The frequency with which condoms are used in sex relations by subjects with HIV was determined by interviewing 132 individuals, 82 men and 50 women, most of them from São Paulo state and some from other regions of the country, all of them seen at an outpatient clinic of the School of Medicine in Botucatu. The women were younger, were of lower educational level and had poorer professional qualification than men. Also, a greater proportion of women were widowed, separated or divorced. We observed that 43.9% of men and 72% of women had been contaminated by the sexual route, but only 41.2% of the men and 31.8% of the women reported the use of a condom after the diagnosis of infection, with most men and women preferring sexual abstinence. The results enable the conclusion that there is still a need to continue to provide information about the use of condoms and to guarantee their free-of-charge distribution due to the low levels of education and professional qualification of the individuals studied. The data also suggest that campaigns for the dissemination of preventive measures should consider the social and cultural differences of infected women.

The occurrence of toxoplasmosis and enteroparasitosis was studied in 434 children from elementary schools in the rural and urban areas of Rolândia, Paraná State, Brazil. Sera and fecal samples from all the students were submitted to IFA for Toxoplasma gondii and coproparasitological tests, respectively. The children were tested by Amsler grid and 72 of them were examined for the presence of lesions compatible with ocular toxoplasmosis. Some variables were tested but none showed increased risk for toxoplasmosis. The distribution according to sex and age and also same other variables are presented and discussed. Correlations between Amsler's grid test, toxoplama RIFI, occurrence of eyes lesions and enteroparasitosis are also considered.

We report the results of a retrospective study on the frequency of intestinal nematodes among 198 alcoholic and 440 nonalcoholic patients at the University Hospital Cassiano Antonio Moraes in Vitória, ES, Brazil. The control sample included 194 nonalcoholic patients matched according to age, sex and neighborhood and a random sample of 296 adults admitted at the same hospital. Stool examination by sedimentation method (three samples) was performed in all patients. There was a significantly higher frequency of intestinal nematodes in alcoholics than in controls (35.3% and 19.2%, respectively), due to a higher frequency of Strongyloides stercoralis (21.7% and 4.1%, respectively). Disregarding this parasite, the frequency of the other nematodes was similar in both groups. The higher frequency of S. stercoralis infection in alcoholics could be explained by immune modulation and/or by some alteration in corticosteroid metabolism induced by chronic ethanol ingestion. Corticosteroid metabolites would mimic the worm ecdisteroids, that would in turn increase the fecundity of females in duodenum and survival of larvae. Consequently, the higher frequency of Strongyloides larvae in stool of alcoholics does not necessarily reflect an increased frequency of infection rate, but only an increased chance to present a positive stool examination using sedimentation methods.

The objective of this study was to investigate the frequency of HIV infection among female sex workers in the port area of Imbituba (State of Santa Catarina), and to identify the viral subtype and its susceptibility to antiretroviral medications. Ninety women were interviewed between December 2003 and February 2004. Six (6.7%) were HIV-positive. Genotyping for HIV, performed on four samples, detected subtype C in three of them, which is predominant in Africa and Asia, and subtype B in one of them, which is prevalent in Brazil, USA and Europe. The results suggest that the Port of Imbituba may be one of the gateways for HIV-1 subtype C to enter Brazil, and for its dissemination to the rest of the country and the Mercosul area, along the highway BR-101. This points towards the need for preventive work to reduce the introduction and dissemination of HIV subtype C in Brazil.

An HIV seroprevalence and molecular study was conducted among 935 subjects: 723 female commercial sex workers, 92 men who have sex with men and 120 HIV-positive volunteers. The reported injection drug use rates were 0.7% in female commercial sex workers and 3% in men who have sex with men. Sexually transmitted infections were reported in 265 (37%) of the female commercial sex workers and 38 (41%) of the men who have sex with men. A total of 20 (2.8%) female commercial sex workers and 12 (13%) men who have sex with men became HIV infected during the study period. A history of sexually transmitted infection increased the risk of subsequent HIV infection twofold (adjusted odds ratio of 2.5) among the female commercial sex workers, while cocaine use had an adjusted odds ratios of 6.61 among men who have sex with men. From 130 samples, and based on heteroduplex mobility assaying for the env gene, with sequencing of part of pol and/or full genomes, subtype B was the predominant subtype identified (66%); followed by subtype F (22%) and subtype C (4%). Recombinant CRF12-BF strains were identified in 6% and CRF17_BF was identified in 2%.

