OBJETIVOS: Traçar o perfil do usuário da Sala de Recursos para Deficiente Visual na cidade de Assis - SP, avaliar a porcentagem de deficientes visuais empregados e comparar o emprego com idade, gênero, tipo de deficiência e doença causadora da deficiência, entre dois períodos: de 1984 a 1996 e de 1997 a 2009. MÉTODOS: Foi realizado estudo retrospectivo dos prontuários médicos e da ficha escolar dos portadores de deficiência visual que frequentaram a Sala de Recursos para Deficiente Visual, no período de 1984 a 2009 na cidade de Assis - SP, divididos em dois períodos de 1984 a 1996 (G1) e de 1997 a 2009 (G2). Analisaram-se dados demográficos, a doença que provocou a baixa visão, o tipo de deficiência visual (cegueira ou baixa visão), escolaridade, recursos ópticos, frequência à Sala de Recursos para Deficiente Visual e taxa de emprego. Foi feita associação da taxa de emprego com: idade, gênero, raça/cor, tipo de deficiência e doença nos dois diferentes períodos. RESULTADOS: Foram encontrados 149 deficientes visuais sendo: 61,07% homens, 38,9% mulheres, 82,5% brancos e 17,4% não brancos e a mediana da idade foi de 18 anos. Dos 149 deficientes visuais, 63,75% eram portadores de baixa visão e 36,24% portadores de cegueira. As principais doenças que levaram à deficiência visual dos 149 pacientes foram em ordem decrescente: retinocoroidite por toxoplasmose (17,40%), atrofia óptica congênita (12,10%), alta hipermetropia (8,72%), retinose pigmentar e alta miopia (6,71% cada uma) e glaucoma congênito e catarata congênita (6,04% cada uma). A frequência à Sala de Recursos para Deficiente Visual foi boa em mais de 50% dos pacientes. Estavam trabalhando regularmente 44,7% e 12,3% dos maiores de 14 anos respectivamente nos períodos de 1984 - 1996 e de 1997 - 2009. Não houve diferença entre os dois períodos quanto às características demográficas, tipo de deficiência e doenças, havendo correlação da taxa de emprego apenas com a idade (pacientes com média de idade maior apresentavam maior porcentagem de emprego). CONCLUSÕES: Os deficientes visuais eram na maioria homens, brancos, portadores de baixa visão e portadores de toxoplasmose ocular. Quase metade (44,7%) dos pacientes com mais de 14 anos encontrava-se trabalhando regularmente até 1996 sugerindo que esforços conjuntos de oftalmologistas e educadores auxilia na inclusão social destes pacientes. A taxa de emprego caiu no período de 1997 a 2009 e a de aposentados aumentou.

OBJETIVO: Dentre os efeitos da poluição ambiental na saúde da criança, destaca-se o aumento de internações por pneumonias. O objetivo do estudo foi estimar a associação dessas internações com o aumento dos poluentes atmosféricos. MÉTODOS: Trata-se de estudo ecológico de séries temporais, realizado na cidade de São José dos Campos, SP, nos anos de 2000 e 2001. Foram utilizados dados diários sobre o número de internações por pneumonia, dados diários de poluentes (SO2, O3 e PM10) e de temperatura e umidade do clima. Foram estimadas as correlações entre as variáveis de interesse pelo coeficiente de Pearson. Para estimar a associação entre as internações por pneumonia e a poluição atmosférica, utilizaram-se modelos aditivos generalizados de regressão de Poisson. Foram estimados os acréscimos das internações por pneumonia para o intervalo interquartil para cada um dos poluentes estudados, com um intervalo de confiança de 95% RESULTADOS: Os três poluentes apresentaram efeitos defasados nas internações por pneumonia, iniciada três a quatro dias após a exposição e decaindo rapidamente. Na estimativa de efeito acumulado de oito dias observou-se ao longo desse período que para aumentos de 24,7 µg/m³ na concentração média de PM10 houve um acréscimo de 9,8% nas internações. CONCLUSÕES: O estudo confirma que o potencial deletério dos poluentes do ar sobre a saúde pode ser detectado, também, em cidades de médio porte. A magnitude do efeito foi semelhante ao observado na cidade de São Paulo. Além disso, mostra a elevada susceptibilidade das crianças aos efeitos adversos advindos da exposição aos contaminantes atmosféricos.

