RESUMO: A entorse do tornozelo é uma das lesões músculo-esqueléticas mais comuns. A limitação da amplitude de dorsiflexão tem sido demonstrada como uma das consequências desta lesão, bem como um dos factores contribuintes para a recorrência. Vários estudos têm demonstrado que o membro lesado de indivíduos com história de entorse, apresenta uma falha posicional anterior do peróneo. Um estudo realizado em cadáveres revelou que um deslizamento póstero-superior ao nível da articulação tibioperoneal inferior pode contribuir para aumentar a amplitude de dorsiflexão. Está descrita uma técnica de terapia manual que realiza o deslizamento póstero-superior do maléolo lateral associada ao movimento activo de flexão dorsal (MWM). No entanto, não existe, até à data, nenhum estudo que investigue a efectividade desta MWM em indivíduos com limitação da FD e história de entorse unilateral do tornozelo. Desenho de estudo: Ensaio clínico aleatorizado e controlado por placebo, duplamente cego. Objectivos: Avaliar os efeitos imediatos da MWM na articulação tibio-peroneal inferior na amplitude de flexão dorsal e no deslizamento posterior do astrágalo em indivíduos com história de entorse unilateral do tornozelo e limitação da flexão dorsal. O protocolo experimental foi aplicado uma única vez e os seus efeitos comparados com uma intervenção placebo. Metodologia: Uma amostra de 30 indivíduos com história de entorse unilateral e limitação da amplitude de flexão dorsal foi aleatoriamente distribuído por dois grupos: grupo MWM e grupo placebo. Foram avaliados o deslizamento posterior do astrágalo e a avaliação da amplitude de flexão dorsal em carga. As avaliações foram realizadas imediatamente antes e após a intervenção. Resultados: Não foram encontradas diferenças significativas entre os grupos na avaliação inicial (baseline). A realização da one-way ANCOVA revelou que, imediatamente após a intervenção, se verificou um aumento na amplitude de flexão dorsal no grupo MWM (aumento de 1.37 cm (DP, 0.97) significativamente superior ao grupo placebo (diminuição de 0.15cm (DP, 0.63) (P<.001). O deslizamento posterior do astrágalo aumentou 1.51º (DP, 1.77) no grupo MWM, no entanto este aumento não foi significativamente superior ao aumento de 0.76º (DP, 1.26) do grupo placebo (P=.113). Conclusão: Os resultados sugerem que a MWM na articulação tibioperoneal inferior produziram um efeito significativo na amplitude de flexão dorsal embora o mesmo não se tenha verificado no deslizamento posterior do astrágalo. Estes resultados fornecem evidência preliminar para a efectividade da MWM como intervenção em indivíduos com história de entorse unilateral e limitação da amplitude de flexão dorsal.---------------ABSTRACT:Background: Ankle sprains are one of the most common musculo-skeletal injuries. Impaired dorsiflexion range of motion has been shown to be one of the consequences of this injury, as well as one of the contributing factors to recurrence. Several studies have shown the presence of an anterior positional fault of the fibula in injuried ankles. A cadaveric study revealed that a posterosuperior glide of the distal tibiofibular may contribute to improve dorsiflexion. There is a manual therapy technique which provides a posterosuperior glide of the lateral malleolus combined with dorsiflexion active movement (MWM). However, there was no study, until now, that investigated the effectiveness of this MWM in individuals with impaired dorsiflexion and history of unilateral ankle sprain. Design: Double-blind randomized placebo controlled trial. Objectives: To determine the immediate effects of a distal tibiofibular MWM in ankle dorsiflexion and talar posterior glide in patients with history of unilateral ankle sprain and limitation of dorsiflexion. The treatment technique was used as a single treatment against a placebo group. Methods: A sample of 30 subjects with a history of unilateral ankle sprain and limitation of dorsiflexion were randomized into two groups: distal tibiofibular MWM or a placebo group. The outcome measures used in this study were the posterior talar glide and weight-bearing (WB) ankle dorsiflexion range of motion. The measures were taken before and immediately after the intervention. Results: No significant differences were found in baseline measures between groups. A one-way ANCOVA revealed that, immediately after the intervention, there was an improvement in ankle dorsiflexion in the MWM group (increase of 1.37 cm (SD, 0.97) significantly superior to the placebo group (decrease of 0.15cm (SD, 0.63) (P<.001). Posterior talar glide increased by 1.51º (SD, 1.77) for the MWM group, which was more than 0.76º (SD, 1.25) for the placebo intervention although there wasn’t a significant difference between groups (P=.113). Conclusion: This investigation’s findings suggest that an inferior tibio-fibular MWM produced a significant effect on WB dorsiflexion range of motion and posterior talar glide. These results provide preliminary evidence for the efficacy of mobilisations with movement in the management of individuals with history of unilateral ankle sprain and limitation of dorsiflexion.

