Hallitsematon ja reaktiivinen kunnossapito on eräs tuotannon suurimpia kustannustekijöistä. Suunnitelmallisesti ja systemaattisesti johdettuna kunnossapito on tuotantotehokkuuden suurin vaikuttaja. Merkittävä osa tuotannon tehokkuuden ylläpidosta saavutetaan laitteiden käyttövarmuudella. Käyttövarmuuden saaminen hallintaan perustuu ennakoivan kunnossapidon määrän kasvattamiseen. Samalla korjaavan kunnossapidon kustannusriski laskee ja siihen käytetty panos vähenee. Huonolla kunnossapidon suunnitelmallisuudella on päinvastaiset vaikutukset. Tavoitteena on määritellä prosessilaitteiden käyttövarmuuksiin perustuva laitekriittisyys. Tutkimuksessa yhdistetään riskien arviointimenetelmiä, joilla keskimääräiset vikavälit ja seuraukset valmistukseen mallinnetaan. Kriittisyystekijöitä ovat käytettävyys, luotettavuus, kustannustekijät, turvallisuus ja ympäristövaikutukset. Tekijöiden arvottamiseen kehitettiin riksianalyysitaulukko. Kriittisyysluokat jaettiin kolmeen kategoriaan, joista A on kriittisin, B keskinkertainen ja C on matalin luokka. Lähtötietojen keräys toteutettiin triangulaatiomenetelmää soveltaen. Empiirisessä osassa HKScan Oy:n lihanjalostustehtaan jauheliha- ja kestomakkaraosastojen laitteet jaettiin A-, B- ja C-luokkiin. Kriittisimpiä laitteita oli 20 prosenttia analysoidusta laitemäärästä. Nämä A-luokkaan sijoitetut laitteet aiheuttavat 80 prosenttia kustannusriskeistä. B-luokkaan kuuluu 50 prosenttia ja C-luokkaan 30 prosenttia laitteista. Luokittelusta erotettiin havaitut turvallisuusriskit riskienhallinnan toimenpiteitä varten. Kustannustietoinen kriittisyysluokittelu on pohja kunnossapitostrategian rakentamiselle. Tämän avuksi esitettiin taulukot huolto-ohjelman luomiseen ja luokituksien hyödyntämiseen päivittäisessä toiminnassa.

A carbon reduction strategy for a historic Grade 1 listed office building in London is presented. The study evaluates the impact of49 different carbon abatement options, quantified using building simulation software, auditing procedures and qualitative methods. The impact of each option is assessed against three criteria: carbon abatement potential, practicality and cost. The strategy comprises of18interventions,integrated within 12 key recommendations. Accumulative reduction of 37% (below a 2009 carbon emissions baseline)appears achievable and only feasible with heavy reliance on changes in occupant behaviour. This theme appears central in achieving realistic and significant carbon savings from listed buildings, where planning constraints relinquish potential for major building fabric alteration and renewable energy installations.

Johne's disease in cattle is a contagious wasting disease caused by Mycobacterium avium subspecies paratuberculosis (MAP). Johne's infection is characterised by a long subclinical phase and can therefore go undetected for long periods of time during which substantial production losses can occur. The protracted nature of Johne's infection therefore presents a challenge for both veterinarians and farmers when discussing control options due to a paucity of information and limited test performance when screening for the disease. The objectives were to model Johne's control decisions in suckler beef cattle using a decision support approach, thus implying equal focus on ‘end user’ (veterinarian) participation whilst still focusing on the technical disease modelling aspects during the decision support model development. The model shows how Johne's disease is likely to affect a herd over time both in terms of physical and financial impacts. In addition, the model simulates the effect on production from two different Johne's control strategies; herd management measures and test and cull measures. The article also provides and discusses results from a sensitivity analysis to assess the effects on production from improving the currently available test performance. Output from running the model shows that a combination of management improvements to reduce routes of infection and testing and culling to remove infected and infectious animals is likely to be the least-cost control strategy.

Providing homeowners with real-time feedback on their electricity consumption through a dedicated display device has been shown to reduce consumption by approximately 6-10%. However, recent advances in smart grid technology have enabled larger sample sizes and more representative sample selection and recruitment methods for display trials. By analyzing these factors using data from current studies, this paper argues that a realistic, large-scale conservation effect from feedback is in the range of 3-5%. Subsequent analysis shows that providing real-time feedback may not be a cost effective strategy for reducing carbon emissions in Australia, but that it may enable additional benefits such as customer retention and peak-load shift.

