The objective of this research was to estimate (co) variance functions and genetic parameters for body weight in Colombian buffalo populations using random regression models with Legendre polynomials. Data consisted of 34,738 weight records from birth to 900 days of age from 7815 buffaloes. Fixed effects in the model were contemporary group and parity order of the mother. Random effects were direct and maternal additive genetic, as well as animal and maternal permanent environmental effects. A cubic orthogonal Legendre polynomial was used to model the mean curve of the population. Eleven models with first to sixth order polynomials were used to describe additive genetic direct and maternal effects, and animal and maternal permanent environmental effects. The residual was modeled considering five variance classes. The best model included fourth and sixth order polynomials for direct additive genetic and animal permanent environmental effects, respectively, and third-order polynomials for maternal genetic and maternal permanent environmental effects. The direct heritability increased from birth until 120 days of age (0.32 +/- 0.05), decreasing thereafter until one year of age (0.18 +/- 0.04) and increased again, reaching 0.39 +/- 0.09, at the end of the evaluated period. The highest maternal heritability estimates (0.11 +/- 0.05), were obtained for weights around weaning age (weaning age range is between 8 and 9.5 months). Maternal genetic and maternal permanent environmental variances increased from birth until about one year of age, decreasing at later ages. Direct genetic correlations ranged from moderate (0.60 +/- 0.060) to high (0.99 +/- 0.001), maternal genetic correlations showed a similar range (0.41 +/- 0.401 and 0.99 +/- 0.003), and all of them decreased as time between weighings increased. Direct genetic correlations suggested that selecting buffalos for heavier weights at any age would increase weights from birth through 900 days of age. However, higher heritabilities for direct genetic weights effects after 600 days of age suggested that selection for these effects would be more effective if done during this age period. A greater response to selection for maternal ability would be expected if selection used maternal genetic predictions for weights near weaning. (C) 2013 Elsevier B.V. All rights reserved.

The sexual abuse suffered in childhood and adolescence, in addition to damage to physical and psychological health of the victim, is considered as an important risk factor for alcohol and drugs addiction, development of psychopathology and psychosocial damage in adulthood. In addition to the pain and humiliation that are submitted by the abuse, children and adolescents also experience shame and guilt which require them to adopt coping strategies to endure those feelings. The use of psychoactive substances is a recognized way of dealing with the pains of living. This work, which is of narrative style, analyses and discusses, through five case reports, chemical dependency as a result of sexual abuse suffered in childhood and/or adolescence. The eight subjects in this study are male and have suffered sexual violence in this age period of life. Their ages range from 23 years to 39 years, and all are admitted to a therapeutic community in a city in the interior of Sao Paulo state, in Brazil, for treatment of chemical dependency, being met by the Department of Psychology. The reasons for the choice of the participants for treatment modality for patients are: difficult to stop using drugs, even unwilling to take it, they have easy access to it; the feeling of losing control over their lives; by successive losses as a result of drug use, and for fear that their lives had a tragic ending. With the exception of two participants, the others do not classify that as a child suffered sexual violence. However, all attribute that facilitated their entry into the world of drugs. Seven participants experienced such violence in childhood (between 7 years and 9 years) and adolescence (age 14). The attackers were people closed to the victims—in the case of two victims, their families, with the exception of one participant who was raped by a stranger. Six participants declared themselves as homosexual. Another participant does not claim to be homosexual, but presents difficulties in terms of sexuality. Two participants are HIV positive. The start of psychoactive substances use occurred during adolescence (12 years to 17 years). The participants see drugs as an anesthetic to the pain of the soul, a way to get pleasure, but they get charged expensively, as it increases the feeling of emptiness, guilt, helplessness, worthlessness and hopelessness. Although participants have sought help to deal with addiction, it is noted that throughout the life course the issue of sexual violence was not treated. It was noted that the patients have a double stigma in society: the issue of drugs addiction and the orientation of sexual desire, because the majority of participants are homosexual. The results reinforce the need for effective action geared to accommodate the victims of sexual violence and effective preventive measures to prevent children and adolescents from being abused.

