Foram obtidos perfis tubulares porosos de polietileno (PE) e de polietileno/fibra de açaí (PE/PA) 80/20 extrudados a partir de partículas granuladas de polietileno de alta densidade reciclado de embalagens pós-consumo de 600 μm, e deste com fibra de açaí de 300 μm. Para o processamento das peças foi desenvolvida uma extrusora mono-rosca de bancada, com sistema mecânico acionado por um motor elétrico de ½ CV (0,37 kw) controlado por um inversor de freqüência, com canhões, roscas, matriz e sistema de aquecimento substituíveis. Para permitir uma visualização didática de condições de operação do equipamento de modo simplificado foram realizados testes com parafina em canhão de vidro variando-se a velocidade de rotação do parafuso e perfil de temperatura, ajustando vazão mássica e pressão na saída. Para a extrusão dos perfis porosos foram realizados ensaios reológicos de PE e PE/FA sendo selecionado rosca, barril e matriz de alumínio; rosca com passo de 9 mm e relação comprimento diâmetro (L/D) 22, composta de um elemento misturador e um elemento de flutuação na zona de controle de vazão; ângulo entre o filete e o eixo da rosca 17º, folga entre a rosca e o barril 0,15 mm; rotação de 1,3 rpm; aquecimento ao longo do canhão de 120ºC; matriz tubular com 21 mm de diâmetro interno e mandril de 19 mm de diâmetro externo. Os perfis PE e PE/FA apresentaram poros com diâmetros médios de 0,7 e 0,6 mm; densidade relativa à água a 28ºC de 0,77 e 0,73; módulo de elasticidade de 1,002 e 2,601 GPa e máximo inchamento aparente do extrudado de 100 e 80%.

The course of leprosy depends of the host immune response which ranges from the lepromatous pole (LL) to the tuberculoid pole (TT). A comparative study was conducted in 60 patients with the LL and TT The results showed a mean expression of TGF-beta of 339 +/- 99.4 cells/field for TT and of 519.2 +/- 68.2 cells/field for LL. Frequency of apoptosis was 6.3 +/- 1.8 in TT and 14.0 +/- 6.1 in LL. A correlation (p = 0.0251) between TGF-beta and caspase-3 in the LL was found. This finding indicates a role of TGF-beta and apoptosis in the immune response in leprosy. (C) 2012 Institut Pasteur. Published by Elsevier Masson SAS. All rights reserved.

Nel tumore combinato epatocolangiocellulare (CHC) le componenti epatocitarie e colangiocitarie sono entrambe presenti. Obiettivo: valutare gli aspetti diagnostici radiologici e caratteristiche clinico-demografiche del CHC su cirrosi. Raccolti pazienti con CHC su cirrosi afferenti a due centri del Nord Italia (Bologna, S. Orsola-Malpighi e Milano,IRCCS Ca’ Granda Maggiore Hospital) tra 2003-2013, con diagnosi istologica di CHC. FASE 1:confronto tra ecografia con mdc (CEUS), TC cmdc e RM cmdc nella diagnosi e caratterizzazione dei noduli di CHC. Casistica di 35 pazienti e 37 noduli (due recidive CHC incluse). Mediana delle dimensioni: 25 mm. Non si è identificato un pattern contrastografico patognomonico per CHC. Pattern di enhancement arterioso ad anello periferico, suggestivo per forma colangiocitaria, atipico per HCC, presente nel 26%,50%,29% dei noduli a CEUS,TC,RM. La CEUS avrebbe portato a una errata diagnosi di HCC tipico in un numero maggiore di casi (48%) vs TC(15%,p=0.005), e RM(18%,p=0.080).L’indicazione della malignità del nodulo (presenza di wash-out dopo enhancement arterioso), era fornita con maggiore accuratezza da parte della CEUS(78%), vs TC (24%,p<0.0001) e RM(29%,p=0.002). FASE 2:analisi degli aspetti clinico-laboratoristici e prognostici del CHC e confronto tramite match 1:2 con HCC su cirrosi (36 CHC,72 HCC). Nel CHC correlano positivamente con sopravvivenza le terapie “curative” (trapianto, resezione chirurgica, terapie ablative percutanee a radiofrequenza/ alcolizzazione), stadio precoce alla diagnosi, dimensioni e essere in sorveglianza per diagnosi precoce di HCC. Correlano indipendentemente con sopravvivenza stadio precoce di malattia (unifocale, ≤ 2 cm) e essere in programma di sorveglianza(multivariata). Sopravvivenze del CHC sovrapponibili al gruppo HCC a 1 anno, e lievemente inferiori a 3/5 anni (81%, 39%, 21% vs 83%, 59% e 40%,p=0.78,p=0.080 e p=0.14). Sopravvivenza mediana per CHC (2.36 anni) inferiore vs HCC (4.09 anni) pur senza significatività statistica.

