This synthesis of the literature provides descriptive analysis and outlines current self-management interventions for African Americans with type 2 diabetes. Specifically, this study describes and explores the design of those studies whose interventions have been shown to lower HbA1C levels in this population by at least 0.5% points, an improvement that provides approximately 10% reduction in long term complications from this disease.^ Results. In total, 37 articles were reviewed and 17 articles met inclusion criteria for analysis. Analysis of each study's methodology and results was performed and selected studies with interventions that resulted in improvements in HbA1C outcomes equal to 0.5% or greater for both group 1 and 2 were summarized by intervention type in table format. Descriptive analysis, outlining the number and characteristics of proximal and distal mediating components addressed in Group 1 studies, was performed in order to determine whether mediating components may have had some relation to effectiveness of intervention on outcome HbA1C. Descriptive analysis revealed that no particular design is substantially more effective than another among Behavioral studies although, there may be an advantage in using culturally sensitive, group interventions that address greater numbers of distal mediating components. Among Process studies, structured approaches (i.e. algorithm care and scheduled follow up), as well as utilization of specialty and group care are represented as effective for African American populations. ^ Conclusions. It may be summarized that by targeting behavior and addressing provider delivery (i.e. algorithm use, group care, home care, and provider follow up) in this population, a greater yield in outcome improvements may be accomplished. However, many gaps exist in a review process that stratifies results and focuses on identifying group specific intervention successes and failures. Further research in different populations will aid researchers and practitioners in discovering the best evidence, and identifying models that could be utilized in practice to achieve the best diabetes management for at risk groups.^

Background. Decision-making on reproductive issues is influenced by an interplay of individual, familial, medical, religious and socio-cultural factors. Women with chronic medical illnesses such an HIV infection and cancers are often fraught with decisional conflicts about child-bearing. With increase in the incidence of these illnesses as well as improvement in survival rates, there is a need to pay due attention to the issue of reproductive decision-making. Examining the prevalence and determinants of fertility desires in the two groups in a comparative manner would help bring to light perception of the medical community and the society in general on the two illnesses and the issue of motherhood. ^ Methods. Systematic literature search was undertaken using databases such as MEDLINE (PubMED), MEDLINE (Ovid), PsycInfo and Web of Science. Articles published in English and English language abstracts for foreign articles were included. Studies that explore ‘fertility desires’ as the outcome variable were included. Quantitative studies which have assessed the prevalence of fertility desires as well as qualitative studies which have provided a descriptive understanding of factors governing reproductive desires were included in the review. ^ Results. A total of 34 articles (29 studies examining HIV and 5 studies examining cancer in relation to fertility desires). Variables such as age, stage of illness, support of spouse and family, perception of the medical community and one’s own view of motherhood were key determinants among both groups. ^ Conclusion. There is a need for uniform, systematic research in this field. It is important that health care workers acknowledge these decisional conflicts, include them as part of the medical care of these patients and provide guidance with the right balance of information, practicality and compassion.^

Background. MRSA (methicillin-resistant Staphylococcus aureus) is a multi-drug resistant bacterium that is quite prevalent in social environments where close person-to-person contact and crowding are an issue. In dental settings, the likelihood of transmission of MRSA may be higher than among other healthcare practitioners because of the close proximity between a patient's nose (where MRSA colonizes) and the field of procedure (the mouth) to the dental professional. Objective. To estimate the prevalence of MRSA nasal colonization among dental professionals (dentists and dental hygienists) in the Greater Houston Metropolitan Area, Texas, and analyze its associations with demographic, professional and personal protective equipment-related variables. Methods. 800 dental professionals (400 dentists and 400 dental hygienists) were randomly selected in the Greater Houston Metropolitan Area. Multiple waves of nasal swab kits and a self-administered questionnaire were mailed to increase the response rate of the study population. The swabs were cultured on chromagenic agar growth medium and bacterial growth results were evaluated after 18 hours. Positively selected bacterial colonies were confirmed as MRSA by further culturing these isolated bacteria on blood agar plates. Associations between positive nasal swabs and self-reported professional practice patterns, personal protective equipment use and demographics were analyzed using multiple logistic regression. Main Results. Completed questionnaires and nasal swabs were received from 496 study participants (68%). Fourteen cultures were positive for MRSA (4.2% among dentists and 1.6% among dental hygienists, p=0.07). After adjusting for gender, dental hygienists had a significantly lower prevalence of nasal colonization of MRSA as compared to dentists (OR: 0.20, 95% CI: 0.05–0.75). No other significant associations or interactions were found. Conclusion. The prevalence of nasal colonization with MRSA among dentists is similar to that reported for health care workers in general, whereas prevalence among dental hygienists is only slightly above that of the general population (1%). Differences in practice patterns and use of personal protective equipment did not explain this difference in this study, and was possibly due either to residual confounding or unexplored risk factors. Increased prevalence of MRSA among dentists warrants further investigation as to the reason for the increased rate and to allow implementation of measures to avoid transmission and progression to disease. ^