RESUMO - Enquadramento: O envelhecimento dos indivíduos nos países mais desenvolvidos e o aumento da incidência de doenças crónicas associadas a estados de dependência e incapacidade têm contribuído para o desenho e implementação de novas políticas de saúde e sociais. Assiste-se, por isso, atualmente, a uma mudança no paradigma da procura de cuidados de saúde, sendo crescente a procura de cuidados de longa duração ou cuidados continuados. O desenvolvimento e implementação de novos modelos de prestação de cuidados de saúde pretendem dar resposta à crescente procura de cuidados continuados, bem como promover a eficiência dos serviços e a disponibilização de camas nos hospitais, retirando dos serviços de agudos as pessoas que não necessitam de cuidados hospitalares, mas sim de cuidados continuados. Neste contexto foi criada em Portugal a Rede Nacional de Cuidados Continuados Integrados (RNCCI), como resposta ao aumento do número de pessoas em situação de dependência, e que necessitam tanto de cuidados de saúde como sociais, e à necessidade de reorganizar e promover a eficiência dos serviços de internamento hospitalar. Objetivo: Determinar o impacto da RNCCI na demora média hospitalar, no período de tempo compreendido entre 1 de Janeiro de 2009 e 31 de Junho de 2011. Métodos: O estudo realizado, com base na revisão da literatura, descreve os principais aspectos referentes ao envelhecimento dos indivíduos e aos cuidados continuados. Foram descritos diferentes modelos e programas organizacionais de prestação de cuidados continuados e o seu impacto na demora média hospitalar. Foi determinada a população em estudo no período de tempo compreendido entre 1 de Janeiro de 2009 e 31 de Junho de 2011. A população foi caraterizada de acordo com o ano e distribuída por dez trimestres para melhor tratamento estatístico e leitura dos dados. Foi considerado o sexo e a faixa etária dos indivíduos sinalizados, de acordo com o GDH de internamento hospitalar e respetiva sub-região de saúde. Foi comparada por trimestre a demora média dos internamentos hospitalares e a demora média hospitalar dos episódios referenciados a nível nacional e ao nível das sub-regiões de saúde. Foram caraterizados os GDH que representam 50% das sinalizações. Foram analisados, por semestre, os três GDH com maior número de referenciações para a RNCCI de acordo com as diferentes regiões de saúde, comparando as respetivas demoras médias nacionais e regionais. Resultados: No periodo de tempo em análise foi verificado que a população com maior utilização dos serviços da RNCCI encontra-se na faixa etária entre 65 ou mais anos, com 79,4% do total de sinalizações efetuadas. Tendo 50% das sinalizações sido referentes aos GDH 14, GDH 211, GDH 533, GDH 818, GDH 810 e GDH 209. Foi apurada uma demora média nacional compreendida entre os 7,3 dias e os 7,7 dias, comparativamente a uma demora média dos episódios referenciados para a RNCCI compreendida entre os 21,9 dias e os 33 dias, para o mesmo período de tempo. Em termos regionais a região de LVT apresenta os valores de demora média mais elevados, com um intervalo entre os 28,8 dias e os 50,3 dias de demora média. Para o GDH 14 foi observada uma demora média dos episódios referenciados compreendida entre os 14,4 dias e os 26,7 dias. No mesmo período de tempo o a demora média nacional para o mesmo GDH situava-se entre os 9,8 dias e os 10,2 dias. Para o GDH 211 foi observada uma demora média dos episódios referenciados compreendida entre os 17,2 dias e os 28,9 dias. Comparativamente a demora média nacional para o mesmo GDH situava-se entre os 12,5 dias e os 13,5 dias. Para o GDH 533 foi observada uma demora média dos episódios referenciados compreendida entre os 23,3 dias e os 52,7 dias. Comparativamente, no mesmo período de tempo, a demora média nacional para o mesmo GDH situava-se entre os 18,7 dias e os 19,7 dias. Conclusões: Foi possível concluir, quanto ao impacto da RNCCI na demora média hospitalar, que a demora média dos episódios referenciados para a Rede é superior à demora média nacional em todo o período de tempo em análise. Relativamente à demora média dos GDH com maior número de referenciações, os GDH 14, 211 e 533, verifica-se que todos eles apresentam uma demora média de referenciação superior à demora média nacional, e demora média regional para o mesmo GDH, em todo o período de tempo do estudo. Ou seja, foi possível verificar que a demora média para indivíduos com o mesmo GDH é superior nos que são referenciados para a RNCCI.