The use of highly active antiretroviral therapy (HAART) in HIV-infected patients has been associated with the development of risk factors for cardiovascular diseases (CD) including dyslipidemia and insulin resistance, hypertriglyceridemia being the most frequent metabolic disturbance in these patients. Fibrates are indicated when hypertriglyceridemia is accentuated and persists for over six months. We evaluated the efficacy and safety of bezafibrate for the treatment of hypertriglyceridemia in HIV-infected individuals on HAART. All patients received 400mg/day of bezafibrate and were evaluated three times: Mo (pre-treatment), M1 (one month after treatment), and M2 (six months after treatment). Fifteen adult individuals, eight males and seven females with mean age = 41.2 ± 7.97 years and triglyceride serum levels ≥400mg/dL were included in the study. Smoking, alcohol ingestion and sedentarism rates were 50%, 6.66% and 60%, respectively. Family history of CD, hypertension and diabetes mellitus was reported in 33.3%, 40% and 46.7% of the cases, respectively, while dyslipidemia was reported by only 13.3%. More than half of the patients were using a protease inhibitor plus a nucleotide analog transcriptase inhibitor. Eutrophy and tendency toward overweight were observed at all three study time points. There were significant reductions in triglyceride serum levels from Mo to M1 and from Mo to M2. No significant changes were observed in the serum levels of creatine phosphokinase, hepatic enzymes, CD4 +, CD8 + and viral load. Therefore, bezafibrate seems to be safe and effective for the reduction of hypertriglyceridemia in HIV-infected patients on HAART. © 2006 by The Brazilian Journal of Infectious Diseases and Contexto Publishing. Five dogs with rectovaginal fistula and atresia ani that had been treated by surgical correction of the malformations were studied retrospectively. Ages at presentation varied from 1 to 3 months and weight from 350 g to 7.5 kg. The histories included voiding of feces through the vulva, with or without tenesmus, usually observed after weaning. Artesia ani, presence of feces in the vaginal canal, abdominal distention, and discomfort on abdominal palpation were observed during clinical examination. Also, 3 dogs had partial tail agenesis. In all dogs, the rectovaginal fistula was isolated and transected, the vulvar and rectal defects were closed separately, and the atresia ani was repaired. Normal defecation was restored, but 1 dog had fecal incontinence that subsequently resolved. One dog died 2.5 months postoperatively, and follow-up was done on the others for periods ranging from 1.6 year to 7.7 years. Surgical correction in dogs with rectovaginal fistula and atresia ani may result in a favorable outcome, if it is done early.

The annual return, seasonal occurrence, and site fidelity of Korean-Okhotsk or western gray whales on their feeding grounds off northeastern Sakhalin Island, Russia, were assessed by boat-based photo-identification studies in 1994-1998. A total of 262 pods were observed, ranging in size from 1 to 9 whales with an overall mean of 2.0'. Sixty-nine whales were individually identified, and a majority of all whales (71.0%) were observed in multiple years. Annual sighting frequencies ranged from 1 to 18 d, with a mean of 5.4 d. The percentage of whales re-identified from previous years showed a continuous annual increase, reaching 87.0% by the end of the study. Time between first and last sighting of identified individuals within a given year was 1-85 d, with an overall mean of 40.6 d. Annual calf proportions ranged from 4.3% (1997) to 13.2% (1998), and mother-calf separations generally occurred between July and September. The seasonal site fidelity and annual return of whales to this part of the Okhotsk Sea emphasize its importance as a primary feeding ground for this endangered population.