The authors examined the development of self-esteem across the life span. Data came from a German longitudinal study with 3 assessments across 4 years of a sample of 2,509 individuals ages 14 to 89 years. The self-esteem measure used showed strong measurement invariance across assessments and birth cohorts. Latent growth curve analyses indicated that self-esteem follows a quadratic trajectory across the life span, increasing during adolescence, young adulthood, and middle adulthood, reaching a peak at age 60 years, and then declining in old age. No cohort effects on average levels of self-esteem or on the shape of the trajectory were found. Moreover, the trajectory did not differ across gender, level of education, or for individuals who had lived continuously in West versus East Germany (i.e., the 2 parts of Germany that had been separate states from 1949 to 1990). However, the results suggested that employment status, household income, and satisfaction in the domains of work, relationships, and health contribute to a more positive life span trajectory of self-esteem. The findings have significant implications, because they call attention to developmental stages in which individuals may be vulnerable because of low self-esteem (such as adolescence and old age) and to factors that predict successful versus problematic developmental trajectories.

As the obesity epidemic continues to increase, the pediatric primary care office setting remains a relatively unexplored arena to offer obesity prevention interventions for children. The increased risk for adult obesity among 10 to 14 year-old children who are overweight, suggests obesity prevention programs should be introduced just before this age or early in this age period. Research is also accumulating on the importance of targeting parents along with children, since parents are in charge of the home environment for children. Therefore, the aim of this project was to develop an obesity prevention program called Helping HAND (Healthy Activity and Nutrition Directions) based on Social Cognitive Theory and authoritative parenting techniques for the pediatric primary care setting and conduct one-on-one interviews with parents as the initial formative evaluation of the intervention material for the obesity prevention intervention. A secondary aim of the project was to determine the feasibility of identifying appropriate subjects for the intervention, and conducting qualitative evaluations of the materials through recruitment through pediatric primary care settings. ^

The growth patterns of weight from birth through the first twelve months of life among rural Taiwanese infants were investigated with the following objectives: (i) compare each of the parameters of the Count model estimated for infants who were nutritionally at risk with those for a reference population from the United States; and (ii) within the Taiwanese infants, account for the variance in the growth patterns in the first and second six months of life on the basis of selected ecological factors.^ The significance between group differences were observed in the patterns of the weight growth in both linear growth and in the timing and the direction of velocity changes. A significant decline in growth velocity was observed among Taiwanese infants at about the fourth month of life. The decline is in keeping with a recent proposal made by J. C. Waterlow regarding the timing of change in growth velocity among nutritionally at risk populations in developing countries. The growth course of a nutritionally at risk infant during the first three months is apparently protected by the nurturance of the mother and innate biological properties of the infant.^ A highly significant portion of the growth variance in the second six months of life was accounted for by exogenous factors and biological factors related to the infant. Conversely, none of the growth variance in the first six months of life was accounted for by predictor variables. The most potent determinant of growth in the second six months of life was seasonality which represents a multiple environmental event.^ The model parameters estimated from the Count model represent different aspect of physical growth; yet the correlation coefficients between parameters b and c are high (r > .80). Clearly, the biological interpretation of the model parameters requires analysis of the whole function in the specific context of a given age period. ^

