Ankle sprains are the most common injuries in sports, usually causing damage to the lateral ligaments. Recurrence has as usual result permanent instability, and thus loss of proprioception. This fact, together with residual symptoms, is what is known as chronic ankle instability, CAI, or FAI, if it is functional. This problem tries to be solved by improving musculoskeletal stability and proprioception by the application of bandages and performing exercises. The aim of this study has been to review articles (meta-analisis, systematic reviews and revisions) published in 2009-2015 in PubMed, Medline, ENFISPO and BUCea, using keywords such as “sprain instability”, “sprain proprioception”, “chronic ankle instability”. Evidence affirms that there does exist decreased proprioception in patients who suffer from CAI. Rehabilitation exercise regimen is indicated as a treatment because it generates a subjective improvement reported by the patient, and the application of bandages works like a sprain prevention method limiting the range of motion, reducing joint instability and increasing confidence during exercise. As podiatrists we should recommend proprioception exercises to all athletes in a preventive way, and those with CAI or FAI, as a rehabilitation programme, together with the application of bandages. However, further studies should be generated focusing on ways of improving proprioception, and on the exercise patterns that provide the maximum benefit.

The aim of this study was to investigate the kinematic, kinetic, and electromyographic pattern before, during and after downward squatting when the trunk movement is restricted in the sagittal plane. Eight healthy subjects performed downward squatting at two different positions, semisquatting (40 degrees knee flexion) and half squatting (70 degrees knee flexion). Electromyographic responses of the vastus medialis oblique, vastus medialis longus, rectus femoris, vastus lateralis, biceps femoris, semitendineous, gastrocnemius lateralis, and tibialis anterior were recorded. The kinematics of the major joints were reconstructed using an optoelectronic system. The center of pressure (COP) was obtained using data collected from one force plate, and the ankle and knee joint torques were calculated using inverse dynamics. In the upright position there were small changes in the COP and in the knee and ankle joint torques. The tibialis anterior provoked the disruption of this upright position initiating the squat. During the acceleration phase of the squat the COP moved posteriorly, the knee joint torque remained in flexion and there was no measurable muscle activation. As the body went into the deceleration phase, the knee joint torque increased towards extension with major muscle activities being observed in the four heads of the quadriceps. Understanding these kinematic, kinetic and EMG strategies before, during and after the squat is expected to be beneficial to practitioners for utilizing squatting as a task for improving motor function. The objective was to determine the reliability of isokinetic strength and endurance testing in the ankle joints of patients with intermittent claudication. Twenty-three patients with peripheral artery disease (PAD) and symptoms of intermittent claudication participated in the study. Isokinetic strength and endurance testing of the ankle joint were performed in symptomatic and asymptomatic legs on 3 separate days. Intraclass coefficient correlation of peak torque (PT) and total work (TW) ranged from 0.77 to 0.92 and 0.89 to 0.96, respectively. PT and TW increased significantly and similarly in both legs from day 1 to day 2 (PT: +42 +/- 84% in the symptomatic leg and +33 +/- 51% in the asymptomatic leg, p < 0.05;TW: +38 +/- 26% in the symptomatic leg and +26 +/- 50% in the asymptomatic leg, p < 0.05). In conclusion, isokinetic strength and endurance testing in the ankle joints of patients with PAD presents reliability coefficients ranging from 0.77 to 0.96. However, strength and endurance increased between the first and the other test sessions performed on separate days, suggesting that two test sessions are necessary for the accurate evaluation of strength and endurance in patients with PAD.

Evaluation of patients for rehabilitation after musculoskeletal injury involves identifying, grading and assessing the injury and its impact on the patient's normal activities. Management is guided by a multidisciplinary team, comprising the patient, doctor and physical therapist, with other health professionals recruited as required. Parallel interventions involving the various team members are specified in a customised management plan. The key component of the plan is active mobilisation utilising strengthening, flexibility and endurance exercise programs. Passive physical treatments (heat, ice, and manual therapy), as well as drug therapy and psychological interventions, are used as adjunctive therapy. Biomechanical devices or techniques (eg, orthotic devices) may also be helpful. Coexisting conditions such as depression and drug dependence are treated at the same time as the injury. Effective team communication, simulated environmental testing and, for those employed, contact with the employer facilitate a staged return to normal living, sports and occupational activities.