This study is designed to compare the monthly continental snow cover and sea ice extent loss in the Arctic with regional atmospheric conditions including: mean sea level pressure, 925 hPa air temperature, and mean wind direction among others during the melt season (March-August) over the 29-year study period 1979-2007. Little research has gone into studying the concurrent variations in the annual loss of continental snow cover and sea ice extent across the land-ocean boundary, since these data are largely stored in incompatible formats. However, the analysis of these data, averaged spatially over three autonomous study regions located in Siberia, North America, and Western Russia, reveals a distinct difference in the response of snow and sea ice to the atmospheric forcing. On average, sea ice extent is lost earlier in the year, in May, than snow cover, in June, although Arctic sea ice is located farther north than continental snow in all three study regions. Once the loss of snow and ice extent begins, snow cover is completely removed sooner than sea ice extent, even though ice loss begins earlier in the melt season. Further, the analysis of the atmospheric conditions surrounding loss of snow and ice cover over the independent study regions indicates that conditions of cool temperatures with strong northeasterly winds in the later melt season months are effective at removing sea ice cover, likely through ice divergence, as are warmer temperatures via southerly winds directly forcing melt. The results of this study set the framework for further analysis of the direct influence of snow cover loss on later melt season sea ice extents and the predictability of snow and sea ice extent responses to modeled future climate conditions

Purpose: To compare 2 methods used to determine the disk position based on sagittal magnetic resonance images. Patients and Methods: A cross-sectional study of patients with the signs and symptoms of temporomandibular disorders was conducted. The patients' ages and gender distributions were collected. The disk position diagnosis from the clinical examination was considered the primary outcome. Three observers evaluated the presence of anterior displacement on magnetic resonance images according to 2 criteria: method 1 (12-o'clock position) and method 2 (location of the intermediate zone). To assess the intraobserver variability of the 2 methods, the examiners evaluated the same magnetic resonance images at the beginning of the study (time 1) and 40 days later (time 2). The intraobserver agreement was assessed using the observed agreement and the kappa statistic. McNemar's test was used to assess the differences between each method and the clinical examination findings (P < .05). The accuracy, sensitivity, specificity, and positive and negative predictive values were calculated by comparing the diagnosis from each method with that from the clinical examination (considered the reference standard). Results: The final sample was composed of 20 subjects with a mean age of 33.0 +/- 33.7 years; 3 were men (15%) and 17 were women (85%). A statistically significant difference between the 2 methods was found. Method 1 yielded a greater percentage of anterior displaced disks (52.5%). The agreement between the clinical diagnosis and method 1 was lower (70.0%) than that between the clinical diagnosis and method 2 (87.5%). No statistically significant difference was found between the clinical diagnosis and method 2. Conclusion: The disk position should be judged according to the intermediate zone criterion. This study focus on pathologies - caused by impoverished uranium and other heavy-metals’ nanoparticles environmental pollution - developed in international military personnel deployed in critical areas; the pathologies are then placed in a general and chronological schema. This study shows an impressive collection of data on impoverished uranium characteristics and its employment in civil and military context and a map of impoverished uranium most polluted areas. The studies on this subject commissioned by two Italian Parliamentary Court of Inquiry and by other nations are then analyzed. Further etiopathogenetic hypothesis are assessed – as multivaccination – comparing vaccination protocols adopted by different NATO nations and their possible effects. Finally the study defines the objectives and the operational protocols of an ongoing epidemiological serial prospective study (time-frame scheduled of 30 years) on military personnel deployed in critical areas for the possible presence of genotoxic agents.