BACKGROUND: Use of polyclonal anti-hepatitis B surface antigen immunoglobulin (HBIg) has been shown to reduce hepatitis B virus (HBV) recurrence after liver transplantation (LT) and to decrease the frequency of acute cellular rejection (ACR). However, the protective role of HBIg against ACR remains controversial, since HBV infection has been also associated with a lower incidence of ACR. AIM: To assess the relationship between HBIg immunoprophylaxis and the incidence of rejection after LT. METHODS: 260 patients (158 males, 43 ± 14 years old) submitted to LT were retrospectively evaluated and divided into three groups, according to the presence of HBsAg and the use of HBIg. Group I was comprised of HBsAg-positive patients (n = 12) that received HBIg for more than 6 months. Group II was comprised of HBsAg-positive patients that historically have not received HBIg or have been treated irregularly for less than 3 months (n = 10). Group III was composed of 238 HBsAg-negative subjects that have not received HBIg. RESULTS: HBIg-treated patients (group I) had significantly less ACR episodes, when compared to group II and III. No differences between groups II and III were observed. CONCLUSIONS: Long-term HBIg administration contributes independently to reduce the number of ACR episodes after LT.

Cerebral phaeohyphomycosis ("chromoblastomycosis") is a rare intracranial lesion. We report the first human culture-proven case of brain abscesses due to Fonsecaea pedrosoi in Brazil. The patient, a 28 year-old immunocompetent white male, had ocular manifestations and a hypertensive intracranial syndrome. Magnetic resonance imaging (MRI) of the brain revealed a main tumoral mass involving the right temporo-occipital area and another smaller apparently healed lesion at the left occipital lobe. A cerebral biopsy was performed and the pathological report was cerebral chromoblastomycosis. The main lesion was enucleated surgically and culture of the necrotic and suppurative mass grew a fungus identified as Fonsecaea pedrosoi. The patient had received a knife wound sixteen years prior to his hospitalization and, more recently, manifested a pulmonary granulomatous lesion in the right lung with a single non-pigmented form of a fungus present. It was speculated that the fungus might have gained entrance to the host through the skin lesion, although a primary respiratory lesion was not excluded. The patient was discharged from the hospital still with ocular manifestations and on antimycotic therapy and was followed for eight months without disease recurrence. Few months after he had complications of the previous neuro-surgery and died. A complete autopsy was performed and no residual fungal disease was found.

