The Laws is generally regarded as Plato’s attempt to engage with the practical realities of political life, as opposed to the more idealistic, or utopian, vision of the Republic. Yet modern scholars have often felt disquieted at the central role of religion in the Laws’ second-best city and regime. There are essentially the two dominant interpretations on offer today: either religion supports a repressive theocracy, which controls every aspect of the citizens’ lives to such an extent that even philosophy itself is discouraged, or religion is an example of the kind of noble lie, which the philosopher must deceive the citizens into believing—viz., that a god, not a man, is the author of the regime’s laws. I argue that neither of these interpretations do justice to the dialogue’s intricately dramatic structure, and therefore to Plato’s treatment of civil religion. What I propose is a third position in which Plato both takes seriously the social and political utility of religion, and views theology as a legitimate, and even necessary, subject of philosophical inquiry without going so far as to advocate theocracy as the second best form of regime.
I conclude that a proper focus on the dialogue form, combined with a careful historical analysis of Plato’s use of social and political institutions, reveals an innovative yet traditional form of civil religion, purified of the harmful influence of the poets, based on the authority of the oracle at Delphi, and grounded on a philosophical conception of god as the eternal source of order, wisdom, and all that is good. Through a union of traditional Delphic theology and Platonic natural theology, Plato gives the city of the Laws a common cult acceptable to philosopher and non-philosopher alike, and thus, not only bridges the gap between religion and philosophy, but also creates a sense of community, political identity, and social harmony—the prerequisites for political order and stability. The political theology of the Laws, therefore, provides a rational defense of the rule of law (νόμος) re-conceived as the application of divine Reason (νοῦς) to human affairs.

This dissertation seeks to identify what makes Cicero’s approach to politics unique. The author's methodology is to turn to Cicero’s unique interpretation of Plato as the crux of what made his thinking neither Stoic nor Aristotelian nor even Platonic (at least, in the usual sense of the word) but Ciceronian. As the author demonstrates in his reading of Cicero’s correspondences and dialogues during the downward spiral of a decade that ended in the fall of the Republic (that is, from Cicero’s return from exile in 57 BC to Caesar’s crossing of the Rubicon in 49 BC), it is through Cicero's reading of Plato that the former develops his characteristically Ciceronian approach to politics—that is, his appreciation for the tension between the political ideal on the one hand and the reality of human nature on the other as well as the need for rhetoric to fuse a practicable compromise between the two. This triangulation of political ideal, human nature, and rhetoric is developed by Cicero through his dialogues "de Oratore," "de Re publica," and "de Legibus."

The aim of this paper is to tackle the role of oblivion in the Plato’s dialogues. It takes as work texts the dialogues where the anamnesis doctrine is exposed, and from these writings, but also attending to several other ones, the connexions between oblivion and other Platonic issues, as, e. g., language, memory, experience or love are explored, in order to clarify their relations of mutual dependence. In this way, we attempt to find out in the Platonic oblivion a philosophical sense, which is not disdainful in comparison with the sense and the relevance that the readers of all times have granted to anamnesis; for anamnesis and oblivion are inseparable indeed.

Uma das causas de insucesso nas artroplastias do joelho está relacionada com a dor que os doentes sentem a curto e/ou longo prazo. A dor tanto pode ser devida a infecção ou descolamento, como pode estar presente sem qualquer um destes factores, provocando a substituição da prótese. Existem estudos de origem clínica que relatam uma percentagem, que se pode considerar relevante, de dor na extremidade da haste, quer na zona da tíbia quer no fémur [1-3]. A nível ósseo, a cintigrafia pode mostrar as alterações metabólicas locais, antes de qualquer tradução radiográfica [4]. O aumento da carga, a nível ósseo, estimula localmente a actividade osteoblástica [4]. Esta estimulação pode originar dor local, como demonstrado num caso clínico por Fonseca et al. [5], referente a um doente com dor na extremidade da haste de uma prótese do joelho e onde a cintigrafia com Tc 99m [5] mostrava uma actividade celular mais intensa em torno da extremidade. O presente estudo teve como objectivo verificar até que ponto é possível estabelecer uma relação entre o sintoma dor e o comportamento biomecânico da haste da prótese do joelho e, particularmente, na sua extremidade. A análise relativa ao nível de tensões no osso, em torno da haste tibial, comparativamente ao valor do osso intacto para o mesmo tipo de carga e localização, foi realizada utilizando a aplicação de análise estrutural HyperWorks (Altair Engineering Inc.). Um estudo posterior, relativo à forma e material da extremidade da haste levou à conclusão de que é possível uma melhor uniformização das tensões no osso, podendo-se desta forma aproximar às tensões fisiológicas. Neste sentido, a forma geometral da haste deverá ser objecto de optimização, que poderá incluir materiais poliméricos, de menor rigidez, na ponta distal do implante.

