Aims: To estimate the prevalence and correlates of cognitive impairment (CI) among the elderly in a general hospital. Methods: A cross-sectional study, including 200 Brazilian inpatients aged 60 years or over, using the Mini Mental State Examination to evaluate CI (dependent variable), and the Geriatric Depression Scale and the Katz and Lawton Index to evaluate basic (BADL) and instrumental activities of daily living (IADL). Results: 56% were women, 29% were dependent for BADL and 77.5% for IADL. The prevalence of CI was 29% and, in the logistic regression, it remained associated with higher age (1 74 years old), number of previous hospitalizations (1 3) and dependency for BADL (being dependent raised the odds of being cognitively impaired). Conclusions: It is essential to train the hospital staff to properly assist these patients, and to orient and support their caregivers. Copyright (C) 2009 S. Karger AG, Basel

AIM: To assess the prevalence of mild cognitive impairment (MCI) in medical inpatients aged 55-85 years without known cognitive deficits, and how often ward physicians mentioned MCI in their discharge notes. Moreover, we aimed to identify variables associated with MCI and to assess the sensitivity and specificity of the Mini-Mental State Examination (MMSE) for MCI. METHODS: Two neuropsychologists administered a 60-min battery of validated tests to evaluate different cognitive domains. The diagnosis of MCI was based on a prespecified algorithm. The sensitivity and specificity of the MMSE for MCI were calculated. RESULTS: Fifteen patients showed a normal cognitive profile (21.4%), while 55 patients (78.6%) showed MCI. Ward physicians, blinded to the results of the neuropsychological evaluation, did not mention MCI in their discharge notes of any of the evaluated patients. The only variable independently associated with MCI was the MMSE. A MMSE score of < or =28 showed a sensitivity of 85.5% and a specificity of 66.7% for MCI. CONCLUSION: MCI is frequent albeit overlooked in elderly medical inpatients without previously known cognitive deficits. In view of therapies preventing the progression of MCI to dementia, MCI screening will be crucial. The MMSE represents a promising screening tool for MCI in medical inpatients.

Objectives- We investigated whether apoE genotypes correlate with cognitive functions in clinically healthy persons. Methods - In 1993 and 1995, we measured information processing speed, delayed free recall and semantic aspects of long-term memory in 227 men and 105 women aged 65 and over, a randomly selected subsample of the prospective Basel Study. Cardiovascular risk factors and education were assessed. Results -E2 were more prevalent in old-old (>75 years, 23.5% vs 15%) compared to E4 than in young-old (<75 years, 19.3% vs 23.5%). Taking into account age and education, subjects with ɛ3/ɛ4 or ɛ4/ɛ4 alleles (E4) performed lowest in all 3 tests compared to those homozygous for ɛ3 (E3) or carriers of one or two ɛ2 alleles (E2) (reaction time P=0.009, free recall P=0.05, WAIS-R vocabulary P<0.05). In old-old there was a significant difference between E2 and E4 for reaction time (P=0.02) and free recall (P<0.02) but not for vocabulary (P=0.086). In all 3 groups there were no significant changes after 2 years. The subgroup with the genotype ɛ2/ɛ4 performed consistently best in the cognitive tests. Cholesterol was significantly increased in the E4 and E3 group compared to the E2 group. Conclusion - ApoE genotype correlates with cognitive performance. The increased prevalence of E2 in the old-old and the significantly lower plasma cholesterol levels suggest differential morbidity and mortality as important factors influencing the prevalence of cognitive disorders in late life.

Descripción y evaluación de sistema de estimulación cognitiva a través de la TDT orientada a personas con enfermedad de Parkinson, con supervisión por parte de sus terapeutas de forma remota. Abstract: This paper details the full design, implementation, and validation of an e-health service in order to improve the community health care services for patients with cognitive disorders. Specifically, the new service allows Parkinson’s disease patients benefit from the possibility of doing cognitive stimulation therapy (CST) at home by using a familiar device such as a TV set. Its use instead of a PC could be a major advantage for some patients whose lack of familiarity with the use of a PC means that they can do therapy only in the presence of a therapist. For these patients this solution could bring about a great improvement in their autonomy. At the same time, this service provides therapists with the ability to conduct follow-up of therapy sessions via the web,benefiting from greater and easier control of the therapy exercises performed by patients and allowing them to customize new exercises in accordance with the particular needs of each patient. As a result, this kind of CST is considered to be a complement of other therapies oriented to the Parkinson patients. Furthermore, with small changes, the system could be useful for patients with a different cognitive disease such as Alzheimer’s or mild cognitive impairment.

