This is the report from the Regional Fisheries Advisory Committee meeting, which was held on the 22nd May, 1989. The report contains sections on fishing licence duties, Ombudsman’s investigations, prosecutions for netting offences, eel netting in River Ribble Estuary, fish mortality incidents and fishing licence sales. The section on Ombudsman’s Report looks at the complaint against the Authority relating to the allocation of haaf net licences for the Solway Firth. The section on fish mortality incidents focuses on fish Mortalities in River Laneshaw and Colne Water on 26/27 March 1989. The Fisheries Advisory Committee was part of the Regional Water Authorities, in this case the North West Water Authority. This preceded the Environment Agency which came into existence in 1996.

Este trabalho aborda a aplicação da Geomática no apoio às ações de rápida resposta às emergências ambientais passíveis de ocorrência, nas rodovias do País, por incidentes com produtos perigosos. A cadeia logística nacional de infraestrutura de transporte de carga está apoiada, principalmente, no modal rodoviário, que é responsável por cerca de 60% do volume total transportado. Seguindo essa tendência, a maioria dos produtos perigosos também é transportada através de estradas. Nessas operações, apesar de todas as medidas de segurança empregadas, tanto na embalagem quanto no manuseio, há sempre a possibilidade da ocorrência de incidentes com esse tipo de carga, no deslocamento entre o ponto de partida e o destino final. Diante dessas potenciais ameaças, ficam evidentes os riscos à integridade física e patrimonial das populações lindeiras, bem como ao meio ambiente, no tocante à qualidade das águas superficiais e subterrâneas, do solo e do ar. Este trabalho sugere, com base na integração dos recursos tecnológicos do Sensoriamento Remoto, do Sistema de Posicionamento Global-GPS e do Geoprocessamento, implantar um sistema de informações georreferenciadas que proporcione os elementos necessários às ações de rápida resposta a esses eventos. A estrutura proposta, denominada Bureau de Informações Territoriais, está moldada para proporcionar uma solução para os problemas relacionados com o posicionamento na superfície terrestre, e que engloba os conceitos da Geomática, através da aplicação das suas quatro etapas básicas: coleta de dados, análise de dados, distribuição da informação e uso da informação. Assim, atendendo ao princípio da mobilidade, o projeto visa, ainda, a disponibilizar na tela de um laptop as informações necessárias ao planejamento e à execução das ações, no menor tempo possível. Por outro lado, considerando que o Bureau também proporciona às organizações que participam das ações de combate aos incidentes rodoviários com produtos perigosos as informações necessárias, através de uma conexão à Internet, espera-se a maximização dos benefícios do planejamento das ações preventivas e corretivas de resposta aos sinistros, assim como a minimização dos efeitos desse tipo de evento sobre a população, as propriedades e o meio ambiente.

