Pesquisou-se a frequência da ocorrência de anticorpos anti-Brucella abortus, Toxoplasma gondii e vírus da artrite encefalite caprina (CAEV) em caprinos de 14 unidades produtoras localizadas dos Estados do Pará e Maranhão. No Estado do Pará foram analisados animais dos municípios de Benevides, Castanhal, Santa Izabel do Pará e Moju e no Estado do Maranhão, o município de Chapadinha. Os testes sorológicos realizados para o diagnóstico da brucelose foi o teste do Antígeno Acidificado Tamponado (AAT), como teste de triagem, e o 2- Mercaptoetanol (2-Me), como teste confirmatório. Para as análises de toxoplasmose foi utilizado a Reação de Imunofluorescência Indireta (RIFI) e para CAEV Imunodifusão de Gel de Agarose (IDGA). O resultado das análises de brucelose mostrou-se negativo para 100,0% das amostras analisadas. Para toxoplasmose e CAEV a frequência obtida foi 23,5% (97/412) e 21,6% (85/393), respectivamente. Foi observada diferença estatística na relação entre a ocorrência de anticorpos anti-Toxoplasma gondii e a faixa etária dos caprinos, mostrando que animais com idade superior a 24 meses tiveram mais risco de estarem infectados quando comparados com animais mais novos OR= 2,15 (IC 95% 1,19 – 3,88). Já os fatores de risco encontrados para CAEV foram: falta de conhecimento da doença OR=6,45 (IC 95% 2,88-14,47); a não utilização de material descartável, OR=10,85 (IC 95% 4,85-24,28); sistema de criação extensivo OR=10,85 (IC 95% 4,85-24,28); sistema de criação semi-extensivo OR=3,71(IC 95% 1,64-8,39) e manejo OR=11,4 (IC 95% 5,51-23,60). Conclui-se que as unidades produtoras de caprinos dos Estados do Pará e Maranhão apresentam positividade em seus rebanhos para toxoplasmose e CAEV.

Background Many medical exams use 5 options for multiple choice questions (MCQs), although the literature suggests that 3 options are optimal. Previous studies on this topic have often been based on non-medical examinations, so we sought to analyse rarely selected, 'non-functional' distractors (NF-D) in high stakes medical examinations, and their detection by item authors as well as psychometric changes resulting from a reduction in the number of options. Methods Based on Swiss Federal MCQ examinations from 2005-2007, the frequency of NF-D (selected by <1% or <5% of the candidates) was calculated. Distractors that were chosen the least or second least were identified and candidates who chose them were allocated to the remaining options using two extreme assumptions about their hypothetical behaviour: In case rarely selected distractors were eliminated, candidates could randomly choose another option - or purposively choose the correct answer, from which they had originally been distracted. In a second step, 37 experts were asked to mark the least plausible options. The consequences of a reduction from 4 to 3 or 2 distractors - based on item statistics or on the experts' ratings - with respect to difficulty, discrimination and reliability were modelled. Results About 70% of the 5-option-items had at least 1 NF-D selected by <1% of the candidates (97% for NF-Ds selected by <5%). Only a reduction to 2 distractors and assuming that candidates would switch to the correct answer in the absence of a 'non-functional' distractor led to relevant differences in reliability and difficulty (and to a lesser degree discrimination). The experts' ratings resulted in slightly greater changes compared to the statistical approach. Conclusions Based on item statistics and/or an expert panel's recommendation, the choice of a varying number of 3-4 (or partly 2) plausible distractors could be performed without marked deteriorations in psychometric characteristics.

A few publications documented the coexistence of epilepsy and obstructive sleep apnea (OSA). The extent, nature, and clinical relevance of this association remain poorly understood. We retrospectively reviewed the database of our sleep center to identify patients with both sleep apnea and epilepsy. Characteristics of epilepsy, sleep history, presence of excessive daytime sleepiness [Epworth Sleepiness Scale (ESS)] and polysomnographic data were assessed. The effect of continuous positive airway pressure (CPAP) on seizure reduction was prospectively analyzed after a median interval of 26 months (range: 2-116 months) from the diagnosis of OSA. OSA was found in 29 epilepsy patients (25 men and 4 women) with a median age of 56 years (range: 37-79). The median apnea hypopnea index was 33 (range: 10-85), the oxygen desaturation index was 12 (range 0-92), and 52% of the patients had an ESS score >10. In 27 patients, epilepsy appeared 1 month to 44 years prior to the diagnosis of OSA. In 21 patients, the appearance of OSA symptoms coincided with a clear increase in seizure frequency or the first appearance of a status epilepticus. Treatment with CPAP was continued with good compliance in 12 patients and led to a significant reduction of both ESS scores and seizure frequency in 4 patients. Our data suggest the importance of considering diagnosis and treatment of OSA in epilepsy patients with poor seizure control and/or reappearance of seizures after a seizure-free interval.

In climate research the interest on carbonaceous particles has increased over the last years because of their influence on the radiation balance of the earth. Nevertheless, there is a paucity of available data regarding their concentrations and sources in the past. Such data would be important for a better understanding of their effects and for estimating their influence on future climate. Here, a technique is described to extract carbonaceous particles from ice core samples with subsequent separation of the two main constituents into organic carbon (OC) and elemental carbon (EC) for analysis of their concentrations in the past. This is combined with further analysis of OC and EC 14C/12C ratios by accelerator mass spectrometry (AMS), what can be used for source apportionment studies of past emissions. We further present how 14C analysis of the OC fraction could be used in the future to date any ice core extracted from a high-elevation glacier. Described sample preparation steps to final analysis include the combustion of micrograms of water–insoluble carbonaceous particles, primary collected by filtration of melted ice samples, the graphitisation of the obtained CO2 to solid AMS target material and final AMS measurements. Possible fractionation processes were investigated for quality assurance. Procedural blanks were reproducible and resulted in carbon masses of 1.3 ± 0.6 μg OC and 0.3 ± 0.1 μg EC per filter. The determined fraction of modern carbon (fM) for the OC blank was 0.61 ± 0.13. The analysis of processed IAEA-C6 and IAEA-C7 reference material resulted in fM = 1.521 ± 0.011 and δ13C = −10.85 ± 0.19‰, and fM = 0.505 ± 0.011 and δ13C = −14.21 ± 0.19‰, respectively, in agreement with consensus values. Initial carbon contents were thereby recovered with an average yield of 93%.

Este artículo tiene como objetivo examinar el impacto y las consecuencias sociales contemporáneas del mantenimiento del sistema de organización social denominado «reserva indígena» en Canadá. Mediante un estudio de caso, se explora cómo la política organizacional y la estructura institucional contemporánea en las reservas indígenas canadienses están alimentando problemas psicosociales y de salud mental como, por ejemplo, abuso de sustancias psicoactivas, depresión, negligencia parental o violencia doméstica. Tres características estructurales se identifican como problemas inherentes en la estructura organizacional de las reservas contemporáneas: el desempleo estructural, la dependencia de programas de asistencia social y el faccionalismo comunitario.