RESUMO - Introdução: As Infeções nosocomiais da corrente sanguínea associada a cateter venoso central (INCS-CVC) provocam um aumento das despesas hospitalares, traduzindo num aumento dos dias de internamento, consumo de antibióticos e de meios complementares de diagnóstico e terapêutica (MCDT). O presente estudo pretende avaliar os custos das INCS-CVC nos serviços de internamento do CHLO, no ano de 2012. Metodologia: Realizou-se um estudo retrospetivo de caso-controlo para determinar os custos adicionais inerentes às INCS-CVC. Foram identificados, em 2012, 32 doentes com infeção e 31 sem infeção. Os controlos foram extraídos da população tendo igual grupo diagnóstico Homogéneo (GDH), idade, sexo, serviço e duração de internamento e presença de CVC. As principais fontes de informação foram os registos da Comissão Controlo de Infeção (CCI) e do processo clinico eletrónico (PCE). A estimativa dos custos teve em consideração a duração de internamento, consumo de antibióticos e de MCDT. Resultados: A idade média dos casos e controlos foi de 66 e 69, respetivamente (p=0,432), 50% dos casos e 51,6% dos controlos eram do sexo masculino. Um total de 22 casos foi comparado com 22 controlos. A duração média de internamento dos casos e controlos foi de 70,8 e 36,6 dias, respetivamente (p=0,000). Em média o custo adicional por doente com antibióticos foi de 256€ (p=0,001). Nos casos o consumo de análises clinica foi 2,5 vezes superior e de exames imagiológicos 2 vezes superior aos controlos. O custo total médio adicional por doente foi de 20.737,6€. Conclusão: A ocorrência de INCS-CVC resultou num aumento significativo de utilização de recursos hospitalares e consequentemente num aumento dos custos hospitalares.

INTRODUCTION: The prevalence of cholelithiasis in the general population ranges from 9 to 18%. This prevalence is known to be higher in the presence of parasympathetic nerve damage of the biliary tract either due to surgery (vagotomy) or neuronal destruction (Chagas disease). The objective of this study was to evaluate the association of cholelithiasis and chagasic or idiopathic megaesophagus. METHODS: The ultrasound scans of 152 patients with megaesophagus submitted to cardiomyotomy and subtotal esophagectomy surgery were evaluated. The presence of cholelithiasis was compared between chagasic and idiopathic esophagopathy and ultrasound and clinical findings were correlated with age, sex and race. RESULTS: A total of 152 cases of megaesophagus, including 137 with chagasic megaesophagus and 15 with idiopathic megaesophagus, were analyzed. The mean age was 56.7 years (45-67) in the 137 patients with chagasic megaesophagus and 35.6 years (27-44) in the 15 cases of idiopathic megaesophagus, with a significant difference between the two groups (p < 0.0001). The group with chagasic megaesophagus consisted of 59 (43%) women and 78 (56.9%) men, while the group with idiopathic megaesophagus consisted of 8 (53.3%) women and 7 (46.6%) men, showing no significant difference between the groups. Of the 137 patients with confirmed chagasic megaesophagus, 39 (28.4%) presented cholelithiasis versus one case (6.6%) in the 15 patients with idiopathic megaesophagus. CONCLUSIONS: The prevalence of cholelithiasis is high in patients with chagasic megaesophagus and preoperative ultrasound should be performed routinely in these patients in order to treat both conditions during the same surgical procedure.

INTRODUCTION: White piedra is a superficial mycosis caused by the genus Trichosporon and characterized by nodules on hair shaft. METHODS: The authors report a family referred to as pediculosis. Mycological culture on Mycosel® plus molecular identification was performed to precisely identify the etiology. RESULTS: A Trichosporon spp. infection was revealed. The molecular procedure identified the agent as Trichosporon inkin. CONCLUSIONS: White piedra and infection caused by T. inkin are rarely reported in Southern Brazil. The molecular tools are essentials on identifying the Trichosporon species.