Because of its simplicity and low cost, arm circumference (AC) is being used increasingly in screening for protein energy malnutrition among pre-school children in many parts of the developing world, especially where minimally trained health workers are employed. The objectives of this study were as follows: (1) To determine the relationship of the AC measure with weight for age and weight for height in the detection of malnutrition among pre-school children in a Guatemalan Indian village. (2) To determine the performance of minimally trained promoters under field conditions in measuring AC, weight and height. (3) To describe the practical aspects of taking AC measures versus weight, age and height.^ The study was conducted in San Pablo La Laguna, one of four villages situated on the shores of Lake Atitlan, Guatemala, in which a program of simplified medical care was implemented by the Institute for Nutrition for Central America and Panama (INCAP). Weight, height, AC and age data were collected for 144 chronically malnourished children. The measurements obtained by the trained investigator under the controlled conditions of the health post were correlated against one another and AC was found to have a correlation with weight for age of 0.7127 and with weight for height of 0.7911, both well within the 0.65 to 0.80 range reported in the literature. False positive and false negative analysis showed that AC was more sensitive when compared with weight for height than with weight for age. This was fortunate since, especially in areas with widespread chronic malnutrition, weight for height detects those acute cases in immediate danger of complicating illness or death. Moreover, most of the cases identified as malnourished by AC, but not by weight for height (false positives), were either young or very stunted which made their selection by AC better than weight for height. The large number of cases detected by weight for age, but not by AC (false negative rate--40%) were, however, mostly beyond the critical age period and had normal weight for heights.^ The performance of AC, weight for height and weight for age under field conditions in the hands of minimally trained health workers was also analyzed by correlating these measurements against the same criterion measurements taken under ideally controlled conditions of the health post. AC had the highest correlation with itself indicating that it deteriorated the least in the move to the field. Moreover, there was a high correlation between AC in the field and criterion weight for height (0.7509); this correlation was almost as high as that for field weight for height versus the same measure in the health post (0.7588). The implication is that field errors are so great for the compounded weight for height variable that, in the field, AC is about as good a predictor of the ideal weight for height measure.^ Minimally trained health workers made more errors than the investigator as exemplified by their lower intra-observer correlation coefficients. They consistently measured larger than the investigator for all measures. Also there was a great deal of variability between these minimally trained workers indicating that careful training and followup is necessary for the success of the AC measure.^ AC has many practical advantages compared to the other anthropometric tools. It does not require age data, which are often unreliable in these settings, and does not require sophisticated subtraction and two dimensional table-handling skills that weight for age and weight for height require. The measure is also more easily applied with less disturbance to the child and the community. The AC tape is cheap and not easily damaged or jarred out of calibration while being transported in rugged settings, as is often the case with weight scales. Moreover, it can be kept in a health worker's pocket at all times for continual use in a widespread range of settings. ^

Este trabajo constituye un estudio sobre el humor verbal en niños y niñas de 4.º curso de Educación Primaria (9-10 años). Tres grandes vías articulan el análisis. En primer lugar, el debate sobre las presuntas diferencias de género en la interpretación y el uso del humor verbal obtiene un interés especial en el caso de los niños, dado su menor nivel de «contaminación» por las convenciones y prejuicios sociales. En segundo lugar, se detecta la práctica ausencia de estudios sobre el humor infantil desde el punto de vista lingüístico, ya que el grueso de las investigaciones sobre el tema proviene de la psicología evolutiva. En tercer lugar, la etapa de edad objeto de este estudio responde a un periodo crucial en el desarrollo lingüístico y, especialmente, metalingüístico del niño.

S’hi presenten les recerques realitzades a l’assentament fortificat ibèric del Cabeçó de Mariola (Alfafara, Alacant; Bocairent, València). S’hi han realitzat prospeccions superficials, geofísica i sondejos arqueològics assistits amb tecnologies geoespacials, a més de la inserció del lloc en el seu entorn territorial mitjançant els SIG. Els resultats mostren una àmplia seqüència d’ocupació, datada entre els segles IX i I aC, d’un important centre fortificat que controla un pas estratègic per a les comunicacions comarcals. Durant el període Ibèric tardà, en els segles II-I, presenta una intensa reestructuració de l’habitatge i les fortificacions que acabarà amb una violenta destrucció en el primer terç del s. I aC.

