Este estudo envolveu 25.891 doadores de sangue que compareceram pela primeira vez ao Hemocentro de Ribeirão Preto, entre 23/06/1996 e 22/06/2001. Seu objetivo foi estudar a positividade de marcadores sorológicos das hepatites B e C em testes de triagem e estimar a prevalência de infe§Ã£o atual ou pregressa pelos vírus de ambas as hepatites, através da análise dos resultados de testes confirmatórios. Estudaram-se dados do Hemocentro e do Hospital das Clínicas da Faculdade de Medicina, coletando-se inform§Ãµes referentes ao doador e aos resultados dos testes sorológicos. A popul§Ã£o foi composta majoritariamente por homens (83,6%) de 26 a 45 (64%) anos de idade. Os valores de positividade nos testes da triagem foram 0,6% (IC95%: 0,54 0,72) para o HBsAg e 1,2% (IC95%: 1,02 1,28), para o anti-HCV. Os valores da prevalência, nos testes confirmatórios, foram 0,2% (IC95%: 0,16 0,28), para a hepatite B, e 0,3% (IC95%: 0,24 0,38) para a hepatite C.

The aim of the present study was to outline the serological profile of hepatitis C among blood donors seen at the Uberaba Regional Blood Center, Hemominas Foundation, over the last 14 years. The frequency of hepatitis C was compared between first-time and repeat donors and the epidemiological characteristics of those with positive and indeterminate ELISA anti-HCV (third and fourth generation) were analyzed based on the donor histories kept in the archives of the Uberaba Regional Blood Center. The serological ineligibility rate was 0.3%, with higher prevalence in the group of first-time donors. We did not find any significant differences regarding age, skin color, marital status or place of residence between eligible and ineligible donors; however, the frequency of positive serology was higher among men. The lower (0.3%) rate of ineligibility due to hepatitis C that was observed at the Uberaba Regional Blood Center, in relation to most Brazilian blood centers, is probably due to the large number of repeat donors (83.3%). This reinforces the importance of achieving donor commitment for increasing transfusion safety.

The role of sexual or intrafamilial transmission of hepatitis C is controversial. A phylogenetic analysis was performed on the non-structural region 5B of the hepatitis C virus (NS5B-HCV). High percentages of homology (mean of 98.3%) were shown between the couples. Twenty (83.3%) of the 24 men but only two of the women (8.3%) reported having had sexually transmitted diseases during their lives. The risk factors for HCV acquisition were blood transfusion (10 couples), use of illegal injected drugs (17), use of inhalants (15), acupuncture (5) and tattoos (5). The shared use of personal hygiene items included toothbrushes between six couples (25%), razor blades between 16 (66.7%), nail clippers between 21 (87.5%) and manicure pliers between 14 (58.3%). The high degree of similarity of the hepatitis C virus genome supports the hypothesis of hepatitis C virus transmission between these couples. The shared use of personal hygiene items suggests the possibility of intrafamilial transmission of infection.

INTRODUCTION: Although various studies have been published regarding the treatment of chronic hepatitis C (CHC) with peginterferon (Peg-IFN) and ribavirin, little is known regarding the real impact of gender on the characteristics that influence the effectiveness and safety of antiviral treatment for CHC patients. The objective of this study was to evaluate the influence of gender on HCV treatment outcomes. METHODS: A retrospective analytical study was conducted among selected carriers of CHC genotype 1, who were treated with Peg-IFN α-2b at a dose of 1.5 μg/kg or Peg-IFN α-2a at a dose of 180 μg/week plus a ribavirin dose of 1,000-1,250 mg/day, according to weight, between 2001 and 2007. RESULTS: Among 181 patients undergoing treatment, the mean age was 46.4 ± 11.0 years and 46% were women. At baseline, 32% of the patients had advanced fibrosis (F3-F4 Scheuer), and 83% of the subjects had viral load > 400,000 IU/ml, without significant difference between the genders (p = 0.428 and p = 0.452, respectively). When compared with men, women had higher incidence of many adverse events such as anemia (p < 0.001) and higher need for dose reduction, for both Peg-IFN (p = 0.004) and ribavirin (p = 0.006). However, the rate of sustained virological response (SVR) did not differ between the genders: 45% (female) vs 41% (male); p=0.464. CONCLUSIONS: This study suggests that women and men react differently to combined therapy, especially in relation to the incidence of adverse events and the need for dose modification. Nevertheless, these differences do not influence the SVR rate.