The association between Social Support, Health Status, and Health Services Utilization of the elderly, was explored based on the analysis of data from the Supplement on Aging to the National Health Interview Survey, 1984 (N = 11,497) using a modified framework of Aday and Andersen's Expanded Behavioral Model. The results suggested that Social Support as operationalized in this study was an independent determinant of the use of health services. The quantity of social activities and the use of community services were the two most consistent determinants across different types of health services use.^ The effects of social support on the use of health services were broken down into three components to facilitate explanations of the mechanisms through which social support operated. The Predisposing and Enabling component of Social Support had independent, although not uniform, effects on the use of health services. Only slight substitute effects of social support were detected. These included the substitution of the use of senior centers for longer stay in the hospital and the substitution of help with IADL problems for the use of formal home care services.^ The effect of financial support on the use of health services was found to be different for middle and low income populations. This differential effect was also found for the presence of intimate networks, the frequencies of interaction with children and the perceived availability of support among urban/rural, male/female and white/non-white subgroups.^ The study also suggested that the selection of appropriate Health Status measures should be based on the type of Health Services Utilization in which a researcher is interested. The level of physical function limitation and role activity limitation were the two most consistent predictors of the volume of physician visits, number of hospital days, and average length of stay in the hospital during the past year.^ Some alternative hypotheses were also raised and evaluated, when possible. The impacts of the complex sample design, the reliability and validity of the measures and other limitations of this analysis were also discussed. Finally, a revised framework was proposed and discussed based on the analysis. Some policy implications and suggestions for future study were also presented. ^

Intensive family preservation services (IFPS), designed to stabilize at-risk families and avert out-of-home care, have been the focus of many randomized, experimental studies. Employing a retrospective “clinical data-mining” (CDM) methodology (Epstein, 2001), this study makes use of available information extracted from client records in one IFPS agency over the course of two years. The primary goal of this descriptive and associational study was to gain a clearer understanding of IFPS service delivery and effectiveness. Interventions provided to families are delineated and assessed for their impact on improved family functioning, their impact on the reduction of family violence, as well as placement prevention. Findings confirm the use of a wide range of services consistent with IFPS program theory. Because the study employs a quasi-experimental, retrospective use of available information, clinical outcomes described cannot be causally attributed to interventions employed as with randomized controlled trials. With regard to service outcomes, findings suggest that family education, empowerment services and advocacy are most influential in placement prevention and in ameliorating unmanageable behaviors in children as well as the incidence of family violence.

The emphasis on "gold-standard" evaluation of IFPS has resulted in fewer "black box" studies that describe actual IFPS service patterns and the fidelity with which they adhere to IFPS program theory. Intervention research is important to the advancement of programs designed to protect the safety of children, improve family functioning, as well as prevent out-of-home placement. Employing a retrospective “clinical data-mining” (CDM) methodology, this exploratory study of Families First, an IFPS program, makes use of available information extracted from client records to describe interventions and service patterns provided over a two year period. This study uncovers actual IFPS service patterns, demonstrates IFPS program fidelity, as well as reveals the usefulness of CDM as a social work research methodology. These findings are particularly valuable for program planning and treatment, policy development and evidence-based practice research.

This evaluation of the first year of an Intensive Family Preservation Service in England is based on the analysis of eighty-six families: fifty-seven families who received the service and a comparison group of twenty-nine families who did not. The study considered whether the program was fulfilling its objectives of reducing the number of children and young people in the public care system; offering a safe, supportive service for children who need protection; integrating the program into family support services as a whole, and improving family functioning. The findings were complex to interpret. Child protection was improved but there was not a reduction in the number of children needing out of home care (indeed there was an increase) meaning that short term savings in costs could not be made. Nor were there lasting improvements in the children’s behavior. There were instead a number of more subtle, arguably more sensitive outcomes: parents’ capacity to tolerate their child’s behavior was greater and overall family functioning was better for most families who received the service. Also families were, on the whole, able to make better use of follow up services.

