In modern society, blood donor motivation and recruitment is a fundamental part of health care delivery. Well defined and documented programmes exist throughout the world but new ideas are always welcome. The situation in the Sudan is different and much remains to be done by way of comparison with elsewhere. This thesis outlines the objectives of a study, how it was supported, sponsored and achieved. It describes briefly the geography of the Sudan, the source of Sudanese economy, climate, culture and historical backgrounds. The problems of existing services in the Sudan are reviewed and a brief account of the demographic characteristics of the Sudanese population is given. Two surveys done in West of Scotland and in the Sudan are described in detail. This work discloses and compares the positive motives that enhances giving of blood and the negative motives that hinders its donation. The comparison is between an Eastern Society with a voluntary motivation not fully activated because of lack of understanding and awareness of the need to give blood voluntarily for strangers and Western Society with a well established voluntary system of donation. An addition to this research was the investigation into the immunity to tetanus and hepatitis in the Sudanese population. An estimate of the percentage of individuals with detectable levels of hepatitis A and B antibodies and tetanus antibodies is included since there is a need to establish a plasmapheresis programme as part of a good Blood Transfusion Service for the procurement of specific immunoglobulin's. This work has revealed major differences between the West of Scotland and the Sudan and suggestions are made for their resolution. The main conclusion and comparison are summarised in Chapter 7. It is hoped that many of the suggestions in this thesis can be introduced in the Sudan at an early date.

Estudio de cohorte prospectiva. Universo: embarazadas de 10 a 19 años y de 20 a 29 años del departamento de Obstetricia, Hospital Vicente Corral Moscoso - Cuenca, Marzo a Mayo del año 2008. Muestra: 391 parturientas. Unidad de análisis: ficha clínica y formulario CLAP. Se registró el nivel de instrucción, estado civil, y paridad, se valoró el tipo de parto, procedimientos durante el parto, patólogías, número de fetos. Se clasificó según peso, talla, edad gestacional, Apgar, perímetro cefálico, patología, necesidad de reanimación y destino del recién nacido. Resultados: la prevalencia del embarazo en adolescentes es de 34.53%. El 46.21% tiene nivel de instrucción primaria. Unión libre 42.42%. Primigestas 65.90%. Parto vaginal 71%. Cesárea 29%. La adolescencia constituye factor de riesgo para la prevalencia de episiotomía (RR: 2.123), así mismo para la eclampsia (RR: 11) y hemorragias del primer trimestre (RR 3.775), no así, es factor protector para amenaza de parto pretérmino (RR 0.95). El recién nacido el 71.21% es adecuado, 24.24% es pequeño, el 4.54% es grande para la edad gestacional. La adolescencia es factor de riesgo para la talla baja del recién nacido (RR 1.436), microcefalia (RR 1.304), Apgar al quinto minuto menor de 73.78%. La adolescencia se asocia a mayor incidencia enfermedad de membrana hialina (RR 4.50). El 10.60% de recién nacidos de madres adolescentes ingresó a neonatología

Leishmania infantum and Trypanosoma cruzi are trypanosomatids of medical importance and are, respectively, the etiologic agents of visceral leishmaniasis (VL) and Chagas disease (CD) in Brazil. People infected with L. infantum or T. cruzi may develop asymptomatically, enabling the transmission of pathogens through blood transfusion and / or organs. The assessment of the infection by T. cruzi is included among the tests performed for screening blood donors in Brazil, however, there is no availability of tests for Leishmania. Serological tests for T. cruzi are very sensitive, but not specific, and may have cross-reactions with other microorganisms. Thus, the aim of this study was to determine the prevalence of Leishmania infection in blood donors and assess whether the serological test for T. cruzi detect L. infantum. Among the 300 blood samples from donors, discarded in 2011, 61 were T. cruzi positive, 203 were from donors with other infections and 36 were from handbags with low blood volume, but without infection. We also assessed 144 samples from donors without infections and able to donate blood, totaling 444 subjects. DNA was extracted from blood samples of all to perform quantitative PCR (qPCR) to detect Leishmania DNA. The buffy coat obtained from all samples was grown in Schneider medium supplemented and NNN. All samples were evaluated for the presence of anti-Leishmania antibody. The serological results indicate a percentage of 22% of Leishmania infection in blood samples obtained from discarded bags. A total of 60% of samples positive in ELISA for T. cruzi were negative by IFI, used as confirmatory test, ie 60% false positive for Chagas. Among these samples false positive for Chagas, 72% were positive by ELISA for Leishmania characterizing the occurrence of cross reaction between serologic assays. Of the 300 cultures performed, 18 grew parasites that were typed by qPCR and specific isoenzymes, found the species Leishmania infantum crops. Among the 18 cultures, 4 were purged from scholarships for low volume and all negative serology blood bank, thus demonstrating that there is a real risk of Leishmania transmission via transfusion. It is concluded that in an area endemic for leishmaniasis in Brazil, serological diagnosis performed to detect infection by T. cruzi among blood donors can identify infection by L. infantum and although cause false positive for Chagas, this cross-reactivity reduces the risk of Leishmania infection via blood transfusion, since tests are not applied specific detection of the parasite. In this way, there remains the need to discuss the implementation of a specific serological screening test for Leishmania in endemic countries such as Brazil

