A incidência de infecções fúngicas invasivas vem aumentando nos últimos anos. Estas infecções, em geral, apresentam altas taxas de mortalidade. A profilaxia com antifúngicos ainda é a estratégia mais comum na contenção da mortalidade e prevenção contra infecções fúngicas invasivas, porém, apresenta baixa eficiência, e relatos de resistência às drogas. Além disso, a terapia antifúngica é limitada a um pequeno grupo de drogas, como os polienos, azóis e equinocandinas. Desta forma, a busca de novos alvos de drogas é fundamental para o desenvolvimento de novos antifúngicos. Estudos in silico indicaram quatro genes como potenciais alvo de drogas em fungos patogênicos. Neste contexto, o objetivo deste trabalho foi verificar a expressão das proteínas codificadas por dois destes possíveis genes alvo, a proteína erg6, na fração microssomal, e trr1, na fração citosólica, em hifas de A. fumigatus. Visando alcançar este objetivo, foram primeiramente padronizadas todas as etapas de fracionamento celular visando isolar estas duas subfrações celulares de A. fumigatus. Posteriormente, foi otimizado o protocolo de extração e reidratação de proteínas microssomais bem como reidratação de proteínas citosólicas. Estes extratos foram submetidos a diferentes protocolos de fracionamento proteico em um sistema de eletroforese OFFGEL (OGE). Os resultados de Western immunoblot mostraram que estas duas proteínas, erg6 e trr1, são de fato expressas na fase filamentosa de A. fumigatus. O extrato proteico da fração microssomal submetido ao OGE em doze subfrações apresentou três subunidades da proteína erg6, reconhecidas pelo anticorpo monoclonal, com massas moleculares e pI distintos: uma subunidade de aproximadamente 79 kDa com pI entre 5,91 e 6,49, e outras duas subunidades de aproximadamente 35 kDa e 32 kDa, ambas com pI entre 6,49 e 7,08. A enzima erg6 foi descrita como um homotetrâmero em outros fungos. Porém, nossos resultados sugerem que, em A. fumigatus, a erg6 possui uma estrutura heterotetramérica. Quanto à proteína trr1, tanto no extrato total quanto nas frações resultantes do fracionamento em OGE, uma banda única de aproximadamente 40 kDa, com pI na faixa de 4,79 e 5,33, foi reconhecida pelo anticorpo policlonal. Desta forma, esta proteína parece ter uma estrutura homodimérica, assim como descrito em outros micro-organismos.

Amphotericin B (AmB) is a popular drug frequently applied in the treatment of systemic fungal infections. In the presence of ruthenium (II) as the maker ion, the behavior of AmB to form ion channels in sterol-free and cholesterol- or ergosterol-containing supported phosphatidylcholine bilayer model membranes were studied by cyclic votammetry, AC impedance spectroscopy, and UV/visible absorbance spectroscopy. Different concentrations of AmB ranging from a molecularly dispersed to a highly aggregated state of the drug were investigated. In a fixed cholesterol or ergosterol content (5 mol %) in glassy carbon electrode-supported model membranes, our results showed that no matter what form of AmB, monomeric or aggregated, AmB could form ion channels in supported ergosterol-containing phosphatidylcholine bilayer model membranes. However, AmB could not form ion channels in its monomeric form in sterol-free and cholesterol-containing supported model membranes. On the one hand, when AmB is present as an aggregated state, it can form ion channels in cholesterol-containing supported model membranes; on the other hand, only when AmB is present as a relatively highly aggregated state can it form ion channels in sterol-free supported phosphatidylcholine bilayer model membranes. The results showed that the state of AmB played an important role in forming ion channels in sterol-free and cholesterol-containing supported phosphatidylcholine bilayer model membranes.

