INTRODUCTION: Late presentation to HIV care leads to increased morbidity and mortality. We explored risk factors and reasons for late HIV testing and presentation to care in the nationally representative Swiss HIV Cohort Study (SHCS). METHODS: Adult patients enrolled in the SHCS between July 2009 and June 2012 were included. An initial CD4 count <350 cells/µl or an AIDS-defining illness defined late presentation. Demographic and behavioural characteristics of late presenters (LPs) were compared with those of non-late presenters (NLPs). Information on self-reported, individual barriers to HIV testing and care were obtained during face-to-face interviews. RESULTS: Of 1366 patients included, 680 (49.8%) were LPs. Seventy-two percent of eligible patients took part in the survey. LPs were more likely to be female (p<0.001) or from sub-Saharan Africa (p<0.001) and less likely to be highly educated (p=0.002) or men who have sex with men (p<0.001). LPs were more likely to have their first HIV test following a doctor's suggestion (p=0.01), and NLPs in the context of a regular check-up (p=0.02) or after a specific risk situation (p<0.001). The main reasons for late HIV testing were "did not feel at risk" (72%), "did not feel ill" (65%) and "did not know the symptoms of HIV" (51%). Seventy-one percent of the participants were symptomatic during the year preceding HIV diagnosis and the majority consulted a physician for these symptoms. CONCLUSIONS: In Switzerland, late presentation to care is driven by late HIV testing due to low risk perception and lack of awareness about HIV. Tailored HIV testing strategies and enhanced provider-initiated testing are urgently needed.

The delivery of library instruction to students in those areas of the sciences and the social sciences dealing with biology and the environment has a long history (Bowden & Di Benedetto 2001; Kutner 2000; Kutner & Danks 2007; Sapp 2006; Sinn 1998). Often these instruction sessions take the form of a one hour lecture or workshop at the start of a semester before the students have begun their projects or papers. This "one-shot" approach, though popular, has its limitations. It may not be offered at a time when the students will actually start making use of library resources, it may not be tied very specifically to a particular assignment, or it may be too general in nature to be of much use to students later on when they need to look at particular topics in much more depth than can be addressed in one such session. The following article describes another approach: the strategic use of drop-in clinics as a method of instruction in which the students themselves determine how the instruction proceeds.

Importancia: el paciente con fibrosis quística después de las complicaciones gastrointestinales y pulmonares debe enfrentar otras comorbilidades como la diabetes relacionada a su condición . Dado el aumento en la esperanza de vida y el hecho de que virtualmente todas los pacientes con esta enfermedad pueden desarrollar alteración en el metabolismo de los carbohidratos, se requiere una sensibilización frente al tema que posibilite una detección temprana de esta entidad y un tratamiento óptimo que evite las complicaciones microvasculares e impacte entre otros el crecimiento pondo-estatural en pacientes en desarrollo y la función pulmonar. Objetivo : realizar una revisión actualizada de la literatura sobre la diabetes relacionada a la fibrosis quística, destacando las indicaciones de tamización y tratamiento. Conclusión : la FQ dentro de su abordaje requiere la detección temprana de la alteración del metabolismo de los carbohidratos con una prueba de tolerancia a la glucosa , el daño del islote pancreático , la disfunción inmune, la resistencia a la insulina, el estrés oxidativo entre otros elementos fisiopatológicos conllevan a un estado de depleción de insulina que producirán un efecto negativo microvascular así como a una reducción marcada de la función pulmonar, mayores tasa de infección e incremento de la mortalidad. La piedra angular del tratamiento en pacientes con o sin hiperglicemia es la insulina que mejora tanto el estado nutricional como la función pulmonar ; nuevos antidiabéticos orales con efecto incretinas y fármacos modificadores de la enfermedad se vislumbran como alternativas al corto plazo

Breast cancer metastasis is a leading cause of death by malignancy in women worldwide. Efforts are being made to further characterize the rate-limiting steps of cancer metastasis, i.e. extravasation of circulating tumor cells and colonization of secondary organs. In this study, we investigated whether angiotensin II, a major vasoactive peptide both produced locally and released in the bloodstream, may trigger activating signals that contribute to cancer cell extravasation and metastasis. We used an experimental in vivo model of cancer metastasis in which bioluminescent breast tumor cells (D3H2LN) were injected intra-cardiacally into nude mice in order to recapitulate the late and essential steps of metastatic dissemination. Real-time intravital imaging studies revealed that angiotensin II accelerates the formation of metastatic foci at secondary sites. Pre-treatment of cancer cells with the peptide increases the number of mice with metastases, as well as the number and size of metastases per mouse. In vitro, angiotensin II contributes to each sequential step of cancer metastasis by promoting cancer cell adhesion to endothelial cells, trans-endothelial migration and tumor cell migration across extracellular matrix. At the molecular level, a total of 102 genes differentially expressed following angiotensin II pretreatment were identified by comparative DNA microarray. Angiotensin II regulates two groups of connected genes related to its precursor angiotensinogen. Among those, up-regulated MMP2/MMP9 and ICAM1 stand at the crossroad of a network of genes involved in cell adhesion, migration and invasion. Our data suggest that targeting angiotensin II production or action may represent a valuable therapeutic option to prevent metastatic progression of invasive breast tumors.

