History of hypertension and eplerenone in patients with acute myocardial infarction complicated by heart failure

In the Eplerenone Post-Acute Myocardial Infarction Heart Failure Efficacy and Survival Study ( n = 6632), eplerenone- associated reduction in all- cause mortality was significantly greater in those with a history of hypertension ( Hx- HTN). There were 4007 patients with Hx- HTN ( eplerenone: n = 1983) and 2625 patients without Hx- HTN ( eplerenone: n = 1336). Propensity scores for eplerenone use, separately calculated for patients with and without Hx- HTN, were used to assemble matched cohorts of 1838 and 1176 pairs of patients. In patients with Hx- HTN, all- cause mortality occurred in 18% of patients treated with placebo ( rate, 1430/ 10 000 person- years) and 14% of patients treated with eplerenone ( rate, 1058/ 10 000 person- years) during 2350 and 2457 years of follow- up, respectively ( hazard ratio [ HR]: 0.71; 95% CI: 0.59 to 0.85; P < 0.0001). Composite end point of cardiovascular hospitalization or cardiovascular mortality occurred in 33% of placebo-treated patients ( 3029/ 10 000 person- years) and 28% of eplerenone- treated patients (2438/10 000 person- years) with Hx- HTN ( HR: 0.82; 95% CI: 0.72 to 0.94; P = 0.003). In patients without Hx- HTN, eplerenone reduced heart failure hospitalization ( HR: 73; 95% CI: 0.55 to 0.97; P = 0.028) but had no effect on mortality ( HR: 0.91; 95% CI: 0.72 to 1.15; P = 0.435) or on the composite end point ( HR: 0.91; 95% CI: 0.76 to 1.10; P = 0.331). Eplerenone should, therefore, be prescribed to all of the post - acute myocardial infarction patients with reduced left ventricular ejection fraction and heart failure regardless of Hx- HTN.

In many respects, A History of Intelligence and Intellectual 'Disability' is a confronting work. At a literal level, it comprises eighteen chapters verging on a total of quarter of a million words, apart from twenty-two pages of primary and secondary sources. Many of its chapters are drawn from Chris Goodey's articles since the early 'nineties in such journals as Ancient Philosophy, Archiv für Geschichte der Philosophie, History of the Human Sciences, and Political Theory. However, he immediately warns us that such articles exist as a "more primitive form" (vii) of the current volume.

Mitochondrial genome sequence of the Brazilian luminescent click beetle Pyrophorus divergens (Coleoptera: Elateridae): Mitochondrial genes utility to investigate the evolutionary history of Coleoptera and its bioluminescence

The Coleoptera order is the richest group among Metazoa, but its phylogenetics remains incompletely understood. Among Coleoptera, bioluminescence is found within the Elateroidea, but the evolution of this character remains a mystery. Mitochondrial DNA has been used extensively to reconstruct phylogenetic relationships, however, the evolution of a single gene does not always correspond to the species evolutionary history and the molecular marker choice is a key step in this type of analysis. To create a solid basis to better understand the evolutionary history of Coleoptera and its bioluminescence, we sequenced and comparatively analyzed the mitochondrial genome of the Brazilian luminescent click beetle Pyrophorus divergens (Coleoptera: Elateridae). © 2007 Elsevier B.V. History of Maltreatment and Psychiatric Impairment in Children in Outpatient Psychiatric Treatment

There is increasing evidence that childhood victimization and attachment disruptions impact a child’s development. In this study, children and adolescents from an outpatient psychiatric clinic were assessed, measuring history of trauma, history of out-of-home placement, initial diagnoses, and CBCL internalizing and externalizing problem scores. Multiple regression analyses showed that both violent abuse trauma (physical/sexual abuse) and victim trauma (physical abuse/sexual abuse/witnessing domestic violence/witnessing community violence) are prevalent among patients with externalizing severity problems; concluding that diagnosis alone may not account for a history of victimization, but externalizing problem severity does. Overall, the study is consistent with past literature that it is important to acknowledge a child’s history of maltreatment and out-of-home placement when understanding their psychiatric development and diagnosis.

Three decades of Space Activities at the Instituto Universitario de Microgravedad "Ignacio Da Riva" / Universidad Politécnica de Madrid (IDR/UPM) en History of Rocketry and Astronautics

Strictly speaking, space related activities at the Escuela Técnica Superior de Ingenieros Aeronáuticos (ETSIA) begun in 1973, when Prof. Ignacio Da Riva got a contract from the European Space Agency (ESA) to compile a handbook on spacecraft thermal control. By the same time, ESA issued an announcement of opportunities offering to the European scientific community the possibility of perform microgravity relevant experiments on board space platform like the European orbital laboratory Spacelab. Prof. Da Riva proposed one of the few selected experiments dealing with fluid physics under microgravity conditions, later flown on Spacelab-1 mission in 1983. These two events were the starting point where Prof. Da Riva, full professor of Aerodynamics at ETSIA, nucleated a small group of young professors and students located at the Laboratorio de Aerodinámica y Mecánica de Fluidos (LAMF) of ETSIA. Such group was leaded by Prof. Da Riva since its creation till 1991, when Prof. Da Riva died, and it was the seed of the more recently created research institute for aerospace science and technology named "Ignacio Da Riva" (IDR) in his honour. In this communication space related activities performed either at LAMF or IDR during the last three decades are briefly described.
