CONTEXT AND OBJECTIVE: Injuries are an important cause of morbidity during adolescence, but can be avoided through learning about some of their characteristics. This study aimed to identify the most frequent injuries among adolescents attended at an emergency service. DESIGN AND SETTING: Retrospective descriptive study on adolescents attended at the emergency service of the Teaching Health Center, Faculdade de Medicina de Ribeirao Preto (FMRP), between January 1, 2009, and September 30, 2009. METHODS: Age, sex, type of injury, site, day and time of occurrence, part of body involved, care received, whether the adolescent was accompanied at the time of injury and whether any type of counseling regarding injury prevention had been given were analyzed. RESULTS: Among 180 adolescents attended, 106 (58.8%) were boys and 74 (41.1%) were girls. Their ages were: 10 to 12 (66/36.6%), 12 to 14 (60/33.3%) and 14 to 16 years (54/30%). The injuries had occurred in public places (47.7%) and at home (21.1%). The main types were bruises (45.1%) and falls (39.2%), involving upper limbs (46.1%), lower limbs (31%) and head/neck (13.1%). The injuries occurred in the afternoon (44.4%) and morning (30%), on Mondays (17.7%) and Thursdays (16.6%). Radiological examinations were performed on 53.8%. At the time of injury, 76.1% of the adolescents were accompanied. Some type of counseling about injury prevention had been received by 39.4%. CONCLUSIONS: Although the injuries were of low severity, preventive attitudes need to be incorporated in order to reduce the risks and provide greater safety for adolescents.

OBJETIVO: Analisar as características dos atendimentos decorrentes de quedas em serviços de urgência e emergência e identificar fatores associados. MÉTODOS: Estudo transversal com 12.617 atendimentos decorrentes de quedas registrados no Sistema de Vigilância de Violências e Acidentes, coletados em 23 capitais e Distrito Federal, de setembro a novembro de 2009, por meio de uma amostra por conglomerado. Foi utilizada a técnica de análise de correspondência, por permitir a observação conjunta de um grande número de variáveis qualitativas. RESULTADOS: A maior parte das vítimas foi do sexo masculino (56,5%), faixa etária de 0 a 19 anos (45,7%) e declarados não brancos (62,2%). A maioria das quedas ocorreu na residência (54,6%) e via pública (17,4%); 14,3% foram relacionadas ao trabalho. Os tipos predominantes foram "queda no mesmo nível" (57,0%) e "queda de escada/degrau" (15,6%). A maioria das lesões foi classificada como entorse, luxação, contusão, corte e laceração (68,3%). Quedas dentre as crianças associaram-se à ocorrência na residência; com os adolescentes na escola; e jovens na prática esportiva. Quedas em adultos estiveram associadas ao local de trabalho, queda de andaimes, telhados, escada/degrau e buracos e uso de álcool. As quedas no mesmo nível resultaram em lesões de menor gravidade, em membros inferiores e superiores, e as quedas de andaime e telhado se associaram com lesões de maior gravidade e internações. CONCLUSÕES: Os resultados mostram que estratégias para a prevenção das quedas devem ser implantadas particularmente em residências, escolas e ambientes de trabalho.

In traffic accidents with pedestrians, cyclists or motorcyclists, patterned impact injuries as well as marks on clothes can be matched to the injury-causing vehicle structure in order to reconstruct the accident and identify the vehicle which has hit the person. Therefore, the differentiation of the primary impact injuries from other injuries is of great importance. Impact injuries can be identified on the external injuries of the skin, the injured subcutaneous and fat tissue, as well as the fractured bones. Another sign of impact is a bone bruise. The bone bruise, or occult bone lesion, means a bleeding in the subcortical bone marrow, which is presumed to be the result of micro-fractures of the medullar trabeculae. The aim of this study was to prove that bleeding in the subcortical bone marrow of the deceased can be detected using the postmortem noninvasive magnetic resonance imaging. This is demonstrated in five accident cases, four involving pedestrians and one a cyclist, where bone bruises were detected in different bones as a sign of impact occurring in the same location as the external and soft tissue impact injuries.

A man wearing no protective helmet was struck by a motor vehicle while riding a bicycle. He was loaded on his left side, and the impact point of his head was his occiput on the car roof girder. He was immediately transported to the general hospital, where he passed away. Postmortem examination using multi-slice computed tomography (MSCT) revealed an extensively comminuted fracture of the posterior part and the base of the skull. Observed were deep direct and contrecoup brain bruises, with the independent fractures of the roof of the both orbits. Massive subdural and subarachnoidal hemorrhage with cerebral edema and shifting of the mid-line towards left side were also detected. MSCT and autopsy results were compared and the body injuries were correlated to vehicle damages. In conclusion, postmortem imaging is a good forensic visualization tool with great potential for documentation and examination of body injuries and pathology.