BACKGROUND: The objective of this study was to link expression patterns of B-cell-specific Moloney murine leukemia virus integration site 1 (Bmi-1) and p16 to patient outcome (recurrence and survival) in a cohort of 252 patients with oral and oropharyngeal squamous cell cancer (OSCC). METHODS: Expression levels of Bmi-1 and p16 in samples from 252 patients with OSCC were evaluated immunohistochemically using the tissue microarray method. Staining intensity was determined by calculating an intensity reactivity score (IRS). Staining intensity and the localization of expression within tumor cells (nuclear or cytoplasmic) were correlated with overall, disease-specific, and recurrence-free survival. RESULTS: The majority of cancers were localized in the oropharynx (61.1%). In univariate analysis, patients who had OSCC and strong Bmi-1 expression (IRS >10) had worse outcomes compared with patients who had low and moderate Bmi-1 expression (P = .008; hazard ratio [HR], 1.82; 95% confidence interval [CI], 1.167-2.838); this correlation was also observed for atypical cytoplasmic Bmi-1 expression (P = .001; HR, 2.164; 95% CI, 1.389-3.371) and for negative p16 expression (P < .001; HR, 0.292; 95% CI, 0.178-0.477). The combination of both markers, as anticipated, had an even stronger correlation with overall survival (P < .001; HR, 8.485; 95% CI, 4.237-16.994). Multivariate analysis demonstrated significant results for patients with oropharyngeal cancers, but not for patients with oral cavity tumors: Tumor classification (P = .011; HR, 1.838; 95%CI, 1.146-2.947) and the combined marker expression patterns (P < .001; HR, 6.254; 95% CI, 2.869-13.635) were correlated with overall survival, disease-specific survival (tumor classification: P = .002; HR, 2.807; 95% CI, 1.477-5.334; combined markers: P = .002; HR, 5.386; 95% CI, 1.850-15.679), and the combined markers also were correlated with recurrence-free survival (P = .001; HR, 8.943; 95% CI, 2.562-31.220). CONCLUSIONS: Cytoplasmic Bmi-1 expression, an absence of p16 expression, and especially the combination of those 2 predictive markers were correlated negatively with disease-specific and recurrence-free survival in patients with oropharyngeal cancer. Therefore, the current results indicate that these may be applicable as predictive markers in combination with other factors to select patients for more aggressive treatment and follow-up. The effect of treatment with eprinomectin on milk yield, milk composition and somatic cell counts (SCCs) was studied in 105 dairy cows located on seven farms in South Tyrol, Italy. On each farm, half of the animals were treated with eprinomectin and the other half were used as an untreated control group. Three test day records per animal were obtained before treatment (days -117, -75 and -33) and another three test day records were obtained after treatment (days 22, 62 and 131). Test day records comprised milk yield, milk composition, SCC and days in milk. On the day of treatment, blood samples and faecal samples were taken for parasitological analysis. Cows with positive faecal egg counts yielded less milk. A significant effect of eprinomectin on milk yield was observed after treatment and was most pronounced on the second and the third test days after treatment (+1.90 kg [P=0.002] and +2.63 kg [P<0.001], respectively). Furthermore, a significant decrease in SCC was observed on the second test day after treatment.

Elevated serum ferritin levels may reflect a systemic inflammatory state as well as increased iron storage, both of which may contribute to an unfavorable outcome of chronic hepatitis C (CHC). We therefore performed a comprehensive analysis of the role of serum ferritin and its genetic determinants in the pathogenesis and treatment of CHC. To this end, serum ferritin levels at baseline of therapy with pegylated interferon-alpha and ribavirin or before biopsy were correlated with clinical and histological features of chronic hepatitis C virus (HCV) infection, including necroinflammatory activity (N = 970), fibrosis (N = 980), steatosis (N = 886), and response to treatment (N = 876). The association between high serum ferritin levels (> median) and the endpoints was assessed by logistic regression. Moreover, a candidate gene as well as a genome-wide association study of serum ferritin were performed. We found that serum ferritin ≥ the sex-specific median was one of the strongest pretreatment predictors of treatment failure (univariate P < 0.0001, odds ratio [OR] = 0.45, 95% confidence interval [CI] = 0.34-0.60). This association remained highly significant in a multivariate analysis (P = 0.0002, OR = 0.35, 95% CI = 0.20-0.61), with an OR comparable to that of interleukin (IL)28B genotype. When patients with the unfavorable IL28B genotypes were stratified according to high versus low ferritin levels, SVR rates differed by > 30% in both HCV genotype 1- and genotype 3-infected patients (P < 0.001). Serum ferritin levels were also independently associated with severe liver fibrosis (P < 0.0001, OR = 2.67, 95% CI = 1.68-4.25) and steatosis (P = 0.002, OR = 2.29, 95% CI = 1.35-3.91), but not with necroinflammatory activity (P = 0.3). Genetic variations had only a limited impact on serum ferritin levels. Conclusion: In patients with CHC, elevated serum ferritin levels are independently associated with advanced liver fibrosis, hepatic steatosis, and poor response to interferon-alpha-based therapy.