RESUMO - Introdução: Actualmente o uso excessivo dos serviços de Urgência Pediátrica hospitalares constitui uma das preocupações de gestores e administradores hospitalares. Esta má utilização pode contribuir para o aumento descomedido dos custos uma vez que a procura não é adequada para este tipo de serviço. A procura de respostas rápidas aos problemas de saúde contribui para a afluência exagerada e por vezes injustificada por parte da população. Objectivo: O objectivo deste estudo foi efectuar a caracterização da utilização da urgência pediátrica num hospital da área metropolitana de Lisboa, no sentido de compreender se existe, ou não, procura injustificada de cuidados de saúde. Material e métodos: Foi utilizado como suporte metodológico uma abordagem quantitativa, num estudo descritivo - correlacional. Foram analisados 36932 episódios de Urgência Pediátrica ocorridos num período de 6 meses (Julho 2010 a Janeiro 2011). Resultados: A amostra é constituída por 53,6% utentes do sexo masculino e por 46,4% do sexo feminino; onde o intervalo de idades mais representativo situa-se entre o 1 ano e 5 anos de idade. O concelho de Sintra apresenta 61% dos episódios, no entanto proporcionalmente o concelho de Amadora recorre mais à Urgência Pediátrica com 7,48% dos episódios. O dia com maior afluência é Segunda-feira com 17% dos episódios; 75% dos epsiódios ocorre entre as 8h - 20h, periodo de funcionamento dos Centros de Saúde. Foram considerados como não urgentes, 54,9% dos episódios, 88,8% destes são encaminhados para o Centro de Saúde. São não referenciados 90% dos episódios. Existe evidência de utilização injustificada do Serviço de Urgência Pediátrica por 49,5% dos utentes. Verifica-se que a utilização do serviço de urgência depende da residência dos utentes (p=0,001), a população da Amadora recorre mais à urgência. Existe evidência de relação entre o dia da semana e o acesso injustificado (p <0,001), quinta-feira apresenta 17,3% dos episódios não justificados. A recorrência injustificada ao serviço de urgência não depende do género (p=0,137). Existe relação entre a idade e a recorrência justificada, sendo estatisticamente significativa (p=0,006). Conclusão: Pode concluir-se que a “sobre utilização” e “inadequação” da procura são fenómenos presentes no Serviço de Urgência Pediátrica. Assim urgem medidas de contenção da utilização indevida, passando estas provavelmente por uma consciencialização e educação da população. - ABSTRACT - Introduction: Currently the overuse of Pediatric Emergency hospital services is a major concern of managers and hospital administrators. This misure may contribute to increased costs because of the inordinate demand is not appropriate for this type of service. The demand for rapid response to health problems contributing to the influx exaggerated and sometimes unjustified by the population. Objective: The aim of this study was carried out to characterize the use of emergency pediatric hospital in the metropolitan area of Lisbon, in order to understand whether there is or not unjustified demand for health care. Methods: We used it as a methodological support a quantitative approach, a descriptive study - correlation. We analyzed 36,932 episodes of pediatric emergency occurring in a period of six months (July 2010 to January 2011). Results: The sample consists of 53.6% of male users and 46.4% female, where the most representative age range is between 1 and 5 years old. The Municipality of Sintra has 61% of the episodes, but the proportion of the county Amadora Pediatric Emergency appeals more to the 7.48% of the episodes. Monday 17% of the episodes is the busiest day, 75% of episodes occurs between 8 am - 20 h period of operation of the Health Centres were considered non-urgent, 54.9% of episodes, 88.8 % of these are referred to the Health Center They are not referenced 90% of episodes. There is evidence of unjustified use of the Pediatric ER for 49.5% of users. It appears that the use of emergency service depends on the residence of users (p = 0.001), the population of Amadora uses more urgency. There is evidence of a relationship between the day of the week and unjustified access (p <0.001), Thursday shows 17.3% of the episodes are not justified. Recurrence unjustified the emergency room does not depend on gender (p = 0.137). There is a relationship between age and recurrence justified statistically significant (p = 0.006). Conclusion: It can be concluded that "on use" and "inadequacy" of demand phenomena are present in the Pediatric ER. So urge measures to curb misuse, these probably passing an awareness and education of the population.
Encerramento Percutâneo de Shunts Interauriculares: Experiência de uma Década de um Centro Terciário

INTRODUCTION: Atrial septal defects (ASD) are among the most common congenital anomalies and account for 10% of congenital heart disease in the pediatric age-group and 30% in adults. Closure is indicated when there is evidence of hemodynamic significance or after a paradoxical embolic event. Ten years ago, percutaneous closure became the treatment of choice in our center for all patients with a clear indication and favorable anatomy. In this paper we report the experience of this first decade. OBJECTIVE: To assess the short- and long-term results of our ten-year experience with percutaneous closure of atrial septal defects. METHODS: We studied retrospectively all patients with ASD treated with a percutaneous approach between November 1998 and December 2008. The pediatric age-group consisted of patients younger than 19 years old. Demographic data, clinical indications, minor and major complication rates, success rate and long-term outcome were assessed. RESULTS: In the first ten years of experience 510 patients, of whom 166 were in the pediatric group, were treated in our center by a team of adult and pediatric cardiologists. The overall success rate of the procedure was 98% (97.5% in ASD and 99.5% in patent foramen ovale (PFO). The minor complication rate was 3% (3.4% in ASD and 2% in PFO). The most frequent complication was supraventricular tachycardia. The major complication rate was 1.2% (0.6% in ASD and 2% in PFO). Two patients developed cardiac tamponade due to hemopericardium that was resolved by pericardiocentesis, without need for surgery. One patient had an arterial pseudoaneurysm corrected by vascular surgery. There was no device embolization and no need for urgent surgery in this population. During follow-up two patients had recurrence of ischemic stroke, one had a transient ischemic attack and another had a hemorrhagic stroke. Mortality was 0.6% (0.6% in ASD and 0.5% in PFO). There were no in-hospital deaths. During follow-up there were two deaths, both in the adult group. DISCUSSION AND CONCLUSION: In this population the success rate was high and most of the complications were minor. The results of this collaboration between adult and pediatric cardiologists in the first ten years of activity confirm the safety and efficacy of percutaneous closure of septal defects, when there is careful patient selection and a standardized technique.