El durazno O'Henry para consumo en fresco es una variedades apta para la exportación. Consecuentemente se propone un método sencillo para determinar las pérdidas de calidad que impiden obtener mejores precios en los mercados.. Las mismas se relacionan con una mala gestión en todos los niveles: producción, empaque y comercialización, y la deficiente capacitación del personal involucrado. En la temporada 1999/2000 el granizo fue la causa más frecuente en precosecha; sus marcas explican el 70 % de los frutos con falta de calidad. En cambio, el principal factor en cosecha fueron las picaduras de grafolita, dando 80 % de frutos sin calidad exportable. En galpón de empaque, el mal manejo poscosecha -revelado en machucones, marcas de uñas y rajaduras- originó el 30 % de las causas de no calidad; si se le agregan las picaduras de grafolita, el porcentaje se aproxima al 60 %. Ambas causas pueden minimizarse con buena planificación y capacitación. Con simulación económica se detectó -en la precitada temporada- 20 % de pérdida de calidad de una partida lo cual motiva una disminución superior al 13 % en los ingresos. Esta diferencia depende no sólo del peso relativo de las distintas calidades sino también del precio pactado.

Apple fruits, cv. Granny Smith, were subjected to mechanical impact and compression loads utilizing a steel rod with a spherical tip 19 mm diameter, 50.6 g mass. Energies applied were low enough to produce enzymatic reaction: 0.0120 J for impact, and 0.0199 J for compression. Bruised material was cut and examined with a transmission electron microscope. In both compression and impact, bruises showed a central region located in the flesh parenchyma, at a distance that approximately equalled the indentor tip radius. The parenchyma cells of this region were more altered than cells from the epidermis and hypodermis. Tissues under compression presented numerous deformed parenchyma cells with broken tonoplasts and tissue degradation as predicted by several investigators. The impacted cells supported different kinds of stresses than compressed cells, resulting in the formation of intensive vesiculation, either in the vacuole or in the middle lamella region between cell walls of adjacent cells. A large proportion of parenchyma cells completely split or had initiated splitting at the middle lamella. Bruising may develop with or without cell rupture. Therefore, cell wall rupture is not essential for the development of a bruise, at least the smallest one, as predicted previously

The recognition of an increasing and worldwide demand for high quality in fruits and vegetables has grown in recent years. Evidence of severe problems of mechanical damage is increasing, and this is affecting the trade of fruits in European and other countries. The potential market for fresh high-quality vegetables and fruits remains restricted by the lack of quality of the majority of products that reach consumers; this is the case for local as well as import/export markets, so a reduction in the consumption of fresh fruits in favour of other fixed-quality products (dairy in particular) may become widespread. In a recent survey (King, 1988, cited in Bellon, 1989), it appears that, for the moment, one third of the surveyed consumers are still continuing to increase their fresh produce consumption. The factors that appear as being most important in influencing the shopping behaviour of these consumers are taste/flavour, freshness/ripeness, appealing look, and cleanliness. Research on mechanical damage in fruit and vegetables has been underway for several years. The first research made on physical properties of fruits was in fact directed towards analysing the response to slow or rapid loading of selected fruits (Fridley et al, 1968; Horsefield et al., 1972). From that time on, research has expanded greatly, and different aspects of the problem have been approached. These include applicable mechanical models for the contact problem, the response of biological tissues to loading, devices for detecting damage causes in machines and equipment, and procedures for sensing bruises in grading and sorting. This chapter will be devoted to the study of actual research results relative to the cause and mechanisms of mechanical damage in fruits (secondarily in vegetables), the development of bruises in these commodities, the models that have been used up to now, and the different factors which have been recognized as influencing the appearance and development of mechanical damage in fruits. The study will be focused mainly on contact-damage - that is, slow or rapid loads applied to the surface of the products and causing bruises. (A bruise is defined as an altered volume of fruit tissues below the skin that is discoloured and softened.) Other types of mechanical damage, like abrasion and scuffing, punctures and cuts, will be also mentioned briefly.

Two instrumented spheres IS 100 were used to evaluate the quality of post-harvest operations. Results obtained from measurements made with both IS (8.8 cm 0 and 6.2 cm 0) show significant differences. Both IS measure the same values of the same variables for soft materials, but not for hard surfaces. Four packing lines belonging to different cooperatives of the region of Murcia (two for stone fruits and two for citrus) were tested. IS values obtained in transfers belonging to the tested lines lay well above 50 g's in most of them Much higher impact intensities are registered in citrus lines than in stone fruit packing lines. To study the incidence of a certain packing line on different products an interaction fruit-packing line test was perf01med. In all cases, more than 50% of fruits belonging to the post-handling sample showed some kind of damage. Bruises evolve after 48 hours storage at room temperature.

Fruit damage during harvesting and handling is a standing problem, particularly for susceptible fruits like peaches and apricots. The resulting mechanical damage is a combination of fruit properties and damage inflicting effects due to procedures and to the equipment. Nine packing lines in the region of Murcia (SE Spain) have been tested with the aid of two different-size electronic fruits IS-100. Probabilities of impacts above three preset thresholds (50 g's, 100 g's and 150 g's) were calculated for each transfer point. Interaction fruit-packing line tests have been also performed in order to study the real incidence of packing lines on natural produce: apricots (1 variety), peaches (3 v.), lemons (1 v.) and oranges (3 v.). Bruises of handled and not handled samples of fruits were compared.
