FUNDAMENTO: Estados congestivos podem ser identificados e manejados através de algoritmos como o Diuretic Treatment Algorithm (DTA) para ajuste de diurético por telefone, com enfoque na avaliação clínica. Porém, o DTA está disponível somente em língua inglesa. OBJETIVO: Adaptar o DTA e testar sua aplicabilidade para uso no Brasil em pacientes ambulatoriais com IC. MÉTODOS: Seguiram-se as etapas de tradução, síntese, retrotradução, avaliação por comitê de especialistas e pré-teste (aplicabilidade clínica por meio de ensaio clínico randomizado). O DTA foi denominado, na versão para o Brasil, algoritmo de ajuste de diurético (AAD). Os pacientes foram randomizados para grupo intervenção (GI) - ajuste de diurético conforme o AAD - ou grupo controle (GC) - ajuste convencional. Foi avaliado o escore clínico de congestão (ECC) e o peso para ambos os grupos. RESULTADOS: Foram realizadas 12 modificações no DTA. Incluíram-se 34 pacientes. Para aqueles congestos, o aumento de diurético guiado pelo AAD resultou em maior resolução da congestão, com redução de dois pontos no ECC para 50% da amostra -2 (-3,5; -1,0), enquanto a mediana para o GC foi 0 (-1,25; -1,0), (p < 0,001). A mediana de variação de peso foi maior no GI -1,4 (-1,7; -0,5) quando comparado ao GC 0,1 (1,2; -0,6), p = 0,001. CONCLUSÕES: O ADD mostrou-se aplicável na prática clínica após adaptação e parece resultar em melhor controle da congestão em pacientes com IC. A efetividade clínica da ferramenta merece ser testada em amostra maior de pacientes visando sua validação para uso no Brasil (Universal Trial Number: U1111-1130-5749) (Arq Bras Cardiol. 2013; [online]. ahead print, PP.0-0).

BOLD APS es un software diseñado para solventar el problema de la planificación de la producción. Como tal, cuenta con un algoritmo, en continuo desarrollo, cuya función es tomar las decisiones oportunas para obtener una buena programación de tareas. Este proyecto consta de dos fases: la principal comprende el diseño e implementación de una nueva sección dentro del algoritmo de planificación de la producción que utiliza la empresa Global Planning Solution, con el objetivo de ofrecer mejoras en la calidad de las soluciones actuales; la fase secundaria consiste en una labor de depuración, limpieza y ordenación del código, para facilitar su comprensión y posterior modificación.

BACKGROUND: The elderly population is particularly at risk for developing vitamin B12-deficiency. Serum cobalamin does not necessarily reflect a normal B12 status. The determination of methylmalonic acid is not available in all laboratories. Issues of sensitivity for holotranscobalamin and the low specificity of total homocysteine limit their utility. The aim of the present study is to establish a diagnostic algorithm by using a combination of these markers in place of a single measurement. METHODS: We compared the diagnostic efficiency of these markers for detection of vitamin B12 deficiency in a population (n = 218) of institutionalized elderly (median age 80 years). Biochemical, haematological and morphological data were used to categorize people with or without vitamin B12 deficiency. RESULTS: In receiver operating curves characteristics for detection on vitamin B12 deficiency using single measurements, serum folate has the greatest area under the curve (0.87) and homocysteine the lowest (0.67). The best specificity was observed for erythrocyte folate and methylmalonic acid (100% for both) but their sensitivity was very low (17% and 53%, respectively). The highest sensitivity was observed for homocysteine (81%) and serum folate (74%). When we combined these markers, starting with serum and erythrocyte folate, followed by holotranscobalamin and ending by methylmalonic acid measurements, the overall sensitivity and specificity of the algorithm were 100% and 90%, respectively. CONCLUSION: The proposed algorithm, which combines erythrocyte folate, serum folate, holotranscobalamin and methylmalonic acid, but eliminate B12 and tHcy measurements, is a useful alternative for vitamin B12 deficiency screening in an elderly institutionalized cohort.

