O período pós-parto pode ser considerado a fase em que a mulher está exposta a retenção de massa corporal (MC) e desenvolvimento da obesidade. Na maioria das situações valorizam-se mais a perda de MC e pouco se conhece a respeito dos componentes que estão sendo mobilizados. Desta forma, há interesse de se validar técnicas acessíveis, como a impedância bioelétrica (BIA), que possam ser aplicadas no acompanhamento nutricional destas mulheres. O presente estudo teve como objetivo validar diferentes modelos de impedância bioelétrica (BIA) para a determinação da composição corporal em mulheres pós-parto tendo como ?padrão ouro? a técnica de absorciometria de dupla energia de raio X(DXA). A amostra foi composta por 46 mulheres no período pós-parto com idade acima de 18anos, inicialmente recrutadas e convidadas a participar da pesquisa, logo após o parto, para posteriormente serem avaliadas. A composição corporal (CC) foi mensurada através de três aparelhos distintos de BIA: RJL , BIO e Tanita e pela DXA. Os componentes101450BC 533corporais medidos foram: massa livre de gordura (MLG), massa gorda (MG) e percentual degordura corporal (%GC). Na análise estatística foram calculadas as médias e desvio-padrãodas variáveis contínuas. A concordância entre os componentes corporais determinados pelaDXA e cada BIA foi avaliada pelo método de Bland & Altman e pelo coeficiente decorrelação de concordância (CCC). Para comparação de médias das variáveis obtidas por BIAe DXA para cada faixa de índice de massa corporal (IMC), foi aplicada a ANOVA e oadotado o teste post-hoc de Tukey. O nível de significância adotado foi de 95% (p=0,05). Amédia ( desvio padrão) de idade foi de 25,5 4,6 anos, a MC de 73,6 12,2kg, o IMC de28,7 4,3kg/m, a MG de 29,87,4kg, a MLG de 43,25,7kg e o %GC de 41,84,3%,obtidos pelo DXA. Ao comparar-se os resultados dos diferentes equipamentos com o método padrão-ouro, verificou-se que houve diferença significativa entre a DXA e as BIA para todos componentes corporais, com exceção da MG fornecida pelo RJL (27,1Kg) e TAN (27,1Kg)em relação à DXA (29,8Kg). Os resultados mostraram haver uma boa reprodutibilidade emrelação aos aparelhos de BIA TAN (r=0,74; r=0,90) e RJL (r=0,78; r=0,89) para MLG e MG.Verificou-se que os equipamentos de BIA TAN e RJL forneceram estimativas semelhantes em relação à MLG, MG e %GC. Em relação aos valores da DXA o equipamento que mais se aproximou foi a TAN e o RJL quando avaliada a MG considerando todas as mulheres, mas quando estratificadas segundo o IMC, a análise foi mais acurada em mulheres obesas.Entretanto, a concordância não foi satisfatória entre os componentes corporais fornecidos pelas BIA em relação à DXA. As BIAs avaliadas no presente estudo subestimaram o %GC e superestimaram a MLG para esse grupo de mulheres no período pós-parto. Conclui-se que as BIA avaliadas não são recomendadas para avaliação da CC neste grupo de mulheres no período pós-parto.

A extração de regras de associação (ARM - Association Rule Mining) de dados quantitativos tem sido pesquisa de grande interesse na área de mineração de dados. Com o crescente aumento das bases de dados, há um grande investimento na área de pesquisa na criação de algoritmos para melhorar o desempenho relacionado a quantidade de regras, sua relevância e a performance computacional. O algoritmo APRIORI, tradicionalmente usado na extração de regras de associação, foi criado originalmente para trabalhar com atributos categóricos. Geralmente, para usá-lo com atributos contínuos, ou quantitativos, é necessário transformar os atributos contínuos, discretizando-os e, portanto, criando categorias a partir dos intervalos discretos. Os métodos mais tradicionais de discretização produzem intervalos com fronteiras sharp, que podem subestimar ou superestimar elementos próximos dos limites das partições, e portanto levar a uma representação imprecisa de semântica. Uma maneira de tratar este problema é criar partições soft, com limites suavizados. Neste trabalho é utilizada uma partição fuzzy das variáveis contínuas, que baseia-se na teoria dos conjuntos fuzzy e transforma os atributos quantitativos em partições de termos linguísticos. Os algoritmos de mineração de regras de associação fuzzy (FARM - Fuzzy Association Rule Mining) trabalham com este princípio e, neste trabalho, o algoritmo FUZZYAPRIORI, que pertence a esta categoria, é utilizado. As regras extraídas são expressas em termos linguísticos, o que é mais natural e interpretável pelo raciocício humano. Os algoritmos APRIORI tradicional e FUZZYAPRIORI são comparado, através de classificadores associativos, baseados em regras extraídas por estes algoritmos. Estes classificadores foram aplicados em uma base de dados relativa a registros de conexões TCP/IP que destina-se à criação de um Sistema de Detecção de Intrusos.