Mundialmente, eventos relatados constituídos por incidentes e acidentes em radioterapia, tem aumentado ao longo dos últimos 25 anos e a maioria destes eventos são resultados de falha humana, além de terem ocorrido com maior frequência em centros sofisticados que utilizam alta tecnologia. Em radioterapia a Gestão da Qualidade necessita de uma abordagem prospectiva através de análise de risco. Esta abordagem é defendida por recentes publicações por ser atualmente uma urgência em virtude do expressivo número de acidentes ocorridos. Dada a complexidade do processo em radioterapia, a busca pela qualidade do tratamento de forma a garantir a segurança do paciente é um dos assuntos mais discutidos mundialmente. Embora apenas 14% dos centros de radioterapia no Brasil ofereçam tratamentos com a técnica de radiocirurgia intracranial (SRS), estudos relativos a qualidade do tratamento relacionada a segurança do paciente submetido à esta técnica, é de relevante importância, pois qualquer desvio da dose prescrita é considerado muito mais crítico que em outras modalidades de tratamento radioterápico, já que são utilizadas altas doses de radiação que em geral variam de 10 Gy a 40 Gy para lesões até 50 mm de diâmetro, que são aplicadas em uma única fração ou até cinco frações. Tendo em vista tais conhecimentos, o objetivo deste trabalho foi atender um novo paradigma de Gestão da Qualidade, através do desenvolvimento de um modelo de análise de risco e de um Índice de Qualidade para a SRS no Brasil, a partir da técnica de Mapa do Processo e FMEA utilizadas pelo TG 100/AAPM. O trabalho foi desenvolvido em três centros de radioterapia de referência em alta tecnologia, dois localizados no Rio de Janeiro e um em São Paulo. Um Mapa do Processo de SRS foi identificado em cada centro de radioterapia e em seguida foi aplicada a técnica FMEA para todos os subprocessos identificados no mapa. A partir dos valores de NPR obtidos pela FMEA foi realizado um ranqueamento dos modos de falha. Modos de falha com NPR ≥ 100 e S ≥ 7 foram escolhidos como prioridade para implementação de estratégias de segurança. A partir das pontuações do parâmetro S atribuídas na aplicação da FMEA foi criado o Índice de Severidade (IS) e um Índice de Qualidade (IQ) foi criado a partir de uma relação entre o NPR e o IS. O resultado deste estudo, indica que as estratégias de segurança sejam implementadas para os 10 primeiros modos de falha do ranking e uma reavaliação do processo deve ocorrer a cada 1 ano. É também indicado que o IQ obtenha uma melhora mínima de 9% após a reavaliação do processo. De forma geral, o estudo mostrou que a adoção da ferramenta FMEA juntamente com o IQ são de fato justificadas, por minimizar os riscos para o paciente, melhorando a qualidade do tratamento aprimorando a segurança, criando mecanismos que permitam a garantia de que a dose será entregue de forma precisa e exata, e consequentemente, aumentando as chances de cura ou do controle local com uma desejada qualidade de vida para estes pacientes.

From 1992 to 1996, 153 bottlenose dolphin stranded in South Carolina, accounting for 73% of all marine mammal strandings during this period. The objectives of our study were to evaluate data from these strandings to deter-mine 1) annual trends in strandings, 2) seasonal and spatial distribution trends, 3) life history parameters such as sex ratio and age classes, 3) seasonal trends in reproduction, and 4) the extent to which humans have played a role in causing these strandings (human inter-actions). The results showed that 49% of the bottlenose dolphin strandings occurred between April and July; the greatest number of strandings occurred in July (n=22). There was a significant seasonal increase in the distribution of bottlenose dolphin strandings in the northern portion of the state from November to March. Bottlenose dolphin neonates stranded in every month of the year, except March and October, and represented 19.6% of the total number of strandings with known length (n=138). Fifty-five percent (n=15) of bottlenose dolphin neonatal strandings occurred between May and July. Bottlenose dolphins determined to have died as the result of human interaction accounted for 23.1% of the total number of bottlenose dolphin strandings (excluding those for which a determination could not be made).Incidents of bottlenose dolphin entanglements in nets accounted for 16 of these cases.

OBJECTIVE: This study identifies the stakeholders who have a role in medical device purchasing within the wider system of health-care delivery and reports on their particular challenges to promote patient safety during purchasing decisions. METHODS: Data was collected through observational work, participatory workshops, and semi-structured qualitative interviews, which were analyzed and coded. The study takes a systems-based and engineering design approach to the study. Five hospitals took part in this study, and the participants included maintenance, training, clinical end-users, finance, and risk departments. RESULTS: The main stakeholders for purchasing were identified to be staff from clinical engineering (Maintenance), device users (Clinical), device trainers (Training), and clinical governance for analyzing incidents involving devices (Risk). These stakeholders display varied characteristics in terms of interpretation of their own roles, competencies for selecting devices, awareness and use of resources for purchasing devices, and attitudes toward the purchasing process. The role of "clinical engineering" is seen by these stakeholders to be critical in mediating between training, technical, and financial stakeholders but not always recognized in practice. CONCLUSIONS: The findings show that many device purchasing decisions are tackled in isolation, which is not optimal for decisions requiring knowledge that is currently distributed among different people within different departments. The challenges expressed relate to the wider system of care and equipment management, calling for a more systemic view of purchasing for medical devices.

