OBJETIVO: Apesar da importância das causas externas como problema de saúde pública, pouco se conhece sobre a demanda de serviços de urgência e emergência. Este estudo tem como objetivo caracterizar a morbidade por causas externas em unidades de urgência e emergência do município de Cuiabá/MT. MÉTODO: Trata-se de um estudo transversal e descritivo. Foram analisadas 3.786 vítimas de causas externas atendidas pelas cinco unidades de urgência e emergência da Secretaria Municipal de Saúde de Cuiabá/MT, no período de 1 de maio a 30 de junho de 2005. RESULTADOS: Aproximadamente 88% dos atendimentos se referiam a vítimas de acidentes, 9% corresponderam a agressões e 2% a lesões autoprovocadas. Os acidentes de transportes representaram 22% dos atendimentos, sendo os motociclistas as principais vítimas (49%); as quedas foram as causas mais freqüentes no grupo de outras causas externas de traumatismos acidentais. A prevalência em homens superou a de mulheres. A maior parte das vítimas era menor de 40 anos (79%). No entanto, a análise por tipo de causa externa apresenta resultados diferentes segundo sexo e faixa etária. Cerca da metade dos eventos ocorreu em casa e, em sua maioria, as vítimas receberam alta após o atendimento, sendo que a taxa de mortalidade foi baixa (0,4%). CONCLUSÃO: Os resultados revelam a importância da análise sistemática dos dados referentes às vítimas de acidentes e violência atendidas em unidades de urgência e emergência, como complemento às informações sobre mortalidade e morbidade hospitalar visando o monitoramento dessas causas.

O estudo tem como ponto de partida a hipótese de que determinadas ocorrências nosológicas atendidas em pronto-socorro, escolhidas com base no conceito de evento sentinela, podem estar relacionadas a falhas da atenção básica e ser utilizadas na formulação de um indicador para o monitoramento desta atenção. Foram utilizados dados quantitativos e qualitativos sobre ocorrências previamente escolhidas e atendidas em pronto-socorro. Usando-se a triangulação de métodos, verificaram-se as diferenças das freqüências dessas ocorrências entre as áreas/unidades básicas de saúde (UBS) de procedência dos pacientes e as motivações determinantes da procura pelo pronto-socorro. As freqüências das ocorrências apresentaram valores de 30% a 42,8% conforme a área/UBS de procedência (Ç2 = 9,19 e p = 0,027). As entrevistas sugeriram a existência de causalidade entre o motivo declarado da procura do pronto-socorro e a atuação das unidades básicas. Conclui-se que: (1) a freqüência das ocorrências escolhidas foi influenciada pelas áreas/UBS de procedência das pessoas; (2) essa influência decorre, em parte, da situação da atenção básica; (3) o instrumental estudado é simples e pode contribuir para o gestor local no acompanhamento cotidiano da situação dos serviços básicos.