BACKGROUND: In the face of the HIV/AIDS epidemic that has contributed to the dramatic increase in orphans and abandoned children (OAC) worldwide, caregiver attitudes about HIV, and HIV-related stigma, are two attributes that may affect caregiving. Little research has considered the relationship between caregiver attributes and caregiver-reported HIV-related stigma. In light of the paucity of this literature, this paper will describe HIV-related stigma among caregivers of OAC in five less wealthy nations. METHODS: Baseline data were collected between May 2006 through February 2008. The sample included 1,480 community-based and 192 institution-based caregivers. Characteristics of the community-based and institution-based caregivers are described using means and standard deviations for continuous variables or counts and percentages for categorical variables. We fit logistic regression models, both for the full sample and separately for community-based and institution-based caregivers, to explore predictors of acceptance of HIV. RESULTS: Approximately 80% of both community-based and institution-based caregivers were female; and 84% of institution-based caregivers, compared to 66% of community-based caregivers, said that they would be willing to care for a relative with HIV. Similar proportions were reported when caregivers were asked if they were willing to let their child play with an HIV-infected child. In a multivariable model predicting willingness to care for an HIV-infected relative, adjusted for site fixed effects, being an institution-based caregiver was associated with greater willingness (less stigma) than community-based caregivers. Decreased willingness was reported by older respondents, while willingness increased with greater formal education. In the adjusted models predicting willingness to allow one's child to play with an HIV-infected child, female gender and older age was associated with less willingness. However, willingness was positively associated with years of formal education. CONCLUSIONS: The caregiver-child relationship is central to a child's development. OAC already face stigma as a result of their orphaned or abandoned status; the addition of HIV-related stigma represents a double burden for these children. Further research on the prevalence of HIV-related acceptance and stigma among caregivers and implications of such stigma for child development will be critical as the policy community responds to the global HIV/AIDS orphan crisis.

Gaussian factor models have proven widely useful for parsimoniously characterizing dependence in multivariate data. There is a rich literature on their extension to mixed categorical and continuous variables, using latent Gaussian variables or through generalized latent trait models acommodating measurements in the exponential family. However, when generalizing to non-Gaussian measured variables the latent variables typically influence both the dependence structure and the form of the marginal distributions, complicating interpretation and introducing artifacts. To address this problem we propose a novel class of Bayesian Gaussian copula factor models which decouple the latent factors from the marginal distributions. A semiparametric specification for the marginals based on the extended rank likelihood yields straightforward implementation and substantial computational gains. We provide new theoretical and empirical justifications for using this likelihood in Bayesian inference. We propose new default priors for the factor loadings and develop efficient parameter-expanded Gibbs sampling for posterior computation. The methods are evaluated through simulations and applied to a dataset in political science. The models in this paper are implemented in the R package bfa.

Quantum teleportation for continuous variables is generally described in phase space by using the Wigner functions. We study quantum teleportation via a mixed two-mode squeezed state in Hilbert-Schmidt space by using the coherent-state representation and operators. This shows directly how the teleported state is related to the original state.

Objective: To examine the association between dietary glycemic index (GI), glycemic load (GL), total carbohydrate, sugars, starch, and fiber intakes and the risk of reflux esophagitis, Barrett’s esophagus, and esophageal adenocarcinoma.
Methods: In an all-Ireland study, dietary information was collected from patients with esophageal adenocarcinoma (n = 224), long-segment Barrett’s esophagus (n = 220), reflux esophagitis (n = 219), and population-based controls (n = 256). Multiple logistic regression analysis examined the association between dietary variables and disease risk by tertiles of intake and as continuous variables, while adjusting for potential confounders.
Results: Reflux esophagitis risk was positively associated with starch intake and negatively associated with sugar intake. Barrett’s esophagus risk was significantly reduced in people in the highest versus the lowest tertile of fiber intake (OR 0.44 95%CI 0.25–0.80). Fiber intake was also associated with a reduced risk of esophageal adenocarcinoma, as was total carbohydrate intake (OR 0.45 95%CI 0.33–0.61 per 50 g/d increase). However, an increased esophageal adenocarcinoma risk was detected per 10 unit increase in GI intake (OR 1.42 95%CI 1.07–1.89).
Conclusions: Our findings suggest that fiber intake is inversely associated with Barrett’s esophagus and esophageal adenocarcinoma risk. Esophageal adenocarcinoma risk is inversely associated with total carbohydrate consumption but positively associated with high GI intakes.