Antifungal compounds produced by Lactic acid bacteria (LAB) metabolites can be natural and reliable alternative for reducing fungal infections pre- and post-harvest with a multitude of additional advantages for cereal-base products. Toxigenic and spoilage fungi are responsible for numerous diseases and economic losses. This thesis includes an overview of the impact fungi have on aspects of the cereal food chain. The applicability of LAB in plant protection and cereal industry is discussed in detail. Specific case studies include Fusarium head blight, and the impact of fungi in the malting and baking industry. The impact of Fusarium culmorum infected raw barley on the final malt quality was part of the investigation. In vitro infected barley grains were fully characterized. The study showed that the germinative energy of infected barley grains decreased by 45% and grains accumulated 199 μg.kg-1 of deoxynivalenol (DON). Barley grains were subsequently malted and fully characterized. Fungal biomass increased during all stages of malting. Infected malt accumulated 8-times its DON concentration during malting. Infected malt grains revealed extreme structural changes due to proteolytic, (hemi)-cellulolytic and starch degrading activity of the fungi, this led to increased friability and fragmentation. Infected grains also had higher protease and β-glucanase activities, lower amylase activity, a greater proportion of free amino and soluble nitrogen, and a lower β-glucan content. Malt loss was over 27% higher in infected malt when compared to the control. The protein compositional changes and respective enzymatic activity of infected barley and respective malt were characterized using a wide range of methods. F. culmorum infected barley grains showed an increase in proteolytic activity and protein extractability. Several metabolic proteins decreased and increased at different rates during infection and malting, showing a complex F. culmorum infection interdependence. In vitro F. culmorum infected malt was used to produce lager beer to investigate changes caused by the fungi during the brewing processes and their effect on beer quality attributes. It was found, that the wort containing infected malt had a lower pH, a higher FAN, higher β-glucan and a 45% increase in the purging rate, and led to premature yeast flocculation. The beer produced with infected malt (IB) had also a significantly different amino acid profile. IB flavour characterization revealed a higher concentration of esters, fusel alcohols, fatty acids, ketones, and dimethylsulfide, and in particular, acetaldehyde, when compared to the control. IB had a greater proportion of Strecker aldehydes and Maillard products contributing to an increased beer staling character. IB resulted in a 67% darker colour with a trend to better foam stability. It was also found that 78% of the accumulated mycotoxin deoxynivalenol in the malt was transferred into beer. A LAB cell-freesupernatant (cfs), produced in wort-base substrate, was investigated for its ability to inhibit Fusarium growth during malting. Wort was a suitable substrate for LAB exhibiting antifungal activity. Lactobacillus amylovorus DSM19280 inhibited 104 spores.mL-1 for 7 days, after 120 h of fermentation, while Lactobacillus reuteri R29 inhibited 105 spores.mL-1 for 7 days, after 48 h of fermentation. Both LAB cfs had significant different organic acid profiles. Acid-base antifungal compounds were identified and, phenyllactic, hydroxy-phenyllactic, and benzoic acids were present in higher concentrations when compared to the control. A 3 °P wort substrate inoculated With L. reuteri R29 (cfs) was applied in malting and successfully inhibited Fusarium growth by 23%, and mycotoxin DON by 80%. Malt attributes resulted in highly modified grains, lower pH, higher colouration, and higher extract yield. The implementation of selected LAB producing antifungal compounds can be used successfully in the malting process to reduce mould growth and mycotoxin production.

In preventing invasive fungal disease (IFD) in patients with acute myelogenous leukemia (AML) or myelodysplastic syndrome (MDS), clinical trials demonstrated efficacy of posaconazole over fluconazole and itraconazole. However, effectiveness of posaconazole has not been investigated in the United States in real-world setting outside the environment of controlled clinical trial. We performed a single-center, retrospective cohort study of 130 evaluable patients ≥18 years of age admitted to Duke University Hospital between 2004 and 2010 who received either posaconazole or fluconazole as prophylaxis during first induction or first reinduction chemotherapy for AML or MDS. The primary endpoint was possible, probable, or definite breakthrough IFD. Baseline characteristics were well balanced between groups, except that posaconazole recipients received reinduction chemotherapy and cytarabine more frequently. IFD occurred in 17/65 (27.0%) in the fluconazole group and in 6/65 (9.2%) in the posaconazole group (P = 0.012). Definite/probable IFDs occurred in 7 (10.8%) and 0 patients (0%), respectively (P = 0.0013). In multivariate analysis, fluconazole prophylaxis and duration of neutropenia were predictors of IFD. Mortality was similar between groups. This study demonstrates superior effectiveness of posaconazole over fluconazole as prophylaxis of IFD in AML and MDS patients. Such superiority did not translate to reductions in 100-day all-cause mortality.