In recent years, the healthcare sector has adopted the use of operational risk assessment tools to help understand the systems issues that lead to patient safety incidents. But although these problem-focused tools have improved the ability of healthcare organizations to identify hazards, they have not translated into measurable improvements in patient safety. One possible reason for this is a lack of support for the solution-focused process of risk control. This article describes a content analysis of the risk management strategies, policies, and procedures at all acute (i.e., hospital), mental health, and ambulance trusts (health service organizations) in the East of England area of the British National Health Service. The primary goal was to determine what organizational-level guidance exists to support risk control practice. A secondary goal was to examine the risk evaluation guidance provided by these trusts. With regard to risk control, we found an almost complete lack of useful guidance to promote good practice. With regard to risk evaluation, the trusts relied exclusively on risk matrices. A number of weaknesses were found in the use of this tool, especially related to the guidance for scoring an event's likelihood. We make a number of recommendations to address these concerns. The guidance assessed provides insufficient support for risk control and risk evaluation. This may present a significant barrier to the success of risk management approaches in improving patient safety. © 2013 Society for Risk Analysis.

There is a need to obtain the hydrologic data including ocean current, wave, temperature and so on in the South China Sea. A new profiling instrument which does not suffer from the damage due to nature forces or incidents caused by passing ships, is under development to acquire data from this area. This device is based on a taut single point mid-water mooring system. It incorporates a small, instrumented vertically profiling float attached via an electromechanical cable to a winch integral with the main subsurface flotation. On a pre-set schedule, the instrument float with sensors is winched up to the surface if there is no strip passing by, which is defined by an on-board miniature sonar. And it can be immediately winched down to a certain depth if the sonar sensor finds something is coming. Since, because Of logistics, the area can only be visited once for a long time and a minimum of 10 times per day profiles are desired, energy demands are severe. To respond to these concerns, the system has been designed to conserve a substantial portion of the potential energy lost during the ascent phase of each profile and subsequently use this energy to pull the instrument down. Compared with the previous single-point layered measuring mode, it is advanced and economical. At last the paper introduces the test in the South China Sea.

The frequency and scale of Harmful Algal Bloom (HAB) and marine algal toxin incidents have been increasing and spreading in the past two decades, causing damages to the marine environment and threatening human life through contaminated seafood. To better understand the effect of HAB and marine algal toxins on marine environment and human health in China, this paper overviews HAB occurrence and marine algal toxin incidents, as well as their environmental and health effects in this country. HAB has been increasing rapidly along the Chinese coast since the 1970s, and at least 512 documented HAB events have occurred from 1952 to 2002 in the Chinese mainland. It has been found that PSP and DSP toxins are distributed widely along both the northern and southern Chinese coasts. The HAB and marine algal toxin events during the 1990s in China were summarized, showing that the HAB and algal toxins resulted in great damages to local fisheries, marine culture, quality of marine environment, and human health. Therefore, to protect the coastal environment and human health, attention to HAB and marine algal toxins is urgently needed from the environmental and epidemiological view.

