The simultaneous presence of infectious organisms within cutaneous lesions of Kaposi sarcoma in persons with AIDS has been demonstrated. We describe a patient with concurrent leprosy and Kaposi sarcoma presenting as an immune reconstitution inflammatory syndrome in the setting of AIDS.

Introduction: The 2D:4D digit ratio is sexually-dimorphic, probably due to testosterone action through the perinatal period. We characterize the 2D:4D ratio in newborn (NB) infants, in between the pre- and postnatal surges of testosterone, and relate it to the mother's 2D:4D and to testosterone levels in the amniotic fluid (AF). Subjects and methods: Testosterone was assayed in samples of maternal plasma and AF collected at amniocentesis. Shortly after birth, 106 NBs and their mothers were measured for 2D:4D ratio. Results: NB males had lower mean 2D:4D ratios than females but this dimorphism was significant only for the left hand (males: 0.927; females: 0.950; p=0.004). Mothers who had sons had lower 2D:4D ratios than those who had daughters and the mother's 2D:4D were higher than those of NBs regardless of sex. Both hands of NB females were negatively correlated with AF testosterone and positively correlated with the mother's 2D:4D, but males showed no significant associations. Maternal plasma testosterone also showed a negative weak correlation with NB's digit ratio in both sexes. Conclusions: Sexual dimorphism at birth was only significant for the left hand, in contrast with reports of greater right hand dimorphism, suggesting that postnatal testosterone is determinant for 2D:4D stabilization. The lower 2D:4D ratios in mothers who had sons support claims that hormone levels in parents are influential for determining their children's sex. NB female's digit ratio, but not males', was associated to the level of AF testosterone. The mother's 2D:4D ratios were positively correlated with their daughters' 2D:4D, but the same was not observed for male NBs, suggesting that prenatal testosterone levels in male fetus lead their 2D:4D ratios to stray from their mothers' with high individual variability.

Background: Anticipatory postural adjustments during gait initiation have an important role in postural stability but also in gait performance. However, these first phase mechanisms of gait initiation have received little attention, particularly in subcortical post-stroke subjects, where bilateral postural control pathways can be impaired. This study aims to evaluate ankle anticipatory postural adjustments during gait initiation in chronic post-stroke subjects with lesion in the territory of middle cerebral artery. Methods: Eleven subjects with post-stroke hemiparesis with the ability to walk independently and twelve healthy controls participated in this study. Bilateral electromyographic activity of tibialis anterior, soleus and medial gastrocnemius was collected during gait initiation to assess the muscle onset timing, period of activation/deactivation and magnitude of muscle activity during postural phase of gait initiation. This phase was identified through centre of pressure signal. Findings: Post-stroke group presented only half of the tibialis anterior relative magnitude observed in healthy subjects in contralesional limb (t=2.38, p=0.027) and decreased soleus deactivation period (contralesional limb, t=2.25, p=0.04; ipsilesional limb, t=3.67, p=0.003) as well its onset timing (contralesional limb, t=3.2. p=0.005; ipsilesional limb, t=2.88, p=0.033) in both limbs. A decreased centre of pressure displacement backward (t=3.45, p=0.002) and toward the first swing limb (t=3.29, p=0.004) was observed in post-stroke subjects. Interpretation: These findings indicate that chronic post-stroke subjects with lesion at middle cerebral artery territory present dysfunction in ankle anticipatory postural adjustments in both limbs during gait initiation.