The primary goal of this paper is to discuss how the leading position of Brazil in South America could contribute to boost security cooperation between the European Union and Mercosur. Both parties share common foreign and security policy concerns, including immigration, terrorism and drug trafficking. Through its great influence on the regional security agenda, Brazil could seek closer bilateral cooperation with Europe in tackling these global challenges, acting at the same time as a representative of regional interests.

Background: To enhance the induction of insert specific immune responses, a new generation of replication competent poxvirus vectors was designed and evaluated against non-replicating poxvirus vectors in a HIV vaccine study in non human primates.Methods: Rhesus macaques were immunized with either the non-replicating variant NYVAC-GagPolNef HIV-1 clade C or the replicating NYVAC-GagPolNef-C-KC, boosted with HIVGag- PolEnv-SLP and immune responses were monitored.Results: Gag-specific T-cell responses were only detected in animals immunized with the replicating NYVAC-GagPolNef-C-KC variant. Further enhancement and broadening of the immune response was studied by boosting the animals with novel T-cell immunogens HIVconsv synthetic long peptides (SLP), which direct vaccine-induced responses to the most conserved regions of HIV and contain both CD4 T-helper and CD8 CTL epitopes. The adjuvanted (Montanide ISA-720) SLP divided into subpools and delivered into anatomically separate sites enhanced the Gag-specific T-cell responses in 4 out of 6 animals, to more than 1000 SFC/106 PBMC in some animals. Furthermore, the SLP immunization broadened the immune response in 4 out of 6 animals to multiple Pol epitopes. Even Env-specific responses, to which the animals had not been primed, were induced by SLP in 2 out of 6 animals.Conclusion: This new immunization strategy of priming with replicating competent poxvirus NYVAC-HIVGagPolNef and boosting with HIVGagPolEnv-SLP, induced strong and broad Tcell responses and provides a promising new HIV vaccine approach. This study was performed within the Poxvirus T-cell Vaccine Discovery Consortium (PTVDC) which is part of the CAVD program.

The immunogenicity of influenza vaccine is suboptimal in lung transplant recipients. Use of a booster dose and vaccine delivery by the intradermal rather than intramuscular route may improve response. We prospectively evaluated the immunogenicity and safety of a 2-dose boosting strategy of influenza vaccine. Sixty lung transplant recipients received a standard intramuscular injection of the 2006-2007 inactivated influenza vaccine, followed 4 weeks later by an intradermal booster of the same vaccine. Immunogenicity was assessed by measurement of geometric mean titer of antibodies after both the intramuscular injection and the intradermal booster. Vaccine response was defined as 4-fold or higher increase of antibody titers to at least one vaccine antigen. Thirty-eight out of 60 patients (63%) had a response after intramuscular vaccination. Geometric mean titers increased for all three vaccine antigens following the first dose (p &lt; 0.001). However, no significant increases in titer were observed after the booster dose for all three antigens. Among nonresponders, 3/22 (13.6%) additional patients responded after the intradermal booster (p = 0.14). The use of basiliximab was associated with a positive response (p = 0.024). After a single standard dose of influenza vaccine, a booster dose given by intradermal injection did not significantly improve vaccine immunogenicity in lung transplant recipients.