In photodynamic antimicrobial chemotherapy (PACT), a combination of a sensitising drug and visible light causes selective destruction of microbial cells. The ability of light-drug combinations to kilt microorganisms has been known for over 100 years. However, it is only recently with the beginning of the search for alternative treatments for antibiotic-resistant pathogens that the phenomenon has been investigated in detail. Numerous studies have shown PACT to be highly effective in the in vitro destruction of viruses and protozoa, as well as Gram-positive and Gram-negative bacteria and fungi. Results of experimental investigations have demonstrated conclusively that both dermatomycetes and yeasts can be effectively killed by photodynamic action employing phenothiazinium, porphyrin and phthatocyanine photosensitisers. Importantly, considerable setectivity for fungi over human cells has been demonstrated, no reports of fungal resistance exist and the treatment is not associated with genotoxic or mutagenic effects to fungi or human cells. In spite of the success of cell culture investigations, only a very small number of in vivo animal. and human trials have been published. The present paper reviews the studies published to date on antifungal applications of PACT and aims to raise awareness of this area of research, which has the potential to make a significant impact in future treatment of fungal infections. Fungal infections are an important cause of morbidity and mortality in patients with acute leukemia (AL). Candidemia, once rare, is now a common nosocomial infection because of the intensity of chemotherapy, prolonged neutropenia, administration of broad-spectrum antibiotics and use of central venous catheters (CVC). We retrospectively identified patients treated for AL from 6/86 to 6/95 who also had candidemia. We describe 28 patients (incidence 6.3%) with a median age of 39 years, 24 of whom were on remission induction and 4 on postremission chemotherapy. All patients had CVC and empiric antimicrobial therapy, 4 had been given prophylactic antifungal drugs, and 2 had parenteral nutrition. Neutropenia was profound (median leukocyte nadir 200/microliters, median duration 19 days). Candida was isolated in blood cultures 10 days (median) after the start of neutropenia. The clinical presentation included fever (100%), respiratory symptoms (71.4%), skin lesions (39.2%) and septic shock (17.8%). Amphotericin B was given to 17 patients and liposomal amphotericin to 5 patients. Infection resolved in 18 patients (64.2%). 10 of whom were in complete remission. Mortality from candidemia was 17.8% (5/28). In conclusion, fungal infections are responsible for death in a significant number of patients. In our series treatment success was related to its rapid onset and to the recovery of neutropenia.

