In the fight against doping, steroid profiling is a powerful tool to detect drug misuse with endogenous anabolic androgenic steroids. To establish sensitive and reliable models, the factors influencing profiling should be recognised. We performed an extensive literature review of the multiple factors that could influence the quantitative levels and ratios of endogenous steroids in urine matrix. For a comprehensive and scientific evaluation of the urinary steroid profile, it is necessary to define the target analytes as well as testosterone metabolism. The two main confounding factors, that is, endogenous and exogenous factors, are detailed to show the complex process of quantifying the steroid profile within WADA-accredited laboratories. Technical aspects are also discussed as they could have a significant impact on the steroid profile, and thus the steroid module of the athlete biological passport (ABP). The different factors impacting the major components of the steroid profile must be understood to ensure scientifically sound interpretation through the Bayesian model of the ABP. Not only should the statistical data be considered but also the experts in the field must be consulted for successful implementation of the steroidal module.

La toxicomanie a des répercussions importantes tant pour les individus qui en souffrent que pour la société en général (MSSS, 2007). L’efficacité des traitements offerts aux toxicomanes est toutefois affaiblie par la difficulté à maintenir et à engager en traitement cette clientèle (Meier et al., 2006). Dans ce contexte, et puisque la qualité de l’alliance thérapeutique est associée aux résultats et à la persévérance en traitement (Martin et al., 2000; Meier et al., 2005a), l’alliance qui se développe entre le client et le thérapeute en cours de thérapie peut être vue comme une piste de solution prometteuse pour améliorer l’efficacité des traitements de la toxicomanie. La présente étude se propose d’identifier les facteurs associés à l’alliance thérapeutique développée par une clientèle toxicomane en traitement (n = 269). L’étude s’intéresse également à l’influence de doubles problématiques (toxico-justice, toxico-santé mentale) sur les relations existantes entre l’alliance et ces facteurs (effets modérateurs). Les liens entre l’alliance thérapeutique et différentes variables (santé mentale, consommation, satisfaction envers les services, motivation, soutien social, pressions légales) ont été examinés à l’aide d’analyses de régression hiérarchique. Les résultats indiquent que le profil de consommation évalué avant l’entrée en traitement covarie avec l’alliance thérapeutique. Les troubles cognitifs et la satisfaction envers les services sont les deux principales variables évaluées lors de la thérapie de groupe qui sont associées à l’alliance. Des effets modérateurs de la présence de problématiques associées à la toxicomanie (justice, santé mentale) sur la relation entre certaines variables indépendantes et l’alliance ont finalement été observés.

Objectives: The aim of the study was to analyze the impact of lifetime panic disorder (PD) diagnosis in a sample of patients with bipolar disorder type I (BPI), evaluating clinical and demographic variables. Methods: Ninety-five outpatients from the Bipolar Disorder Research Program at the Institute of Psychiatry of the University of Sao Paulo Medical School were enrolled. Twenty-seven BPI patients with PD were compared to 68 BPI patients without any anxiety disorders regarding clinical and demographic variables. Results: Compared to BPI patients without any anxiety disorders, patients with BPI + PD presented significantly higher number of mood episodes (18.9 +/- 13.8 vs 8.5 +/- 7.8; P < .001), depressive episodes (10.8 +/- 8.2 vs 4.6 +/- 4,8; P = .001), and manic episodes (7.4 +/- 7.3 vs 3.6 +/- 3.6; P = .008). Patients with BPI + PD had more frequently a depressive episode as their first one compared to BPI patients without anxiety disorders (94.1% vs 57.5%; P = .011). Patients with BPI + PD had more comorbidity with lifetime diagnosis of drug abuse or dependence (33.3% vs 8.8%; P = .010) and eating disorders (29.6% vs 6.0%; P = .004). Conclusions: The higher number of mood episodes in general presented by patients with BPI + PD when compared with BPI patients without any anxiety disorders, along with the higher frequencies of drug misuse and eating disorders, indicates that PD comorbidity is associated with a poorer Course and outcome of BPI. The higher frequency of depression as the onset mood episode and the higher number of manic episodes in the group with PD may have important treatment implications and should be further investigated. Introduction: Risk-taking behaviors, family criminality, poverty, and poor parenting have been frequently associated with an earlier onset of criminal activities and a longer criminal career among male convicts. Objective: This study aims to identify factors related to the onset and recurrence of criminal behavior among female robbers in the State of Sao Paulo - Brazil. Method: It was a cross-sectional study carried out inside a feminine penitentiary in Sao Paulo. From June 2006 to June 2010, 175 inmates convicted only for robbery were recruited to be evaluated about family antecedents of criminal conviction, alcohol and drug misuse, impulsiveness, depressive symptoms, and psychosocial features. Results: Having family antecedents of criminal conviction consistently predicted an earlier onset of criminal activities and a longer criminal career among female robbers. Drug use in youth and the severity of drug misuse were significantly related to the initiation and recurrence of criminal behavior, respectively. Discussion: Prisons must systematically screen detainees and provide treatments for those with health problems in general. Children of inmates should obtain help to modify the negative consequences of their parents' incarceration in order to mitigate the negative consequences of pursuing this 'static' factor.

