A anemia falciforme é uma doença genética caracterizada por uma anemia hemolítica crônica e fenômenos vaso-oclusivos, que levam a crises dolorosas e à lesão tecidual crônica e progressiva. Tem sido relatado que pacientes com anemia falciforme apresentam aumento dos níveis circulantes de citocinas, incluindo fator de necrose tumoral-α (TNF-α). O principal fármaco utilizado no tratamento dessa anemia é a hidroxiuréia (HU), fonte exógena de óxido nítrico (NO) e responsável pela inibição da agregação de plaquetas e aumento dos níveis de hemoglobina fetal (HbF). Trabalhos prévios têm demonstrado a importância da subunidade 1,2,5-oxadiazol-N-óxido como doadora de óxido nítrico. Nesse contexto, o presente trabalho tem como objetivo sintetizar um novo derivado híbrido do 1,2,5-oxadiol-N-óxido para a formação de um composto útil para o tratamento dos processos preventivos contra agregação plaquetária exacerbado em pacientes falciformes

O Programa Nacional de Triagem Neonatal (PNTN), popularmente conhecido como “Teste do Pezinho”, é responsável pelo rastreamento das seguintes doenças nos recém-nascidos brasileiros: Fenilcetonúria, Hipotiroidismo Congênito, Doenças Falciformes e demais Hemoglobinopatias, e Fibrose Cística. Seu objetivo é o diagnóstico precoce dessas doenças congênitas em fase assintomática, buscando prevenir o aparecimento de sequelas neurológicas e outras complicações por meio do acompanhamento e tratamento adequados. O PNTN, além de realizar o diagnóstico pré-clínico de doenças que constituem problemas de saúde pública, gera informações que podem ser usadas em estudos epidemiológicos fundamentais para o planejamento de programas de saúde. Para que esse sistema seja completo, no entanto, é necessário haver o treinamento dos profissionais da saúde envolvidos no assunto, bem como a implantação de atividades educativas para estes e para o público em geral. Neste contexto, este estudo retrospectivo propõe investigar a relação de resultados alterados das doenças detectadas pelo PNTN entre os meses de abril e dezembro de 2009 e janeiro e dezembro de 2010 no município de Araraquara, verificar se as primeiras coletas de sangue estão sendo feitas segundo as recomendações do Ministério da Saúde e capacitar agentes comunitários da saúde nesse assunto. Foram analisados 4.116 resultados. Não houve diferença significativa entre as prevalências obtidas em 2009 e 2010, ou seja, as freqüências das patologias estudadas se mantiveram praticamente constantes ao longo dos dois períodos

The suppression of erythropoiesis by Hydroxyurea (HU) therapy is associated with increase in mean corpuscular volume, in addition to the increase in Hb F. Monitoring the mean corpuscular volume values and the presence of macrocytosis are effective tools of adherence to the treatment with HU in patients with sickle cell anemia. The aim of this study is to monitor the mean corpuscular volume values after starting treatment with HU to determine if macrocytosis can be used as a surrogate marker of compliance with therapy. We conducted a prospective cohort study over one year with measurements of blood counts and mean corpuscular volume after starting therapy with HU in 95 patients with sickle cell anemia who were regularly followed in our ambulatory outpatient unit. In one-year of successful use of HU the mean value of the mean corpuscular volume increased significantly. The Andersen and Gill model demonstrated that the increase of one unit of MCV implies a 5% reduction in the risk of visiting the emergency room. Monitoring mean corpuscular volume values after prescribing HU alerts the provider of noncompliance in order to counsel the patient in question for better adherence to the use of HU that could improve the quality of care and to reduce morbidity and the frequency of acute pain crises and associated healthcare costs.

Objective: to evaluate the use of hydroxyurea with regard to effectiveness and toxicity in people with sickle cell anemia. Method: this is a retrospective descriptive study, developed with 57 medical records of patients with sickle cell anemia, treated at the University Hospital Center of Campo Grande (Mato Grosso do Sul, Brazil), from 1993 to 2005. Inclusion criteria: electrophoresis of hemoglobin in medical record; regular use of drugs, for an average of 196 weeks; dosage; and hematological analyses before starting treatment. Exclusion criteria: living with other hemoglobinopathies. The variables evaluated were: neutrophils count; platelets; leukocytes; hemoglobin; time using hydroxyurea; drug response to the optimal dosage; and number and type of episodes of hospitalization. The research protocol was approved by the Ethics Committee of Universidade Federal de Mato Grosso do Sul, under the Protocol 645. Results: of the 57 medical records, 3 cases were evaluated. Comparing the hematological values, according to Portaria 872, enacted on 11/12/2002, it was found that: cases A, B, and C present an use of hydroxyurea (500 mg/day) for four years, with an average of 196 weeks. Case A, female, decreased painful episodes and frequency of hospitalization, keeping hematological values with no toxicity. In Case B, female, there was one hospitalization due to pain crises and important hemolysis. It stood out, in case C, male, neutropenia with hematological values < 2,000/mm3 . Conclusion: in the cases analyzed, we observed a drop in the number of hospitalizations with the decrease in painful crises from three to one a year, and there was no toxicity with regard to the dosage and time using hydroxyurea, in all three cases. For more comprehensive results, one suggests further study on this therapy with significant samples of this clientele.