Rhesus macaques (Macaca mulatta) have played a valuable role in the development of human immunodeficiency virus (HIV) vaccine candidates prior to human clinical trials. However, changes and/or improvements in immunogen quality in the good manufacturing practice (GMP) process or changes in adjuvants, schedule, route, dose, or readouts have compromised the direct comparison of T-cell responses between species. Here we report a comparative study in which T-cell responses from humans and macaques to HIV type 1 antigens (Gag, Pol, Nef, and Env) were induced by the same vaccine batches prepared under GMP and administered according to the same schedules in the absence and presence of priming. Priming with DNA (humans and macaques) or alphavirus (macaques) and boosting with NYVAC induced robust and broad antigen-specific responses, with highly similar Env-specific gamma interferon (IFN-gamma) enzyme-linked immunospot assay responses in rhesus monkeys and human volunteers. Persistent cytokine responses of antigen-specific CD4(+) and CD8(+) T cells of the central memory as well as the effector memory phenotype, capable of simultaneously eliciting multiple cytokines (IFN-gamma, interleukin 2, and tumor necrosis factor alpha), were induced. Responses were highly similar in humans and primates, confirming earlier data indicating that priming is essential for inducing robust NYVAC-boosted IFN-gamma T-cell responses. While significant similarities were observed in Env-specific responses in both species, differences were also observed with respect to responses to other HIV antigens. Future studies with other vaccines using identical lots, immunization schedules, and readouts will establish a broader data set of species similarities and differences with which increased confidence in predicting human responses may be achieved.

Testes de significância de contrastes de efeitos de níveis de um fator ou de combinações de níveis de dois ou mais fatores requerem o conhecimento das estimativas das variâncias dos estimadores desses contrastes. As expressões dessas estimativas para experimentos com delineamentos complexos, em geral, não são disponíveis em textos. Sua derivação algébrica a partir da equação do modelo estatístico é trabalhosa. Expõe-se um algoritmo prático para a obtenção da variância do estimador de qualquer contraste de parâmetros para delineamentos completos balanceados. O algoritmo baseia-se nos valores esperados dos quadrados médios.

Despite major progress in T lymphocyte analysis in melanoma patients, TCR repertoire selection and kinetics in response to tumor Ags remain largely unexplored. In this study, using a novel ex vivo molecular-based approach at the single-cell level, we identified a single, naturally primed T cell clone that dominated the human CD8(+) T cell response to the Melan-A/MART-1 Ag. The dominant clone expressed a high-avidity TCR to cognate tumor Ag, efficiently killed tumor cells, and prevailed in the differentiated effector-memory T lymphocyte compartment. TCR sequencing also revealed that this particular clone arose at least 1 year before vaccination, displayed long-term persistence, and efficient homing to metastases. Remarkably, during concomitant vaccination over 3.5 years, the frequency of the pre-existing clone progressively increased, reaching up to 2.5% of the circulating CD8 pool while its effector functions were enhanced. In parallel, the disease stabilized, but subsequently progressed with loss of Melan-A expression by melanoma cells. Collectively, combined ex vivo analysis of T cell differentiation and clonality revealed for the first time a strong expansion of a tumor Ag-specific human T cell clone, comparable to protective virus-specific T cells. The observed successful boosting by peptide vaccination support further development of immunotherapy by including strategies to overcome immune escape.

O objetivo deste trabalho foi avaliar o mapeamento de área de cana‑de‑açúcar por meio de série temporal, de seis anos de dados do índice de vegetação por diferença normalizada (NDVI), oriundos do sensor Vegetation, a bordo do satélite "système pour l'observation de la Terre" (SPOT). Três classes de cobertura do solo (cana‑de‑açúcar, pasto e floresta), do Estado de São Paulo, foram selecionadas como assinaturas espectro‑temporais de referência, que serviram como membros extremos ("endmembers") para classificação com o algoritmo "spectral angle mapper" (SAM). A partir desta classificação, o mapeamento da área de cana‑de‑açúcar foi realizado com uso de limiares na imagem-regra do SAM, gerados a partir dos valores dos espectros de referência. Os resultados mostram que o algoritmo SAM pode ser aplicado a séries de dados multitemporais de resolução moderada, o que permite eficiente mapeamento de alvo agrícola em escala mesorregional. Dados oficiais de áreas de cana‑de‑açúcar, para as microrregiões paulistas, apresentam boa correlação (r² = 0,8) com os dados obtidos pelo método avaliado. A aplicação do algoritmo SAM mostrou ser útil em análises temporais. As séries temporais de NDVI do sensor SPOT Vegetation podem ser utilizadas para mapeamento da área de cana‑de‑açúcar em baixa resolução.