Acquired Brain Injury (ABI) has become one of the most common causes of neurological disability in developed countries. Cognitive disorders result in a loss of independence and therefore patients? quality of life. Cognitive rehabilitation aims to promote patients? skills to achieve their highest degree of personal autonomy. New technologies such as interactive video, whereby real situations of daily living are reproduced within a controlled virtual environment, enable the design of personalized therapies with a high level of generalization and a great ecological validity. This paper presents a graphical tool that allows neuropsychologists to design, modify, and configure interactive video therapeutic activities, through the combination of graphic and natural language. The tool has been validated creating several Activities of Daily Living and a preliminary usability evaluation has been performed showing a good clinical acceptance in the definition of complex interactive video therapies for cognitive rehabilitation.

New technologies such as virtual reality or interactive video allow developing rehabilitation therapies based on reproducible Activities of Daily Living (ADLs), increasing the ecological validity of the therapy. However, the lack of frameworks to formalize and represent the definition of this kind of therapies can be a barrier for widespread use of interactive virtual environments in clinical routine.

Les déficits cognitifs sont présents chez les patients atteints de cancer. Les tests cognitifs tels que le Montreal Cognitive Assessment se sont révélés peu spécifiques, incapables de détecter des déficits légers et ne sont pas linéaires. Pour suppléer à ces limitations nous avons développé un questionnaire cognitif simple, bref et adapté aux dimensions cognitives atteintes chez les patients avec un cancer, le FaCE « The Fast Cognitif Evaluation », en utilisant la modélisation Rasch (MR). La MR est une méthode mathématique probabiliste qui détermine les conditions pour qu’un outil soit considéré une échelle de mesure et elle est indépendante de l’échantillon. Si les résultats s’ajustent au modèle, l’échelle de mesure est linéaire avec des intervalles égaux. Les réponses sont basées sur la capacité des sujets et la difficulté des items. La carte des items permet de sélectionner les items les plus adaptés pour l’évaluation de chaque aspect cognitif et d’en réduire le nombre au minimum. L’analyse de l’unidimensionnalité évalue si l’outil mesure une autre dimension que celle attendue. Les résultats d’analyses, conduites sur 165 patients, montrent que le FaCE distingue avec une excellente fiabilité et des niveaux suffisamment différents les compétences des patients (person-reliability-index=0.86; person-separation-index=2.51). La taille de la population et le nombre d’items sont suffisants pour que les items aient une hiérarchisation fiable et précise (item-reliability=0.99; item-séparation-index=8.75). La carte des items montre une bonne dispersion de ceux-ci et une linéarité du score sans effet plafond. Enfin, l’unidimensionnalité est respectée et le temps d’accomplissement moyen est d’environ 6 minutes. Par définition la MR permet d’assurer la linéarité et la continuité de l’échelle de mesure. Nous avons réussi à développer un questionnaire bref, simple, rapide et adapté aux déficits cognitifs des patients avec un cancer. Le FaCE pourrait, aussi, servir de mesure de référence pour les futures recherches dans le domaine.