Objectives: to identify the demographic profile and frequency of anemia and hemoglobinopathies, as a basis for future implementation of actions aimed at pregnant women in the public health domain. Method: this is a cross-sectional study developed with pregnant women attended in a university hospital at Mato Grosso do Sul, Brazil. Blood samples were collected for the erythrogram analysis for detection of anemia and selective and specific tests for abnormal hemoglobin. The patients regarded as indigenous and mentally ill, as well as inmates, were excluded from the research, as they represent a vulnerable population which needs a cohort different from that of the sample. For data collection, a particular questionnaire was used. The research was approved by the Ethics Committee of Universidade Federal de Mato Grosso do Sul (UFMS), under the Protocol 873/2006. Results: of the 215 pregnant women under study, 20% were adolescents; 36.3% had incomplete primary education; 53.0% were non-Caucasian; 43.3% were from Campo Grande, Mato Grosso do Sul, Brazil; and 21.1% were of European descent. 17.7% had some type of anemia and, in the evaluation of hemoglobinopathies, 4.7% of patients were detected with some abnormal hemoglobin, with the following frequencies: 3.3% with HbAS; 0.9% with HbAC; and 0.5% with intermediate β-thalassemia. Conclusion: the frequencies of anemia and hemoglobinopathy found in these pregnant women showed the importance of early diagnosis, revealing indicators able to provide a basis for preventive and assistance actions for adequate clinical monitoring, reducing maternal and neonatal morbimortality in the public health services. This study aimed to assess antioxidant effects of melatonintreatment compared to N-acetylcysteine (NAC) and to their combination in asickle cell suspension. Sickle erythrocytes were suspended in phosphate-buffered saline, pH 7.4, composing external control group. They were alsosuspended and incubated at 37°C either in the absence (experimental controlgroup) or in the presence of NAC, melatonin and their combination atconcentrations of 100 pM, 100 nM and 100 lM for 1 hr (treatment groups).The melatonin influences were evaluated by spectrophotometric [hemolysisdegree, catalase (CAT), glutathione S-transferase (GST), glutathioneperoxidase (GPx), glutathione reductase (GR), glucose-6-phosphatedehydrogenase (G6PDH), and superoxide dismutase (SOD) activities] andchromatographic methods [glutathione (GSH) and malondialdehyde (MDA)levels]. Incubation period was able to cause a rise about 64% on hemolysisdegree as well as practically doubled the lipid peroxidation levels (P < 0.01).However, almost all antioxidants tested treatments neutralized this incubationeffect observed in MDA levels. Among the antioxidant biomarkers evaluated,we observed a modulating effect of combined treatment on GPx and SODactivities (P < 0.01), which showed ~25% decrease in their activities. Inaddition, we found an antioxidant dose-dependent effect for melatonin onlipid peroxidation (r = 0.29; P = 0.03) and for combined antioxidanttreatments also on MDA levels (r = 0.37; P = 0.01) and on SOD activity(r = 0.54; P < 0.01). Hence, these findings contribute with important insightthat melatonin individually or in combination with NAC may be useful forsickle cell anemia management.