In this essay I review a recent research study from Italy, “Le Radici nel Futuro – La Continuita’ della Relazione Genitoriale oltre la Crisi Familiare,” edited by Paola Dallanegra (2005). The contributors focus on “Spazio Neutro,” a multi-purpose child welfare agency in southern Italy that facilitates parent-child visiting and relationships between children placed in out-of-home care and their families. They delineate and illustrate, through comments from family members, selected principles and strategies for maintaining such continuity throughout the out-of-home placement.

The presented work aims to contribute towards the standardization and the interoperability off the Future Internet through an open and scalable architecture design. We present S³OiA as a syntactic/semantic Service-Oriented Architecture that allows the integration of any type of object or device, not mattering their nature, on the Internet of Things. Moreover, the architecture makes possible the use of underlying heterogeneous resources as a substrate for the automatic composition of complex applications through a semantic Triple Space paradigm. Created applications are dynamic and adaptive since they are able to evolve depending on the context where they are executed. The validation scenario of this architecture encompasses areas which are prone to involve human beings in order to promote personal autonomy, such as home-care automation environments and Ambient Assisted Living.

El presente trabajo recoge los resultados de una investigación sobre la «vivienda productiva», es decir, de la casa como lugar de trabajo. Dicha investigación se ha ocupado de explorar cómo influyen las cotidianeidades doméstico-laborales actuales —junto con sus arquitecturas y los urbanismos que estas generan— en la sociedad, en especial en temas sociales centrales como la construcción de las subjetividades, y de una vida en comunidad. Asimismo, explora cómo podrían influir en un futuro. La hipótesis sobre la que se trabaja, y que finalmente se intenta demostrar, consiste en que, en la territorialidad múltiple de lo laboral, la vivienda, como plataforma doméstica multiusos, productiva y reproductiva de la que muchas personas disponen, desempeña un papel nodal, y es posible que en el futuro su papel sea aún más importante y más visible. La vivienda productiva funciona como una infraestructura urbana desde la cual se construyen diferentes tipos de bienes materiales e inmateriales, pero sobre todo formas de relación, sociales y productivas con otros, además de imaginarios y afectos. La vivienda como infraestructura productiva y reproductiva es tanto «fábrica fundamental de lo social», como nos enseña Silvia Federici, como «fábrica de lo urbano». El trabajo en casa, como parte de diferentes dinámicas «dispersas» del trabajo que se dan en la actualidad, presenta tanto problemáticas y desafíos importantes, como capitales y posibilidades sociales. Por un lado, puede facilitar una absorción de tipo «24/7» vinculada a las lógicas de producción dispersa y consumo dominantes además de la que se da por las dinámicas patriarcales tradicionales, y en múltiples casos situaciones de evidente asimetría y de explotación laboral. También puede propiciar situaciones socialmente extendidas o impuestas de soledad y aislamiento. Sin embargo, está asimismo vinculado al surgimiento de espacios de experimentación tanto en soledad como en compañía, de cooperación y de intercambio, además de a la evidente posibilidad de construir rutinas propias y, por tanto, una subjetividad laboral propia. Por otro lado, frente a la idea general y homogénea del trabajador casero como un sujeto mayoritariamente aislado de su entorno, las jornadas de trabajo de los trabajadores caseros estudiados presentan una gran diversidad. Estas pueden conllevar aislamiento y exclusión, así como acompañar diferentes formas de construcción de una ciudadanía activa, tanto en solitario, como mediante formas de participación activa en la construcción de una vida en comunidad. En esta investigación se analizan, pues, desafíos y oportunidades de estas espacialidades del trabajo y, a través de este análisis, se indaga en el papel de los marcos físicos, organizativos y simbólicos actuales para la vivienda y para la ciudad pensada desde los trabajadores caseros. Al mismo tiempo se exploran nuevos tipos de vivienda y urbanismos que puedan quizás acompañar procesos sociales de inclusión, así como de emancipación, cooperación e intercambio y, en general, nuevas formas de vecindad y bienestar compartido en los ámbitos doméstico-productivos. Metodológicamente, las singularidades presentes en las realidades estudiadas invitan además a explorar protocolos de análisis, métodos de diseño, de gestión y gobierno que, desde lo micro, puedan dialogar con lo cotidiano y sus singularidades. Diego Barajas, autor de la tesis, es arquitecto dedicado a la práctica, a la investigación y la docencia. Su trabajo se focaliza en explorar el carácter mediador de la arquitectura frente a lo cotidiano y sus microrrealidades. Es profesor de proyectos de urbanismo en la IE School of Architecture y es miembro de Husos, una plataforma de investigación y diseño orientada a la innovación en arquitectura y urbanismo, con sede en Madrid. Es el autor del libro Dispersion, A Study of Global Mobility and the Dynamics of a Fictional Urbanism (Episode Publishers, Róterdam, 2003). Sus trabajos teóricos y de investigación han aparecido en libros y revistas internacionales tales como The Domestic and The Foreign in Architecture (010 Publishers, 2008), Public Spheres, a Europan Discussion (Europan9, 2007), Photoespaña 05 (La Fábrica, Madrid, 2005), Domus, Volume, Abitare, Architese, Plot, y Summa+. Su trabajo se ha expuesto en la Bienal de Venecia (selección principal), la Bienal de Róterdam, la Fundación Tapies, Photoespaña, entre otros y es parte de la colección permanente del FRAC Centre en Orleans y del Historisch Museum de Róterdam entre otros. Antes de estudiar en Róterdam, se gradúa con Honores en la Universidad de los Andes en 1999 y su tesis