Objectives: To evaluate the feasibility of a randomized-controlled trial (RCT) investigating the effects of adding auricular acupuncture (AA) to exercise for participants with chronic low-back pain (CLBP). Methods: Participants with CLBP were recruited from primary care and a university population and were randomly allocated (n=51) to 1 of 2 groups: (1) "Exercise Alone (E)"-12-week program consisting of 6 weeks of supervised exercise followed by 6 weeks unsupervised exercise (n=27); or (2) "Exercise and AA (EAA)"-12-week exercise program and AA (n=24). Outcome measures were recorded at baseline, week 8, week 13, and 6 months. The primary outcome measure was the Oswestry Disability Questionnaire. Results: Participants in the EAA group demonstrated a greater mean improvement of 10.7% points (95% confidence interval, -15.3,-5.7) (effect size=1.20) in the Oswestry Disability Questionnaire at 6 months compared with 6.7% points (95% confidence interval, -11.4,-1.9) in the E group (effect size=0.58). There was also a trend towards a greater mean improvement in quality of life, LBP intensity and bothersomeness, and fear-avoidance beliefs in the EAA group. The dropout rate for this trial was lower than anticipated (15% at 6 mo), adherence with exercise was similar (72% E; 65% EAA). Adverse effects for AA ranged from 1% to 14% of participants. Discussion: Findings of this study showed that a main RCT is feasible and that 56 participants per group would need to be recruited, using multiple recruitment approaches. AA was safe and demonstrated additional benefits when combined with exercise for people with CLBP, which requires confirmation in a fully powered RCT.

Evidence supports the use of exercise for chronic low back pain (CLBP); however, adherence is often poor due to ongoing pain. Auricular acupuncture is a form of pain relief involving the stimulation of points on the outer ear corresponding with specific body parts. It may be a useful adjunct to exercise in managing CLBP; however, there is only limited evidence to support its use with this patient group.

Introduction: Many cancer patients experience sleeping difficulties which can persist several years after the completion of cancer treatment. Previous research suggests that acupuncture, and variants of acupuncture (acupressure, auricular therapy) may be effective treatment options for sleep disturbance. However, current evidence is limited for cancer patients.
Methods: Feasibility study with 3 arms. Seven cancer patients with insomnia randomised to receive either auricular therapy (attaching semen vaccariae seeds to ear acupoints) (n=4), self-acupressure (n=1) or no treatment (n=2). Participants assigned to receive auricular therapy or self-acupressure stimulated the acupoints each night an hour before retiring to bed. The duration of participant involvement was 5 weeks. Subjective sleep quality was measured at baseline and post-treatment using the Pittsburgh Sleep Quality Index (PSQI). The impact of treatment on concerns of importance to the participants themselves was measured using the Measure Yourself Concerns and Wellbeing (MYCaW). Each participant also completed a treatment log book.
Results: All participants completed their treatment. All auricular therapy and self-acupressure participants recorded clinically significant improvements in global PSQI scores. In the auricular therapy arm mean global PSQI reduced from 12.5 at baseline to 8 following completion of treatment. In the self-acupressure arm PSQI reduced from 15 to 11. While in the no treatment arm the mean PSQI score was 14.5 at both baseline and follow up.
Conclusions: Despite the limited sample size, both auricular therapy and self-acupressure may represent potentially effective treatments for cancer patients with insomnia. The positive findings suggest further research is warranted into both treatment modalities.