Cold-formed steel portal frames are a popular form of construction for low-rise commercial, light industrial and agricultural buildings with spans of up to 20 m. In this article, a real-coded genetic algorithm is described that is used to minimize the cost of the main frame of such buildings. The key decision variables considered in this proposed algorithm consist of both the spacing and pitch of the frame as continuous variables, as well as the discrete section sizes.A routine taking the structural analysis and frame design for cold-formed steel sections is embedded into a genetic algorithm. The results show that the real-coded genetic algorithm handles effectively the mixture of design variables, with high robustness and consistency in achieving the optimum solution. All wind load combinations according to Australian code are considered in this research. Results for frames with knee braces are also included, for which the optimization achieved even larger savings in cost.

We demonstrate genuine three-mode nonlocality based on phase-space formalism. A Svetlichny-type Bell inequality is formulated in terms of the s-parametrized quasiprobability function. We test such a tool using exemplary forms of three-mode entangled states, identifying the ideal measurement settings required for each state. We thus verify the presence of genuine three-mode nonlocality that cannot be reproduced by local or nonlocal hidden variable models between any two out of three modes. In our results, GHZ- and W-type nonlocality can be fully discriminated. We also study the behavior of genuine tripartite nonlocality under the effects of detection inefficiency and dissipation induced by local thermal environments. Our formalism can be useful to test the sharing of genuine multipartite quantum correlations among the elements of some interesting physical settings, including arrays of trapped ions and intracavity ultracold atoms. We address the distribution of quantum information among many parties in the presence of noise. In particular, we consider how to optimally send to m receivers the information encoded into an unknown coherent state. On one hand, a local strategy is considered, consisting in a local cloning process followed by direct transmission. On the other hand, a telecloning protocol based on nonlocal quantum correlations is analysed. Both the strategies are optimized to minimize the detrimental effects due to losses and thermal noise during the propagation. The comparison between the local and the nonlocal protocol shows that telecloning is more effective than local cloning for a wide range of noise parameters. Our results indicate that nonlocal strategies can be more robust against noise than local ones, thus being suitable candidates for playing a major role in quantum information networks.

We address the generation of fully inseparable three-mode entangled states of radiation by interlinked nonlinear interactions in chi((2)) media. We show how three-mode entanglement can be used to realize symmetric and asymmetric telecloning machines, which achieve optimal fidelity for coherent states. An experimental implementation involving a single nonlinear crystal in which the two interactions take place simultaneously is suggested. Preliminary experimental results showing the feasibility and the effectiveness of the interaction scheme with a seeded crystal are also presented. (C) 2004 Optical Society of America.

We introduce a scheme to reconstruct arbitrary states of networks composed of quantum oscillators-e. g., the motionalstate of trapped ions or the radiation state of coupled cavities. The scheme involves minimal resources and minimal access, in the sense that it (i) requires only the interaction between a one-qubit probe and a single node of the network; (ii) provides the Weyl characteristic function of the network directly from the data, avoiding any tomographic transformation; (iii) involves the tuning of only one coupling parameter. In addition, we show that a number of quantum properties can be extracted without full reconstruction of the state. The scheme can be used for probing quantum simulations of anharmonic many-body systems and quantum computations with continuous variables. Experimental implementation with trapped ions is also discussed and shown to be within reach of current technology.

We analyse a proposal that we have recently put forward for an interface between matter-wave and optomechanical technologies from the perspective of macroscopic quantumness. In particular, by making use of a measure of macroscopicity in quantum superpositions that is particularly well suited for continuous variables systems, we demonstrate the existence of working points for our interface at which a quantum mechanical superposition of genuinely mesoscopic states is achieved. Our proposal thus holds the potential to affirm itself as a viable atom-to-mechanics transducer of quantum coherences.

This paper considers the optimal design of fabricated steel beams for long-span portal frames. The design optimisation takes into account ultimate as well as serviceability limit states, adopting deflection limits recommended by the Steel Construction Institute (SCI). Results for three benchmark frames demonstrate the efficiency of the optimisation methodology. A genetic algorithm (GA) was used to optimise the dimensions of the plates used for the columns, rafters and haunches. Discrete decision variables were adopted for the thickness of the steel plates and continuous variables for the breadth and depth of the plates. Strategies were developed to enhance the performance of the GA including solution space reduction and a hybrid initial population half of which is derived using Latin hypercube sampling. The results show that the proposed GA-based optimisation model generates optimal and near-optimal solutions consistently. A parametric study is then conducted on frames of different spans. A significant variation in weight between fabricated and conventional hot-rolled steel portal frames is shown; for a 50 m span frame, a 14–19% saving in weight was achieved. Furthermore, since Universal Beam sections in the UK come from a discrete section library, the results could also provide overall dimensions of other beams that could be more efficient for portal frames. Eurocode 3 was used for illustrative purposes; any alternative code of practice may be used.