A anemia falciforme (AF) é a doença hematológica hereditária crônica de maior prevalência no Brasil, causada por uma mutação pontual no gene da β-globina, levando as hemácias a adquirirem formato de foice quando em estado desoxigenado. Essa alteração estrutural causa aumento da adesão celular, hipóxia local, vaso-oclusão e alterações na coagulação. O quadro clínico dos pacientes é diversificado, predominando a dor causada pelo processo inflamatório agudo e crônico, além de complicações que podem ocasionar infarto de tecidos e órgãos, levando à morte. O único fármaco aprovado pelo FDA disponível para o tratamento da doença é a hidroxiuréia (HU), mas, devido a sua toxicidade, têm sido intensificadas as buscas por novas possibilidades terapêuticas. O fator de necrose tumoral alfa (TNF-α) é uma citocina pró-inflamatória presente em grande quantidade nos pacientes portadores da doença, e o óxido nítrico pode contribuir para vasodilatação e proteção vascular. Assim, propôs-se a síntese e avaliação farmacológica de candidatos a fármacos para o tratamento da AF, desenhados com o propósito de apresentar propriedades inibidoras do TNF-α e doadoras de óxido nítrico (NO). Foi avaliada a estabilidade química dos compostos sintetizados, em água, pH 1,2 e pH 7,4, sendo o Composto II o mais estável tanto em pH ácido como em água. Em pH básico, todos os compostos se mostraram instáveis. Os compostos III e VI apresentaram uma maior inibição da agregação plaquetária, atividade também atribuída à capacidade de doação de NO. O composto III foi o único a mostrar-se inibidor de TNF-α e IL-1β, em todas as concentrações avaliadas, e, também, mostrou-se inibidor de NO, no teste induzido por LPS. Diante de todos os ensaios realizados, o composto III é o mais promissor candidato a fármaco, entre os compostos sintetizados, para o tratamento dos sintomas da anemia falciforme.

Background and ObjectivesHypokalemia has been consistently associated with high mortality rate in peritoneal dialysis. However, studies investigating if hypokalemia is acting as a surrogate marker of comorbidities or has a direct effect in the risk for mortality have not been studied. Thus, the aim of this study was to analyze the effect of hypokalemia on overall and cause-specific mortality.Design, Setting, Participants and MeasurementsThis is an analysis of BRAZPD II, a nationwide prospective cohort study. All patients on PD for longer than 90 days with measured serum potassium levels were used to verify the association of hypokalemia with overall and cause-specific mortality using a propensity match score to reduce selection bias. In addition, competing risks were also taken into account for the analysis of cause-specific mortality.ResultsThere was a U-shaped relationship between time-averaged serum potassium and all-cause mortality of PD patients. Cardiovascular disease was the main cause of death in the normokalemic group with 133 events (41.8%) followed by PD-non related infections, n=105 (33.0%). Hypokalemia was associated with a 49% increased risk for CV mortality after adjustments for covariates and the presence of competing risks (SHR 1.49; CI95% 1.01-2.21). In contrast, in the group of patients with K < 3.5mEq/L, PD-non related infections were the main cause of death with 43 events (44.3%) followed by cardiovascular disease (n=36; 37.1%). For PD-non related infections the SHR was 2.19 (CI95% 1.52-3.14) while for peritonitis was SHR 1.09 (CI95% 0.47-2.49).ConclusionsHypokalemia had a significant impact on overall, cardiovascular and infectious mortality even after adjustments for competing risks. The causative nature of this association suggested by our study raises the need for intervention studies looking at the effect of potassium supplementation on clinical outcomes of PD patients.

During the acute phase response, there is an increased production and release of certain proteins known as acute phase proteins (APPs) which can be produced by hepatocytes and peripheral tissues such as C-reactive protein (CRP), serum amyloid A (SAA), haptoglobin (Hp), alpha-1 acid glycoprotein (AGP). These proteins have been investigated as markers of various infectious diseases in small animals and the purpose of this review is to update the current knowledge about APPs in infectious diseases in dogs and cats.

Intra-abdominal adhesions constitute a significant clinical and surgical problem that can lead to complications such as pain and bowel occlusion or subocclusion. These adhesions are frustrating and potentially fatal, representing a major postoperative complication in abdominal surgery. It is estimated that 32% of horses undergoing laparotomy will present clinical symptoms due to adhesions, but the true prevalence is not known because a large proportion of animals with postoperative recurrent colics are medically treated or submitted to euthanasia without necropsy. Adhesions are highly cellular, vascularized, dynamic structures that are influenced by complex signaling mechanisms. Understanding their pathogenesis could assist in applying better therapeutic strategies and in developing more effective antiadhesion products. Currently, there are no definitive strategies that prevent adhesion formation, and it is difficult to interpret the results of existing studies due to nonstandardization of an induction model and evaluation of their severity. The best clinical results have been obtained from using minimally traumatic surgical techniques, anti-inflammatory agents, antimicrobials, anticoagulants, and mechanical separation of serosal surfaces by viscous intraperitoneal solutions or physical barriers. This paper aims to review adhesion formation pathogenesis, guide the understanding of major products and drugs used to inhibit adhesion formation, and address their effectiveness in the equine species.