Venous ulcers in patients with post thrombotic syndromes are complex situations with multiple therapeutic options. They are responsible for high morbidity rates, conservative treatment is very slow and recurrences are very common. Deep venous reconstructive surgery is an alternative, but it should be based on the morphologic and functional aspects of the venous system and only adopted after a very careful study, including venography. The authors describe a morphological "pattern", found in some of these patients and related to the competence of the saphenous femoral junction, rendering possible to perform a valvular transposition. Seven patients with post thrombotic ulcers who have been treated during the last 6 years in which the pattern already described was detect, underwent a transposition of the superficial femoral vein, to the great saphenous vein and when necessary complemented with skyn grafts. Before the operation all patients had ulcers with more than 3 cm in size (3.2-5.4 cm) and with more than 4 months duration (4-16 months). All ulcers healed in the postoperative period (mean time 28 days). All patients have been reevaluated in 2003 and it was diagnosed the thrombosis of two procedures, one of them with the recurrence of the ulcer who healed with conservative treatment. The authors consider this method as a very easy technique to perform, although rarely used, and a valid alternative in a highly selected group of patients.

Acute Generalized Exanthematous Pustulosis (AGEP) is a drug-induced dermatosis characterized by an acute episode of sterile pustules over erythematous-edematous skin. It is accompanied by an episode of fever, which regresses a few days after discontinuation of the drug that caused the condition or as a result of corticosteroid treatment. The main triggering drugs are antibiotics, mainly beta-lactam ones. Other medications, such as antifungal agents, non steroid anti-inflammatory drugs, analgesics, antiarrhythmic, anticonvulsant and antidepressant drugs, may also be responsible. Histologically, it is characterized by the existence of vasculitis, associated with non-follicular subcorneal pustules. A case of a Caucasian female outpatient unit of Dermatology with AGEP, who presented with generalized pustulosis lesions after the use of cephalosporin for urinary infection is related. The diagnosis was confirmed by the clinical and pathological correlations, the resolution of the dermatosis after discontinuation of the drug and use of systemic corticosteroid treatment, and the recurrence of the disorder after the introduction of a similar drug. The importance of the recognition of this drug-induced dermatosis is given by its main differential clinical and histological diagnoses: generalized pustular psoriasis and subcorneal pustulosis.

INTRODUCTION: Carotid body tumours (CBT) are neoplasms that develop from paragangionic cells of this structure. They are rare, with an estimated incidence of 1:30000 and can be associated with other neuro-endocrine neoplasia. The authors report their experience in the management of the disease, in the last 10 years. MATERIAL AND METHODS: Eight patients (with eight tumours) were treated, all submitted to tumour resection. 75% were female and the mean age was 56 years. We report a 12,5% incidence of neurological sequelae from surgery, and no mortality. In the follow-up (which varied between 1 and 10 years), no local or contralateral recurrence or metastasis were registered. Also, we did not found family cases of this disease. CONCLUSIONS: The authors noticed an unusually high proportion of female patients. The tumour resection was curative in all patients, with a rate of neurological complications inferior to that reported in other published series. These neurological sequelae were reported in patients with large tumours, thus reinforcing the outmost importance of an early diagnosis. Pre-operative selective embolization of these tumours can be helpful in the resection of large tumours, allowing a potential reduction in neurological complications.