Coronary heart disease is a leading cause of death in Australia with the Coalfields district of New South Wales having one of the country's highest rates. Identification of the Coalfields epidemic in the 1970's led to the formation of a community awareness program in the late 1980's (the healthy heart support group) followed by a more intense community action program in 1990, the Coalfields Healthy Heartbeat (CHHB). CHHB is a coalition of community members, local government officers, health workers and University researchers. We evaluate the CHHB program, examining both the nature and sustainability of heart health activities undertaken, as well as trends in risk factor levels and rates of coronary events in the Coalfields in comparison with nearby local government areas. Process data reveal difficulties mobilising the community as a whole; activities had to be selected for interested subgroups such as families of heart disease patients, school children, retired people and women concerned with family nutrition and body maintenance. Outcome data show a significantly larger reduction in case fatality for Coalfields men (although nonfatal heart attacks did not decline) while changes in risk factors levels were comparable with surrounding areas. We explain positive responses to the CHHB by schools, heart attack survivors and women interested in body maintenance in terms of the meaning these subgroups find in health promotion discourses based on their embodied experiences. When faced with a threat to one's identity, health discourse suddenly becomes meaningful along with the regimens for health improvement. General public disinterest in heart health promotion is examined in the context of historical patterns of outsiders criticising the lifestyle of miners, an orientation toward communal lather than individual responsibility for health (i.e, community 'owned' emergency services and hospitals) and anger about risks from environmental hazards imposed by industrialists. Background: Duration of untreated psychosis (DUP) depends on several factors, including socio-demographic, socioeconomic, clinical and contextual circumstances, such as availability of mental health services. Living arrangements may also play a role, especially in low- and middle-income countries, where most people who develop psychosis live with their relatives. Methods: Population-based study of first-episode psychosis in Sao Paulo, Brazil. Participants were aged 18-64 years, lived in a defined geographic area of the city and had a first contact in life with mental health services due to a psychotic episode. Duration of untreated psychosis was defined as the period between onset of first psychotic symptom and first contact with health service due to psychosis. The median DUP was used to classify participants into short and long DUP. Psychopathology, social adjustment and psychiatric diagnoses were made with standardized assessments. Type of service sought and living arrangements were examined. Results: Two hundred participants were included (52% women, 61% non-affective psychoses). The median DUP was 4.1 weeks (inter-quartile range: 1.9-11.4), and was shorter for affective psychoses. Most participants had their first contact with psychiatric emergency services. Those who did not live with a relative (children older than 18 years, parents, partner) were more likely to present long DUP (OR: 2.63; 95%Cl: 0.98-7.04); p = 0.05). Conclusion: The DUP in Sao Paulo was shorter than expected. Living arrangements may play an important role in shortening the DUP in urban centres of low- and middle income countries that have a network of mental health services. (C) 2009 Elsevier B.V. OBJECTIVE To describe the lack of access and continuity of health care in adults.METHODS A cross-sectional population-based study was performed on a sample of 12,402 adults aged 20 to 59 years in urban areas of 100 municipalities of 23 states in the five Brazilian geopolitical regions. Barriers to the access and continuity of health care and were investigated based on receiving, needing and seeking health care (hospitalization and accident/emergency care in the last 12 months; care provided by a doctor, by other health professional or home care in the last three months). Based on the results obtained by the description of the sample, a projection is provided for adults living in Brazilian urban areas.RESULTS The highest prevalence of lack of access to health services and to provision of care by health professionals was for hospitalization (3.0%), whilst the lowest prevalence was for care provided by a doctor (1.1%). The lack of access to care provided by other health professionals was 2.0%; to accident and emergency services, 2.1%; and to home care, 2.9%. As for prevalences, the greatest absolute lack of access occurred in emergency care (more than 360,000 adults). The main reasons were structural and organizational problems, such as unavailability of hospital beds, of health professionals, of appointments for the type of care needed and charges made for care.CONCLUSIONS The universal right to health care in Brazil has not yet been achieved. These projections can help health care management in scaling the efforts needed to overcome this problem, such as expanding the infrastructure of health services and the workforce.

RESUMO - As mudanças na saúde são cada vez mais rápidas e os serviços de saúde têm cada vez mais dificuldade em dar resposta aos problemas de saúde dos portugueses. Responsáveis por grande parte da despesa em saúde, os idosos são a população que mais utiliza os serviços de saúde e as respetivas unidades hospitalares e serviços de urgência. Estes têm estadias mais prolongadas e consomem mais recursos durante essas permanências nas instituições de saúde. Sabendo isto revelou-se oportuno encontrar as principais causas de internamento hospitalar, os principais diagnósticos secundários, demoras médias e a sua relação com as principais causas de morte na população portuguesa com mais de 65 anos no período de 2003-2012. Para tal, optou-se por uma análise descritiva de 3375817 episódios de internamento referentes a dez anos. Daqui retirou-se que os diagnósticos principais mais frequentes para todos os anos e todas as faixas etárias são o acidente vascular cerebral isquémico e a pneumonia, sendo que o primeiro é o mais frequente até 2006, passando depois a ser a pneumonia o mais frequente. A demora média é maior quanto mais diagnósticos secundários associados houver e aumenta com a idade. Os diagnósticos secundários mais frequentes são a hipertensão essencial e a diabetes mellitus. Estes dados são relevantes para o conhecimento da saúde em Portugal, podendo-se alterar e uniformizar e melhorar práticas hospitalares e com isso progredir na qualidade dos tratamentos e aumentar a qualidade de vida com hipótese de diminuição da demora média.

El projecte exposat té com a propòsit definir i implementar un model de simulació basat en la coordinació i assignació dels serveis d’emergència en accidents de trànsit. La definició del model s’ha realitzat amb l’ús de les Xarxes de Petri Acolorides i la implementació amb el software Rockwell Arena 7.0. El modelatge de la primera simulació ens mostra un model teòric basat en cues mentre que el segon, mostra un model més complet i real gràcies a la connexió mitjançant la plataforma Corba a una base de dades amb informació geogràfica de les flotes i de les rutes. Com a resultat de l’estudi i amb l’ajuda de GoogleEarth, podem realitzar simulacions gràfiques per veure els accidents generats, les flotes dels serveis i el moviment dels vehicles des de les bases fins als accidents.