Aujourd'hui, les problèmes des maladies infectieuses concernent l'émergence d'infections difficiles à traiter, telles que les infections associées aux implants et les infections fongiques invasives chez les patients immunodéprimés. L'objectif de cette thèse était de développer des stratégies pour l'éradication des biofilms bactériens (partie 1), ainsi que d'étudier des méthodes innovantes pour la détection microbienne, pour l'établissement de nouveaux tests de sensibilité (partie 2). Le traitement des infections associées aux implants est difficile car les biofilms bactériens peuvent résister à des niveaux élevés d'antibiotiques. A ce jour, il n'y a pas de traitement optimal défini contre des infections causées par des bactéries de prévalence moindre telles que Enterococcus faecalis ou Propionibacterium acnés. Dans un premier temps, nous avons démontré une excellente activité in vitro de la gentamicine sur une souche de E. faecalis en phase stationnaire de croissance Nous avons ensuite confirmé l'activité de la gentamicine sur un biofilm précoce en modèle expérimental animal à corps étranger avec un taux de guérison de 50%. De plus, les courbes de bactéricidie ainsi que les résultats de calorimétrie ont prouvé que l'ajout de gentamicine améliorait l'activité in vitro de la daptomycine, ainsi que celle de la vancomycine. In vivo, le schéma thérapeutique le plus efficace était l'association daptomycine/gentamicine avec un taux de guérison de 55%. En établissant une nouvelle méthode pour l'évaluation de l'activité des antimicrobiens vis-à-vis de micro-organismes en biofilm, nous avons démontré que le meilleur antibiotique actif sur les biofilms à P. acnés était la rifampicine, suivi par la penicilline G, la daptomycine et la ceftriaxone. Les études conduites en modèle expérimental animal ont confirmé l'activité de la rifampicine seule avec un taux de guérison 36%. Le meilleur schéma thérapeutique était au final l'association rifampicine/daptomycine avec un taux de guérison 63%. Les associations de rifampicine avec la vancomycine ou la levofloxacine présentaient des taux de guérisons respectivement de 46% et 25%. Nous avons ensuite étudié l'émergence in vitro de la résistance à la rifampicine chez P. acnés. Nous avons observé un taux de mutations de 10"9. La caractérisation moléculaire de la résistance chez les mutant-résistants a mis en évidence l'implication de 5 mutations ponctuelles dans les domaines I et II du gène rpoB. Ce type de mutations a déjà été décrit au préalable chez d'autres espèces bactériennes, corroborant ainsi la validité de nos résultats. La deuxième partie de cette thèse décrit une nouvelle méthode d'évaluation de l'efficacité des antifongiques basée sur des mesures de microcalorimétrie isotherme. En utilisant un microcalorimètre, la chaleur produite par la croissance microbienne peut être-mesurée en temps réel, très précisément. Nous avons évalué l'activité de l'amphotéricine B, des triazolés et des échinocandines sur différentes souches de Aspergillus spp. par microcalorimétrie. La présence d'amphotéricine Β ou de triazole retardait la production de chaleur de manière concentration-dépendante. En revanche, pour les échinochandines, seule une diminution le pic de « flux de chaleur » a été observé. La concordance entre la concentration minimale inhibitrice de chaleur (CMIC) et la CMI ou CEM (définie par CLSI M38A), avec une marge de 2 dilutions, était de 90% pour l'amphotéricine B, 100% pour le voriconazole, 90% pour le pozoconazole et 70% pour la caspofongine. La méthode a été utilisée pour définir la sensibilité aux antifongiques pour d'autres types de champignons filamenteux. Par détermination microcalorimétrique, l'amphotéricine B s'est avéré être l'agent le plus actif contre les Mucorales et les Fusarium spp.. et le voriconazole le plus actif contre les Scedosporium spp. Finalement, nous avons évalué l'activité d'associations d'antifongiques vis-à-vis de Aspergillus spp. Une meilleure activité antifongique était retrouvée avec l'amphotéricine B ou le voriconazole lorsque ces derniers étaient associés aux échinocandines vis-à-vis de A. fumigatus. L'association échinocandine/amphotéricine B a démontré une activité antifongique synergique vis-à-vis de A. terreus, contrairement à l'association échinocandine/voriconazole qui ne démontrait aucune amélioration significative de l'activité antifongique. - The diagnosis and treatment of infectious diseases are today increasingly challenged by the emergence of difficult-to-manage situations, such as infections associated with medical devices and invasive fungal infections, especially in immunocompromised patients. The aim of this thesis was to address these challenges by developing new strategies for eradication of biofilms of difficult-to-treat microorganisms (treatment, part 1) and investigating innovative methods for microbial detection and antimicrobial susceptibility testing (diagnosis, part 2). The first part of the thesis investigates antimicrobial treatment strategies for infections caused by two less investigated microorganisms, Enterococcus faecalis and Propionibacterium acnes, which are important pathogens causing implant-associated infections. The treatment of implant-associated infections is