Introdução: A fibrilhação auricular é uma arritmia comum e com risco tromboembólico bem documentado, estando definidas nas recomendações internacionais indicações referentes ao uso de anticoagulantes orais. Existem, contudo, lacunas de informação nomeadamente no que se refere à duração dos episódios de fibrilhação auricular e sua relação com o risco de tromboembolismo. Esta questão tem particular interesse em doentes com dispositivos electrónicos cardíacos implantados com documentação contínua da duração de episódios de taquidisritmias auriculares, que são frequentemente curtos e assintomáticos. Material e Métodos: Foi feita uma análise crítica da evidência disponível sobre a relação da duração dos episódios de FA paroxística e a ocorrência de eventos embólicos, com base numa pesquisa na base de dados bibliográfica PubMed. Resultados: Foram selecionados oito artigos com abordagens diferentes no estudo deste tema; sete com recurso a monitorização cardíaca com dispositivos electrónicos cardíacos implantados (pacemakers, cardioversores-desfibilhadores implantáveis e ressincronizadores cardíacos) e um com base em registo de Holter. Metade destas publicações, correspondendo globalmente às maiores amostragens, aborda a questão do ponto de vista do somatório diário de episódios de fibrilhação auricular (carga diária) e não da duração de cada episódio. O risco tromboembólico aumenta gradualmente com a carga arrítmica, tendo sido demonstrado um aumento significativo do risco quando esta ultrapassa os cinco minutos num dia. Discussão: A formação de um trombo intracavitário, e consequente potencial embólico, é um processo dinâmico que resulta da interacção de várias condicionantes anatómicas e funcionais. O risco individual dependerá da interacção destes factores. A associação entre fenómenos embólicos e curtos períodos de fibrilhação auricular é inequívoca, apesar do mecanismo não ser óbvio, tendo em conta a discrepância frequentemente observada entre os períodos de fibrilhação auricular e os eventos clínicos. Conclusões: O risco de eventos tromboembólicos aumenta significativamente mesmo para períodos curtos de fibrilhação auricular (≥ cinco minutos de fibrilhação auricular em um dia), apesar da relação causa-efeito não estar definida. A decisão final sobre o recurso à anticoagulação oral deve basear-se na avaliação clínica individualizada.

A Fibrilhação Auricular é uma alteração do ritmo cardíaco designada por arritmia. Esta patologia é considerada a forma de arritmia mais frequentemente observada na prática clínica e que constitui uma importante causa de morbilidade pelo risco inerente de desenvolvimento de AVC. Em 2010 foi realizado um estudo epidemiológico na população Portuguesa com o objectivo determinar a prevalência de Fibrilhação Auricular na população portuguesa com idade igual ou superior a 40 anos, sob o acrónimo de FAMA. Os dados publicados indicaram uma estimativa de prevalência de 2,5%, com um aumento da prevalência em função da classe etária. A nível regional não foram observadas diferenças na taxa de prevalência. Estudos de mapeamento de doenças mostraram que a determinação de taxas de prevalência por região, quando o número de casos observados é relativamente baixo, apresentam sobredispersão e, consequentemente, uma falta de precisão nas estimativas obtidas através um método frequencista clássico. A utilização de modelos Bayesianos hierárquicos no mapeamento de doenças tem apresentado vantagem na estimação de valores de risco da doença comparativamente à abordagem clássica. Assim, é objectivo deste trabalho determinar a prevalência de Fibrilhação Auricular na população Portuguesa por região, ao nível da NUTS III, usando modelos hierárquicos Bayesianos. Os dados utilizados neste estudo são os dados referentes ao estudo FAMA, pós-estratificados para correcção dos ponderadores. O modelo Bayesiano proposto por Besag, York e Mollié (1991) foi usado para modelar os dados, covariando para a idade e índice de massa corporal. A revisão do desenho do estudo e o recálculo dos ponderadores foi realizado com recurso ao software R, survey, e a análise Bayesiana efectuada através do WinBugs. iii Os resultados deste estudo demonstram que o uso de modelos Bayesianos são uma melhor opção para a estimação de valores risco relativo e prevalência da doença. Contudo, a utilização de covariáveis não resultou numa melhoria considerável ao contrário do que seria esperado. Conclui-se que a Fibrilhação Auricular apresenta variações regionais significativas, a nível de NUTS III, que não devem ser desvalorizadas na determinação de políticas de saúde pública para controle da doença.