de grado de arquitectura recibe la máxima distinción como proyecto de Grado Meritorio. ABSTRACT The present study records an investigation into the «productive house», or the home as a workplace. This investigation looks at how working from home, its architectures, and the urban dynamics generated around it influence daily life and the construction of society. It also explores what influences home-based work might have in the future, particularly in central themes such as the biopolitical construction of subjectivities and community life. The central hypothesis revolves around the idea that the home, as a multi-use infrastructure that most of us have access to, plays a fundamental role in the contemporary work sphere, due to —or even in spite of— the omnipresence of work in practically every moment and area of our daily lives, and might play a main role in the future. The home functions as a kind of hub from which we create different kinds of material and immaterial goods, but above all relationships with others, both social and productive, as well as imaginaries and affections. The home, as a machine for production and reproduction, is as much a main «factory of the social» as Silvia Federici describes it, as a «factory of the urban». Working from home, as one of several «dispersed» work dynamics that are part of the contemporary world of work, presents as many important problematics and challenges as it does possibilities and social capital. On the one hand, it can contribute to kind of «24/7» absorption linked both to the logistics of the current dispersed production and consumption, and to that of traditional patriarchal dynamics, and in some cases even clearly asymmetrical situations that exploit the workforce. It can also lead to the socially widespread phenomena of isolation and loneliness, at times imposed upon home-based workers. However, by the same token, it is also linked to the growth of experimental spaces of cooperation and exchange, both solitary and in company, as well as the clear possibility of constructing individualised routines and, therefore, a personal laboural subjectivity. On the other hand, contrary to the generalised, homogenous idea of a home-based worker who is mostly isolated from his or her environment, the working days of those we studied went hand in hand with different ways of building subjectivities. This could be as much in exclusion and isolation as through building an active citizenship, at times in solitary and at times actively participating in the construction of communal life. In this investigation, therefore, the challenges and opportunities of the home as workplace have been analysed, and through this analysis we have inquired into the role of current physical, organisational and symbolic frameworks for the home and for the city from the perspective of home workers. At the same time we have explored new types of homes and of town planning that could perhaps accompany social processes of diversity and inclusion as well as emancipation, cooperation and exchange and, in general, new forms of shared welfare in productive domestic environments. Methodologically, the singularities present in the cases studied also invite us to explore protocols of analysis, methods of design, management and government that, from the micro level, could dialogue with the peculiarities of day-to-day life. The author of this thesis, Diego Barajas, is an architect dedicated to practice, investigation and teaching. His work is focussed on exploring the mediating nature of architecture and the built environment in terms of daily life and its microrealities. He is professor of urban projects at the IE School of Architecture and member of Husos, a platform for investigation and town planning based in Madrid. He is the author of the book Dispersion, A Study of Global Mobility and the Dynamics of a Fictional Urbanism (Episode Publishers, Rotterdam, 2003) and his theoretical works and investigations have appeared in books such as The Domestic and The Foreign in Architecture (010 Publishers, 2008), Public Spheres, a Europan Discussion (Europan9, 2007), Photoespaña 05 (La Fábrica, Madrid, 2005), and in international magazines such as Domus, Volume, Abitare, Architese, Plot and Summa+. His work has been exhibited in places such as the Venice Biennial (main selection), the Rotterdam Biennial, the Tapies Foundation and Photoespaña, among others, and forms part of the permanent collection of the FRAC Centre in Orleans and the Historisch Museum of Rotterdam. Entre instituições e atividades ligadas à igreja como reuniões de oração, cultos, ensaios de corais, ministério infantil, casa, filhos, vida conjugal, etc. está presente uma figura ainda não estudada pelas Ciências da Religião, a da esposa do pastor batista. O objetivo principal desta pesquisa foi o de analisar o processo de construção e de manutenção das representações sociais dessas mulheres. Para isso, no primeiro capítulo identificamos e analisamos as representações de gênero presentes na estrutura e no cotidiano eclesiástico da denominação e seus reflexos na elaboração e na conservação do perfil das esposas de pastores. No segundo capítulo realizamos uma análise do poder e do papel das instituições de formação teológica e ministerial da denominação batista como formas de idealização, de construção e de manutenção das representações sociais das esposas de pastores. Investigamos no terceiro capítulo as formas do trânsito das esposas de pastores entre os espaços públicos e privados - imbricados um no outro, e as formas em que o poder institucional atua nestes lugares. Para este fim, utilizamos o método etnográfico, observando as igrejas batistas selecionadas, aplicando questionários aos membros das igrejas, líderes institucionais e estudantes da Faculdade Teológica Batista do Paraná FTBP e realizando entrevistas semi-estruturadas com os pastores e suas esposas. As teorias das Ciências da Religião e Sociais nos proporcionaram o suporte teórico necessário para o desenvolvimento do texto e para a análise dos dados empíricos. (AU)