Aims: To estimate dementia prevalence and describe the etiology of dementia in a community sample from the city of Sao Paulo, Brazil. Methods: A sample of subjects older than 60 years was screened for dementia in the first phase. During the second phase, the diagnostic workup included a structured interview, physical and neurological examination, laboratory exams, a brain scan, and DSM-IV criteria diagnosis. Results: Mean age was 71.5 years (n = 1,563) and 58.3% had up to 4 years of schooling (68.7% female). Dementia was diagnosed in 107 subjects with an observed prevalence of 6.8%. The estimate of dementia prevalence was 12.9%, considering design effect, nonresponse during the community phase, and positive and negative predictive values. Alzheimer`s disease was the most frequent cause of dementia (59.8%), followed by vascular dementia (15.9%). Older age and illiteracy were significantly associated with dementia. Conclusions: The estimate of dementia prevalence was higher than previously reported in Brazil, with Alzheimer`s disease and vascular dementia being the most frequent causes of dementia. Dementia prevalence in Brazil and in other Latin American countries should be addressed by additional studies to confirm these higher dementia rates which might have a sizable impact on countries` health services. Although several reports have demonstrated physiological and behavioral changes in adult rats due to neonatal immune challenges, little is known about their effects in adolescence. Since neonatal exposure to lipopolysaccharide (LPS) alters the neural substrates involved in cognitive disorders, we tested the hypothesis that it may also alter the response to novel environments in adolescent rats. At 3 and 5 days of age, male Wistar rats received intraperitoneal injections of either vehicle solution or E. coli LPS (0.05 mg/kg) or were left undisturbed. In the mid-adolescent period, between 40 and 46 days of age, the rats were exposed to the following behavioral tests: elevated plus-maze, open-field, novel-object exploration task, hole-board and the modified Porsolt forced swim test. The results showed that, in comparison with control animals, LPS-treated rats exhibited (1) less anxiety-related behaviors and enhanced patterns of locomotion and rearing in the plus-maze and the open-field tests, (2) high levels of exploration of both objects in the novel-object task and of corner and central holes in hole-board test, and (3) more time spent diving, an active behavior in the forced swim test. The present findings suggest that neonatal LPS exposure has long-lasting effects on the behavior profile adolescent rats exhibit in response to novelty. This behavioral pattern, characterized by heightened exploratory activity in novel environments, also suggests that early immune stimulation may contribute to the development of impulsive behavior in adolescent rats. RESUMO: Várias intervenções psiquiátricas e psicossociais têm demonstrado ser eficazes na redução da depressão e da ansiedade em indivíduos que sofreram um acidente coronário agudo. A possiblidade de modificarem a evolução da doença coronária e de reduzirem a mortalidade cardíaca continua, no entanto, por comprovar. Este estudo teve como principal objectivo avaliar a efectividade de uma intervenção de Psiquiatria de Ligação (PL) em doentes com cardiopatia isquémica aguda. Métodos: um grupo inicial de 129 doentes consecutivos, internados por Enfarte Agudo do Miocárdio (EAM) ou Angina Instável, numa unidade de cuidados intensivos foi avaliado com a Hospital Anxiety e Depression Scale (HADS). Os doentes que apresentaram uma pontuação ≥8 nas subescalas da Depressão ou da Ansiedade (n=72) foram aleatoriamente distribuídos por grupo de intervenção (GI) (n=37) e grupo de controlo (GC) (n=35). O GI foi sujeito a uma intervenção de PL, realizada durante o período hospitalar, que se iniciou nos primeiros dias de internamento e consistiu em pelo menos 3 sessões individuais (incluindo avaliação psiquiátrica, psicoterapia de suporte, intervenção psicoeducativa e medicação, quando necessário). A sessão pré-alta envolveu o cônjuge e abordou a modificação de comportamentos de risco, a adesão terapêutica e o regresso ao trabalho. O GC recebeu os cuidados habituais da unidade de internamento. Todos os doentes completaram uma entrevista inicial para avaliação do estado cognitivo (Mini Mental State Examination – MMSE), do ajustamento social (Social Problems Questionnaire – SPQ) e de aspectos sociodemográfi cos e clínicos. Os doentes foram reavaliados antes da alta, aos 45 dias, 3 e 6 meses com a HADS, o SPQ e ainda com o Nottingham Health Profi le (NHP) para avaliação da qualidade de vida. No follow-up de 6 meses foi colhida informação sobre sobrevivência, número e duração de reinternamentos, número de dias de baixa e regresso ao trabalho. Resultados: na amostra de 129 doentes avaliados no início do internamento, 20,9% apresentavam níveis de depressão ≥8 na subescala da Depressão (HADS), 53,5% níveis de ansiedade ≥8 na subescala da Ansiedade (HADS) e 9,3% perturbações cognitivas (MMSE). A avaliação longitudinal desta amostra mostrou que os níveis de depressão, inicialmente baixos, aumentaram nos 45 dias após o internamento, para depois diminuírem até ao fi nal do follow-up. Os níveis de ansiedade, que eram inicialmente altos, aumentaram nos 45 dias seguintes e antiveram- se estáveis, mas altos, até ao fi m do estudo. O GI apresentou uma pontuação média na subescala da depressão signifi cativamente inferior à do GC no follow-up de 6 meses (5,8±4,1 no GI vs. 7,9±4,3 no GC, p=0,04). O número de doentes deprimidos foi signifi cativamente menor no GI nas avaliações realizadas aos 3 meses (11 vs. 18 no GC, p=0,04) e aos 6 meses (12 vs. 18 no GC, p= 0,05). O mesmo aconteceu com o número de doentes ansiosos aos 3 meses (15 no GI vs. 23 no GC, p=0,01). As dimensões do NHP “Isolamento social” aos 45 dias e “Reacção emocional” aos 45 dias e aos 3 meses, bem como a qualidade de vida geral (NHP 2ª parte) aos 3 meses, mostraram melhoria signifi cativa no grupo de intervenção. Embora a intervenção tenha reduzido o nível médio da ansiedade nas várias avaliações após a alta, esta redução não atingiu signifi cância estatística. A intervenção realizada não teve impacto na mortalidade ou nas variáveis relacionadas com a evolução da doença cardíaca no período do follow-up. Conclusões: Os resultados do presente estudo mostram a alta prevalência de depressão e de ansiedade após um acidente coronário agudo e a manutenção de níveis altos de ansiedade nos 6 meses seguintes. Os resultados comprovam também a efectividade de uma intervenção em PL no tratamento da depressão e da ansiedade em doentes que sofreram um acidente coronário agudo. Estes resultados apontam para a necessidade de desenvolvimento de programas de PL para este tipo de doentes, tanto no hospital geral como nos cuidados de saúde primários. Sugerem ainda a necessidade de desenvolvimento de investigação que permita estabelecer o impacto específi co dos diversos tipos de intervenção, assim como compreender os mecanismos subjacentes à associação da depressão e da ansiedade com a doença coronária.----------ABSTRACT:Different types of psychiatric and psychochosocial interventions have proven effi cacy in decreasing anxiety and depression in coronary heart disease. There is, however, an ongoing discussion about the impact these interventions may have on the clinical outcome and on cardiac mortality. The main objective of the current study was to evaluate the effectiveness of a consultation liaison psychiatry (CL) intervention on a group of patients admitted with Myocardial Infarction or Unstable Angina, to a Coronary Care Unit. Methods: The study had a prospective, randomised, controlled design, with a 6-month follow-up. One hundred and twenty-nine consecutive patients were assessed during the first 48 hours of admission with the Hospital Anxiety and Depression Scale (HADS). Those with a score of ≥8 on the Depression or the Anxiety subscales (n=72) were randomly allocated to intervention (n=37) and usual care (n=35). The CL intervention, started during the fi rst days of admission, had a minimum of 3 (60 minutes) sessions, and included a psychiatric evaluation, supportive psychotherapy, a psychoeducational intervention, when necessary, psychotropic drugs. The last session, shortly before discharge, included the spouse and was focused on compliance, modifi cation of behavioral risk factors, and possible diffi culties upon returning to work. Cognitive status (Mini-Mental State Examination - MMSE), social adjustment (Social Problems Questionnaire - SPQ), and demographic and clinical characteristics were also assessed at baseline. Patients were reassessed before discharge, and at 45 days, 3 and 6 months after admission with HADS, SPQ, and with Nottingham Health Profile (NHP) for quality of life. Survival, number of readmissions and days of readmission, number of sickleave days and return to work were assessed at six months. Results: The initial sample of 129 patients, presented a 20.9% prevalence of depressive symptoms, 53.5% of anxiety symptoms, and 9.3% of cognitive disorders. The longitudinal evaluation of this sample showed that the initially low levels of depression were increased 45 days later, and slowlly decreased afterwards till the 6-month follow-up. Initially high anxiety levels, somewhat decreased before discharge, had increased 45 days later, and stayed stable and high till the end of the study. The intervention group showed a signifi cantly lower depression mean score at 6 months (5.8±4.1 vs. 7.9±4.3 in the controls, p=0.04). The number of patients considered depressed was lower in the intervention group at 3(11 vs. 18 controls, p=0.04) and 6 months (12vs. 18 controls, p=0,05). The number of anxious patients was also lower in the intervention.