Abstract: The first metatarsal sesamoid bones are not always taken into consideration when making a diagnosis, in pathologies that affect the region of the first metatarsal head. This is due to the insufficient knowledge of all the pathologies that can affect the sesamoids and the relative little incidence that they have. With the increment of sports activities, in particular the running, increasingly affects of the symptoms concerning this region are observed. Methods: A literature search was performed in 5 databases (Medline, PubMed, Scopus, Cochrane Library and BUCEA). The terms included in the search were: sesamoids, anatomy, biomechanics, sesamoids review and sesamoids pathology. In the initial search articles with no more than 10 years, only humans and revision texts are considered. Results: 24 articles were selected and include different pathologies with diagnosis using imaging tests and treatments, both conservative and surgical; as well as aspects from the biomechanics of the metatarsal-sesamoid joint. Conclusion: Sesamoids due of his anatomy, topography and function can be involved in a lot of pathologies; with similar signs and symptoms that can confuse the podiatry when he has to make a correct diagnosis or treatment.

A descrição do papel da medicina no Livro III da República pode facilmente deixar o leitor constrangido. O trecho defende que a medicina deveria se espelhar nas práticas do tempo de Asclépio. Neste tempo, a medicina tinha um papel político de não prolongar a vida dos cidadãos tomados pela doença, nem deixá-los procriar. Frente a estas declarações, surgiram várias interpretações que vão desde o totalitarismo de Popper até a ironia de Strauss. Para uma melhor compreensão do texto, seria necessária a interlocução com a medicina hipocrática e com as práticas médicas do tempo dos séculos V e IV aEN. Contudo, a passagem em questão não se reduz ao contexto histórico, mas apresenta uma crítica válida ainda na atualidade. _______________________________________________________________________________ ABSTRACT

Sexual dysfunction (SD) affects up to 80% of multiple sclerosis (MS) patients and pelvic floor muscles (PFMs) play an important role in the sexual function of these patients. The objective of this paper is to evaluate the impact of a rehabilitation program to treat lower urinary tract symptoms on SD of women with MS. Thirty MS women were randomly allocated to one of three groups: pelvic floor muscle training (PFMT) with electromyographic (EMG) biofeedback and sham neuromuscular electrostimulation (NMES) (Group I), PFMT with EMG biofeedback and intravaginal NMES (Group II), and PFMT with EMG biofeedback and transcutaneous tibial nerve stimulation (TTNS) (Group III). Assessments, before and after the treatment, included: PFM function, PFM tone, flexibility of the vaginal opening and ability to relax the PFMs, and the Female Sexual Function Index (FSFI) questionnaire. After treatment, all groups showed improvements in all domains of the PERFECT scheme. PFM tone and flexibility of the vaginal opening was lower after the intervention only for Group II. All groups improved in arousal, lubrication, satisfaction and total score domains of the FSFI questionnaire. This study indicates that PFMT alone or in combination with intravaginal NMES or TTNS contributes to the improvement of SD.

This study involved a retrospective evaluation of patients subjected to surgery for dentofacial deformities treated without induced controlled hypotension (group I, n=50) and a prospective evaluation of patients who were subjected to surgery under hypotensive general anaesthesia (group II, n=50). No statistical differences were found between the study groups with regard to the duration of surgery. However, there were statistically significant differences in the need for blood transfusion and the occurrence of bradycardia during the maxillary down-fracture. Hypotensive anaesthesia decreased the need for a blood transfusion and the occurrence of bradycardia, and is therefore considered highly beneficial for patients undergoing orthognathic surgery.