O contexto batista é predominantemente marcado por lideranças masculinas, destinando às mulheres apenas lugares e comportamentos socialmente estabelecidos, como a casa, o cuidado, a maternidade, a submissão, entre outras características que enfatizam a hierarquia de gênero. Mesmo diante do desenvolvimento econômico e da ocupação que as mulheres estão conquistando no campo público, a igreja e principalmente as igrejas batistas, permanecem fundadas em alicerces que exaltam o poder masculino em detrimento do lugar que deve ser ocupado pelas mulheres, ou seja, onde elas decidirem atuar. Caso elas decidam atuar num campo predominantemente masculino, terão que lidar com a desconstrução de um pensamento socialmente permeado de dominação masculina e com a árdua construção de um pensamento que vise a igualdade de gênero. O objeto desta pesquisa é o ministério pastoral feminino no contexto batista brasileiro. O texto analisa o discurso das Pastoras Batistas do Estado de São Paulo e o discurso dos líderes da Ordem dos Pastores Batistas de São Paulo (OPBB-SP) a respeito do ministério pastoral feminino e a não filiação de mulheres na OPBB-SP. A importância deste trabalho é a de demostrar as relações de micro poder existentes entre pastores e pastoras e concomitantemente as desigualdades dentro do contexto batista com relação ao ministério pastoral feminino. Essa afirmação se consolida por meio das análises das entrevistas semiestruturadas que realizei na pesquisa de campo, com sete pastoras batistas do Estado de São Paulo, bem como com três líderes da OPPB-SP. Esta é uma pesquisa qualitativa, em que foram analisados documentos oficiais da igreja, como pautas de convenções, atas, sites institucionais, periódicos e documentos não oficiais encontrados em redes sociais, blogs, jornais online, entre outros. Posso afirmar que as pastoras batistas estão se mobilizando para cumprir sua vocação, usando argumentos transcendentes que impedem qualquer pessoa de desafiar ou duvidar de seu chamado pastoral, pois: “O vento sopra onde quer; ouve-se o ruído, mas não sabes de onde vem, nem para onde vai. Assim acontece com aquele(a) que nasceu do Espírito.” (João 3.8).