ABSTRACT: Background. In India, prevalence rates of dementia and prodromal amnestic Mild Cognitive Impairment (MCI) are 3.1% and 4.3% respectively. Most Indians refer to the full spectrum of cognitive disorders simply as ‘memory loss.’ Barring prevention or cure, these conditions will rise rapidly with population aging. Evidence-based policies and practices can improve the lives of affected individuals and their caregivers, but will require timely and sustained uptake. Objectives. Framed by social cognitive theories of health behavior, this study explores the knowledge, attitudes and practices concerning cognitive impairment and related service use by older adults who screen positive for MCI, their primary caregivers, and health providers. Methods. I used the Montreal Cognitive Assessment to screen for cognitive impairment in memory camps in Mumbai. To achieve sampling diversity, I used maximum variation sampling. Ten adults aged 60+ who had no significant functional impairment but screened positive for MCI and their caregivers participated in separate focus groups. Four other such dyads and six doctors/ traditional healers completed in-depth interviews. Data were translated from Hindi or Marathi to English and analyzed in Atlas.ti using Framework Analysis. Findings. Knowledge and awareness of cognitive impairment and available resources were very low. Physicians attributed the condition to disease-induced pathology while lay persons blamed brain malfunction due to normal aging. Main attitudes were that this condition is not a disease, is not serious and/or is not treatable, and that it evokes stigma toward and among impaired persons, their families and providers. Low knowledge and poor attitudes impeded help-seeking. Conclusions. Cognitive disorders of aging will take a heavy toll on private lives and public resources in developing countries. Early detection, accurate diagnosis, systematic monitoring and quality care are needed to compress the period of morbidity and promote quality of life. Key stakeholders provide essential insights into how scientific and indigenous knowledge and sociocultural attitudes affect use and provision of resources.