Segundo a DSM IV a Deficiência Mental (DM) define-se como o funcionamento intelectual global inferior à média (QI < 70) associado a perturbações do comportamento adaptativo com início antes dos 18 anos. Procurou-se caracterizar retrospectivamente a população de crianças com DM observadas no Centro de Desenvolvimento do Hospital de Dona Estefânia (CDHDE), entre Janeiro 2005 e Junho 2007. Foram avaliados os dados epidemiológicos, gravidade, etiologia, co-morbilidade e intervenção proposta. Do total de 232 processos clínicos observados, 185 apresentavam DM. Classificaram-se em DM ligeira 112 (61%), DM moderada 54 (29%), DM grave 17 (9%) e profunda 2 (1%). Foram definidas etiologias em 86 crianças (46%) sendo a taxa de diagnóstico mais elevada na DM de maior gravidade. Observou-se uma elevada variabilidade de etiologias: as mais frequentemente encontradas foram as doenças genéticas, prematuridade e patologia associada. Foi detectada co-morbilidade em 123 crianças (66%), sendo a mais frequente as do foro oftalmológico (57 crianças, 46%). Foram propostas e sinalizadas para apoio a totalidade das crianças com DM, 47% em intervenção precoce e 58% em educação especial, das quais 5% usufruiram, por curto período, do apoio simultaneo de educadora de Intervenção Precoce e de docente do Ensino Especial, durante o período inicial de integração em jardim de infância. Observou-se um predomínio do sexo masculino. Foi efectuada caracterização clínica e funcional das crianças seguidas no CDHDE com o diagnóstico de DM e encontraram-se semelhanças entre os dados presentes e os descritos na literatura. Contudo alguns dados diferem de outras casuísticas decorrente, muito provavelmente decorrente da heterogeneidade da população estudada, quer do ponto de vista etiológico, quer no referente aos grupos etários, provavelmente condicionada, pela política assistencial.

A autora aborda o historial, génese e objectivos da Classificação Internacional de Funcionalidade para crianças e jovens, implementada pela Organização Mundial de Saúdeem 2007. Sublinha o papel de complementaridadade da CIF relativamente a outras classificações como a Classificação Internacional de Doenças (CID), em que a primeira visa caracterizar as capacidades e dificuldades de crianças e jovens com deficiência, ou seja, caracterizar do ponto de vista funcional cada criança e jovem, independentemente do diagnóstico etiológico médico. Trata-se da mudança de paradigma bio-médico para a do indivíduo que apresenta determinadas competências e dificuldades, em que estas últimas são definidas em função do ambiente – facilitador ou actuando como barreira. Assim, é dado enfoque ao ambiente para que este seja modificado transformando os factores que actuam como barreira em facilitadores, que anulem ou atenuem as dificuldades. Sendo a deficiência e ou doença uma experiência universal, a CIF vem colmatar uma importante lacuna na dificuldade de comunicação e articulação entre os diversos actores intervenientes no apoio socio educativo e médico destas crianças,criando uma linguagem acessível aos técnicos envolvidos – educadores, terapeutas e outros profissionais ligados à infância, por força envolvidos, dando uma maior ênfase à interacção criança/meio numa perspectiva holística de bem estar bio-psico-social.

O Lúpus Eritematoso Sistémico (LES) é uma doença multissistémica, auto-imune, caracterizada por inflamação vascular e do tecido conjuntivo, com anticorpos antinucleares. As manifestações clínicas são variáveis, com uma história natural progressiva e imprevisível. Apresentamos o caso de uma adolescente, com antecedentes de artrite dos joelhos, febre e astenia de etiologia não esclarecida, com um ano de evolução. Internada na Unidade de Cuidados Intensivos Pediátricos por pneumonia bilateral a Streptococcus pneumoniae complicada com derrame pleural, anasarca e hematúria macroscópica. Iniciou antibioticoterapia, com evolução favorável, após o que se verifica agravamento clínico, com reaparecimento do derrame pleural, lesões vesiculares disseminadas sugestivas de etiologia herpética, hipertensão arterial sintomática e convulsão tónico-clónica generalizada. Da investigação, destaca-se estudo imunológico compatível com LES em actividade, poliserosite, proteinúria nefrótica, nefrite lúpica classe IV e anemia grave.