In this study, we assessed whether the white-coat effect (difference between office and daytime blood pressure (BP)) is associated with nondipping (absence of BP decrease at night). Data were available in 371 individuals of African descent from 74 families selected from a population-based hypertension register in the Seychelles Islands and in 295 Caucasian individuals randomly selected from a population-based study in Switzerland. We used standard multiple linear regression in the Swiss data and generalized estimating equations to account for familial correlations in the Seychelles data. The prevalence of systolic and diastolic nondipping (<10% nocturnal BP decrease) and white-coat hypertension (WCH) was respectively 51, 46, and 4% in blacks and 33, 37, and 7% in whites. When white coat effect and nocturnal dipping were taken as continuous variables (mm Hg), systolic (SBP) and diastolic BP (DBP) dipping were associated inversely and independently with white-coat effect (P < 0.05) in both populations. Analogously, the difference between office and daytime heart rate was inversely associated with the difference between daytime and night-time heart rate in the two populations. These results did not change after adjustment for potential confounders. The white-coat effect is associated with BP nondipping. The similar associations between office-daytime values and daytime-night-time values for both BP and heart rate suggest that the sympathetic nervous system might play a role. Our findings also further stress the interest, for clinicians, of assessing the presence of a white-coat effect as a means to further identify patients at increased cardiovascular risk and guide treatment accordingly.

Cette thèse avait pour but premier d’évaluer la douleur chronique endométriosique et ses concomitants (dépression, anxiété et stress), les conséquences de la douleur sur le physique, les activités et le travail, sur la relation maritale et les séquelles sur la qualité de vie chez des participantes souffrant de douleurs pelviennes chroniques diagnostiquées endométriose (laparoscopie). En deuxième lieu, il s’agissait d’évaluer et de comparer l’efficacité des techniques psychologiques de contrôle de la douleur (Hypnose, Cognitif-behavioral) en ajout aux traitements médicaux à un groupe contrôle (Attention thérapeute). L’échantillon était composé de 60 femmes réparties aléatoirement soit à l’un des deux groupes de traitement ou au groupe contrôle. Les instruments de mesure étaient tous des questionnaires déjà traduits en français et validés pour la population francophone québécoise. Des tests du khi-carré ont été effectués pour les variables nominales et des analyses de variances (ANOVA) ont été faites pour les variables continues. Dans des modèles ANOVA estimant l’effet du traitement, du temps et de leur interaction, une différence significative (effet de Groupe ou traitement) a été trouvée pour les variables suivantes : Douleur (McGill :composante évaluative p = 0.02), au moment « présent » de l’Échelle visuelle analogique (EVA, p = 0.05) et dans l’Échelle de Qualité de vie (douleur, p = 0,03) ainsi qu’à la dimension Fonctionnement social de cette dernière échelle (SF-36; p = 0,04). En comparant les données en pré et post-traitement, des résultats significatifs au niveau du Temps ont aussi été mis en évidence pour les variables suivantes : Douleur McGill: Score total, (p = 0,03), Affective (p = 0,04), Évaluative (p = 0,01); Douleur (ÉVA) moment Fort (p < 0,0005), Dépression (p = 0,005), Anxiété (situationnelle/état (p = 0,002), Anxiété/trait (p < 0,001), Stress (p = 0, 003) ainsi que pour quatre composantes de la Qualité de vie (Fonctionnement social, (p = 0,05), Vitalité (p = 0,002), Douleur, (p = 0,003) et Changement de la santé (p < 0,001) et ceci pour les trois groupes à l’exception du groupe Hypnose sur cette dernière variable. Des effets d’Interaction (Groupe X Temps) sont ressortis sur les variables « Conséquences physiques » de la douleur mais sur la dimension « Activités » seulement (p = 0,02), sur l’anxiété situationnelle (État : p = 0,007). Un effet d’interaction se rapprochant de la signification (p = 0,08) a aussi été analysé pour la variable Fonctionnement social (SF-36). L’étude montre une légère supériorité quant au traitement Cognitif-behavioral pour l’anxiété situationnelle, pour le Fonctionnement social et pour la douleur mesurée par le SF-36. L’étude présente des forces (groupe homogène, essai clinique prospectif, répartition aléatoire des participantes et groupe contrôle) mais aussi des lacunes (faible échantillon et biais potentiels reliés à l’expérimentateur et à l’effet placebo). Toute future étude devrait tenir compte de biais potentiels quant au nombre d’expérimentateur et inclure un groupe placebo spécifique aux études à caractère psychologique. Une future étude devrait évaluer le schème cognitif « catastrophisation » impliqué dans la douleur, les traits de personnalité des participantes ainsi que le rôle du conjoint. De plus, des techniques psychologiques (entrevues motivationnelles) récentes utilisées dans plusieurs études devraient aussi être prises en considérations. Tout de même des résultats significatifs offrent des pistes intéressantes pour un essai clinique comportant un échantillon plus élevé et pour un suivi à long terme.