Researcher allegiance (RA) is widely discussed as a risk of bias in psychotherapy outcome research. The relevance attached to RA bias is related to meta-analyses demonstrating an association of RA with treatment effects. However, recent meta-analyses have yielded mixed results. To provide more clarity on the magnitude and robustness of the RA-outcome association this article reports on a meta-meta-analysis summarizing all available meta-analytic estimates of the RA-outcome association. Random-effects methods were used. Primary study overlap was controlled. Thirty meta-analyses were included. The mean RA-outcome association was r=.262 (p=.002, I(2)=28.98%), corresponding to a moderate effect size. The RA-outcome association was robust across several moderating variables including characteristics of treatment, population, and the type of RA assessment. Allegiance towards the RA bias hypothesis moderated the RA-outcome association. The findings of this meta-meta-analysis suggest that the RA-outcome association is substantial and robust. Implications for psychotherapy outcome research are discussed.

Abstract We explored the feasibility of unrelated donor haematopoietic stem cell transplant (HSCT) upfront without prior immunosuppressive therapy (IST) in paediatric idiopathic severe aplastic anaemia (SAA). This cohort was then compared to matched historical controls who had undergone first-line therapy with a matched sibling/family donor (MSD) HSCT (n = 87) or IST with horse antithymocyte globulin and ciclosporin (n = 58) or second-line therapy with unrelated donor HSCT post-failed IST (n = 24). The 2-year overall survival in the upfront cohort was 96 ± 4% compared to 91 ± 3% in the MSD controls (P = 0·30) and 94 ± 3% in the IST controls (P = 0·68) and 74 ± 9% in the unrelated donor HSCT post-IST failure controls (P = 0·02).The 2-year event-free survival in the upfront cohort was 92 ± 5% compared to 87 ± 4% in MSD controls (P = 0·37), 40 ± 7% in IST controls (P = 0·0001) and 74 ± 9% in the unrelated donor HSCT post-IST failure controls (n = 24) (P = 0·02). Outcomes for upfront-unrelated donor HSCT in paediatric idiopathic SAA were similar to MSD HSCT and superior to IST and unrelated donor HSCT post-IST failure. Front-line therapy with matched unrelated donor HSCT is a novel treatment approach and could be considered as first-line therapy in selected paediatric patients who lack a MSD. © 2015 John Wiley & Sons Ltd.

BACKGROUND Type D personality (Type D) is an independent psychosocial risk factor for poor cardiac prognosis and increased mortality in patients with cardiovascular disease (CVD), but the involved mechanisms are poorly understood. Macrophages play a pivotal role in atherosclerosis, the process underlying coronary artery disease (CAD). We investigated macrophage superoxide anion production in production in CAD patients with and without Type D. METHODS AND RESULTS We studied 20 male CAD patients with Type D (M:66.7±9.9years) and 20 age-matched male CAD patients without Type D (M:67.7±8.5years). Type D was measured using the DS14 questionnaire with the two subscales 'negative affectivity' and 'social inhibition'. We assessed macrophage superoxide anion production using the WST-1 assay. All analyses were controlled for potential confounders. CAD patients with Type D showed higher superoxide anion production compared to CAD patients without Type D (F(1,38)=15.57, p<0.001). Complementary analyses using the Type D subscales 'negative affectivity' and 'social inhibition', and their interaction as continuous measures, showed that both Type D subscales (negative affectivity: (ß=0.48, p=0.002, R(2)=0.227); social inhibition: (ß=0.46, p=0.003, R(2)=0.208)) and their interaction (ß=0.36, p=0.022, R(2)=0.130) were associated with higher WST-1 reduction scores. Results remained significant when controlling for classical CVD risk factors (i.e. body mass index, mean arterial blood pressure), atherosclerosis severity (i.e. intima media thickness, presence of carotid plaques), and psychological factors (depressive symptom severity, chronic stress). CONCLUSIONS Our results indicate higher macrophage superoxide anion production in CAD patients with Type D compared to those without Type D. This may suggest a mechanism contributing to increased morbidity and mortality in CAD patients with Type D.

