BACKGROUND: Antibiotics are frequently prescribed for older adults who reside in long-term care facilities. A substantial proportion of antibiotic use in this setting is inappropriate. Antibiotics are often prescribed for asymptomatic bacteriuria, a condition for which randomized trials of antibiotic therapy indicate no benefit and in fact harm. This proposal describes a randomized trial of diagnostic and therapeutic algorithms to reduce the use of antibiotics in residents of long-term care facilities. METHODS: In this on-going study, 22 nursing homes have been randomized to either use of algorithms (11 nursing homes) or to usual practise (11 nursing homes). The algorithms describe signs and symptoms for which it would be appropriate to send urine cultures or to prescribe antibiotics. The algorithms are introduced by inservicing nursing staff and by conducting one-on-one sessions for physicians using case-scenarios. The primary outcome of the study is courses of antibiotics per 1000 resident days. Secondary outcomes include urine cultures sent and antibiotic courses for urinary indications. Focus groups and semi-structured interviews with key informants will be used to assess the process of implementation and to identify key factors for sustainability.

Introduction
PET-computed tomography (PET-CT) is a useful staging imaging modality in colorectal liver metastases (CRLM). This study aimed to determine whether PET-CT parameters, standardized uptake value (SUV) and reconstructed tumour volume (RTV), are predictors of prognosis and survival.
Methods
A study of all resectable CRLM patients in the regional HPB unit from 2007–2009 was performed. Preoperative PET-CT scans were retrospectively reviewed; SUV, diameter and RTV for each lesion was recorded. Correlation analysis was performed with other pathological and biochemical parameters, by Pearson’s correlation analysis. Survival analysis was performed using Cox regression hazard model. A P value of less than 0.05 was considered statistically significant.
Results
A total of 79 patients were included. SUV moderately correlated with tumour diameter, both PET-CT (r=0.4927; P<0.0001) and histology (r=0.4513; P=0.0003); RTV (r=0.4489; P<0.001), preoperative carcinoembryonic antigen (CEA) (r=0.4977; P=0.0001), and postoperative CEA (r=0.3727; P=0.004). Multivariate analysis found that an independent predictor of SUVmax was preoperative CEA (P=0.03). RTV strongly correlated with preoperative CEA (r=0.9389; P<0.0001). SUV and RTV had a negative effect on survival.
Conclusion
PET-CT, in the setting of CRLM, may have a prognostic role in assessing survival. Although no definite conclusions can be drawn regarding the prognostic role of SUV and RTV, it acts to reinforce the need for further prospective studies to validate these findings.

[CoCl(-Cl)(Hpz(Ph))(3)](2) (1) and [CoCl2(Hpz(Ph))(4)] (2) were obtained by reaction of CoCl2 with HC(pz(Ph))(3) and Hpz(Ph), respectively (Hpz(Ph)=3-phenylpyrazole). The compounds were isolated as air-stable solids and fully characterized by IR and far-IR spectroscopy, MS(ESI+/-), elemental analysis, cyclic voltammetry (CV), controlled potential electrolysis, and single-crystal X-ray diffraction. Electrochemical studies showed that 1 and 2 undergo single-electron irreversible (CoCoIII)-Co-II oxidations and (CoCoI)-Co-II reductions at potentials measured by CV, which also allowed, in the case of dinuclear complex 1, the detection of electronic communication between the Co centers through the chloride bridging ligands. The electrochemical behavior of models of 1 and 2 were also investigated by density functional theory (DFT) methods, which indicated that the vertical oxidation of 1 and 2 (that before structural relaxation) affects mostly the chloride and pyrazolyl ligands, whereas adiabatic oxidation (that after the geometry relaxation) and reduction are mostly metal centered. Compounds 1 and 2 and, for comparative purposes, other related scorpionate and pyrazole cobalt complexes, exhibit catalytic activity for the peroxidative oxidation of cyclohexane to cyclohexanol and cyclohexanone under mild conditions (room temperature, aqueous H2O2). Insitu X-ray absorption spectroscopy studies indicated that the species derived from complexes 1 and 2 during the oxidation of cyclohexane (i.e., Ox-1 and Ox-2, respectively) are analogous and contain a Co-III site. Complex 2 showed low invitro cytotoxicity toward the HCT116 colorectal carcinoma and MCF7 breast adenocarcinoma cell lines.