BACKGROUND: Prospective testing for posttransplant circulating anti-HLA antibodies seems to be a critical noninvasive tool, but confirmatory data are lacking. MATERIALS AND METHODS: Over the last 3 years, peritubular capillary (PTC) C4d deposition was prospectively sought by an immunofluorescence technique applied to frozen tissue in biopsies obtained for allograft dysfunction. Screening for circulating anti-HLA class I/II alloantibodies (AlloAb) by the flow cytometric test was performed simultaneously. RESULTS: We evaluated 132 sets of biopsies and simultaneous serum samples. PTC C4d deposition was demonstrated in 15.9% (21/132) of biopsies. Circulating anti-HLA I/II AlloAb were detected in 25% (33/132) of serum samples. Employing receiver-operator characteristic (ROC) curves for all C4d-positive biopsies, screening for AlloAb showed a global specificity of 82% and sensitivity of 61.9%. When this analysis was restricted to biopsies obtained in the first month posttransplantation, the sensitivity increased to 81.8%, but the specificity decreased to 76.9%. After the first month posttransplantation, we observed sensitivity of 40.0% and a specificity of 86.4%. In the first month posttransplantation, all patients with a diagnosis of acute antibody-mediated rejection displayed circulating anti-HLA class I/II, but not always at the same time as the C4d-positive biopsy. CONCLUSIONS: In the first month posttransplantation, prospective monitoring of anti-HLA antibodies may be useful. The high sensitivity allows the identification of patients at risk, affording an earlier diagnosis of antibody-mediated rejection. After the first month, the test can be used to evaluate allograft dysfunction episodes, since positivity is highly suggestive of an antibody-mediated process.

Irá igualmente contribuir para dar maior visibilidade ao impacto dos cuidados de enfermagem nos ganhos em saúde das populações.

Introduction: Paediatric patients who undergo posterior spinal fusion surgery to correct scoliosis often require multiple blood transfusions. Tranexamic acid is a synthetic antifibrinolytic drug that reduces transfusion requirements in scoliosis surgery (1),(2),(3). Methods: To evaluate the efficacy of prophylactic tranexamic acid (TA) (initial dose of 10mg/kg and infusion of 1mg.kg(-1).h(-1)) in reducing perioperative blood transfusion requirements, we reviewed patients files and compared the amount of blood lost and blood transfused in the perioperative period of 12 patients (54.5%) that received TA and 10 patients (45.5%) who did not received TA. T-Student test was applied. Results: The average difference of blood losses (2,67 +/- 6,06ml) and blood transfused (212,9 +/- 101,1ml) between the two groups was not statistically significant (p>0.05). No thrombotic complications were detected in either group. Discussion: Results of the current study showed that prophylactic low dose of TA did not have a significant effect in the management of intraoperative blood loss and transfusion requirements in children undergoing scoliosis surgery. It is important to emphasize that our study is retrospective and that the size of the sample is small. Further studies are needed to evaluate the efficacy and safety of TA on paediatric scoliosis surgery.

Introduction: Hysterectomy is the commonest gynecologic operation, performed for malignant and benign conditions. There are many approaches to hysterectomy for benign disease. Studies comparing the techniques have showed that vaginal hysterectomy has benefits in terms of reduced hospital stay, faster recovery and less operating time. Objective: The purpose of this study is to compare the surgical and immediate postoperative outcomes of Laparoscopic Assisted Vaginal Hysterectomy (LAVH) with those of Vaginal Hysterectomy (VH). Methods: Retrospective descriptive study, comparing two groups of women who underwent LAVH or VH in our department during a 24 months period, from January 2009 to December 2010. The two groups were compared regarding age, vaginal deliveries, previous abdominal surgery, uterine and adnexal pathology, intra-operative and post-operative complications, uterus weight, blood loss and number of days until discharge. Results: In our study 42 LAVH and 99 VH were included, with a patient mean age of 47 and 59, respectively. The most frequent indication for hysterectomy was fibroids (80%) for LAVH and POP(58.6%) for HV. In LAVH group 47.6% of patients had previous abdominal surgery, vs 28.2% in VH group. The medium operative time was 167 minutes for LAVH vs 99 minutes for HV. The intra-operative complications were one case (2%) of accidental incision of rectum in LAVH, and one bladder incision in the VH (1%). There were 3 conversions to laparotomy for difficult technique (7%) in LAVH group. There were no significant post-operative complications for LAVH. In VH group there were 2 cases of haemoperitoneum (2%) and 1 case requiring blood transfusion (1%). The mean time for discharge was 4.23 days for LAVH and 4.46 days for VH. Conclusions: In our study, the main advantage for VH was the reduced operative time. In terms of time to discharge there was no difference between the 2 groups. The main intra-operative complication of LAVH was the risk of conversion to laparotomy, but post-operatively this procedure had fewer complications than VH. In conclusion, LAVH is a safe option for women requiring hysterectomy in cases where VH is anticipated to be technically difficult.