Die vorliegende Arbeit befasst sich mit den veränderten Studienbedingungen, die durch die europaweite Einführung eines einheitlichen Systems von Bachelor- und Masterstudiengängen entstanden sind. In Deutschland wurde der Bologna-Prozess von umfangreichen Protesten begleitet und in der medialen Öffentlichkeit unter anderem die zeitliche und inhaltliche Überlastung der Studierenden und die verschulten Studiengänge kritisiert, die keinen Raum für die persönliche Entwicklung ließen. rnIm Rahmen des BMBF-geförderten Projekts ZEITLast wurde die zeitliche Belastung von Studierenden untersucht. Der scheinbare Widerspruch zwischen dem objektiv messbaren Zeitaufwand und dem subjektiven Belastungsempfinden der Studierenden stellt den Ausgangspunkt der vorliegenden Arbeit dar. Es wird der Frage nachgegangen, welche Strategien Studierende im Umgang mit den neuen Bachelorstudienstrukturen entwickeln und in welchem Zusammenhang diese mit dem subjektiven Belastungsempfinden stehen. Zur Beantwortung der Fragestellung werden quantitative und qualitative Forschungsmethoden kombiniert. Neben der Auswertung von umfangreichen Zeitbudgetanalysen und Befragungen aus dem Forschungsprojekt ZEITLast werden mit ausgewählten Probanden des Studiengangs Erziehungswissenschaft an der Universität Mainz problemzentrierte Interviews geführt und objektiv hermeneutisch ausgewertet. Eine zeitliche Überlastung der Studierenden lässt sich empirisch nicht nachweisen, dennoch fühlen sich Studierende subjektiv stark belastet Es zeigt sich, dass unterschiedliche subjektive Perspektiven auf die individuelle Studienorganisation und das Belastungsempfinden der Studierenden wirken. Vor allem spielt das individuelle Zeit- und Selbstmanagement bei der erfolgreichen Bewältigung der Anforderungen im Studium eine bedeutende Rolle. Insgesamt wird deutlich, dass Vorgaben im Studium abhängig von individuellen Dispositionen sowohl als Entlastung als auch als Belastung wahrgenommen werden. Die Aufgabe der Hochschule ist es, Studiengänge so zu gestalten, dass sie den unsicheren Studierenden Orientierung und Hilfestellung bieten, während sie den selbstbestimmten Studierenden Freiräume zur individuellen Entwicklung lassen. Aus den Ergebnissen der Arbeit werden abschließend hochschuldidaktische Maßnahmen entwickelt, die zu einer Entlastung der Studierenden beitragen können.

The goal of the current investigation was to compare two monitoring processes (judgments of learning [JOLs] and confidence judgments [CJs]) and their corresponding control processes (allocation of study time and selection of answers to maximize accuracy, respectively) in 5- to 7-year-old children (N=101). Children learned the meaning of Japanese characters and provided JOLs after a study phase and CJs after a memory test. They were given the opportunity to control their learning in self-paced study phases, and to control their accuracy by placing correct answers into a treasure chest and incorrect answers into a trash can. All three age groups gave significantly higher CJs for correct compared to incorrect answers, with no age-related differences in the magnitude of this difference, suggesting robust metacognitive monitoring skills in children as young as 5. Furthermore, a link between JOLs and study time was found in the 6- and 7-year-olds, such that children spent more time studying items with low JOLs compared to items with high JOLs. Also, 6- and 7-year-olds but not 5-year-olds spent more time studying difficult items compared to easier items. Moreover, age-related improvements were found in children's use of CJs to guide their selection of answers: although children as young as 5 placed their most confident answers in the treasure chest and least confident answers in the trash can, this pattern was more robust in older children. Overall, results support the view that some metacognitive judgments may be acted upon with greater ease than others among young children.

Children typically hold very optimistic views of their own skills but so far, only a few studies have investigated possible correlates of the ability to predict performance accurately. Therefore, this study examined the role of individual differences in performance estimation accuracy as a global metacognitive index for different monitoring and control skills (item-level judgments of learning [JOLs] and confidence judgments [CJs]), metacognitive control processes (allocation of study time and control of answers), and executive functions (cognitive flexibility, inhibition, working memory) in 6-year-olds (N=93). The three groups of under estimators, realists and over estimators differed significantly in their monitoring and control abilities: the under estimators outperformed the over estimators by showing a higher discrimination in CJs between correct and incorrect recognition. Also, the under estimators scored higher on the adequate control of incorrectly recognized items. Regarding the interplay of monitoring and control processes, under estimators spent more time studying items with low JOLs, and relied more systematically on their monitoring when controlling their recognition compared to over estimators. At the same time, the three groups did not differ significantly from each other in their executive functions. Overall, results indicate that differences in performance estimation accuracy are systematically related to other global and item-level metacognitive monitoring and control abilities in children as young as six years of age, while no meaningful association between performance estimation accuracy and executive functions was found.