INTRODUƒO: A hepatite C é uma das principais causas de doença hepática em todo mundo. Apresenta um curso evolutivo dinâmico e influenciável por diversos co-fatores. Dentre eles, a infe§Ã£o pregressa pelo vírus B (anti-HBcAg [+] e HBsAg [-]) tem se associado a pior prognóstico histológico e terapêutico. Este trabalho teve como objetivo analisar a associ§Ã£o entre a infe§Ã£o pregressa pelo vírus B e fibrose hepática em portadores de hepatite C crônica, de maneira independente. MÉTODOS: Foram revistos retrospectivamente prontuários médicos de pacientes infectados cronicamente pelo vírus C, atendidos consecutivamente durante um ano no ambulatório de Doenças Infecciosas e Parasitárias - HC FMUSP, quanto aos dados epidemiológicos, clínicos, laboratoriais e histológicos. A análise de independência do impacto da infe§Ã£o pregressa pelo vírus B foi realizada através de modelo estatístico de regressão logística multivariado, considerando a dete§Ã£o do anti-HBcAg como variável de exposição, sendo o desfecho a alter§Ã£o estrutural histopatológica graus 3 e 4 (septos com form§Ã£o de nódulos e cirrose).0 RESULTADOS: 145 indivíduos foram avaliados pelo estudo, 47.2% com anti-HBcAg (+). O fator de risco mais comumente relatado foi transfusão de sangue e hemoderivados (35,9%). Embora necrose em saca-bocado tenha sido encontrada com maior frequência no grupo de infe§Ã£o pregressa, a sorologia anti-HBcAg (+) não se associou à fibrose hepática avançada. CONCLUSÕES: A infe§Ã£o pregressa pelo vírus B não parece acentuar a lesão estrutural desencadeada pela hepatite C crônica, após controle estatístico para outros co-fatores sabidamente capazes de influenciar a história natural desta infe§Ã£o.

INTRODUCTION: Approximately 30% of hepatitis C virus (HCV) monoinfected patients present persistently normal alanine aminotransferase (ALT) levels. Most of these patients have a slow progression of liver fibrosis. Studies have demonstrated the rate of liver fibrosis progression in hepatitis C virus-human immunodeficiency virus (HCV-HIV) coinfected patients is faster than in patients infected only by HCV. Few studies have evaluated the histological features of chronic hepatitis C in HIV-infected patients with normal ALT levels. METHODS: HCV-HIV coinfected patients (HCV-RNA and anti-HIV positive) with known time of HCV infection (intravenous drugs users) were selected. Patients with hepatitis B surface antigen (HBsAg) positive or hepatitis C treatment before liver biopsy were excluded. Patients were considered to have a normal ALT levels if they had at least 3 normal determinations in the previous 6 months prior to liver biopsy. All patients were submitted to liver biopsy and METAVIR scale was used. RESULTS: Of 50 studied patients 40 (80%) were males. All patients were treated with antiretroviral therapy. The ALT levels were normal in 13 (26%) patients. HCV-HIV co-infected patients with normal ALT levels had presented means of the liver fibrosis stages (0.77±0.44 versus 1.86±1.38; p<0.001) periportal inflammatory activity (0.62±0.77 versus 2.24±1.35; p<0.001) and liver fibrosis progression rate (0.058±0.043 fibrosis unit/year versus 0.118±0.102 fibrosis unit/year) significantly lower as compared to those with elevated ALT. CONCLUSIONS: HCV-HIV coinfected patients with persistently normal ALTs showed slower progression of liver fibrosis. In these patients the development of liver cirrhosis is improbable.