A articulação tíbio-társica (tornozelo) é considerada uma das articulações mais relevantes e mais susceptível à lesão, sendo que a entorse do tornozelo consiste na lesão mais comum em desportistas, com uma percentagem de ocorrência de 35% no andebol. Este trabalho centra-se, assim, na problemática das entorses do tornozelo nos jogadores de andebol e da sua relação com o treino proprioceptivo como medida preventiva. O objectivo do presente trabalho foi a realização de uma revisão sistemática que estuda o treino proprioceptivo na prevenção da entorse do tornozelo, para isso, tentámos identificar a possível relação entre entorse do tornozelo e treino proprioceptivo. A metodologia utilizada neste trabalho foi uma revisão da literatura, sendo realizada uma pesquisa electrónica na base de dados da B-On, utilizando combinações de palavras-chave. No total resultaram 758 artigos, tendo sido pela sua relevância selecionados 29. Neste trabalho ficou evidente a relação do treino proprioceptivo com a prevenção da entorse do tornozelo. De todas as medidas preventivas o treino proprioceptivo é o que apresenta melhores resultados, pois, este tipo de treino está direccionado para uma estruturação da zona lesionada, fortalecendo os músculos e ligamentos, restaurando a componente proprioceptiva das estruturas danificadas e permitindo uma resposta mais rápida do Sistema Nervoso Central, aos vários estímulos a que está sujeito o atleta. Foi possível descrever em que consiste o treino proprioceptivo e os seus constituintes, adaptando-o ao andebol. Com esta revisão da literatura não foi possível obter um consenso sobre o efeito do treino proprioceptivo na prevenção de entorses do tornozelo.

regula a posição do corpo no espaço, sendo um pré-requisito para o movimento. À periferia este processo de Controlo Postural pode ser identificado também através da variação do stiffness. O Acidente Vascular Encefálico apresenta-se como a patologia onde os sujeitos são referenciados como tendo alteração do stiffness, e poderão verificar-se modificações nesta variável no âmbito da reabilitação neuro-motora. Objetivo: Descrever o comportamento do stiffness da tibiotársica, nos dois membros inferiores, em indivíduos pós Acidente Vascular Encefálico, face a uma intervenção em fisioterapia baseada num processo de raciocínio clínico. Métodos: 5 sujeitos participaram no estudo, tendo sido implementado um programa de reabilitação para cada um dos sujeitos, por um período de 3 meses, com 2 momentos de avaliação (M0 e M1). O torque e a amplitude articular da tibiotársica foi monitorizada, através do dinamómetro isocinético, durante o movimento passivo de dorsiflexão, a diferentes velocidades (5º/s, 1º/s e 0,25º/s) A atividade eletromiográfica dos músculos Gastrocnémio Interno e Solear foi também recolhida. O valor de stiffness foi calculado através da relação torque/posição. Resultados: Em todos os sujeitos em estudo verificou-se que de uma forma geral o stiffness do membro contralateral à lesão apresentou uma modificação no sentido da diminuição em todas as amplitudes em M1. Nos sujeitos A e C, verificou-se que o stiffness do membro ipsilateral apresentou uma modificação no sentido da diminuição em M1 (em amplitudes intermédias). Nos sujeitos B, D e E o stiffness não apresentou modificações. O stiffness não variou com a velocidade. Conclusão: O stiffness apontou para uma diminuição, nos sujeitos em estudo no membro contralateral à lesão e no membro ipsilateral à lesão nos sujeitos A e C em amplitudes intermédias.

This study investigated the influence of wearing unstable shoe construction (WUS) on compensatory postural adjustments (CPA) associated with external perturbations. Thirty-two subjects stood on a force platform resisting an anterior-posterior horizontal force applied to a pelvic belt via a cable, which was suddenly released. They stood under two conditions: barefoot and WUS. The electromyographic (EMG) activity of gastrocnemius medialis, tibialis anterior, rectus femoris, biceps femoris, rectus abdominis, and erector spinae muscles and the center of pressure (CoP) displacement were acquired to study CPA. The EMG signal was used to assess individual muscle activity and latency, antagonist co-activation and reciprocal activation at joint and muscle group levels. Compared to barefoot, WUS led to: (1) increased gastrocnemius medialis activity, (2) increased total agonist activity, (3) decreased antagonist co-activation at the ankle joint and muscle group levels, (4) increased reciprocal activation at the ankle joint and muscle group levels, and (5) decrease in all muscle latencies. No differences were observed in CoP displacement between conditions. These findings demonstrate that WUS led to a reorganization of the postural control system associated to improved performance of some components of postural control responses.