Incessant ovulation is thought to be one of the primary causes of epithelial ovarian cancer. However, the effects of ovulation at different ages and of the various exposures or events that suppress ovulation have not been established. We used data from an Australian case-control study of 791 ovarian cancer cases and 853 controls to examine the effect of ovulation on ovarian cancer risk. The total number of lifetime ovulations was calculated using information provided in a monthly contraceptive/reproductive calendar, as well as incorporating other information such as average menstrual cycle length. An increase of I year's worth of ovulation was associated with a 6% increase in risk of ovarian cancer (95% confidence interval [CI] = 4-8%). Ovulations in the 20-29-year age group were associated with the greatest risk, with a 20% increase in risk associated with each year of ovulation during this age period (95% Cl = 13-26%). When the effects of different exposures that suppress ovulation were compared, there was an indication that some factors may have a greater effect than others. These findings support the theory that incessant ovulation is a major contributor to the occurrence of ovarian cancer and suggest that ovulations during the 20s may be those most associated with disease risk. OBJECTIVE: The Thrombolysis in Myocardial Infarction (TIMI) score is a validated tool for risk stratification of acute coronary syndrome. We hypothesized that the TIMI risk score would be able to risk stratify patients in observation unit for acute coronary syndrome. METHODS: STUDY DESIGN: Retrospective cohort study of consecutive adult patients placed in an urban academic hospital emergency department observation unit with an average annual census of 65,000 between 2004 and 2007. Exclusion criteria included elevated initial cardiac biomarkers, ST segment changes on ECG, unstable vital signs, or unstable arrhythmias. A composite of significant coronary artery disease (CAD) indicators, including diagnosis of myocardial infarction, percutaneous coronary intervention, coronary artery bypass surgery, or death within 30 days and 1 year, were abstracted via chart review and financial record query. The entire cohort was stratified by TIMI risk scores (0-7) and composite event rates with 95% confidence interval were calculated. RESULTS: In total 2228 patients were analyzed. Average age was 54.5 years, 42.0% were male. The overall median TIMI risk score was 1. Eighty (3.6%) patients had 30-day and 119 (5.3%) had 1-year CAD indicators. There was a trend toward increasing rate of composite CAD indicators at 30 days and 1 year with increasing TIMI score, ranging from a 1.2% event rate at 30 days and 1.9% at 1 year for TIMI score of 0 and 12.5% at 30 days and 21.4% at 1 year for TIMI ≥ 4. CONCLUSIONS: In an observation unit cohort, the TIMI risk score is able to risk stratify patients into low-, moderate-, and high-risk groups.

A multiproxy data set of an AMS radiocarbon dated 46 cm long sediment core from the continental margin off western Svalbard reveals multidecadal climatic variability during the past two millennia. Investigation of planktic and benthic stable isotopes, planktic foraminiferal fauna, and lithogenic parameters aims to unveil the Atlantic Water advection to the eastern Fram Strait by intensity, temperatures, and salinities. Atlantic Water has been continuously present at the site over the last 2,000 years. Superimposed on the increase in sea ice/icebergs, a strengthened intensity of Atlantic Water inflow and seasonal ice-free conditions were detected at ~ 1000 to 1200 AD, during the well-known Medieval Climate Anomaly (MCA). However, temperatures of the MCA never exceeded those of the 20th century. Since ~ 1400 AD significantly higher portions of ice rafted debris and high planktic foraminifer fluxes suggest that the site was located in the region of a seasonal highly fluctuating sea ice margin. A sharp reduction in planktic foraminifer fluxes around 800 AD and after 1730 AD indicates cool summer conditions with major influence of sea ice/icebergs. High amounts of the subpolar planktic foraminifer species Turborotalia quinqueloba in size fraction 150-250 µm indicate strengthened Atlantic Water inflow to the eastern Fram Strait already after ~ 1860 AD. Nevertheless surface conditions stayed cold well into the 20th century indicated by low planktic foraminiferal fluxes. Most likely at the beginning of the 20th century, cold conditions of the terminating Little Ice Age period persisted at the surface whereas warm and saline Atlantic Water already strengthened, hereby subsiding below the cold upper mixed layer. Surface sediments with high abundances of subpolar planktic foraminifers indicate a strong inflow of Atlantic Water providing seasonal ice-free conditions in the eastern Fram Strait during the last few decades.