This study was carried out to determine the prevalence of malaria parasite among blood donors at the Police Clinic Port Harcourt, Rivers State, Nigeria. The standard parasitological techniques using both thick and thin blood films from the donors for the detection of malaria parasite was followed. Venous blood was collected from 200 blood donors and films were made on clean greese-free glass slide and stained with 10%Giemsa stains and viewed under the microscope using the oil immersion objective. Of the 200 samples examined, 56 (28.00%) were positive with Plasmodium falciparium . The highest prevalence among the males 53(26.50%) and between the ages 21-30years and only 3 (1.50%) of females were positive. Donors having the blood group O were more infected (60.70%) than the other blood groups and the lowest was blood group AB (5.40%). This result shows that there is a relatively high prevalence of malaria parasite among the blood donors in Port Harcourt, Nigeria. It is, therefore, recommended that malaria parasite screening test be included among other blood screening tests before any transfusion to avert the deleterious effects of malaria on recipients.

The ABO and Rhesus blood group systems are very important clinical tools that are commonly used in blood transfusion and their associations with various disease conditions have been widely reported. This study investigated the distribution of these blood group systems and assessed the association of malaria infection with the ABO blood groups among children in Federal Capital Territory, Abuja. Blood specimens from deep finger pricks of 730 children aged between 0-2 years were examined for malaria parasites using Field stains method. ABO and Rhesus blood group antigens tests were also performed using standard tile protocols. Of all the children admitted into the study, 445 were sick while 285 were apparently healthy. The prevalence of malaria parasites was significantly higher (P = 0.00047) among the sick children (69.8%) than the apparently healthy children (30.2%). The most prevalent blood group was O (55.7%) and the Rhesus D antigen was positive for 98.4% of all the children. The prevalence of blood group B among the sick children was significantly lower (P = 0.00373) than the other blood group types. There is no association between malaria infection and ABO blood groups but the prevalence of higher malaria parasite density was significantly greater (P = 0.0404) in children with blood group A (7.7%). In conclusion, blood group O was the most prevalent blood group in the study and children with blood group A appeared to be more susceptible to higher level of malaria parasitemia.

OBJETIVO. La presente investigación pretendió determinar complicaciones posnatales en los embarazos gemelares Monocorial y Bianmiotico en mujeres de 15 a 45 años del Hospital Homero Castanier Crespo de la ciudad de Azogues. MATERIAL Y MÉTODOS. Es una investigación cuantitativa y retrospectiva, se trabajó con una muestra de 41 historias clínicas, se utilizó un formulario elaborado y validado por las autoras. La fuente información fue secundaria mediante la revisión de archivos estadísticos y registros estadísticos de los embarazos gemelares que acudieron al Hospital Homero Castanier Crespo. La información fue procesada en el programa estadístico SPSS versión 1.5 y los resultados son presentados en tablas simples de frecuencias y porcentajes. RESULTADOS En la investigación se enconcontrò un 12.1 % del 100% presenta un diagnóstico de preclampsia ,en cuanto a la instrucción tenemos un 36.6% dando lugar 15 usuarias en cuanto al estado civil tenemos un porcentaje del 70.7% dando lugar a 29 usuarias el lugar de residencia tenemos el 56.1% que equivale a 23 usuarias en la placenta amniótico tenemos 65.9% con un total de 27 usuarias en cuanto los pesos de los Recién nacidos tenemos de bajo peso de 62.2% que equivale 51 en cuanto al Apgar tenemos 95.1%. CONCLUSIÓN. La investigación permitió determinar complicaciones posnatales en los embarazos gemelares Monocorial y Bianmiotico en mujeres de 15 a 45 años, verificamos que el gemelo número dos nace con bajo peso ya que el gemelo uno recibe todos los beneficios durante la gestación también se encontró hiperbilirrubinemia mas SDR.