This work deals with the numerical simulation of air stripping process for the pre-treatment of groundwater used in human consumption. The model established in steady state presents an exponential solution that is used, together with the Tau Method, to get a spectral approach of the solution of the system of partial differential equations associated to the model in transient state.

Le Popol Vuh, récit historique du peuple maya quiché, a été traduit des dizaines de fois. Jusqu’au milieu du 20e siècle, bon nombre de ces traductions se fondaient sur la version réalisée en 1861 par Brasseur de Bourbourg, un missionnaire français. Pour souligner le travail du traducteur, nous avons étudié sa traduction non pas d’un point de vue comparatif des deux textes, mais du point de vue du paratexte, c’est-à-dire ce qui entoure le texte (page de titre, préface, notes, illustrations, etc.). Pour ce faire, nous avons dressé le cadre théorique du paratexte à l’appui des écrits de Genette et de Lane, puis nous l’avons appliqué à celui de la traduction du Popol Vuh de Brasseur de Bourbourg. D’une taille colossale, ce paratexte nous renseigne sur ce qui a motivé le travail du traducteur et sur ce qu’il a fait. L’étude de son avant-propos nous indique clairement que son but est de faire connaître la culture des Amériques sous un jour nouveau, et le Popol Vuh est pour lui l’exemple parfait d’une richesse littéraire, historique et culturelle jusque-là largement ignorée. Cette partie du paratexte de Brasseur de Bourbourg nous prépare à la lecture, alors que les nombreuses notes de bas de page nous guident pendant celle-ci. Force est toutefois d’admettre que le paratexte de cet ouvrage est si imposant qu’il porte ombrage à la traduction. Bref, l’étude du paratexte nous amène à aborder la traduction de Brasseur de Bourbourg d’un oeil critique, en fonction de ce que nous dit le paratexte. La lecture du paratexte et la connaissance de ses tenants et aboutissants devraient donc faire d’un simple lecteur un véritable lecteur averti, qu’il s’agisse d’une traduction ou de tout autre texte.

Plants and microorganisms provide the pharmaceutical industry with some of the most important sources of components for the research of new medications This thesis involves the study of three medicinal plants belonging to three different important families viz, Cyperus rotundus (Cyperaceae), Stereospermum colais (Bignoniaceae) as well as the well known medicinal plant Zingiber officinale (Zingiberaceae) as the third. The first chapter gives an overview of biologically active natural products with special reference to antioxidant, antidiabetic, anti-inflammatory and antimicrobial molecules from terrestrial sources. Chapter 2 of the thesis deals with the isolation of phytochemical constituents of the medicinal plant Cyperus rotundus and its antioxidant and radical scavenging potential. Chapter 3 of the thesis describes the studies on the roots of Stereospermum colais, A Bignoniaceae plant belonging to the genus Stereospermum which is used extensively. Chapter 4 describes the biological potential of rhizomes of Zingiber officinale. Ethyl acetate extract of ginger (EAG) possessed antioxidant activity as is evident from the results of various in vitro assays compared to other extracts .In conclusion, medicinal plants Cyperus rotundus and Stereospermum colais have been analysed for their phytochemical constituents. Also, the positive results obtained from biological activity studies such as antioxidant, anti-inflammatory and antimicrobial activity on the isolated compounds/extracts add on to the medicinal properties of these plants. Apart from that, ethyl acetate extract of Zingiber officinale (ginger) rhizomes has been shown to have very good biological potential including glucose lowering and adipocyte differentiation inhibitory effect.