Individualized measures are relevant for the assessment of therapeutic results and allow patients to identify the aspects that they value the most in assessing their clinical improvement. PSYCHLOPS, as an individualized measure, invite the patient to create their own items. The items created by the patients were compared with the contents of standardized measures. A sample composed by 107 patients admitted for psychological treatment in Hospital Espírito Santo (Évora), and three institutions for drug misuse fill in the PSYCHLOPS and two standardized measures presented in a random order: CORE-OM and PHQ-9. 279 items were created in PSYCHLOPS and later recoded, by thematic analysis, in 51 subthemes. Work-related problems was the most common subtheme identified by patients as relevant to the clinical improvement assessment. From the 51 subthemes, 17 (33.3%) were not represented in CORE-OM and 43 (84.3%) were not represented in PHQ-9. The majority of our sample indicated at least one subtheme that was not represented in CORE-OM and PHQ-9. These results show us the importance of individualized measures in identifying the patients’ most value concerns, which may have implications in the therapeutic process.

This paper examines variations in suicide in the eight Health Boards of the Republic of Ireland for the years 1976 to 1995. It is found that while all have experienced a rise in male suicide, it has been much less pronounced in the Eastern Health Board which is somewhat surprising when one considers Dublin’s much-publicised problems with homelessness and hard drug misuse. Since the mid-eighties, female rates have been somewhat higher in the southern half of the country, comprising the Southern, Mid-Western and South-Eastern Health Boards. This variation may reflect a difficulty with contacting services for psychological distress in rural areas, either because of stigma or simple practical problems associated with transport. The development of appropriate services, especially in rural areas, should be at the top of the agenda of any Resource Officer to be appointed subsequent to the Final Report of the Task Force.

La conciliación de medicamentos es la adecuada combinación de conocimientos y evidencias científicas de las reacciones, interacciones y necesidades de los pacientes, constituye en esencial el buen uso de los medicamentos. Objetivo general: Establecer la conciliación de medicamentos e identificar los tipos de discrepancias existentes al ingreso, durante la hospitalización y al alta en las pacientes del área de ginecología del Hospital Vicente Corral Moscoso. Cuenca, durante los meses noviembre – diciembre 2015. Metodología: Se diseñó un estudio descriptivo, con un población de 200 pacientes hospitalizadas en el área de ginecología del Hospital Vicente Corral Moscoso, durante 2 meses del 2015, recolectamos los datos mediante un formulario de dos etapas para la conciliación, a partir de las prescripciones de la historia clínica y entrevista a las pacientes, los que fueron ingresados en el software SPSS 15.0 para su tabulación, análisis, y presentación en tablas. Resultados: Se encontró 161 errores de conciliación y 42 discrepancias justificadas, en promedio 1,87discrepancias no justificadas por paciente. El error de conciliación más frecuente al ingreso corresponde a diferente dosis, vía y frecuencia de administración con un 84,6%, durante la hospitalización y al alta, correspondió a prescripciones incompletas con el 40% y 60,3% respectivamente. Conclusiones: La frecuencia con la que se realiza la conciliación de medicamentos en el Hospital Vicente Corral Moscoso fue del 15%. El 52% de pacientes están expuestos a riesgo por discordancias en las prescripciones, de ellos 43% son errores en la conciliación y un 9 % son discordancias justificadas