Background In 2009 Malawi introduced a new protocol to screen potential blood donors for anaemia, using the WHO Haemoglobin Colour Scale (HCS) for initial screening. Published studies of the accuracy of the HCS to screen potential blood donors show varying levels of accuracy and opinion varies whether this is an appropriate screening test. The aim of the study was to assess the validity of the HCS, as a screening test, by comparison to HemoCue in potential blood donors in Malawi. Study design and Methods This was a blinded prospective study in potential blood donors aged over 18 years, at Malawi Blood Transfusion Service in Blantyre, Malawi. Capillary blood samples were analysed using the HCS and HemoCue, independent of each other. The sensitivity and specificity of correctly identifying ineligible blood donors (Hb≤12g/dL) were calculated. Results From 242 participants 234 (96.7%) were correctly allocated and 8 (3.3%), were wrongly allocated on the basis of the Haemoglobin Colour Scale (HCS) compared to HemoCue, all were subjects that were wrongly accepted as donors when their haemoglobin results were ≤12.0g/dL. This gave a sensitivity of 100% and specificity of 96.7% to detect donor eligibilty. The negative predictive value of the HCS was 100% but the positive predictive value to identify ineligible donors on the basis of anaemia was only 20%. Conclusions Initial screening with the HCS correctly predicts eligibility for blood donation in the majority of potential blood donors at considerable cost saving compared with use of HemoCue as the first line anaemia screening test, however, by this method a small number of anaemic patients were allowed to donate blood.

Aim To determine the distribution of haematological parameters in healthy individuals residing in Blantyre, Malawi. We also examined the effect of sociodemographic and nutritional factors on the haematological variables. Methods We conducted a proof-of-concept cross-sectional study, involving 105 healthy blood donors at Malawi Blood Transfusion Service in Blantyre. Eligible participants were HIV-negative males and females, aged 19 to 35 years, who did not have any evidence of acute or chronic illness, or bloodborne infection. We performed the haematological tests at the Malawi-Liverpool Wellcome Trust laboratory in Blantyre, and the screening tests at Malawi Blood Transfusion Service laboratories. Results Out of 170 consenting healthy volunteers, haematological results were available for 105 participants. The proportions of results which were below the lower limit of the manufacturer’s reference ranges were 35.2% (37/105) for haemoglobin, 15.2% (16/105) for neutrophils, 23.8% (25/105) for eosinophils, and 88.6 % (93/105) for basophils. The proportions of results that were above the upper limit of the manufacturer’s reference ranges were 9.5% (10/105) for platelets and 12.4% (13/105) for monocytes. We also observed that the mean leucocyte and basophil counts were significantly higher in males than females (p = 0.042 and p = 0.015, respectively). There were no statistically significant differences in haematological results observed among different ethnic, age, and body mass index groups. Conclusions Over half of otherwise healthy study participants had at least one abnormal haematological result, using previously established foreign standards. More detailed studies are needed to establish locally relevant normal ranges for different age groups and other demographic characteristics of the Malawian population. This will lead to accurate interpretation of laboratory results.

This article considers a comparison study between different non-normal process capability estimation methods and utilizing themin the leukocyte filtering process in blood service sectors. Since the amount of leukocyte in a unit of the blood is a critical issue inthe blood transfusion process and patient safety, estimating and monitoring the capability of the leukocyte filtering process to meetthe target window is very important for blood service sectors. However, observed data from the leukocyte filtering process showthat the leukocyte levels after filtering demonstrate a right skewed distribution and applying conventional methods with a normalityassumption fails to provide trustful results. Hence, we first conduct a simulation study to compare different methods in estimating theprocess capability index of non-normal processes and then we apply these techniques to obtain the process capability of the leukocytefiltering process. The study reveals that the Box-Cox transformation method provides reliable estimation of the process capability ofthe leukocyte filtering process.