Cotrimoxazole reduces mortality in HIV-infected adults with tuberculosis (TB), and in vitro data suggest potential anti-mycobacterial activity of cotrimoxazole. We aimed to evaluate whether prophylaxis with cotrimoxazole is associated with a decreased risk of incident TB in SHCS participants. We determined the incidence of TB per 1000 person-years from January 1992 to December 2012. Rates were analyzed separately in participants with current or no previous antiretroviral treatment (ART) using Poisson regression adjusted for CD4 cell count, sex, region of origin, injecting drug use, and age. 13,431 cohort participants contributed 107,549 person-years follow-up; 182 patients had incident TB; 132 (73%) before and 50 (27%) after ART initiation. The multivariable incidence rate ratios for cumulative cotrimoxazole exposure per year for persons with no previous and current ART were 0.70 (95% CI 0.55-0.89) and 0.87 (0.74-1.0) respectively. Cotrimoxazole may prevent the development of TB among HIV-positive persons, especially among those with no previous ART.

PURPOSE The implementation of genomic-based medicine is hindered by unresolved questions regarding data privacy and delivery of interpreted results to health-care practitioners. We used DNA-based prediction of HIV-related outcomes as a model to explore critical issues in clinical genomics. METHODS We genotyped 4,149 markers in HIV-positive individuals. Variants allowed for prediction of 17 traits relevant to HIV medical care, inference of patient ancestry, and imputation of human leukocyte antigen (HLA) types. Genetic data were processed under a privacy-preserving framework using homomorphic encryption, and clinical reports describing potentially actionable results were delivered to health-care providers. RESULTS A total of 230 patients were included in the study. We demonstrated the feasibility of encrypting a large number of genetic markers, inferring patient ancestry, computing monogenic and polygenic trait risks, and reporting results under privacy-preserving conditions. The average execution time of a multimarker test on encrypted data was 865 ms on a standard computer. The proportion of tests returning potentially actionable genetic results ranged from 0 to 54%. CONCLUSIONS The model of implementation presented herein informs on strategies to deliver genomic test results for clinical care. Con el fin de conocer las últimas interpretaciones acerca de nuestro pasado regional, los videos de la serie "Mendoza, crónica de nuestra identidad", pretenden llevar adelante la tarea constante de construir y reconstruir una identidad que se mantenga abierta a los valores del pasado, pero también a la inevitable reformulación crítica de los mismos principios. La propuesta es generar un proceso de actualización y perfeccionamiento en el campo de la historia regional. En el documental LA LITERATURA DE MENDOZA, se describen las tendencias literarias en la provincia desde la época de la colonia a la actualidad. Los temas abordados son: - Los cronistas - El siglo XVIII - El Siglo XIX - El regionalismo literario - Vanguardia en Mendoza - Las últimas tendencias

Regiones y Departamentos. Relatos de nuestra identidad es una serie que aborda las características turísticas, culturales e históricas de la provincia de Mendoza. En los 14 programas se han destacado y revalorizado las particularidades de cada uno de los departamentos de esta provincia argentina que se dividen en distintas regiones: Valle de Uco, Gran Mendoza, Zona Sur, Zona Este y Zona Nordeste GRAN MENDOZA IV, es un documental que resalta las particularidades de los departamentos de Godoy Cruz, Guaymallén y Las Heras. En el bloque 1 conoceremos la historia del departamento de Las Heras y cómo se dividió la sociedad después del terromoto de 1861. En el bloque 2 reconoceremos las características sociales y económicas del departamento de Guaymallén así como sus principales figuras. En el bloque 3 reconoceremos la oferta turística y cultural que ofrece el departamento de Godoy Cruz, un lugar propio de bodegas y espacios para disfrutar. Finalmente el último bloque es un destacado de Armando Tejada Gomez por Mercedes Sosa.

Regiones y Departamentos. Relatos de nuestra identidad es una serie que aborda las características turísticas, culturales e históricas de la provincia de Mendoza. En los 14 programas se han destacado y revalorizado las particularidades de cada uno de los departamentos de esta provincia argentina que se dividen en distintas regiones: Valle de Uco, Gran Mendoza, Zona Sur, Zona Este y Zona Nordeste. ZONA SUR II, es un documental que recorre los departamentos de General Alvear, Malargüe y San Rafael rescatando sus características sociales, culturales y turísticas. El bloque 1destaca la historia de General Alvear la cual comienza con la llegada de los mapuches. Pueblo que además sufrió persecuciones, y exterminio como fue la Campaña del Desierto impulsada por Julio A. Roca. La historia de Malargue es también parte de este recorrido que culmina con el mito del tan conocido bandido social Juan Bautista Vairoleto. En el bloque 2 se analizan las características sociales y económicas del departamento de San Rafael, siendo una de las principales actividades el desarrollo agrícola. Finalmente en el bloque 3 se resaltan las particularidades turísticas que ofrece el sur mendocino y la cual es mundialmente reconocida.

Establecido como objeto de estudio para el presente trabajo un conjunto de obras incluidas dentro de lo que provisoriamente puede denominarse "género policial", surge como problemática la categoría misma de lo policial en su capacidad abarcadora de distintas manifestaciones que cada vez se hacen más complejas y diversas. Se plantea así como hipótesis de trabajo la posiblidad de llegar a una redefinición del género en función de un creciente énfasis puesto en el papel del lector como co-creador de las ficciones detectivescas. Ello impone una revisión de las invariantes del género, como así también de las distintas actitudes o "modelos genológicos" con que los autores argentinos han asumido su pertenencia a una tradición determinada. Igualmente, dentro de ese corpus narrativo que se reconoce por la presencia de una trama centrada en un enigma a resolver por distintas vías, se pueden establecer algunas modalidades particulares; el policial paródico, el policial metafisico teológico, el policial psicológico, etc., ejemplificadas por la obra de Jorge Luis Borges, Leonardo Castellani, Antonio Di Benedetto y Marco Denevi, entre otros.