Principal Findings: Over the period of 35 years, the risk of hospitalization for cardiovascular diseases and respiratory diseases decreased. Hospitalization for musculoskeletal diseases increased whereas mental and behavioral hospitalizations slightly decreased. The risk of cancer hospitalization decreased marginally in men, whereas in women an upward trend was observed.

Conclusions/Significance: A considerable health transition related to hospitalizations and a shift in the utilization of health care services of working-age men and women took place in Finland between 1976 and 2010.

Background: The health transition theory argues that societal changes produce proportional changes in causes of disability and death. The aim of this study was to identify long-term changes in main causes of hospitalization in working-age population within a nation that has experienced considerable societal change.

Methodology: National trends in all-cause hospitalization and hospitalizations for the five main diagnostic categories were investigated in the data obtained from the Finnish Hospital Discharge Register. The seven-cohort sample covered the period from 1976 to 2010 and consisted of 3,769,356 randomly selected Finnish residents, each cohort representing 25% sample of population aged 18 to 64 years.

Background While survival rates of extremely preterm infants have improved over the last decades, the incidence of neurodevelopmental disability (ND) in survivors remains high. Representative current data on the severity of disability and of risk factors associated with poor outcome in this growing population are necessary for clinical guidance and parent counselling. Methods Prospective longitudinal multicentre cohort study of preterm infants born in Switzerland between 240/7 and 276/7 weeks gestational age during 2000–2008. Mortality, adverse outcome (death or severe ND) at two years, and predictors for poor outcome were analysed using multilevel multivariate logistic regression. Neurodevelopment was assessed using Bayley Scales of Infant Development II. Cerebral palsy was graded after the Gross Motor Function Classification System. Results Of 1266 live born infants, 422 (33%) died. Follow-up information was available for 684 (81%) survivors: 440 (64%) showed favourable outcome, 166 (24%) moderate ND, and 78 (11%) severe ND. At birth, lower gestational age, intrauterine growth restriction and absence of antenatal corticosteroids were associated with mortality and adverse outcome (p < 0.001). At 360/7 weeks postmenstrual age, bronchopulmonary dysplasia, major brain injury and retinopathy of prematurity were the main predictors for adverse outcome (p < 0.05). Survival without moderate or severe ND increased from 27% to 39% during the observation period (p = 0.02). Conclusions In this recent Swiss national cohort study of extremely preterm infants, neonatal mortality was determined by gestational age, birth weight, and antenatal corticosteroids while neurodevelopmental outcome was determined by the major neonatal morbidities. We observed an increase of survival without moderate or severe disability.

OBJECTIVE: To investigate adaptive changes in bone and muscle parameters in the paralysed limbs after detraining or reduced functional electrical stimulation (FES) induced cycling following high-volume FES-cycling in chronic spinal cord injury. SUBJECTS: Five subjects with motor-sensory complete spinal cord injury (age 38.6 years, lesion duration 11.4 years) were included. Four subjects stopped FES-cycling completely after the training phase whereas one continued reduced FES-cycling (2-3 times/week, for 30 min). METHODS: Bone and muscle parameters were assessed in the legs using peripheral quantitative computed tomography at 6 and 12 months after cessation of high-volume FES-cycling. RESULTS: Gains achieved in the distal femur by high-volume FES-cycling were partly maintained at one year of detraining: 73.0% in trabecular bone mineral density, 63.8% in total bone mineral density, 59.4% in bone mineral content and 22.1% in muscle cross-sectional area in the thigh. The subject who continued reduced FES-cycling maintained 96.2% and 95.0% of the previous gain in total and trabecular bone mineral density, and 98.5% in muscle cross-sectional area. CONCLUSION: Bone and muscle benefits achieved by one year of high-volume FES-cycling are partly preserved after 12 months of detraining, whereas reduced cycling maintains bone and muscle mass gained. This suggests that high-volume FES-cycling has clinical relevance for at least one year after detraining.