Introdução:O Controlo Postural é um processo neural complexo envolvido na organização da estabilidade e orientação da posição do corpo no espaço. A Instabilidade Funcional (IF) do tornozelo é descrita como uma perceção subjetiva de instabilidade articular, que afeta o controlo postural. Apesar de vários estudos terem investigado os fatores inerentes à IF ainda existe inconsistência nos resultados da literatura sobre os mecanismos envolvidos nesta. Objetivo (s):avaliar os ajustes posturais envolvidos na resposta a uma perturbação externa realizada de forma previsível e imprevisível em indivíduos com IF. Métodos:Estudo observacional analítico transversal, teve uma amostra de 20 indivíduos, que foram divididos em grupo com IF e grupo de controlo. Foi recolhida atividade eletromiográfica bilateral dos músculos longo e curto peroneal (PL e PC), tibial anterior (TA) e solear (SOL) associado a uma perturbação externa aplicada de forma previsível e imprevisível. Os ajustes posturais foram avaliados através da análise do início da atividade muscular, da magnitude global dos ajustes posturais compensatórios e antecipatórios e magnitude das respostas de curta e média latência Resultados: Na perturbação imprevisível não se verificaram diferenças significativas no início da atividade muscular (p>0,05). Enquanto na magnitude das respostas de curta e média latência verificou-se diferenças nos músculos TA (Ia,p=0,000; II, p=0,011), CP (Ia,p=0,029; II, p=0,001) e LP (Ia, p=0,030) entre o membro com IF e o controlo e no LP (II, p=0,011) entre o membro sem IF do grupo com IF e o controlo. Na perturbação previsível observaram-se diferenças nos ajustes posturais antecipatórios (APA) dos músculos TA (p=0,006) e LP (p=0,020) entre o membro sem IF do grupo com IF e o controlo. Conclusão: Os indivíduos com IF apresentam défices na magnitude das respostas de média e curta latência numa perturbação imprevisível e nos APA na perturbação previsível.

INTRODUCTION: Ankle arthrodesis in an intervention that may be performed using a variety of surgical approaches. The joint may be stabilized using diferent systems, with or without bone graft. MATERIAL AND METHODS: Retrospective study that collected the results of 40 patients who underwent ankle arthrodesis; the complications that arose were analyzed, results according to the AOFAS clinical rating system, global patient satisfaction and the relationship with the articular approach route, the !xation system or the usage of grafts.RESULTS: 55% of the patients presented one or more complication that motivated a total of 12 re-interventions in 10 patients (excludes removal of osteosynthesis material). The absence of consolidation, which required a new arthrodesis was more frequent in patients where osteosynthesis was not used (two out of three cases) than in those using screws (four out of 24 cases), while there was no register of this in 13 patients in which intramedullary pin. The use of the distal extreme of the fibula or the iliac crest as bone graft has failed to improve the consolidation. Arthroscopic approaches avoid the appearance of soft tissue complications, although the consolidation disorders were more frequent. The average score on the AOFAS clinical rating system was 56.4±21.2 and 55% of the patients were satisfied. CONCLUSIONS: Although the case histories fail to provide significant results, it is a demanding intervention, with an elevated complication rate. Intramedullary pins seem to favor improved consolidation.

This study aims to design a wearable system for kinetics measurement of multi-segment foot joints in long-distance walking and to investigate its suitability for clinical evaluations. The wearable system consisted of inertial sensors (3D gyroscopes and 3D accelerometers) on toes, forefoot, hindfoot, and shank, and a plantar pressure insole. After calibration in a laboratory, 10 healthy elderly subjects and 12 patients with ankle osteoarthritis walked 50m twice wearing this system. Using inverse dynamics, 3D forces, moments, and power were calculated in the joint sections among toes, forefoot, hindfoot, and shank. Compared to those we previously estimated for a one-segment foot model, the sagittal and transverse moments and power in the ankle joint, as measured via multi-segment foot model, showed a normalized RMS difference of less than 11%, 14%, and 13%, respectively, for healthy subjects, and 13%, 15%, and 14%, for patients. Similar to our previous study, the coronal moments were not analyzed. Maxima-minima values of anterior-posterior and vertical force, sagittal moment, and power in shank-hindfoot and hindfoot-forefoot joints were significantly different between patients and healthy subjects. Except for power, the inter-subject repeatability of these parameters was CMC>0.90 for healthy subjects and CMC>0.70 for patients. Repeatability of these parameters was lower for the forefoot-toes joint. The proposed measurement system estimated multi-segment foot joints kinetics with acceptable repeatability but showed difference, compared to those previously estimated for the one-segment foot model. These parameters also could distinguish patients from healthy subjects. Thus, this system is suggested for outcome evaluations of foot treatments.

