BACKGROUND: Transanal endoscopic microsurgery may represent appropriate diagnostic and therapeutic procedure in selected patients with distal rectal cancer following neoadjuvant chemoradiation. Even though this procedure has been associated with low rates of postoperative complications, patients undergoing neoadjuvant chemoradiation seem to be at increased risk for suture line dehiscence. In this setting, we compared the clinical outcomes of patients undergoing transanal endoscopic microsurgery with and without neoadjuvant chemoradiation. METHODS: Thirty-six consecutive patients were treated by transanal endoscopic microsurgery at a single institution. Twenty-three patients underwent local excision after neoadjuvant chemoradiation therapy for rectal adenocarcinoma, and 13 patients underwent local excision without any neoadjuvant treatment for benign and malignant rectal tumors. Chemoradiation therapy included 50.4 to 54Gy and 5-fluorouracil-based chemotherapy. All patients underwent transanal endoscopic microsurgery with primary closure of the rectal defect. Complications (immediate and late) and readmission rates were compared between groups. RESULTS: Overall, median hospital stay was 2 days. Immediate (30-d) complication rate was 44% for grade II/III complications. Patients undergoing neoadjuvant chemoradiation therapy were more likely to develop grade II/III immediate complications (56% vs 23%; P = .05). Overall, the 30-day readmission rate was 30%. Wound dehiscence was significantly more frequent among patients undergoing neoadjuvant chemoradiation therapy (70% vs 23%; P = .03). Patients undergoing neoadjuvant chemoradiation therapy were at significantly higher risk of requiring readmission (43% vs 7%; P = .02). CONCLUSION: Transanal local excision with the use of endoscopic microsurgical approach may result in significant postoperative morbidity, wound dehiscence, and readmission rates, in particular, because of rectal pain secondary to wound dehiscence. In this setting, the benefits of this minimally invasive approach either for diagnostic or therapeutic purposes become significantly restricted to highly selected patients that can potentially avoid a major operation but will still face a significantly morbid and painful procedure.

Background: Most cases of congenital clubfoot treated with the Ponseti technique require percutaneous Achilles tenotomy to correct the residual equinus. Clinical evidence suggests that complete healing occurs between the cut tendon stumps, but there have not yet been any detailed studies investigating this reparative process. This study was performed to assess Achilles tendon repair after percutaneous section to correct the residual equinus of clubfoot treated with the Ponseti method. Method: A prospective study analyzed 37 tenotomies in 26 patients with congenital clubfoot treated with the Ponseti technique, with a minimum follow-up of 1 year after the section. The tenotomy was performed percutaneously with a large-bore needle bevel with patient sedation and local anesthesia. Ultrasonographic scanning was performed after section to ascertain that the tenotomy had been completed and to measure the stump separation. In the follow-up period, the reparative process was followed ultrasonographically and assessed at 3 weeks, 6 months, and 1 year posttenotomy. Results: The ultrasonography performed immediately after the procedure showed that in some cases, residual strands between the tendon ends persisted, and these were completely sectioned under ultrasound control. A mean retraction of 5.65 mm +/- 2.26 mm (range, 2.3 to 11.0 mm) between tendon stumps after section was observed. Unusual bleeding occurred in one case and was controlled by digital pressure, with no interference with the final treatment. After 3 weeks, ultrasonography showed tendon repair with the tendon gap filled with irregular hypoechoic tissue, and also with transmission of muscle motion to the heel. Six months after tenotomy, there was structural filling with a fibrillar aspect, mild or moderate hypoechogenicity, and tendon scar thickening when compared with a normal tendon. One year after tenotomy, ultrasound showed a fibrillar structure and echogenicity at the repair site that was similar to a normal tendon, but with persistent tendon scarring thickness. Conclusions: There is a fast reparative process after Achilles tendon percutaneous section that reestablishes continuity between stumps. The reparative tissue evolved to tendon tissue with a normal ultrasonographic appearance except for mild thickening, suggesting a predominantly intrinsic repair mechanism.