Durante a gravidez podem ser diagnosticados tumores abdominais ou pélvicos. De entre estes, os mais comuns no grupo etário em idade reprodutiva, são os tumores uterinos e anexiais. A ecografia é o meio mais comum para apoiar o diagnóstico; contudo, o crescimento uterino e a sua relação com os anexos, podem levar por vezes a diagnósticos enganadores. É a propósito de uma dessas situações, rara na gravidez, que se descreve o seguinte caso clínico.

Aim: To characterise clinically the patients with C4d in peritubular capillaries deposits (C4dPTCD) and/or circulating anti-HLA class I/II alloantibodies. To determine the correlation between positive C4dPTCD and circulating anti-HLA class I/II alloantibodies during episodes of graft dysfunction. Subjects and Methods: C4d staining was performed in biopsies with available frozen tissue obtained between January 2004 and December 2006. The study was prospective from March 2005, when a serum sample was obtained at the time of biopsy to detect circulating anti-HLA class I/II alloantibodies. Results: We studied 109 biopsies in 86 cadaver renal transplant patients. Sixteen of these (14.7%) presented diffuse positive C4dPTCD. There was a 13.5% rate of +C4dPTCD incidence within the first six months of transplantation and 16% after six months (p>0.05). Half of the +C4dPTCD in the first six months was associated with acute humoral rejection. After six months, the majority of +C4dPTCD (n=7/8) was present in biopsies with evidence of interstitial fibrosis/tubular atrophy and/or transplant glomerulopathy. The C4dPTCD was more frequent in patients with positive anti-HCV antibodies(p<0.0001), a previous renal transplant (p=0.007), and with a panel reactivity antibody (PRA) ≥ 50%(p=0.0098). The anti-HCV+ patients had longer time on dialysis (p=0.0019) and higher PRA(p=0.005). Circulating anti-HLA I/II alloantibodies were screened in 46 serum samples. They were positive in 10.9% of samples, all obtained after six months post transplant. Circulating alloantibodies were absent in 92.5% of the C4d negative biopsies. Conclusion: We found an association between the presence of C4dPTCD and 2nd transplant recipients,higher PRA and the presence of anti-HCV antibodies. The presence of HCV antibodies is not a risk factor for C4dPTCD per se, but appears to reflect longer time on dialysis and presensitisation. In renal dysfunction a negative alloantibody screening is associated with a reduced risk of C4dPTCD (<10%).

Este estudo surgiu em contexto de trabalho na Unidade de Transplantes do Hospital Curry Cabral na sequência do contacto directo com doentes com Paramiloidose. A elevada morbilidade motivada por esta patologia e a ausência de terapêuticas eficazes com excepção do transplante hepático, motivou um grupo de cinco Enfermeiros para a realização de um estudo de investigação sobre o tema "Percepções e sentimentos que o doente com Paramiloidose vivencia após o transplante hepático: Que qualidade de Vida?". O presente estudo teve como objectivos analisar as percepções e sentimentos que os doentes com Paramiloidose vivenciam após o transplante hepático e avaliar se o transplante lhes proporcionou uma melhor qualidade de vida. Dada a natureza do fenómeno a estudar, optámos por um estudo exploratório, descritivo, retrospectivo com abordagem qualitativa, numa amostra de carácter intencional, não probabilística, constituída por seis doentes submetidos a transplante hepático há mais de um ano, com o diagnóstico de Polineuropatia Amiloidótica Familiar (PAF). os dados foram recolhidos através de entrevista semiestruturada. Os resultados do estudo evidenciaram que após o transplante hepático, ocorreu na percepção destes doentes, uma melhoria significativa da sua qualidade de vida, embora haja referência ao impacto associado às interferências inerentes ao transplante em si, como sejam adaptação ao regime terapêutico da imunossupressão.