Atraumatic osteonecrosis of the talus can be extremely painful and lead to significant functional impairment. Although clinical, radiographic, and demographic characteristics of atraumatic osteonecrosis of the talus have been well documented, the diagnosis is frequently missed or delayed; the most common causes are use of corticosteroids and the presence of immune disorders. Operative treatment of large osteochondral lesions of the talus is difficult because the blood supply is poor in the talar dome. Microvascular reconstruction of the talar dome with iliac crest autografts is a complex but functionally excellent therapeutic option. We present a 48-year-old man, who developed an extensive atraumatic avascular necrosis of the talar dome without collapse. Except for insulin dependent diabetes mellitus no further comorbidities were known. A microvascular iliac crest bone flap was inserted into the talus. A follow-up 16 years postoperatively showed a clinically as well as radiographically stable reconstruction of the talar dome and an excellent mobility of the ankle joint. The AOFAS hindfoot scale had improved from initially 33 points to 100 on the last follow-up. Free microvascular bony reconstruction of the talar dome should not only be considered in younger patients but also for middle aged active patients, since our follow-up shows an excellent long term result. Early reconstruction can prevent collapse of the talar bone.

This paper is concerned with the design of robust feedback H~-control systems for the control of the upright posture of paraplegic persons standing. While the subject stands in a special apparatus, stabilising torque at the ankle joint is generated by electrical stimulation of the paralyzed calf muscles. Since the muscles acting as actuators in this setup show a significant degree of nonlinearity, a robust H~-control design is used. The design approach is implemented in experiments with a paraplegic subject. The results demonstrate good performance and closed loop stability over the whole range of operation.

Background: Down syndrome (DS) is a genetic alteration characterized by being a nonprogressive congenital encephalopathy. Children with DS have hypotonia and developmental delays that interfere in the movement`s acquisition for these children. Objective: Analyze the effects of treadmill inclination on angle and spatiotemporal gait characteristics of these individuals. Methodology: We studied 23 subjects of both sexes, with ages ranged between 05 and 11 years, they presented ability to walk on level 5 classified according to the Functional Ambulation Category (FAC). Initially held a subjective evaluation of balance through a questionnaire (Berg Balance Scale-BBS) then the kinematic gait analysis was realized on a treadmill first, without inclination and then, with inclination of 10%, using the motion system analysis Qualisys System. Data analysis was done using BioStat 5.0 attributing significance level of 5%. Normality of data was verified using D'Agostino test and later was applied paired t-test to compare data in two experimental conditions. Results: There was a statistically significant difference in the spatiotemporal variables: reduction in the cadence (from 108.92 ± 39.07 to 99.11 ± 27.51, p <0.04), increase in cycle time (from 1.24 ± 0.27 to 1.36 ± 0.34, p = 0.03 ) and increase in time to take stock (from 0.77 ± 0.15 to 0.82 ± 0.18, p <0.001). Angular variables that showed statistically significant increasing were: the hip in the initial contact (12.23 ± 4.63 to 18.49 ± 5.17, p <0.0001) and max. flexion in balance (12.96±4:32 to 19.50 ± 4.51, p <0.0001 ), knee in the initial contact (15.59 to ± 6.71 to 21.63 ± 6.48, p <0.0001), the ankle in the initial contact (-2.79 ± 9.8 to 2.25 ± 8.79, p <0.0001), max dorsiflexion in stance (4.41 ± 10.07 to 7.13 ± 11.58, p <0.0009), maximum plantar flexion in the pre-assessment of the ankle joint (increase of -6.33 ± 8.77 to -2.69 ± 8.62, p <0.0004).Conclusions: The inclination acts in a positive way for angular and spatiotemporal features gait of children with Down syndrome, demonstrating possible benefit of using this surface in the gait rehabilitation of children with Down Syndrome