In the present study we characterized titanium (Ti) surfaces submitted to different treatments and evaluated the response of osteoblasts derived from human alveolar bone to these surfaces. Five different surfaces were evaluated: ground (G), ground and chemical etched (G1-HF for 60 s), sand blasted (SB-Al2O3 particles 65 pm), sand blasted and chemical etched (SLA1-HF for 60 s and SLA2-HF for 13 s). Surface morphology was evaluated under SEM and roughness parameters by contact scanning instrument. The presence of Al2O3 was detected by EDS and the amount calculated by digital analyses. Osteoblasts, were cultured on these surfaces and it was evaluated: cell adhesion, proliferation, and viability, alkaline phosphatase activity, total protein content, and matrix mineralization formation. Physical and chemical treatments produced very different surface morphologies. Al2O3 residues were detected on SB and SLA2 surfaces. Only matrix mineralization formation was affected by different surface treatments, being increased on rough surface (SLA1) and reduced on surface with high amount of Al2O3 residues (SB). On the basis of these findings, it is possible to conclude that high concentration of residual Al2O3 negatively interfere with the process of matrix mineralization formation in contact with Ti implant surfaces. This paper examines why practitioners and researchers get different estimates of equity value when they use a discounted cash flow (CF) model versus a residual income (RI) model. Both models are derived from the same underlying assumption -- that price is the present value of expected future net dividends discounted at the cost of equity capital -- but in practice and in research they frequently yield different estimates. We argue that the research literature devoted to comparing the accuracy of these two models is misguided; properly implemented, both models yield identical valuations for all firms in all years. We identify how prior research has applied inconsistent assumptions to the two models and show how these seemingly small errors cause surprisingly large differences in the value estimates. The author investigated how training in small-group and interpersonal behaviors affected children's behavior and interactions as they worked in small groups 2 years later. The authors assigned 52 fifth graders, who had been trained 2 years previously in cooperative group behaviors, to the trained condition and 36 fifth graders, who had not previously been trained, to the untrained condition. Both were reconstituted from the pool of students who had participated previously in group activities. The results showed a residual training effect, with the children in the trained groups being more cooperative and helpful than their untrained peers.

For dynamic simulations to be credible, verification of the computer code must be an integral part of the modelling process. This two-part paper describes a novel approach to verification through program testing and debugging. In Part 1, a methodology is presented for detecting and isolating coding errors using back-to-back testing. Residuals are generated by comparing the output of two independent implementations, in response to identical inputs. The key feature of the methodology is that a specially modified observer is created using one of the implementations, so as to impose an error-dependent structure on these residuals. Each error can be associated with a fixed and known subspace, permitting errors to be isolated to specific equations in the code. It is shown that the geometric properties extend to multiple errors in either one of the two implementations. Copyright (C) 2003 John Wiley Sons, Ltd.

For the first time it was possible to observe regular quasiperiodic scintillations (QPS) in VHF radio-satellite transmissions from orbiting satellites simultaneously at short (2.1 km) and long (121 km) meridional baselines in the vicinity of a typical mid-latitude station (Brisbane; 27.5degreesS and 152.9degreesE geog. and 35.6degrees invar.lat.), using three sites (St. Lucia-S, Taringa-T in Brisbane and Boreen Pt.-B, north of Brisbane). A few pronounced quasiperiodic (QP) events were recorded showing unambiguous regular structures at the sites which made it possible to deduce a time displacement of the regular fading minimum at S, T and B. The QP structure is highly dependent on the geometry of the ray-path from a satellite to the observer which is manifested as a change of a QP event from symmetrical to non-symmetrical for stations separated by 2.1 km, and to a radical change in the structure of the event over a distance of 121 km. It is suggested the short-duration intense QP events are due to a Fresnel diffraction (or a reflection mechanism) of radio-satellite signals by a single ionospheric irregularity in a form of an ellipsoid with a large ionization gradient along the major axis. The structure of a QP event depends on the angle of viewing of the irregular blob from a radio-satellite. In view of this it is suggested that the reported variety of the ionization formation, responsible for different types of QPS, is only apparent but not real. (C) 2003 Elsevier Science Ltd. Visando a aplicação do aço UNS S32304 em embalados para transporte de material radioativo, amostras soldadas por processo TIG, com diferentes gases de proteção, foram submetidas a tratamentos térmicos nas temperaturas de 475°C, 600°C e 750°C por 8 horas, seguidas de resfriamento ao ar, a fim de analisar o efeito de temperaturas críticas no perfil de tensões residuais e microestrutura. A difratometria de raios X foi utilizada para determinação das tensões residuais, em diferentes condições (amostras como recebidas e apenas tratadas termicamente) e o perfil de tensões residuais total das amostras soldadas é apresentado para cada fase (austenita e ferrita). As tensões residuais das fases foram determinadas pela técnica sen2ψ, utilizando um difratômetro com fonte de radiação CuKα (<λ