In order to validate the use of 238U/235U as a paleoredox proxy in carbonates, we examined the incorporation and early diagenetic evolution of U isotopes in shallow Bahamian carbonate sediments. Our sample set consists of a variety of primary precipitates that represent a range of carbonate producing organisms and components that were important in the past (scleractinian corals, calcareous green and red algae, ooids, and mollusks). In addition, four short push cores were taken in different depositional environments to assess the impact of early diagenesis and pore water chemistry on the U isotopic composition of bulk carbonates. We find that U concentrations are much higher in bulk carbonate sediments (avg. 4.1 ppm) than in primary precipitates (avg. 1.5 ppm). In almost all cases, the lowest bulk sediment U concentrations were as high as or higher than the highest concentrations found in primary precipitates. This is consistent with authigenic accumulation of reduced U(IV) during early diagenesis. The extent of this process appears sensitive to pore water H2S, and thus indirectly to organic matter content. d238/235U values were very close to seawater values in all of the primary precipitates, suggesting that these carbonate components could be used to reconstruct changes in seawater U geochemistry. However, d238/235U of bulk sediments from the push cores was 0.2-0.4 per mil heavier than seawater (and primary precipitates). These results indicate that authigenic accumulation of U under open-system sulfidic pore water conditions commonly found in carbonate sediments strongly affects the bulk U concentrations and 238U/235U ratios. We also report the occurrence of dolomite in a tidal pond core which contains low 234U/238U and 238U/235U ratios and discuss the possibility that the dolomitization process may result in sediments depleted in 238U. From this initial exploration, it is clear that 238U/235U variations in ancient carbonate sediments could be driven by changes in global average seawater, by spatial and temporal variations in the local deposition environment, or subsequent diagenesis. To cope with such effects, proxies for syndepositional pore water redox conditions (e.g., organic matter content, iron speciation, and trace metal distributions) and careful consideration of possible post-deposition alteration will be required to avoid spurious interpretation of 238U/235U data from ancient carbonate sediments.

Clay mineralogy and geotechnical properties of Tarras clay, basin clays and tills from some parts of Schleswig-Holstein: Tarras clay of lower Eocene age, Quaternary till containing various admixtures of Tarras clay as well as basin clay and varve-clay from Schleswig-Holstein were investigated. Grain size distribution and soil mechanic characteristics were determined, which indicated different geotechnical properties for each sediment type.

Two sealed borehole hydrologic observatories (CORKs) were installed in two active hydrogeochemical systems at the Costa Rica subduction zone to investigate the relationship between tectonics, fluid flow, and fluid composition. The observatories were deployed during Ocean Drilling Program (ODP) Leg 205 at Site 1253, ~ 0.2 km seaward of the trench, in the upper igneous basement, and at Site 1255, ~ 0.5 km landward of the trench, in the décollement. Downhole instrumentation was designed to monitor formation fluid flow rates, composition, pressure, and temperature. The two-year records collected by this interdisciplinary effort constitute the first co-registered hydrological, chemical, and physical dataset from a subduction zone, providing critical information on the average and transient state of the subduction thrust and upper igneous basement. The continuous records at ODP Site 1253 show that the uppermost igneous basement is highly permeable hosting an average fluid flow rate of 0.3 m/yr, and indicate that the fluid sampled in the basement is a mixture between seawater (~ 50%) and a subduction zone fluid originating within the forearc (~ 50%). These results suggest that the uppermost basement serves as an efficient pathway for fluid expelled from the forearc that should be considered in models of subduction zone hydrogeology and deformation. Three transients in fluid flow rates were observed along the décollement at ODP Site 1255, two of which coincided with stepwise increases in formation pressure. These two transients are the result of aseismic slip dislocations that propagated up-dip from the seismogenic zone over the course of ~ 2 weeks terminating before reaching ODP Site 1255 and the trench. The nature and temporal behavior of strain and the associated hydrological response during these slow slip events may be an analog for the response of the seaward part of the subduction prism during or soon after large subduction zone earthquakes.

This study is about the nature of persons and personal identity. It belongs to a tradition that maintains that in order to understand what it is to be a person we must clarify what personal identity consists in. In this pursuit, I differentiate between the problems (i) How do persons persist? and (ii) What facts, if any, does personal identity consist in? Concerning the first question, I argue that persons persist three-dimensionally (the endurance view), and not four-dimensionally (the perdurarne view), on the ground that objects must always fall under some substance sortal concept S (the sortal dependency of individuation), and that the concept person entails that objects falling under it are three-dimensional. Concerning the second question, I differentiate between Criterianists, who maintain that it is possible to specify a non-circular and informative criterion for personal identity, and Non-Criterianists, who deny that such a specification is possible. I argue against Criterianist accounts of personal identity on the ground that they are either (i) circular, (ii) violate the intrinsicality of identity or (iii) do not adequately represent what we are essentially. I further criticise three Psychological Non-Criterianist accounts of personal identity on the ground that they wrongly assume that 'person' refers to mental entities. Instead I formulate the Revised Animal Attribute View where person is understood as a basic sortal concept which picks out a biological sort of enduring animals. In this, I claim that the real essence of a person is determined by the real essence of the kind of animal he is, without thereby denying that persons have a real essence as persons.
