On the whole research has concluded that most young people navigate adolescence with few difficulties, but a minority do find this stage of life challenging, and their ways of coping may have negative consequences for both themselves and others. For some the experience of adversity in earlier childhood, and the cumulative impact of multiple adversities has the potential to compromise a young person’s natural resilience. In some cases this may result in an adolescent feeling overwhelmed, and dying through suicide or other high risk behaviours. This article explores these issues with reference to eight young people who died by suicide in Northern Ireland. The article argues that young people exposed to multiple adversities in childhood require earlier, more sustained and better co-ordinated services from professionals if their psychological and emotional well-being is to be promoted.

The renewed concern in assessing risks and consequences from technological hazards in industrial and urban areas continues emphasizing the development of local-scale consequence analysis (CA) modelling tools able to predict shortterm pollution episodes and exposure effects on humans and the environment in case of accident with hazardous gases (hazmat). In this context, the main objective of this thesis is the development and validation of the EFfects of Released Hazardous gAses (EFRHA) model. This modelling tool is designed to simulate the outflow and atmospheric dispersion of heavy and passive hazmat gases in complex and build-up areas, and to estimate the exposure consequences of short-term pollution episodes in accordance to regulatory/safety threshold limits. Five main modules comprising up-to-date methods constitute the model: meteorological, terrain, source term, dispersion, and effects modules. Different initial physical states accident scenarios can be examined. Considered the main core of the developed tool, the dispersion module comprises a shallow layer modelling approach capable to account the main influence of obstacles during the hazmat gas dispersion phenomena. Model validation includes qualitative and quantitative analyses of main outputs by the comparison of modelled results against measurements and/or modelled databases. The preliminary analysis of meteorological and source term modules against modelled outputs from extensively validated models shows the consistent description of ambient conditions and the variation of the hazmat gas release. Dispersion is compared against measurements observations in obstructed and unobstructed areas for different release and dispersion scenarios. From the performance validation exercise, acceptable agreement was obtained, showing the reasonable numerical representation of measured features. In general, quality metrics are within or close to the acceptance limits recommended for ‘non-CFD models’, demonstrating its capability to reasonably predict hazmat gases accidental release and atmospheric dispersion in industrial and urban areas. EFRHA model was also applied to a particular case study, the Estarreja Chemical Complex (ECC), for a set of accidental release scenarios within a CA scope. The results show the magnitude of potential effects on the surrounding populated area and influence of the type of accident and the environment on the main outputs. Overall the present thesis shows that EFRHA model can be used as a straightforward tool to support CA studies in the scope of training and planning, but also, to support decision and emergency response in case of hazmat gases accidental release in industrial and built-up areas.

Benzodiazepines are group of drugs used mainly as sedatives, hypnotics, muscle relaxants, and anti-epileptics. Tapering off benzodiazepines is, for some users, a painful, traumatic, and protracted process. In this article, I use an autoethnographic approach, adopting the metaphor of water, to examine heuristically my experience of iatrogenic illness and recovery. I draw on personal journals and blog entries and former users’ narratives to consider the particular form of biographical disruption associated with benzodiazepines and the processes involved in identity reconstruction. I emphasize the role of the online community in providing benzodiazepine users such as myself with a co-cultural community through which to share a voice and make sense of our experiences. I explain how the success stories of former users provided me with the hope that I, the “medical victim,” could become the “victor” and in the process construct a new life and fresh identity.

El Contrato de Seguro siendo una relación de dos personas en la que prevalece la buena fe de las partes por excepción una de ellas y por lo general la contratante altera la realidad del hecho fáctico con el objeto de que opere a su favor los beneficios a titulo de indemnizaciones. La historia de este contrato se ha visto empañada por sutilezas hasta grotescas falsificaciones de documentos de salud para lograr sus indemnizaciones. Este trabajo analiza a plenitud estas subjetividades que por ser dolosas inciden en el campo penal; en consecuencia los cuatro capítulos que conlleva esta tesis están orientados a analizar en derecho como recoge nuestra legislación positiva estos dolos, para finalizar analizando un caso concreto en el que convergen todas las artimañas de que se vale el contratado-beneficiario, para lograr el fin indemnizatorio en detrimento de los intereses y buena fe de la aseguradora. En el caso fáctico observamos que si existe vacío de ley en materia de seguros, concluimos que la agudeza de la parte contratante-beneficiario ha detectado este vacío y utilizando últimas técnicas obituarias destruyen la evidencia, dejando a la interpretación mas o menos legal las evidencias recabadas oportunamente por una parte procesal frente a la decidía total y absoluta de la otra.

Estuaries are coastal environments ephemeral life in geological time, derived from the drowning of the shoreline as a function of elevation relative sea level. Such parallel systems is characterized by having two sources of sediment, the river and the sea. The study area comprises the Acu River estuary, located on the northern coast of Rio Grande do Norte State, in a region of intense economic activity, mainly focused on the exploration of oil onshore and offshore, likely to accidental spills. In the oil sector are developed for salt production, shrimp farming, agriculture, fisheries and tourism, which by interacting with sensitive ecosystems, such as estuaries, may alter the natural conditions, thus making it an area susceptible to contamination is essential in understanding the morphodynamic variables that occur in this environment to obtain an environmental license. Information about the submarine relief the estuaries are of great importance for the planning of the activity of environmental monitoring, development and coastal systems, among others, allowing an easy management of risk areas, and assist in the creation of thematic maps of the main aspects of landscape. Morphodynamic studies were performed in this estuary in different seasonal periods in 2009 to observe and quantify morphological changes that have occurred and relate these to the hydrodynamic forcing from the river and its interaction with the tides. Thus, efforts in this area is possible to know the bottom morphology through records of good quality equipment acquired by high resolution geophysical (side-scan sonar and profiler current by doppler effect). The combination of these data enabled the identification of different forms of bed for the winter and summer that were framed in a lower flow regime and later may have been destroyed or modified forms of generating fund scheme than the number according Froude, with different characteristics due mainly to the variation of the depth and type of sedimentary material they are made, and other hydrodynamic parameters. Thus, these features background regions are printed in the channel, sandy banks and muddy plains that border the entire area

Ingestion of a foreign object, including a dental object, can lead to a trip to the emergency room. This article describes the accidental swallowing of a key that was used to activate a rapid maxillary expander. An orthodontic patient swallowed the key while trying to activate the appliance at home. The object's trajectory was followed on radiographs until it was eliminated. Possible clinical complications, legal implications of this situation, and practices for prevention are described. (Am J Orthod Dentofacial Orthop 2011;140:266-8)

This report describes a protocol for managing the accidental swallowing of dental instruments in implant dentistry, illustrated by a patient who accidentally swallowed a hexagonal wrench. The first step was to refer the patient to the medical emergency hospital service for radiographic and clinical evaluation. The hexagonal wrench was located in the stomach and was immediately removed with an endoscopic procedure. The gastric mucosa was sampled via biopsy and the sample submitted to the urease test, which was positive for Helicobacter pylori. Triple treatment was instituted for gastritis caused by H pylori to avoid exposing the patient to unnecessary risk. Removal of a foreign body by means of an endoscopic procedure constitutes a safe and effective treatment.