A rotação de culturas é uma prática essencial, no cultivo de hortaliças, para controle de pragas e doenças e tem sido usada, também, visando ao aproveitamento dos resíduos de adubação. O presente trabalho teve por objetivo avaliar o efeito residual da adubação orgânica, aplicada na cultura do quiabo, sobre o rendimento de espigas de milho-verde em cultivo subsequente. O quiabeiro foi plantado com duas populações de plantas e adubado com biofertilizante líquido de suínos, nas doses: 0, 6, 12, 24, e 48 m³ ha-1. Após a retirada dos restos culturais do quiabeiro, sementes de milho híbrido AG 1051 e da variedade UFVM 100 foram semeadas, em linhas duplas, no espaçamento de 1,0 m x 0,40 m e 0,25 m entre plantas após o desbaste. O delineamento experimental foi em blocos casualizados com quatro repetições, no esquema fatorial 2 x 5, sendo dois cultivares de milho e resíduo de cinco doses de biofertilizante. O resíduo da adubação com biofertilizante de suínos em quiabeiro não foi suficiente para nutrir as plantas de milho-verde; consequentemente obteve-se baixa produtividade comercial. O plantio do milho-verde em sucessão ao quiabeiro, visando à rotação de culturas e aproveitamento de resíduos orgânicos, é promissor, sendo necessária adubação complementar de cobertura, para suprimento de N e K. O híbrido AG 1051 apresentou maior altura de plantas, número e produtividade de espigas comerciais despalhadas e peso médio de espigas comerciais. A variedade UFVM100 apresentou maior teor de P e K nas folhas, número de espigas com palha, número e produtividade de espigas não comerciais despalhadas.

Este trabalho foi desenvolvido em campo, com o objetivo de avaliar a atividade residual de herbicidas recomendados para a cultura da cana-de-açúcar, quando aplicados ao solo na época seca. Os tratamentos foram organizados em esquema de parcelas subdivididas, em que, no âmbito das parcelas, foram aplicados seis herbicidas e, no âmbito das subparcelas, foram considerados três períodos de permanência dos produtos no solo (130 dias, 70 dias e testemunha, sem aplicação). Os tratamentos herbicidas foram (g ha-1): amicarbazone a 1260; clomazone a 1250; hexazinone a 450; imazapic a 147; isoxaflutole a 187,5; e sulfentrazone a 900. Além da infestação natural de plantas daninhas, para avaliação da atividade residual dos produtos, semeou-se pepino na área, considerado como bioindicador. A mesma molécula herbicida teve avaliação diferenciada em razão da espécie bioindicadora considerada. Os herbicidas amicarbazone, hexazinone, isoxaflutole e sulfentrazone têm propriedades positivas para aplicação ao solo em época seca, tais como elevada solubilidade, baixo Kow e baixa fotodegradação. A aplicação de imazapic em pré-emergência tem eficácia consistente em época seca, controlando adequadamente o pepino e a comunidade natural de plantas daninhas. O herbicida clomazone não controlou adequadamente o pepino e a comunidade natural de plantas daninhas.