difficult in general due to reduced susceptibility of bacteria when present in biofilms. We demonstrated an excellent in vitro activity of gentamicin against E. faecalis in stationary growth- phase and were able to confirm the activity against "young" biofilms (3 hours) in an experimental foreign-body infection model (cure rate 50%). The addition of gentamicin improved the activity of daptomycin and vancomycin in vitro, as determined by time-kill curves and microcalorimetry. In vivo, the most efficient combination regimen was daptomycin plus gentamicin (cure rate 55%). Despite a short duration of infection, the cure rates were low, highlighting that enterococcal biofilms remain difficult to treat despite administration of newer antibiotics, such as daptomycin. By establishing a novel in vitro assay for evaluation of anti-biofilm activity (microcalorimetry), we demonstrated that rifampin was the most active antimicrobial against P. acnes biofilms, followed by penicillin G, daptomycin and ceftriaxone. In animal studies we confirmed the anti-biofilm activity of rifampin (cure rate 36% when administered alone), as well as in combination with daptomycin (cure rate 63%), whereas in combination with vancomycin or levofloxacin it showed lower cure rates (46% and 25%, respectively). We further investigated the emergence of rifampin resistance in P. acnes in vitro. Rifampin resistance progressively emerged during exposure to rifampin, if the bacterial concentration was high (108 cfu/ml) with a mutation rate of 10"9. In resistant isolates, five point mutations of the rpoB gene were found in cluster I and II, as previously described for staphylococci and other bacterial species. The second part of the thesis describes a novel real-time method for evaluation of antifungals against molds, based on measurements of the growth-related heat production by isothermal microcalorimetry. Current methods for evaluation of antifungal agents against molds, have several limitations, especially when combinations of antifungals are investigated. We evaluated the activity of amphotericin B, triazoles (voriconazole, posaconazole) and echinocandins (caspofungin and anidulafungin) against Aspergillus spp. by microcalorimetry. The presence of amphotericin Β or a triazole delayed the heat production in a concentration-dependent manner and the minimal heat inhibition concentration (MHIC) was determined as the lowest concentration inhibiting 50% of the heat produced at 48 h. Due to the different mechanism of action echinocandins, the MHIC for this antifungal class was determined as the lowest concentration lowering the heat-flow peak with 50%. Agreement within two 2-fold dilutions between MHIC and MIC or MEC (determined by CLSI M38A) was 90% for amphotericin B, 100% for voriconazole, 90% for posaconazole and 70% for caspofungin. We further evaluated our assay for antifungal susceptibility testing of non-Aspergillus molds. As determined by microcalorimetry, amphotericin Β was the most active agent against Mucorales and Fusarium spp., whereas voriconazole was the most active agent against Scedosporium spp. Finally, we evaluated the activity of antifungal combinations against Aspergillus spp. Against A. jumigatus, an improved activity of amphotericin Β and voriconazole was observed when combined with an echinocandin. Against A. terreus, an echinocandin showed a synergistic activity with amphotericin B, whereas in combination with voriconazole, no considerable improved activity was observed.

Les maladies constituent présentement la cause la plus importante de perte économique en aquaculture moderne. Chez certaines espèces, notamment les salmonidés (Oncorhynchus sp. et Salmo sp.), on rapporte des pertes annuelles atteignant cinquante pour cent de la production. À l’heure actuelle, les infections fongiques occupent le second rang derrière les maladies bactériennes en fonction de leur importance économique. Ces poissons sont particulièrement vulnérables à une infection fongique causée par Saprolegnia sp. qui infecte habituellement les oeufs morts. Le saprophyte ubiquitaire se propage ensuite aux oeufs sains et aux individus matures. Malheureusement, le traitement efficace de cette infection, souvent primaire et parfois secondaire, est de plus en plus difficile en raison de nouvelles réglementations restrictives entourant le vert de malachite. Jadis, ce colorant constituait le fongicide le plus efficace dans la lutte contre la saprolégniose, mais son potentiel cancérigène en limite maintenant l’utilisation. Jusqu'à présent, aucun traitement disponible n’est aussi efficace que le vert de malachite pour le contrôle de la saprolégniose. Récemment, nous sommes parvenus à isoler trois bactéries capables d’inhiber la croissance de Saprolegnia sp. in vitro. Ces trois Pseudomonas fluorescens proviennent d’une pisciculture dans laquelle survenaient des cas d’infections à Saprolegnia parasitica. En poussant la caractérisation de l’activité grâce à des analyses de chromatographie liquide haute performance et de spectrométrie de masse, nous avons réussi à isoler et à identifier la molécule responsable. L’acide phénazine-1-carboxylique (PCA), sécrété par deux de nos trois souches, cause l’inhibition de la croissance de Saprolegnia.