Atrial electrical remodeling plays a part in recurrence of atrial fibrillation (AF). It has been related to an increase in heterogeneity of atrial refractoriness that facilitates the occurrence of multiple reentry wavelets and vulnerability to AF. AIM: To examine the relationship between dispersion of atrial refractoriness (Disp_A) and vulnerability to AF induction (A_Vuln) in patients with clinical paroxysmal AF (PAF). METHODS: Thirty-six patients (22 male; age 55+/-13 years) with > or =1 year of history of PAF (no underlying structural heart disease--n=20, systemic hypertension--n=14, mitral valve prolapse--n=1, surgically corrected pulmonary stenosis--n=1), underwent electrophysiological study (EPS) while off medication. The atrial effective refractory period (AERP) was assessed at five different sites--high (HRA) and low (LRA) lateral right atrium, high interatrial septum (IAS), proximal (pCS) and distal (dCS) coronary sinus--during a cycle length of 600 ms. AERP was taken as the longest S1-S2 interval that failed to initiate a propagation response. Disp_A was calculated as the difference between the longest and shortest AERP. A_Vuln was defined as the ability to induce AF with 1-2 extrastimuli or with incremental atrial pacing (600-300 ms) from the HRA or dCS. The EPS included analysis of focal electrical activity based on the presence of supraventricular ectopic beats (spontaneous or with provocative maneuvers). The patients were divided into group A--AF inducible (n=25) and group B--AF not inducible (n=11). Disp_A was analyzed to determine any association with A_Vuln. Disp_A and A_Vuln were also examined in those patients with documented repetitive focal activity. Logistic regression was used to determine any association of the following variables with A_Vuln: age, systemic hypertension, left ventricular hypertrophy, left atrial size, left ventricular function, duration of PAF, documented atrial flutter/tachycardia and Disp_A. RESULTS: There were no significant differences between the groups with regard to clinical characteristics and echocardiographic data. AF was inducible in 71% of the patients and noninducible in 29%. Group A had greater Disp_A compared to group B (105+/-78 ms vs. 49+/-20 ms; p=0.01). Disp_A was >40 ms in 50% of the patients without A_Vuln and in 91% of those with A_Vuln (p=0.05). Focal activity was demonstrated in 14 cases (39%), 57% of them with A_Vuln. Disp_A was 56+/-23 ms in this group and 92+/-78 ms in the others (p=0.07). Using logistic regression, the only predictor of A_Vuln was Disp_A (p=0.05). CONCLUSION: In patients with paroxysmal AF, Disp_A is a major determinant of A_Vuln. Nevertheless, the degree of nonuniformity of AERP appears to be less important as an electrophysiological substrate for AF due to focal activation.

Os autores apresentam um caso clínico de derrame pleural direito, num doente do sexo feminino com 68 anos de idade, surgindo fora do contexto de patologia cardiovascular ou infecciosa previamente conhecida. O diagnóstico final foi de neoplasia pleural secundária a carcinoma adenoquístico, com localização primitiva no palato duro, diagnosticado e tratado 8 anos antes, e que nunca apresentara recidiva local. Salienta-se a raridade deste diagnóstico e o facto de que, estando descrita a metastização pulmonar com possível envolvimento secundário da pleura, é muito raro o aparecimento de metástase pleural isolada.

Primary tumours of the heart are uncommon entities, cardiac myxomas being the most frequent. However, mitral valve myxomas are exceptionally rare. In the last 12 years, there have been 25 myxomas diagnosed at our institution, with only two of them originating from the mitral valve. Both patients were female, the first, 25, and the second, 72 years old. The younger patient was very symptomatic with a large mass, 4 cm long, which involved both leaflets causing significant obstruction to the left ventricular inflow. The second one had a smaller mass located at the atrial side of the posterior leaflet that only produced some flow divergence. Neither of them had constitutional nor embolic symptoms. Both patients were submitted to emergent surgical resection that in the first case involved the mitral valve and replacement with mechanical prosthesis. The macroscopic appearance of these tumours suggested a malignant aetiology which may represent somewhat different features of the myxomas when originating from the cardiac valves. Both patients are well reflecting the good prognosis of this illness after resection, although the younger patient was re-operated because of prosthetic valve obstruction and suspicion of recurrence that was not confirmed. Because of the illustrative images and different presentations, we found it interesting to report and discuss them together.

BACKGROUND: Before the introduction of highly active antiretroviral therapy (HAART), CMV retinitis was a common complication in patients with advanced HIV disease and the therapy was well established; it consisted of an induction phase to control the infection with ganciclovir, followed by a lifelong maintenance phase to avoid or delay relapses. METHODS: To determine the safety of CMV maintenance therapy withdrawal in patients with immune recovery after HAART, 35 patients with treated CMV retinitis, on maintenance therapy, with CD4+ cell count greater than 100 cells/mm³ for at least three months, but almost all patients presented these values for more than six months and viral load < 30000 copies/mL, were prospectively evaluated for the recurrence of CMV disease. Maintenance therapy was withdrawal at inclusion, and patients were monitored for at least 48 weeks by clinical and ophthalmologic evaluations, and by determination of CMV viremia markers (antigenemia-pp65), CD4+/CD8+ counts and plasma HIV RNA levels. Lymphoproliferative assays were performed on 26/35 patients. RESULTS: From 35 patients included, only one had confirmed reactivation of CMV retinitis, at day 120 of follow-up. No patient returned positive antigenemia tests. No correlation between lymphoproliferative assays and CD4+ counts was observed. CONCLUSION: CMV retinitis maintenance therapy discontinuation is safe for those patients with quantitative immune recovery after HAART.