In this paper, We analyzed the geological and geographical settings of dinosaurs extinction at the end of Cretaceous, especially the effect of the change of the elements contents on dinosaurs extinction. We studied basis on the two typical sections-Cretaceous-Paleocene boundary (Baishantou section (in Jiayin, Heilongjiang province of China) and Arkhara-Boguchan Coal Mine section (in Far East of Russian)) and Longgushan section (in Jiayin, Heilongjiang province of China) mainly. This work provided some evidences for forecasting the effects of global environmental change on bio-circle. The followings are the primary gains: According to the paleo-climate indexes (CaO/MgO,Sr/Ba) and the results of Factor Analysis, we found that there were similar climate in Baishantou section and Arkhara-Boguchan Coal Mine section near the K/E boundary, and both of them took on the trend of temperature declining and precipitation heightening after transitory high-temperature and drought. There are similar change and evlution rule of the elements contents near the boundary in the both sections (Baishantou section and Arkhara-Boguchan Coal Mine section). Both iron group elements and chalcophile elements appeared obvious abnormity. There are not visible correlation between the change of elements contents and climate indexes. This shows that the elements abnormity maybe came from the factors excluding climate or the factors were too many to conceal the influence of climate. --The result of cluster analysis showed that the boundary between BST3-8 and BST3-9 may be the K/E boundary of Baishantou section, and the top of twofold coal were the K/E boundary of Arkhara-Boguchan Coal Mine section which was consistent with accepted conclusion formerly. By contrast of elements contents in dinosaur bones and general organism, in surrounding rock and general sand stone, the regulation of the change of elements contents in dinosaur bones and surrounding rock, we confirmed that dinosaur extinction in Jiayin were relative with the high abnormities of Sr, Ba, Pb, Cr and the low abnormity of Zn, at least, it was them which speeded up dinosaurs extinction. After a series of analysis, we concluded that dinosaurs extinction of this areas tied up with the relative high background values of geo-chemical elements , paleo-climate and disaster incidents. First of all, high background values provided the necessary condition for the accumulation of the elements. Secondly, the drought climate adverse to the survival of dinosaurs, and led them to extinct gradually. finally, disaster incidents, the eruption of volcano or the collision of aerolites, made them exit this planet.

This research aims to discuss it is the complexity of interpersonal association and job autonomy that influence the predictive validity of personality for job performance. In addition, for service profession, incumbents' personality can predict not only contextual performance, but also task performance. Salesclerks in shopping center and life insurance agents are selected as subjects. The job performance rating scale is produced by using Critical Incidents Technique. The research method is measuring NEO-PI and collecting direct supervisors' rating of salespeople's job performance. The research results are as follows: 1. The factor analysis result of job performance is different from the west. That is to say, the support for organizations which belongs to contextual performance in the west can not be distinguished from task performance. Therefore, in China, or to say in the shopping center selected, task performance includes both technical proficiency and the support for organizations, and contextual performance includes job dedication and interpersonal facilitation. 2. For salespeople, personality can be the antecedent of contextual performance and task performance as well. However, the predictive validity for task performance is very low. 3. The more complexity of interpersonal association, the stronger relationship between personality and job performance. 4a. The correlation between job performance and facets of Big Five is higher than the one between job performance and factors of Big Five, such as Agreeableness, whose facets have different impacts on job performance, some positive and others negative. 4b. The correlation between personality and the items of job performance rating scale is higher than the one between personality and the factors of job performance. 4. Working experience is the moderator of the relationship between personality and job performance. For salesclerks, only if the working experience of subjects is less than 3 years, achievement striving-one facet of conscientiousness-is significantly correlated with the ratio of finished sales volume at 0.01 level.

Bullying incidents in traditional and online settings are a cause for concern to many parties. The goal of the current study was to explore the extent to which a bystander would intervene in a bullying incident and the degree to which this behavior is influenced by group size (the number of other witnesses), the setting (traditional or cyberbullying), and gender of the victim. Using an online survey method, participants were presented with eight bullying scenarios, each of which involved verbal bullying of a victim. Participants (N = 82) were asked to report how likely they would be to intervene in each of these scenarios. Results showed that female victims were more likely to be helped than male victims. Furthermore, female participants were more willing to intervene than the male participants in the cyberbullying scenarios. Altruism was a positive predictor of participants’ willingness to intervene. The present findings suggest that certain gender differences in helping behavior may depend on the context in which bullying is observed (traditional or cyberbullying).

From 2008-2012, a dramatic upsurge in incidents of maritime piracy in the Western Indian Ocean led to renewed global attention to this region: including the deployment of multi national naval patrols, attempts to prosecute suspected pirates, and the development of financial interdiction systems to track and stop the flow of piracy ransoms. Largely seen as the maritime ripple effect of anarchy on land, piracy has been slotted into narratives of state failure and problems of governance and criminality in this region.

