Crianças e adolescentes freqüentemente são vítimas de trauma nas regiões oral e maxilofacial. O objetivo deste estudo foi determinar as características do trauma na região oral e maxilofacial que resultaram em registros policiais, em crianças e adolescentes na faixa etária de 0 a 16 anos, por um período de 5 anos. Dos 28.200 laudos avaliados, 463 foram incluídos na pesquisa. A taxa homem:mulher observada foi de 1,6:1 e a faixa etária de maior ocorrência foi a de 15-16 anos (44,40%). A maioria dos casos de trauma foi decorrente de agressão física (64,50%) e resultou em lesão de tecido mole (80,36%). Entre os tipos mais freqüentes, destacam-se as escoriações (28,64%) e quanto à localização, a região maxilar (22,63%). O tipo de lesão dental mais comum foi trauma dental (54,76%), e as fraturas ósseas predominaram nas regiões nasal (36,7%). Os resultados encontrados podem auxiliar no planejamento e execução de medidas preventivas e direcionar medidas curativas dirigidas a este grupo populacional.

Pachydermodactyly is a rare and benign form of acquired digital fibromatosis characterized by the expansion of soft tissue around proximal phalanges and interphalangeal joints. The etiology remains unknown, although it is suggested that repetitive mechanic trauma like the interlacing or rubbing of the fingers can lead to skin thickening. We report a case of a young man with skin thickening around interphalangeal joints and compulsive finger manipulation habit. The histopathology disclosed hyperkeratosis, discrete papillomatosis, mild increase of fibroblasts and dermal mucinosis. The lesions regressed partially, after finger manipulation was stopped.

In this study, the physicochemical characteristics of calcium phosphate based bioactive ceramics of different compositions and blends presenting similar micro/nanoporosity and micrometer scale surface texture were characterized and evaluated in an in vivo model. Prior to the animal experiment, the porosity, surface area, particle size distribution, phase quantification, and dissolution of the materials tested were evaluated. The bone regenerative properties of the materials were evaluated using a rabbit calvaria model. After 2, 4, and 8 weeks, the animals were sacrificed and all samples were subjected to histologic observation and histomorphometric analysis. The material characterization showed that all materials tested presented variation in particle size, porosity and composition with different degrees of HA/TCP/lower stoichiometry phase ratios. Histologically, the calvarial defects presented temporal bone filling suggesting that all material groups were biocompatible and osteoconductive. Among the different materials tested, there were significant differences found in the amount of bone formation as a function of time. At 8 weeks, the micro/nanoporous material presenting similar to 55,TCP:45%,HA composition ratio presented higher amounts of new bone regeneration relative to other blends and a decrease in the amount of soft tissue infiltration. Objective: To compare with pristine sites bone resorption and soft tissue adaptation at implants placed immediately into extraction sockets (IPIES) in conjunction with deproteinized bovine bone mineral (DBBM) particles and a collagen membrane.Material and methods: The mesial root of the third premolar in the left side of the mandible was endodontically treated (Test). Flaps were elevated, the tooth hemi-sectioned, and the distal root removed to allow the immediate installation of an implant into the extraction socket in a lingual position. DBBM particles were placed into the defect and on the outer contour of the buccal bony ridge, concomitantly with the placement of a collagen membrane. A non-submerged healing was allowed. The premolar on the right side of the mandible was left in situ (control). Ground sections from the center of the implant as well as from the center of the distal root of the third premolar of the opposite side of the mandible were obtained. The histological image from the implant site was superimposed to that of the contralateral pristine distal alveolus, and dimensional variation evaluated for the hard tissue and the alveolar ridge.Results: After 3 months of healing, both histological and photographic evaluation revealed a reduction of hard and soft tissue dimensions.Conclusion: The contour augmentation performed with DBBM particles and a collagen membrane at the buccal aspects of implants placed IPIES was not able to maintain the tissue volume.

Piezosurgery is a new and modern technique of bone surgery in implantology. Selective cutting is possible for different ultrasonic frequencies acting only in hard tissues (mineralized), saving vital anatomical structures. With the piezoelectric osteotomy technique, receptor site preparation for implants, autogenous bone graft acquistition (particles and blocks), osteotomy for alveolar bone crest expansion, maxillary sinus lifting, and dental implant removal can be performed accurately and safely, providing excellent clinical and biological results, especially for osteocyte viability. The aim of this review was, through literature review, to present clinical applications of piezosurgery in implant dentistry and outline their advantages and disadvantages over conventional surgical systems. Moreover, this study addressed the biological aspects related to piezosurgery that differentiate it from those of bone tissue approaches. Overall, piezosurgery enables critical operations in simple and fully executable procedures; and effectively, areas that are difficult to access have less risk of soft tissue and neurovascular tissue damage via piezosurgery.