Objectives: Literature review of classification, epidemiology, pathophysiology, microbiology, clinical presentation, diagnosis, complications, treatment and prevention of urinary tract infections (UTI) in pregnancy. Data Sources and Review Methods: Bibliographic research in Medline, through PubMed and Medscape, of systematic reviews, observational studies, clinical guidelines, meta-analyses and randomized controlled trials published between January 1992 and December 2010. Results: Asymptomatic bacteriuria occurs in 2 to 10% of pregnant women, 30% of which will develop acute pyelonephritis if left untreated. Treatment of asymptomatic bacteriuria has been shown to reduce the risk of pyelonephritis in pregnancy. Therefore, screening and treatment of this form of UTI has become a standard of obstetrical care, although the ideal duration of the treatment is still controverse. Acute pyelonephritis is one of the most common medical complications of pregnancy and may be associated with maternal, obstetric and perinatal complications. There is no clear consensus in the literature regarding antibiotic choice or duration of therapy for symptomatic UTI. Increasing antibiotic resistance complicates the choice of empirical regimens and local resistance rates need to be taken into account when deciding the therapy. Considering the high rate of recurrence of UTI during pregnancy, prophylactic measures need to be taken in pregnant women who have already experienced UTI during their current pregnancy. Conclusions: Although UTI is a generally benign condition in non-pregnant women, it may be a potentially serious complication during pregnancy. Early diagnosis and treatment of UTI during pregnancy are mandatory and can prevent severe maternal and perinatal complications.

INTRODUCTION: Synovial sarcoma is a high-grade, soft-tissue sarcoma that most frequently is located in the vicinity of joints, tendons or bursae, although it can also be found in extra-articular locations. Most patients with synovial sarcoma of the hand are young and have a poor prognosis, as these tumors are locally aggressive and are associated with a relatively high metastasis rate. According to the literature, local recurrence and/or metastatic disease is found in nearly 80% of patients. Current therapy comprises surgery, systemic and limb perfusion chemotherapy, and radiotherapy. However, the 5-year survival rate is estimated to be only around 27% to 55%. Moreover, most authors agree that synovial sarcoma is one of the most commonly misdiagnosed malignancies of soft tissues because of their slow growing pattern, benign radiographic appearance, ability to change size, and the fact that they may elicit pain similar to that caused by common trauma. CASE PRESENTATION: We describe an unusual case of a large synovial sarcoma of the hand in a 63-year-old Caucasian woman followed for 12 years by a multidisciplinary team. In addition, a literature review of the most pertinent aspects of the epidemiology, diagnosis, treatment and prognosis of these patients is presented. CONCLUSION: Awareness of this rare tumor by anyone dealing with hand pathology can hasten diagnosis, and this, in turn, can potentially increase survival. Therefore, a high index of suspicion for this disease should be kept in mind, particularly when evaluating young people, as they are the most commonly affected group.

Introduction: Rhinoscleroma is a rare, chronic, granulomatous disease that most frequently affects the upper respiratory tract, especially the nasal cavity and sometimes extends through the lower respiratory tract. Is associated with Klebsiella rhinoscleromatis, which is endemic in certain geographic regions namely Central America. The pathogenesis and risk factors remain unclear. Clinical case: We report a five years Old Portuguese boy, previously healthy, brought to the Emergency Department with epistaxis, without other accompanying signs or symptoms. The Otorhinolaryngologist (ORL) performed rhinoscopy and identified an intranasal bleeding mass. The MRI revealed an intranasal mass with extension to the ethmoid bone sinus, and performed biopsy. The histopathology was vital, making the diagnosis of Rhinoscleroma. The child had traveled abroad for the first time on vacations a year before to Dominican Republic. The bacteriologic exam identified a Klebsiella spp. sensible to the association of amoxicillin and clavulanic acid. Blood test performed excluded association of immunodeficiency. Since it’s a rare disease genetic study are under course. Monthly evaluation by ORL and pediatrician was performed which documented progressive reduction until total disappearing of the macro and microscopic lesion, and negative bacteriologic exam. Six months of antibiotic therapy were completed without any known secondary effects. The child remained asymptomatic up to the last visit, 3 months following treatment and has shown no evidence of recurrence. Conclusion: Globalization and free transit of people to areas far from origin countries here some rare diseases are endemic brings a new challenge to modern medicine. Sometimes vacations bring more than memories.