A Reforma Gerencial do Estado levou as Universidades Federais de Ensino Superior a buscar tecnologia para modernizar a Gestão acadêmica aumentando a qualidade no atendimento. O Sistema Integrado de Ensino (SIE) foi implantado em 2006. Lança-se os seguintes problemas: Quais gargalos encontrados na implantação do SIE foram responsáveis pelas não implementações do estabelecido? Em que medida o modelo do SIE para gerenciamento administrativo dos assuntos acadêmicos contribuiu para a eficiência da gestão na UFPA, assim fornececendo suporte para o avanço do ensino e da pesquisa? O processo de implantação foi suficiente para dizer que estava dentro das exigências da GesPública? Para atender o objetivo Geral de analisar se os modelos de gestão praticados pelos gestores na implantação do SIE estão de acordo com o instrumento de aplicação do Plano Nacional de Gestão Pública e Desburocratização – GesPública (PBQP), o Plano Diretor Institucional e os Planos Anuais de Gestão da UFPA utilizou-se de entrevistas, questionários e de análise documental. As categorias liderança, estratégia e Plano, cidadãos e sociedade, processos e pessoas; resultados; informações e conhecimentos são da GesPública. As hipóteses validadas parcialmente dizem que a instituição avançou na utilização das Teorias Organizacionais nos aparatos administrativos e tecnológicos estando em consonância com o contexto da Reforma do Estado. As práticas de qualidade são adequadas, em início de implantação, com pontos que precisam ser melhorados. Deixam enormes espaços na modernização da gestão acadêmica. Os líderes deveriam ter inseridos os usuários no processo e trabalhado suas experiências e necessidades buscando resultados conforme prevê a Teoria Neo-Institucional e a Teoria da Qualidade que envolve as pessoas nas tomadas de decisões, pois são elas que lidam diretamente com os equipamentos e com os clientes.

In order to maintain the competitiveness the companies seek for cost reduction and resource optimization. A product or service price is calculated considering direct and indirect costs involved in its fabrication, which means that if an intern indirect process is optimized, the final price of the product is also reduced and becomes more interesting to the final costumer. Considering this reasoning, the companies must evaluate each of their processes and optimize them. It is common to outsource intern products manufacturing as a cost reduction strategy, nevertheless when there are already resources and capability to produce the own goods, it becomes necessary to evaluate the economic gain that each strategy - make or buy - generates. This paper aims to evaluate the necessary support to take a decision regarding making or buying in a production cell of hoses based in the value of economic cost, system cost and decision process

Background: The mechanisms by which humans regulate pro-and anti-inflammatory responses on exposure to different malaria parasites remains unclear. Although Plasmodium vivax usually causes a relatively benign disease, this parasite has been suggested to elicit more host inflammation per parasitized red blood cell than P. falciparum. Methodology/Principal Findings: We measured plasma concentrations of seven cytokines and two soluble tumor necrosis factor (TNF)-alpha receptors, and evaluated clinical and laboratory outcomes, in Brazilians with acute uncomplicated infections with P. vivax (n = 85), P. falciparum (n = 30), or both species (n = 12), and in 45 asymptomatic carriers of low-density P. vivax infection. Symptomatic vivax malaria patients, compared to those infected with P. falciparum or both species, had more intense paroxysms, but they had no clear association with a pro-inflammatory imbalance. To the contrary, these patients had higher levels of the regulatory cytokine interleukin (IL)-10, which correlated positively with parasite density, and elevated IL-10/TNF-alpha, IL-10/interferon (IFN)-gamma, IL-10/IL-6 and sTNFRII/TNF-alpha ratios, compared to falciparum or mixed-species malaria patient groups. Vivax malaria patients had the highest levels of circulating soluble TNF-alpha receptor sTNFRII. Levels of regulatory cytokines returned to normal values 28 days after P. vivax clearance following chemotherapy. Finally, asymptomatic carriers of low P. vivax parasitemias had substantially lower levels of both inflammatory and regulatory cytokines than did patients with clinical malaria due to either species. Conclusions: Controlling fast-multiplying P. falciparum blood stages requires a strong inflammatory response to prevent fulminant infections, while reducing inflammation-related tissue damage with early regulatory cytokine responses may be a more cost-effective strategy in infections with the less virulent P. vivax parasite. The early induction of regulatory cytokines may be a critical mechanism protecting vivax malaria patients from severe clinical complications.