Introduction : La néphro-urétérectomie radicale (NUR) représente le traitement primaire pour les patients atteints d’une tumeur des voies excrétrices supérieures (TVES) non métastatique. Une approche ouverte ou laparoscopique peut être considérée. Malgré la présence de plusieurs études comparant les résultats périopératoires et oncologiques entre ces deux approches, aucunes études se basent sur une cohorte populationnelle. Objectif : Notre but est d’évaluer la morbidité péri-opératoire entre la NUR ouverte et laparoscopique en utilisant une cohorte populationnelle. Méthode : Nous avons utilisé la base de donnée Nationwide Inpatient Sample (NIS) pour identifier tous les patients atteints d’une TVES non métastatique, traités par NUR ouverte ou laparoscopique, entre 1998 et 2009. Au total, 7401 (90,8%) et 754 (9,2%) patients ont subi une NUR ouverte et laparoscopique, respectivement. Dans le but de contrôler les différences inhérentes entre les deux groupes, nous avons utilisé une analyse par appariement sur les scores de propension. Ainsi, 3016 (80%) patients avec NUR ouverte étaient appariés à 754 (20%) patients avec NUR laparoscopique. Intervention : Tous les patients ont subi une NUR. Mesures : Les taux de complications intra-opératoires et post-opératoires, de transfusions sanguines, d’hospitalisation prolongée et de mortalité intrahospitalière ont été mesurés. Des analyses de régression logistique on été utilisées pour notre cohorte, après appariement sur les scores de propension. Résultats et Limitations : Pour les patients traités par approche ouverte vs. laparoscopique, les taux suivants furent calculés : transfusions sanguines : 15 vs. 10% (p<0,001); complications intra-opératoires : 4,7 vs. 2,1% (p=0,002); complications post-opératoires : 17 vs. 15% (p=0,24); durée d’hospitalisation prolongée (≥ 5 jours) : 47 vs. 28% (p<0,001); mortalité intra-hospitalière 1,3 vs. 0,7% (p=0,12). Sur les analyses par régression logistique, les patients ayant été traités par NUR laparoscopique avaient moins de chance de recevoir une transfusion sanguine (odds ratio [OR]: 0,6, p<0,001), de subir une complication intra-opératoire (OR: 0,4, p=0,002), et d’avoir une durée prolongée d’hospitalisation (OR: 0,4, p<0,001). Globalement les taux de complications postopératoires étaient équivalents. Toutefois, l’approche laparoscopique était associée à moins de complications pulmonaires (OR: 0,4, p=0,007). Cette étude est limitée par sa nature rétrospective. Conclusion: Après ajustement de potentiels biais de sélection, la NUR par approche laparoscopique est associée à moins de complications intraopératoires et péri-opératoires comparée à la NUR par approche ouverte.

Cette revue systématique évalue l’effet d’une utilisation libérale de plaquettes (ratio élevé plaquettes:culots globulaires) en comparaison à une utilisation traditionnelle (faible ratio plaquettes:culots globulaires) dans la réanimation initiale des polytraumatisés. Parmi 6123 références identifiées, nous avons sélectionné 7 études observationnelles comparatives incluant au total 4230 patients. Quatre études ont rapporté une diminution de la mortalité avec des ratios plaquettes:culots globulaires élevés chez des patients transfusés massivement. Une étude sur des patients sans hémorragie massive et une étude sur un nouveau protocole de transfusion massive n’ont rapporté aucune différence. L’hétérogénéité clinique et les failles méthodologiques des études n’ont pas permis d’effectuer une méta-analyse. Les données probantes actuelles sont insuffisantes pour appuyer l’utilisation d’un ratio plaquettes:culots globulaires spécifique dans la réanimation des polytraumatisés, surtout en considérant le biais de survie et les hémorragies non massives. Des essais cliniques randomisés évaluant la sécurité et l’efficacité d’un ratio élevé plaquettes:culots globulaires sont nécessaires avant de recommander leur utilisation.

Increased risk of bleeding after major orthopedic surgery (MOS) has been widely documented in general population. However, this complication has not been studied in elderly patients. The purpose of this study is to determine whether the risk of major bleeding after MOS is higher in elderly patients, compared with those operated at a younger age. Methods: This retrospective cohort study included total hip and total knee arthroplasty patients operated during 5 consecutive years. The main outcome was the occurrence of major bleeding. Patients with other causes of bleeding were excluded. Relative risks (RRs) and confidence intervals (CIs) were calculated, anda multivariate analysis was performed. Results: A total of 1048 patients were included, 56% of patients were hip arthroplasties. At the time of surgery, 553 (53%) patients were older than 70 years. Patients aged >70 years showed an increased risk of major bleeding (RR: 2.42 [95% CI: 1.54-3.81]). For hip arthroplasty, the RR of bleeding was 2.61 (95%CI: 1.50-4.53) and 2.25 (95% CI: 1.03-4.94) for knee arthroplasty. After multivariate analysis, age was found to be independently associated with higher risk of major bleeding. Conclusion: According to European Medicines Agency criteria, patients aged 70 years are at a higher risk of major bleeding after MOS, result of a higher frequency of blood transfusions in this group of patients. Standardized protocols for blood transfusion in these patients are still required.