Introduction: Rotenone is a botanical pesticide derived from extracts of Derris roots, which is traditionally used as piscicide, but also as an industrial insecticide for home gardens. Its mechanism of action is potent inhibition of mitochondrial respiratory chain by uncoupling oxidative phosphorylation by blocking electron transport at complex-I. Despite its classification as mild to moderately toxic to humans (estimated LD50, 300-500 mg/kg), there is a striking variety of acute toxicity of rotenone depending on the formulation (solvents). Human fatalities with rotenone-containing insecticides have been rarely reported, and a rapid deterioration within a few hours of the ingestion has been described previously in one case. Case report: A 49-year-old Tamil man with a history of asthma, ingested 250 mL of an insecticide containing 1.24% of rotenone (3.125 g, 52.1-62.5 mg/kg) in a suicide attempt at home. The product was not labeled as toxic. One hour later, he vomited repeatedly and emergency services were alerted. He was found unconscious with irregular respiration and was intubated. On arrival at the emergency department, he was comatose (GCS 3) with fixed and dilated pupils, and absent corneal reflexes. Physical examination revealed hemodynamic instability with hypotension (55/30 mmHg) and bradycardia (52 bpm). Significant laboratory findings were lactic acidosis (pH 6.97, lactate 17 mmol/L) and hypokalemia (2 mmol/L). Cranial computed tomography (CT) showed early cerebral edema. A single dose of activated charcoal was given. Intravenous hydration, ephedrine, repeated boli of dobutamine, and a perfusor with 90 micrograms/h norepinephine stabilized blood pressure temporarily. Atropine had a minimal effect on heart rate (58 bpm). Intravenous lipid emulsion was considered (log Pow 4.1), but there was a rapid deterioration with refractory hypotension and acute circulatory failure. The patient died 5h after ingestion of the insecticide. No autopsy was performed. Quantitative analysis of serum performed by high-resolution/accurate mass-mass spectrometry and liquid chromatography (LC-HR/AM-MS): 560 ng/mL rotenone. Other substances were excluded by gas chromatography-mass spectrometry (GC-MS) and liquid chromatography-mass spectrometry (LC-MS/MS). Conclusion: The clinical course was characterized by early severe symptoms and a rapidly fatal evolution, compatible with inhibition of mitochondrial energy supply. Although rotenone is classified as mild to moderately toxic, physicians must be aware that suicidal ingestion of emulsified concentrates may be rapidly fatal. (n=3): stridor, cyanosis, cough (one each). Local swelling after chewing or swallowing soap developed at the earliest after 20 minutes and persisted beyond 24 hours in some cases. Treatment with antihistamines and/or steroids relieved the symptoms in 9 cases. Conclusion: Bar soap ingestion by seniors carries a risk of severe local reactions. Half the patients developed symptoms, predominantly swellings of tongue and/or lips (38%). Cognitive impairment, particularly in the cases of dementia (37%), may increase the risk of unintentional ingestion. Chewing and intraoral retention of soap leads to prolonged contact with the mucosal membranes. Age-associated physiological changes of oral mucosa probably promote the irritant effects of the surfactants. Medical treatment with antihistamines and corticosteroids usually leads to rapid decline of symptoms. Without treatment, there may be a risk of airway obstruction.

L'objectiu del projecte és desenvolupar un Sistema d'Informació Geogràfica (SIG) d'ús senzill, amb la finalitat de generar cartografia actualitzada de manera gairebé instantània que serveixi de recolzament a l'ajuda internacional desplegada en un país després d'una catàstrofe natural. La cartografia serà d'Haití, com a exemple de zona afectada per un terratrèmol. Amb el SIG els equips d'emergència podran indicar aquelles vies tallades, per poder tenir cartografia actualitzada. Igualment podran indicar un punt origen i un punt destí i el SIG els indicarà la ruta més curta, tenint en compte les carreteres tallades. El programari utilitzat per desenvolupar el projecte és GeoMedia Professional, Microsoft Access i Microsoft Visual Studio .NET. S'ha generat cartografia d'Haití en format vectorial.

There are exceptional situations where emergency services are required Primary Care in the application of material used by drug-dependent patients, being the response to this demand is something that many of the cases, to individual discretion and the randomness and variability every situation leads to an answer. It calls for a response commensurate to public services and preventive health philosophy in most cases will be carried out by the nurse to perform assistance Devices Critical Care (DCCU), often this first contact these patients and slots at the supply of resources diminishes the possibilities of acquisition of such material to them. That is why, and in the absence in this area of patient safety and professional, a workflow model and according to the prevailing philosophy of working in primary care in terms of prevention policies and recruitment of patients concerned, this project raises guidance for the development of a needle exchange program from the triage consultations DCCU.