Beef cow herd owners can benefit from incorporating price signals into their heifer retention decisions. Whereas a perfect forecast of calf prices over the productive life of the heifer added to the herd would be ideal, such information is not available. However, simple decision rules that incorporate current or recent prices and the knowledge that the cattle cycle likely will repeat itself can help producers improve their investment decisions. A dollar cost averaging strategy that retains the same dollar value of heifers each year and a rolling average value strategy that retains a 10-year average value of heifers out performed strategies that sought to maintain a constant herd size or a constant cash flow.

The study assessed the economic efficiency of different strategies for the control of post-weaning multi-systemic wasting syndrome (PMWS) and porcine circovirus type 2 subclinical infection (PCV2SI), which have a major economic impact on the pig farming industry worldwide. The control strategies investigated consisted on the combination of up to 5 different control measures. The control measures considered were: (1) PCV2 vaccination of piglets (vac); (2) ensuring age adjusted diet for growers (diets); (3) reduction of stocking density (stock); (4) improvement of biosecurity measures (bios); and (5) total depopulation and repopulation of the farm for the elimination of other major pathogens (DPRP). A model was developed to simulate 5 years production of a pig farm with a 3-weekly batch system and with 100 sows. A PMWS/PCV2SI disease and economic model, based on PMWS severity scores, was linked to the production model in order to assess disease losses. This PMWS severity scores depends on the combination post-weaning mortality, PMWS morbidity in younger pigs and proportion of PCV2 infected pigs observed on farms. The economic analysis investigated eleven different farm scenarios, depending on the number of risk factors present before the intervention. For each strategy, an investment appraisal assessed the extra costs and benefits of reducing a given PMWS severity score to the average score of a slightly affected farm. The net present value obtained for each strategy was then multiplied by the corresponding probability of success to obtain an expected value. A stochastic simulation was performed to account for uncertainty and variability. For moderately affected farms PCV2 vaccination alone was the most cost-efficient strategy, but for highly affected farms it was either PCV2 vaccination alone or in combination with biosecurity measures, with the marginal profitability between 'vac' and 'vac+bios' being small. Other strategies such as 'diets', 'vac+diets' and 'bios+diets' were frequently identified as the second or third best strategy. The mean expected values of the best strategy for a moderately and a highly affected farm were £14,739 and £57,648 after 5 years, respectively. This is the first study to compare economic efficiency of control strategies for PMWS and PCV2SI. The results demonstrate the economic value of PCV2 vaccination, and highlight that on highly affected farms biosecurity measures are required to achieve optimal profitability. The model developed has potential as a farm-level decision support tool for the control of this economically important syndrome.

The Personal Response System Program at Huffington Center on Aging, Baylor College of Medicine, provides emergency call systems for elderly people living independently in Houston, Texas. The goal of the project was to complete a formative evaluation of the Personal Response System Program. The specific aims of the evaluation were three-fold. One aim was to evaluate participant health status and level of disability. The second aim was to develop a health care cost estimation strategy. Finally, a preliminary cost-effectiveness analysis was completed to evaluate the program's impact on health care costs and health status among the elderly target population. ^ The evaluation was a longitudinal, randomized experimental design. After the screening of 120 volunteers for eligibility, clients were asked to complete a written questionnaire and a monthly health service contact diary. Volunteers were contacted by telephone interviewers to collect health status information from 100 eligible clients (83%) on three occasions during the six months of follow-up. ^ Initially, volunteers were randomized to two experimental groups. The two groups were found to be comparable at the beginning of the study. No significant differences were detected related to health status, level of disability, or history of physician visits at baseline. However, the group with the Personal Response System (PRS) device had more adverse health events, higher IADL scores, more frequent use of walkers, lower average health status scores, and fewer community volunteers hours than the usual care comparison group. ^ The health care costs were estimated based on an algorithm adapted from the American Medical Association guidelines. Average total health care costs for the group with the PRS device ($912) were greater than the usual care group ($464). However, median health care values for the PRS group ($263) were similar to the usual care comparison group ($234). The preliminary findings indicated that the use of the PRS device was not associated with health care cost savings. ^ In the preliminary cost-effectiveness analysis, use of the personal response system was found to be associated with increased mental health status among elderly clients. The cost-effectiveness evaluation indicated that the associated cost for six months was $710 per unit increase in mental component score when the $150 PRS subscription was included. However, clients with the PRS device were found to have a greater decline in physical health status during the six-month follow-up. The beneficial effect on mental health status was found to be in contrast to negative findings associated with changes in physical health status. The implications for future research relate to the need to identify risk factors among geriatric populations to better target groups that would most likely benefit from PRS Program enrollment. ^