Les travaux effectués dans le cadre de cette thèse de doctorat avaient pour but de mettre au point des nouvelles formulations d’antifongiques sous forme de nanoparticules polymériques (NP) en vue d’améliorer l’efficacité et la spécificité des traitements antifongiques sur des souches sensibles ou résistantes de Candida spp, d’Aspergillus spp et des souches de Candida albicans formant du biofilm. Dans la première partie de ce travail, nous avons synthétisé et caractérisé un polymère à base de polyester-co-polyéther branché avec du poly(éthylène glycol) (PEG-g-PLA). En plus d’être original et innovant, ce co-polymère a l’avantage d’être non-toxique et de posséder des caractéristiques de libération prolongée. Trois antifongiques couramment utilisés en clinique et présentant une biodisponibilité non optimale ont été choisis, soient deux azolés, le voriconazole (VRZ) et l’itraconazole (ITZ) et un polyène, l’amphotéricine B (AMB). Ces principes actifs (PA), en plus des problèmes d’administration, présentent aussi d’importants problèmes de toxicité. Des NP polymériques encapsulant ces PA ont été préparées par une technique d’émulsion huile-dans-l’eau (H/E) suivie d’évaporation de solvant. Une fois fabriquées, les NP ont été caractérisées et des particules de d’environ 200 nm de diamètre ont été obtenues. Les NP ont été conçues pour avoir une structure coeur/couronne avec un coeur constitué de polymère hydrophobe (PLA) et une couronne hydrophile de PEG. Une faible efficacité de chargement (1,3% m/m) a été obtenue pour la formulation VRZ encapsulé dans des NP (NP/VRZ). Toutefois, la formulation AMB encapsulée dans des NP (NP/AMB) a montré des taux de chargement satisfaisants (25,3% m/m). En effet, le caractère hydrophobe du PLA a assuré une bonne affinité avec les PA hydrophobes, particulièrement l’AMB qui est le plus hydrophobe des agents sélectionnés. Les études de libération contrôlée ont montré un relargage des PA sur plusieurs jours. La formulation NP/AMB a été testée sur un impacteur en cascade, un modèle in vitro de poumon et a permis de démontrer le potentiel de cette formulation à être administrée efficacement par voie pulmonaire. En effet, les résultats sur l’impacteur en cascade ont montré que la majorité de la formulation s’est retrouvée à l’étage de collecte correspondant au niveau bronchique, endroit où se situent majoritairement les infections fongiques pulmonaires. Dans la deuxième partie de ces travaux, nous avons testé les nouvelles formulations d’antifongiques sur des souches planctoniques de Candida spp., d’Aspergillus spp. et des souches de Candida albicans formant du biofilm selon les procédures standardisées du National Committee for Clinical Laboratory Standards (NCCLS). Les souches choisies ont démontré des résistances aux azolés et aux polyènes. Les études d’efficacité in vitro ont permis de prouver hors de tout doute que les nouvelles formulations offrent une efficacité nettement améliorée comparée à l’agent antifongique libre. Pour mettre en lumière si l’amélioration de l’efficacité antifongique était due à une internalisation des NP, nous avons évalué le comportement des NP avec les cellules de champignons. Nous avons procédé à des études qualitatives de microscopie de fluorescence sur des NP marquées avec de la rhodamine (Rh). Tel qu’attendu, les NP ont montré une localisation intracellulaire. Pour exclure la possibilité d’une simple adhésion des NP à la surface des levures, nous avons aussi confirmé leur internalisation en microscopie confocale de fluorescence. Il est important de noter que peu d’études à ce jour ont mis l’accent sur l’élaboration de nouvelles formulations d’antifongiques à base de polymères non toxiques destinées aux traitements des mycoses, donnant ainsi une grande valeur et originalité aux travaux effectués dans cette thèse. Les résultats probants obtenus ouvrent la voie vers une nouvelle approche pour contourner les problèmes de résistances fongiques, un problème de plus en plus important dans le domaine de l’infectiologie.