Mechanically evoked reflexes have been postulated to be less sensitive to presynaptic inhibition (PSI) than the H-reflex. This has implications on investigations of spinal cord neurophysiology that are based on the T-reflex. Preceding studies have shown an enhanced effect of PSI on the H-reflex when a train of ~10 conditioning stimuli at 1 Hz was applied to the nerve of the antagonist muscle. The main questions to be addressed in the present study are if indeed T-reflexes are less sensitive to PSI and whether (and to what extent and by what possible mechanisms) the effect of low frequency conditioning, found previously for the H-reflex, can be reproduced on T-reflexes from the soleus muscle. We explored two different conditioning-to-test (C-T) intervals: 15 and 100 ms (corresponding to D1 and D2 inhibitions, respectively). Test stimuli consisted of either electrical pulses applied to the posterior tibial nerve to elicit H-reflexes or mechanical percussion to the Achilles tendon to elicit T-reflexes. The 1 Hz train of conditioning electrical stimuli delivered to the common peroneal nerve induced a stronger effect of PSI as compared to a single conditioning pulse, for both reflexes (T and H), regardless of C-T-intervals. Moreover, the conditioning train of pulses (with respect to a single conditioning pulse) was proportionally more effective for T-reflexes as compared to H-reflexes (irrespective of the C-T interval), which might be associated with the differential contingent of Ia afferents activated by mechanical and electrical test stimuli. A conceivable explanation for the enhanced PSI effect in response to a train of stimuli is the occurrence of homosynaptic depression at synapses on inhibitory interneurons interposed within the PSI pathway. The present results add to the discussion of the sensitivity of the stretch reflex pathway to PSI and its functional role.

The extract of stevia leaves (Stevia rebaudiana Bertoni) is the only sweetener utilized in sucrose substitution which can be produced totally in Brazil. The objective of this study, was determine the temporal characteristic of sweet and bitter taste of stevia and compare with sucrose at 3 and 10% in the same equi-sweet. The time-intensity curves (T-I) for each substance were collected through the software Sistema de Coleta de Dados Tempo-Intensidade - SCDTI for Windows, where the judges recorded through of mouse the perception of each stimuli inside function of time, for each sample. The parameters of T-I curves collected were: time for intensity maxim (TImax), intensity maxim (Imax), time of decay (Td), time of plato (Platô), area under curve (Area) and total time of stimuli duration (Ttot). The parameters Td, Ttot, Area e Plato of T-I curves, for stimuli sweet in both sweetness level, were significativelly superior for stevia, while Timax e Imax were significativelly inferior (p£0,05), at differences between value for both substances were superior DESS at 10%. Sucrose didn?t present any record for simuli bitter as 3 as 10%, while stevia presented a characteristic T-I curve with intensity and total time of stimuli duration dependent of concentration.

The case of a 15 years old white man, diabetic in cetoacidosis, with a orbit-rhino-cerebral phycomycosis is reported. The illness had an acute onset and the treatment was iniciated early with Amphotericin-B and unilateral osteotomy of maxillary and ethmoidal sinus. With this treatment the patient did well with residuals of ophtalmoplegia and amaurosis on the right. Interesting investigation aspects are the occluded internal carotid on the same side of the affected orbit and the CAT-SCAN finding of moderated ventricular dilatation (two month after hospital admission). Mycology, pathophysiology, histopathology, clinical aspects, diagnosis and therapy are discussed, comparing the findings of this case with available literature. An increased number of survivors can be expected with earlier recognition and more aggressive therapy. Treatment of the underlying debilitating disease, Amphotericin-B and surgical debridement of necrotic tissue, are frequently necessary such as observed in the case reported. The favorable results ootaineü with the proposed managment are stressed.

OBJETIVO: avaliar as alterações nas inclinações dos dentes anteriores causadas pelo tratamento ortodôntico, utilizando-se aparelho Straight-Wire, prescrição II Capelozza, antes e após a fase de nivelamento com fios ortodônticos de aço de secção retangular. MÉTODOS: foram selecionados 17 indivíduos adultos de padrão facial II, má oclusão Classe II, indicados para tratamento ortodôntico compensatório. As inclinações dos dentes anteriores foram avaliadas em três tempos clínicos, após o uso dos fios ortodônticos de diâmetros 0,020" (T1); 0,019" x 0,025" (T2) e 0,021" x 0,025" (T3), através de exames de tomografia computadorizada. Empregou-se a análise de variância de Friedman, com nível de significância de 5%, na comparação entre os tempos. RESULTADOS: observou-se que o fios retangulares empregados não foram capazes de produzir uma mudança significativa na mediana da inclinação dentária, exceto por uma discreta alteração nos incisivos laterais inferiores (p<0,05). Por outro lado, constatou-se que a variação das inclinações observadas era menor no fio retangular 0,021" x 0,025", principalmente para os incisivos superiores (p<0,001). CONCLUSÃO: fios retangulares 0,021" x 0,025" produzem uma maior homogeneidade no grau de variação na inclinação dos incisivos superiores, embora sem mudança significativa na sua mediana.