This view fails to account for a number of factors that were crucial in making possible the unprecedented rise of Somali piracy and its contemporary transformation. Instead of an emphasis on failed states and crises of governance, my dissertation approaches maritime piracy within a historical and regional configuration of actors and relationships that precede this round of piracy and will outlive it. The story I tell in this work begins before the contemporary upsurge of piracy and closes with a foretaste of the itineraries beyond piracy that are being crafted along the East African coast.

Beginning in the world of port cities in the long nineteenth century, my dissertation locates piracy and the relationship between trade, plunder, and state formation within worlds of exchange, including European incursions into this oceanic space. Scholars of long distance trade have emphasized the sociality engendered through commerce and the centrality of idioms of trust and kinship in structuring mercantile relationships across oceanic divides. To complement this scholarship, my work brings into view the idiom of protection: as a claim to surety, a form of tax, and a moral claim to authority in trans-regional commerce.

To build this theory of protection, my work combines archival sources with a sustained ethnographic engagement in coastal East Africa, including the pirate ports of Northern Somalia, and focuses on the interaction between land-based pastoral economies and maritime trade. This connection between land and sea calls attention to two distinct visions of the ocean: one built around trade and mobility and the other built on the ocean as a space of extraction and sovereignty. Moving between historical encounters over trade and piracy and the development of a national maritime economy during the height of the Somali state, I link the contemporary upsurge of maritime piracy to the confluence of these two conceptualizations of the ocean and the ideas of capture, exchange, and redistribution embedded within them.

The second section of my dissertation reframes piracy as an economy of protection and a form of labor implicated within other legal and illegal economies in the Indian Ocean. Based on extensive field research, including interviews with self-identified pirates, I emphasize the forms of labor, value, and risk that characterize piracy as an economy of protection. The final section of my dissertation focuses on the diverse international, regional, and local responses to maritime piracy. This section locates the response to piracy within a post-Cold War and post-9/11 global order and longer attempts to regulate and assuage the risks of maritime trade. Through an ethnographic focus on maritime insurance markets, navies, and private security contractors, I analyze the centrality of protection as a calculation of risk and profit in the contemporary economy of counter-piracy.

Through this focus on longer histories of trade, empire, and regulation my dissertation reframes maritime piracy as an economy of protection straddling boundaries of land and sea, legality and illegality, law and economy, and history and anthropology.

We asked 1004 undergraduates to estimate both the probability that they would enter therapy and the probability that they experienced but could not remember incidents of potentially life-threatening childhood traumas or physical and sexual abuse. We found a linear relation between the expectation of entering therapy and the belief that one had, but cannot now remember, childhood trauma and abuse. Thus individuals who are prone to seek psychotherapy are also prone to accept a suggested memory of childhood trauma or abuse as fitting their expectations. In multiple regressions predicting the probability of forgotten memories of childhood traumas and abuse, the expectation of entering therapy remained as a substantial predictor when self-report measures of mood, anxiety, post-traumatic stress disorder symptom severity, and trauma exposure were included.

While incidents requiring the rapid egress of passengers from trains are infrequent, perhaps the most challenging scenario for passengers involves the evacuation from an overturned carriage subjected to fire. In this paper we attempt to estimate the flow rate capacity of an overturned rail carriage end exit. This was achieved through two full-scale evacuation experiments, in one of which the participants were subjected to non-toxic smoke. The experiments were conducted as part of a pilot study into evacuation from rail carriages. In reviewing the experimental results, it should be noted that only a single run of each trial was undertaken with a limited — though varied — population. As a result it is not possible to test the statistical significance of the evacuation times quoted and so the results should be treated as indicative rather than definitive. The carriage used in the experiments was a standard class Mark IID which, while an old carriage design, shares many features with those carriages commonly found on the British rail network. In the evacuation involving smoke, the carriage end exit was found to achieve an average flow rate capacity of approximately 5.0 persons/min. The average flow rate capacity of the exit without smoke was found to be approximately 9.2 persons/min. It was noted that the presence of smoke tended to reduce significantly the exit flow rate. Due to the nature of the experimental conditions, these flow rates are considered optimistic. Finally, the authors make several recommendations for improving survivability in rail accidents.