A transformação nos modos de organizar e produzir atenção em saúde exige a invenção de dispositivos com potência para proporcionar espaços coletivos de análise acerca da produção da gestão e do cuidado. O apoio é tecido por múltiplas relações, interesses, projetos e agenciamentos. Fabricado no encontro, é um intercessor que pode favorecer reflexões sobre a micropolítica do trabalho, sobre os encontros entre trabalhadores e usuários, entre trabalhadores e entre trabalhadores e gestão, agenciando possibilidades de análise do cotidiano e interferências sobre os modos de cuidar e de gerir. As interrogações sobre o cuidado podem abrir zonas de visibilidade aos processos de trabalho, discursos, práticas e relações de poder. A pesquisa procurou analisar o processo de fabricação do apoio na rede de atenção à saúde no município de São Bernardo do Campo e seus efeitos. O município organizou nove núcleos territoriais em saúde, cada qual com um grupo de até cinco apoiadores constituído por trabalhadores com formações variadas. Ademais, conta com uma dupla de facilitadores de educação permanente, ligados à gestão nos diferentes departamentos da Secretaria de Saúde e orientadores de educação permanente que tem a função de dar suporte aos apoiadores e facilitar os processos de educação permanente nos territórios. No desenho de São Bernardo, o apoio é uma ferramenta estratégica para a construção do cuidado em rede e para a análise das práticas de saúde. Há uma forte aposta da gestão na criação de espaços coletivos e dispositivos de conexão entre os departamentos, serviços, gestores e trabalhadores na intenção de suscitar transversalidade e combater a estrutura vertical de sua organização. O município vive uma intensa criação de dispositivos mobilizadores de encontros, propostos para a construção de redes e de gestão compartilhada do cuidado. Por meio de andanças com os apoiadores em variados territórios, conversas, afecções, registros em diário de campo, narrativas e documentos, tecemos uma cartografia: composição de cenários, de perspectivas e de analisadores, sentidos abertos, múltiplos e conectáveis. A abordagem cartográfica acompanha processos, persegue rastros e traçados, se movimenta entre linhas, sustenta problemáticas; nela, somos pesquisadores in-mundos, nos infectamos, nos misturamos, sempre implicados e em produção com os mundos pesquisados. Mesmo sendo uma aposta de governo, tensionamentos e conflitos acontecem no cotidiano, relacionados a diferentes prioridades, agendas, quebra de acordos, interrupção de projetos. As produções do apoio vêm fomentando conexões entre os serviços, contribuindo com o matriciamento de saúde mental e de outras especialidades na atenção básica e entre os trabalhadores da atenção especializada, fortalecendo redes, estimulando análises coletivas sobre o cuidado, criando estratégias e ferramentas, transformações em fluxos e na regulação. O apoio não é função somente do apoiador, pode ser agenciado por variados atores. Por fim, a auto-análise, quando acontece, potencializa o apoio como dispositivo, provisório, ativador de processos e de protagonismo coletivo.
Resumo:
Purpose – Many international retirement migrants are amenity movers undertaking the first move in the late life course model of migration. The purpose of this paper is to examine second moves within the retirement destination community to test whether the model of late life course migration accurately portrays the motivations and housing choices local movers make after retiring to another country. Design/methodology/approach – The paper combines secondary data and survey results to examine the composition of the retiree migrant population in the Alicante province of Spain. The socioeconomic characteristics and housing choices of those who have made a second move since retiring to Spain are compared with those who have not moved through a series of t-tests and chi-square tests. Findings – The paper finds that those who have made a second move within Spain are somewhat typical of second movers in the late life course. They are likely to cite mobility or health problems as a reason for moving and appear to recognize the need for a home that provides living area on one floor. Yet, they are choosing to move within an area that does not provide them with access to informal family care givers. Research limitations/implications – The data are restricted to retirees of two nationalities in one province of Spain. Further research is suggested in other locations and with retirees of other nationalities for comparison. Practical implications – Because many international retirees do not plan to return to their countries of origin, they will create demand for formal in-home care services and supportive retiree housing in the near future in their retirement destination countries. Originality/value – This paper provides understanding of a growing consumer housing segment in retirement destinations.