Este trabalho teve como objetivo a análise das micoses sistémicas, no que respeita à sua identificação, morfologia dos agentes patogénicos que lhes dão origem, onde são endémicas, os sintomas que apresentam, os métodos de diagnóstico e as terapêuticas disponíveis para cada patologia. As micoses sistémicas são infeções causadas por fungos patogénicos primários que têm o trato respiratório como porta de entrada, e a partir daí podem disseminar-se por todo o organismo. Os agentes antifúngicos são usados no tratamento destas infeções, em que, no caso das infeções sistémicas, predominam o uso de cetoconazol (polieno), fluconazol e itraconazol (azóis). Existem ainda estudos que demonstram que as vacinas atenuadas podem ser usadas na profilaxia destas infeções. Com este trabalho conclui-se que as infeções fúngicas estão longe de ser extintas, uma vez que se verifica cada vez mais a existência de resistências por parte dos fungos aos fármacos, sendo o diagnóstico e a terapêutica usada dois parâmetros fundamentais para um tratamento eficaz.

A Mastite é a afeção mais comum do úbere e consiste na reação inflamatória dos tecidos secretores da glândula mamária, como resposta a lesões traumáticas, infeções bacterianas e fúngicas. É uma das doenças mais observadas em explorações leiteiras, com grande impacto e perdas a nível económico. O objetivo do presente estudo foi a análise da eficácia da utilização de uma terapia combinada com Cefquinoma para o tratamento de mastites, comparando a terapêutica de Cefquinoma e outra terapêutica alternativa, o tempo de tratamento aplicado e a evolução do caso clínico. O estudo foi realizado num total de 80 vacas com mastite clínica em 3 explorações na área de grande Lisboa, tendo sido registados e analisados, para cada caso, qual o tratamento usado (a terapia combinada com Cefquinoma ou a terapêutica protocolar estabelecida pela exploração), o tempo de tratamento e a sua evolução clínica (se obteve cura, morte, refugo ou perda de ¼ mamário). O estudo da eficácia da terapia combinada com Cefquinoma pretende contribuir para um melhor conhecimento da eficácia das terapias existentes como coadjuvantes da melhoria da rentabilidade das explorações leiteiras. Os resultados obtidos permitem estabelecer uma relação positiva entre a taxa de cura e a terapêutica usada.

Amphotericin B (AmB) is widely used in the treatment of systemic fungal infections, despite its toxic effects. Nephrotoxicity, ascribed as the most serious toxic effect, has been related to the state of aggregation of the antibiotic. In search of the increase in AmB antifungal activity associated with low toxicity, several AmB-amphiphile formulations have been proposed. This work focuses on the structural characterization of a specific AmB formulation: AmB associated with sonicated dioctadecyl dimethylammonium bromide (DODAB) aggregates. Here, it was confirmed that sonicated DODAB dispersion is constituted by DODAB bicelles, and that monomeric AmB is much more soluble in bicelles than in DODAB vesicles. A new optical parameter is proposed for the estimation of the relative amount of amphiphile-bound monomeric AmB. With theoretical simulations of the spectra of spin labels incorporated in DODAB bicelles it was possible to prove that monomeric AmB binds preferentially to lipids located at the edges of DODAB bicelles, rigidifying them, and decreasing the polarity of the region. That special binding of monomeric AmB along the borders of bicelles, where the lipids are highly disorganized, could be used in the formulation of other carriers for the antibiotic, including mixtures of natural lipids which are known to form bicelles.