Leishmaniasis is a disease emerging or re-emerging worldwide distribution (Sousa, 2008), a major impact on public health. The canine visceral leishmaniasis is an infectious parasitic zoonosis of worldwide distribution (Troncarelli, 2009), caused by a flagellate protozoan called Leishmania chagasi (Costard, 2009). Osteomyelitis can develop when the parasite reaches the bone tissue of the host via blood or continuity of adjacent soft tissue infection (Baltenperger, 2004). The histopathology of the lesions has 100% specificity when viewing the amastigote form of the parasite (Feitosa et al., 2000). A dog breed poodle, female, five years, with chronic lameness four months ago was attended by a veterinary, proceeded to the general clinical examination, radiographic evaluation of the hindquarters and the laboratory tests of enzyme immunoassay (ELISA) and indirect immunofluorescence (RIFI) for Leishmania sp. On examination, besides the enlargement of lymph nodes, the animal showed pain on flexion and extension of hind limbs. In radiographic lytic bone lesions were observed in bilateral ischial board and greater trochanter of the femur, suggestive of osteomyelitis. In specific laboratory tests for diagnosis of leishmaniasis ELISA reagent and RIFI reagent 1:40. As recommended by the Ministry of Health, the animal was euthanized. No macroscopic findings were reported during the necropsy, usually associated with leishmaniasis. The ischium bone fragments were sent for histopathological examination. There was intense proliferation of mononuclear inflammatory cells, mainly macrophages and lymphocytes. Amastigotes of Leishmania sp, were identified in the cytoplasm of some macrophages and bone tissue. In endemic areas for canine leishmaniasis, dogs with a history of intermittent lameness, and radiographic lytic bone lesions suggestive of osteomyelitis should be directed to realization of the histopathology and serologic tests for the differential diagnosis of Leishmania sp. Even without evidence of cutaneous or visceral lesions, usually associated with this disease.

The distal sesamoid bone, also known as navicular bone, is located inside the hoof, palmar (thoracic limbs) or plantar (pelvic limbs) to distal interphalangeal joint. Its extremities are fixed by collateral medial and lateral sesamoidean ligament and distal impar sesamoidean ligament. Navicular disease diagnostic is made through a thorough clinical exam, nerve blocks and imaging exams. Even though imaging exams are not conclusive, they are essential to evaluate the extension of soft tissue lesions. Radiographic projections used to evaluate navicular bone are lateromedial (LM); dorsoproximal-palmaro/plantarodistal oblíqua (D30Pr-PaDiO); dorsoproximal-palmaro/plantarodistal oblíqua (D60Pr-PaDiO) e a palmaro/plantaroproximal-palmaro/plantarodistal oblíqua (PaPr-PaDiO). This exam allows to identify number and shape alterations of synovial invaginations on the distal (foramem nutricio) in the distal margin of distal sesamoid, osteophytes, enthesophytes and periarticular lesions. There are four ultrasonographic accesses described in literature to evaluate podotroclear apparatus, they are: palmar or plantar distal do the pastern, through the heel bulbs, through coronary band and transcuneal. These images allow a beeter the evaluation of soft tissue next to the distal sesamoid, because it is more sensitive than radiographic exam to evaluate acute lesions in soft tissues and perioesteum

Germline mutations in TP53 gene are associated with Li-Fraumeni syndrome (LFS) and its variants Li-Fraumeni-like (LFL). They predispose carriers to a wide variety of early onset tumors. In Brazil, there is a high frequency of a germline mutation in this gene (NC_000017.9: c.1010G>A; p.R337H) in Southern and Southeastern regions, due to a founder effect. It is estimated to be present in 0,3% ofthe local population, but only few families have been detected. Due to this significant divergence, the purpose of this study was to verify the effectiveness of wider criteria for detection of these individuals. Herein, clinical criteria were established, DNA samples were collected, analyzed by Restriction Fragment Length Polymorphism (RFLP) and sequenced. Thus, assessing the prevalence of this mutation in families with multiple cases of cancer. Based on our proposed criteria, one out of 31 patients (3,22%) was found to carry p.R337H mutation. The patient developed ductal invasive breast cancer at age 47, invasive adenocarcinoma of the lung at age 48 and soft-tissue sarcoma at age 49. In addition, an extensive cancer family history was referred, atypical for LFS, including a case of Ewing’s sarcoma. These outcomes indicate that the proposed criteria may detect probable carriers who did not fit previous LFS criteria. Nevertheless, additional studies, which might include a larger number of families and more stringent parameters, will be useful to improve screening sensibility