INTRODUCTION: Little information regarding hepatitis B virus (HBV) and hepatitis C virus (HCV) infections among Brazilian female prisoners exists. This study investigated the prevalence and risk factors associated with HBV and HCV infections and identified viral genotypes among female prisoners in Goiás, Central Brazil. METHODS: Women incarcerated in the largest prison in the State of Goiás were invited to participate in the study. All female prisoners were interviewed and tested for the detection of hepatitis B surface antigen (HBsAg), antibodies against HBsAg (anti-HBs), against hepatitis B core antigen (anti-HBc), and antibody against HCV (anti-HCV) by ELISA. HBsAg and anti-HCV positive samples were tested for HBV DNA and HCV RNA and genotyped, respectively. RESULTS: Participants (n=148; 98.6%) completed the study with an overall HBV prevalence of 18.9%. Age >30 years, a low education level, sex with a sexually transmitted diseases carrier, and a male sexual partner serving in the same penitentiary were associated with HBV infections. Only 24% of the women were anti-HBs positive suggesting previous HBV vaccination. Nine female prisoners (6.1%) were anti-HCV positive. Age >40 years, injecting drug use and length of incarceration were statistically associated with anti-HCV antibodies. Five samples were HCV RNA positive and classified as genotypes 1 (subtypes 1a; n=3 and 1b; n=1) and 3 (subtype 3a; n=1). The HBsAg-reactive sample was HBV DNA positive and genotype A. CONCLUSIONS: These findings highlight the necessity of public policies to control hepatitis B and C infections and emphasize the importance of hepatitis B vaccination in prison environments.

Introduction Sex workers (SWs) are vulnerable to HIV, hepatitis, and syphilis coinfection. Methods A cross-sectional study was conducted in Tubarão, Laguna, and Imbituba, Southern Brazil. We surveyed 147 SWs using face-to-face interviews and blood sampling for serological evaluation. Results Prevalence of hepatitis B (HBV) was 23.1%, syphilis 19.7%, hepatitis C (HCV) 8.8%, and HIV 8.8%. Of 13 HIV-infected patients, 3 were co-infected with HCV, 4 with syphilis, and 5 with HBV. Conclusions SWs had high HIV infection rates, and coinfection with viral hepatitis and syphilis.

RESUMO - Portugal continental, como outros países europeus, foi afectado por uma onda de calor de grande intensidade no Verão de 2003, com efeitos na mortalidade da população. O excesso de óbitos associados à onda de calor foi estimado pela comparação do número de óbitos observados entre 30 de Julho e 15 de Agosto de 2003 e o número de óbitos esperados se a população tivesse estado exposta às taxas de mortalidade médias do biénio 2000-2001 no respectivo período homólogo. Os óbitos esperados foram calculados com ajustamento para a idade. O número de óbitos observados (O) foi superior ao número esperado (E) em todos os dias do período estudado e o seu excesso global foi estimado em 1953 óbitos (excesso relativo de 43%), dos quais 1317 (61%) ocorreram no sexo feminino e 1742 no grupo de 75 e + anos (89%). A nível distrital, Portalegre teve o maior aumento relativo do número de óbitos (+89%) e Aveiro o menor (+18%). Numa área geográfica contínua do interior do território (Guarda, Castelo Branco, Portalegre e Évora) houve aumentos relativos superiores a 80%. Em termos absolutos, o maior excesso de óbitos ocorreu no distrito de Lisboa (mais cerca de 396) e no do Porto (mais cerca de 183). As causas de morte «golpe de calor» e «desidratação e outros distúrbios metabólicos» tiveram os aumentos relativos mais elevados (razões O/E de, respectivamente, 70 e 8,65). Os maiores aumentos absolutos do número de óbitos ocorreram no grupo das «doenças do aparelho circulatório» (mais 758), nas «doenças do aparelho respiratório» (mais 255) e no conjunto de «todas as neoplasias malignas» (mais 131). No período da onda de calor e no período de comparação, a percentagem dos óbitos que ocorreu nos hospitais (52% e 56%), no domicílio (32 e 33%) e em «outros locais» foi semelhante. A discussão sobre os factores que condicionaram a obtenção dos valores apresentados, relativos ao excesso de óbitos por sexo, grupo etário, distrito, causa e local da morte, permite concluir que os mesmos se afiguram adequados para medir a ordem de grandeza e caracterizar o efeito da onda de calor na mortalidade. O erro aleatório, medido pelos intervalos de confiança, e alguns possíveis erros sistemáticos associados ao período de comparação escolhido não deverão afectar de modo relevante as estimativas.