INTRODUCTION: The geriatric depression (GD) represents one of the most frequent psychiatric disorders in outpatient services specialized in old-age treatment. OBJECTIVE: The course of two illustrative cases of GD is discussed, highlighting its clinical picture after antidepressant treatment and underlining variables related to disease prognosis, treatment effectiveness and conversion to major cognitive disorders such as vascular dementia (VD). METHODS: The cognitive performance, depressive symptoms, autonomy and brain structural measurements as white matter hyperintensities (WMH) and hippocampal size, and microstructural integrity of WM with diffusion tensor imaging were followed during four years. RESULTS: Case 1, with a severe degree of WMH, was associated with worsening cognition and increasing functional disability. Case 2, with mild WMH, an improvement of cognitive functioning could be seen. CONCLUSIONS: The existence of different subtypes of GD, as presented in this report, points a pathophysiological heterogeneity of GD, and suggests a possible continuum vascular depression (VaDp) and vascular cognitive impairment (VCI).

The most frequent clinical manifestation of borreliosis in Switzerland is erythema migrans, with about 2500 patients each year. Neurological manifestations are rare, mostly hyperalgesic radiculitis (Bannwarth syndrome), aseptic meningitis or cranial nerve involvement. We report the first Swiss patient with meningovasculitis due to neuroborreliosis, with recurrent multiple ischemic strokes in multiple vascular territories. The treatment with ceftriaxone stopped the progression, but the patient is still suffering from severe invalidating cognitive disorders. We also comment on the pathophysiology and review the literature of other clinical cases.
