epithelial cells still remains to be elucidated.

The members of the genus Acinetobacter are Gram-negative cocobacilli that are frequently found in the environment but also in the hospital setting where they have been associated with outbreaks of nosocomial infections. Among them, Acinetobacter baumannii has emerged as the most common pathogenic species involved in hospital-acquired infections. One reason for this emergence may be its persistence in the hospital wards, in particular in the intensive care unit; this persistence could be partially explained by the capacity of these microorganisms to form biofilm. Therefore, our main objective was to study the prevalence of the two main types of biofilm formed by the most relevant Acinetobacter species, comparing biofilm formation between the different species. Findings: Biofilm formation at the air-liquid and solid-liquid interfaces was investigated in different Acinetobacter spp. and it appeared to be generally more important at 25°C than at 37°C. The biofilm formation at the solid-liquid interface by the members of the ACB-complex was at least 3 times higher than the other species (80-91% versus 5-24%). In addition, only the isolates belonging to this complex were able to form biofilm at the air-liquid interface; between 9% and 36% of the tested isolates formed this type of pellicle. Finally, within the ACB-complex, the biofilm formed at the air-liquid interface was almost 4 times higher for A. baumannii and Acinetobacter G13TU than for Acinetobacter G3 (36%, 27% & 9% respectively). Conclusions: Overall, this study has shown the capacity of the Acinetobacter spp to form two different types of biofilm: solid-liquid and air-liquid interfaces. This ability was generally higher at 25°C which might contribute to their persistence in the inanimate hospital environment. Our work has also demonstrated for the first time the ability of the members of the ACB-complex to form biofilm at the air-liquid interface, a feature that was not observed in other Acinetobacter species.

Carbapenemase production is an important mechanism of carbapenem resistance among nonfermentative Gram-negative isolates. This study aimed to report the detection of blaOXA-58 gene in multiresistant clinical isolates of Acinetobacter baumannii recovered from inpatients in a public hospital. Polymerase chain reaction tests were performed to detect the blaOXA-23-like, blaOXA-24-like, blaOXA-58-like and blaOXA-51-like genes. The blaOXA-58 and blaOXA-23 genes were detected in one and three isolates, respectively. Sequencing of the blaOXA-58-like amplicon revealed 100% identity with the A. baumannii blaOXA-58 gene listed in the GenBank database. This is the first report of an OXA-58-producing A. baumannii isolate in Rio de Janeiro, Brazil.

The aim of the current study was to describe the occurrence of the blaOXA-23 gene and the ISAba1 element in imipenem-susceptible Acinetobacter baumannii strains. By performing the polymerase chain reaction mapping using combinations of ISAba1 forward primers and the blaOXA-23-like gene reverse primers, we demonstrated that the ISAba1 element did not occur upstream of the blaOXA-23 gene in five of 31 isolates, which explained the lack of resistance to imipenem despite the presence of the blaOXA-23 gene. All of the blaOXA-23-positive isolates were susceptible to imipenem and meropenem with minimal inhibitory concentration < 4 µg/mL. Pulsed-field gel electrophoresis analysis revealed four genotypes among the five blaOXA-23-positive isolates. The current report of the blaOXA-23 gene in imipenem-susceptible isolates provided evidence that this gene may be silently spread in a hospital environment and highlighted the threat of undetected reservoirs of carbapenemase genes.

Adhesion to host cells is an initial and important step in Acinetobacter baumannii pathogenesis. However, there is relatively little information on the mechanisms by which A. baumannii binds to and interacts with host cells. Adherence to extracellular matrix proteins, such as fibronectin, affords pathogens with a mechanism to invade epithelial cells. Here, we found that A. baumannii adheres more avidly to immobilized fibronectin than to control protein. Free fibronectin used as a competitor resulted in dose-dependent decreased binding of A. baumannii to fibronectin. Three outer membrane preparations (OMPs) were identified as fibronectin binding proteins (FBPs): OMPA, TonB-dependent copper receptor, and 34 kDa OMP. Moreover, we demonstrated that fibronectin inhibition and neutralization by specific antibody prevented significantly the adhesion of A. baumannii to human lung epithelial cells (A549 cells). Similarly, A. baumannii OMPA neutralization by specific antibody decreased significantly the adhesion of A. baumannii to A549 cells. These data indicate that FBPs are key adhesins that mediate binding of A. baumannii to human lung epithelial cells through interaction with fibronectin on the surface of these host cells.

Although analysis of toxin-antitoxin (TA) systems can be instructive, to date, there is no information on the prevalence and identity of TA systems based on a large panel of Acinetobacter baumannii clinical isolates. The aim of the current study was to screen for functional TA systems among clinical isolates of A. baumannii and to identify the systems’ locations. For this purpose, we screened 85 A. baumannii isolates collected from different clinical sources for the presence of the mazEF, relBE and higBA TA genes. The results revealed that the genes coding for the mazEF TA system were commonly present in all clinical isolates of A. baumannii. Reverse transcriptase-polymerase chain reaction analysis showed that transcripts were produced in the clinical isolates. Our findings showed that TA genes are prevalent, harboured by chromosomes and transcribed within A. baumannii. Hence, activation of the toxin proteins in the mazEF TA system should be investigated further as an effective antibacterial strategy against this bacterium.