Two searches for supersymmetric particles in final states containing a same-flavour opposite-sign lepton pair, jets and large missing transverse momentum are presented. The proton--proton collision data used in these searches were collected at a centre-of-mass energy s√=8 TeV by the ATLAS detector at the Large Hadron Collider and corresponds to an integrated luminosity of 20.3 fb−1. Two leptonic production mechanisms are considered: decays of squarks and gluinos with Z bosons in the final state, resulting in a peak in the dilepton invariant mass distribution around the Z-boson mass; and decays of neutralinos (e.g. χ~02→ℓ+ℓ−χ~01), resulting in a kinematic endpoint in the dilepton invariant mass distribution. For the former, an excess of events above the expected Standard Model background is observed, with a significance of 3 standard deviations. In the latter case, the data are well-described by the expected Standard Model background. The results from each channel are interpreted in the context of several supersymmetric models involving the production of squarks and gluinos.

A low-background inclusive search for new physics in events with same-sign dileptons is presented. The search uses proton--proton collisions corresponding to 20.3 fb−1 of integrated luminosity taken in 2012 at a centre-of-mass energy of 8 TeV with the ATLAS detector at the LHC. Pairs of isolated leptons with the same electric charge and large transverse momenta of the type e±e±,e±μ±, and μ±μ± are selected and their invariant mass distribution is examined. No excess of events above the expected level of Standard Model background is found. The results are used to set upper limits on the cross sections for processes beyond the Standard Model. Limits are placed as a function of the dilepton invariant mass within a fiducial region corresponding to the signal event selection criteria. Exclusion limits are also derived for a specific model of doubly charged Higgs boson production.

Purpose. To analyze dry eye disease (DED) tests and their consistency in similar nonsymptomatic population samples living in two geographic locations with different climates (Continental vs. Atlantic). Methods. This is a pilot study including 14 nonsymptomatic residents from Valladolid (Continental climate, Spain) and 14 sex-matched and similarly aged residents from Braga (Atlantic climate, Portugal); they were assessed during the same season (spring) of two consecutive years. Phenol red thread test, conjunctival hyperemia, fluorescein tear breakup time, corneal and conjunctival staining, and Schirmer test were evaluated on three different consecutive visits. Reliability was assessed using the intraclass correlation coefficient and weighted kappa (J) coefficient for quantitative and ordinal variables, respectively. Results. Fourteen subjects were recruited in each city with a mean (TSD) age of 63.0 (T1.7) and 59.1 (T0.9) years (p = 0.08) in Valladolid and Braga, respectively. Intraclass correlation coefficient and J values of the tests performed were below 0.69 and 0.61, respectively, for both samples, thus showing moderate to poor reliability. Subsequently, comparisons were made between the results corresponding to the middle and higher outdoor relative humidity (RH) visit in each location as there were no differences in mean temperature (p Q 0.75) despite RH values significantly differing (p e 0.005). Significant (p e 0.05) differences were observed between Valladolid and Braga samples on tear breakup time (middle RH visit, 2.76 T 0.60 vs. 5.26 T 0.64 seconds; higher RH visit, 2.61 T 0.32 vs. 5.78 T 0.88 seconds) and corneal (middle RH, 0.64 T 0.17 vs. 0.14 T 0.10; higher RH, 0.60 T 0.22 vs. 0.0 T 0.0) and conjunctival staining (middle RH, 0.61 T 0.17 vs. 0.14 T 0.08; higher RH, 0.57 T 0.15 vs. 0.18 T 0.09). Conclusions. This pilot study provides initial evidence to support that DED test outcomes assessing the ocular surface integrity and tear stability are climate dependent. Future large-sample studies should support these outcomes also in DED patients. This knowledge is fundamental for multicenter clinical trials. Lack of consistency in diagnostic clinical tests for DED was also corroborated. (Optom Vis Sci 2015;92:e284Ye289)