In recent years research explored different acupuncture stimulation techniques but interest has focused primarily on somatic acupuncture and on a limited number of acupoints. As regards ear Acupuncture (EA) there is still some criticism about the clinical specificity of auricular points/areas representing organs or structures of the body. The aim of this study was to verify through (Functional magnetic resonance imaging) fMRI the hypothesis of EA point specificity using two auricular points having different topographical locations and clinical significance. Six healthy volunteers underwent two experimental fMRI sessions: the first was dedicated to the stimulation of Thumb Auricular Acupoint (TAA) and the second to the stimulation of Brain Stem Auricular Acupoint (BSAA). The stimulation of the needle placed in the TAA of the left ear produced an increase in activation bilaterally in the parietal operculum, region of the secondary somatosensory area SII. Stimulation of the needle placed in the BSAA of the left ear showed a pattern that largely overlapped regions belonging to the pain matrix, as shown to be involved in previous somatic acupuncture studies but with local differences in the left amygdala, anterior cingulate cortex, and cerebellum. The differences in activation patterns between TAA and BSAA stimulation support the specificity of the two acupoints. Moreover, the peculiarity of the regions involved in BSAA stimulation compared to those involved in the pain matrix, is in accordance with the therapeutic indications of this acupoint that include head pain, dizziness and vertigo. Our results provide preliminary evidence on the specificity of two auricular acupoints; further research is warranted by means of fMRI both in healthy volunteers and in patients carrying neurological/psychiatric syndromes.

Implants in craniofacial reconstructions improve prostheses retention and stability, comfort, and safety for the patient. According to biomechanical principles, the treatment success regarding osseointegration maintenance depends on an adequate surgery technique associated to a retention system that provides favorable tension distribution to implants. Implants in the mastoid area are a very important aid for retention of auricular prostheses. Color stability of resin and silicone is an important factor for longevity of auricular prostheses, and the high degree of satisfaction of patients with head and neck defects receiving epithesial reconstruction in the maxillofacial region is demonstrated.

Presently, acupuncture is a technique considered to be capable of stimulating the regulatory systems of the organism, such as the central nervous system, the endocrine system and the immunological system. The median frequency of the upper trapezium muscle with 40% and 60% of maximal voluntary contraction (MVC) of 15 healthy volunteers, was analyzed after the individuals were submitted to the AA treatment. The non-parametric Friedman test was used to compare median frequency values. In this exploratory study, the level of significance of each comparison was set to p < 0.05. The intraclass analyses indicate a significant increase of the median frequency muscle at 60% of the MVC (Wicoxon test). Based on the results found, the AA peripheral stimulus can act as a modulator mechanism of muscle activity and was possible to verify correspondence of the auricular acupoint with the trapezius muscle. © 2009 Elsevier Ltd. The advancement of knowledge in neurophysiology has demonstrated that acupuncture is a method of peripheral neural stimulation that promotes local and systemic reflexive responses. The purpose of this study was to determine if surface electromyography can be used as a tool to study the action of auricular acupuncture on striated skeletal muscle. The electromyographic amplitudes of the anterior, middle and posterior deltoid muscle and the upper trapezium muscle with 20%, 40% and 60% of maximal voluntary contraction of 15 healthy volunteers, were analyzed after the individuals were submitted to the auricular acupuncture treatment. The non-parametric Friedman test was used to compare Root Mean Square values estimated by using a 200 ms moving window. Significant results were further analyzed using the Wilcoxon signed rank test. In this exploratory study, the level of significance of each comparison was set to p < 0.05. It was concluded in this study that a surface electromyography can be used as a tool to investigate possible alterations of electrical activity in muscles after auricular acupuncture. However there is still a lack of adequate methodology for its use in this type of study, being that the method used to record the electromyographic signal can also influence the results. © 2008 Elsevier Ltd. Among the harmful health agents, it is given to the noise, one of the most present in urban and social environments. There are efficient measures to control the noise and the auricular Individual Protection Equipment (IPEa) is the most used. It has been observed the shortage in the comfortable and functional individual auditory protection products, related to durability, face to the efficiency right of the equipment. The main objective of this study was to recognize if there is effectiveness loss toward the used time on the Auditory Equipment, intra-auricular Individual Protection, of foam (3M), model 1110. It has been verified that the equipments suffer efficiency alteration according to the time of use, and the material consuming initiates reliable loss, mainly in audible frequencies going up to 100Hz, and from 16 days for sonorous pressure suffered started in 101dB 20Hz. For sonorous pressure suffered from 107dB to 20Hz, the reliable loss for the usage was of 8 days. The IPEas do not lose effectiveness on important attenuation by the daily use in frequencies from 1KHz until 20KHz. It has been concluded that there is effectiveness loss, according to the daily and frequent use of the IPEa studied.