INTRODUCTION: The aim of this study was to evaluate the therapeutic response of hepatitis C in patients coinfected with human immunodeficiency virus (HIV-1). METHODS: A retrospective study of 20 patients coinfected with HIV-1/HCV who were treated in the outpatient liver clinic at the Sacred House of Mercy Foundation Hospital of Pará (Funda&#231;ão Santa Casa de Miseric&#243;rdia do Pará - FSCMPA) from April 2004 to June 2009. Patients were treated with 180µg PEG interferon-α2a in combination with ribavirin (1,000 to 1,250mg/day) for 48 weeks. The end point was the sustained virological response (SVR) rate (HCV RNA negative 24 weeks after completing treatment). RESULTS: The mean age of the patients was 40±9.5 years, of which 89% (n=17) were male, and the HCV genotypes were genotype 1 (55%, n=11/20), genotype 2 (10%, n=2/20) and genotype 3 (35%, n=7/20). The mean CD4+ lymphocyte count was 507.8, and the liver fibrosis stages were (METAVIR) F1 (25%), F2 (55%), F3 (10%) and F4 (10%). The early virological response (EVR) was 60%, the end-of-treatment virological response (EOTVR) was 45% and the SVR was 45%. CONCLUSIONS: The median HCV viral load was high, and in 85% of cases in which highly active antiretroviral therapy (HAART) was used, none of the patients with F3-F4 fibrosis responded to treatment. Of the twenty patients treated, 45% achieved SVR and 45% achieved EOTVR. Studies that include cases from a wider region are needed to better evaluate these findings.

Introduction Previous studies have shown high residual risk of transfusing a blood donation contaminated by human immunodeficiency virus (HIV) or hepatitis C virus (HCV) in Brazil and motivated the development of a Brazilian platform for simultaneous detection of both viruses by nucleic acid amplification test (NAT) denominated HIV/HCV Bio-Manguinhos/Funda&#231;ão Oswaldo Cruz (FIOCRUZ). The objective of this study was to verify seroprevalence, incidence and residual risk for both viruses before and after the implementation of NAT. Methods Over 700,000 blood samples from all blood banks in the southern Brazilian State of Santa Catarina were analyzed during the period between January 2007 and July 2013. Results Compared with the period preceding the NAT screening, HIV prevalence increased from 1.38 to 1.58 per 1,000 donors, HIV incidence rate increased from 1.22 to 1.35 per 1,000 donor-years, and HIV residual risk dropped almost 2.5 times during the NAT period. For HCV, seroprevalence increased from 1.22 to 1.35 per 1,000 donors, incidence dropped from 0.12 to 0.06 per 1,000 donor-years, and residual risk decreased more than 3 times after the NAT implementation. Conclusions NAT reduced the duration of the immunologic window for HIV and HCV, thus corresponding to approximately 2.5- and 3-fold respective residual risk reductions.

INTRODUCTION: The present study investigated the prevalence of two single-nucleotide polymorphisms (SNPs) in the Toll-like receptor 3 (TLR3) gene in patients infected with hepatitis B virus (HBV) and hepatitis C virus (HCV). METHODS: Samples collected from HCV (n = 74) and HBV (n = 35) carriers were subjected to quantitative real-time PCR (qPCR) to detect the presence of the SNPs rs5743305 and rs3775291 in TLR3 and to measure the following biomarkers: alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transpeptidase (GGT), and prothrombin time (PT). A healthy control group was investigated and consisted of 299 HCV- and HBV-seronegative individuals. RESULTS: No significant differences in allele, genotype and haplotype frequencies were observed between the investigated groups, and no association was observed between the polymorphisms and histopathological results. Nevertheless, genotypes TA/AA (rs5743305) and GG (rs3775291) appear to be associated with higher levels of ALT (p<0.01), AST (p<0.05) and PT (p<0.05). In addition, genotypes TT (rs5743305; p<0.05) and GG (rs3775291; p<0.05) were associated with higher GGT levels. CONCLUSIONS: This genetic analysis revealed the absence of an association between the polymorphisms investigated and susceptibility to HBV and HCV infection; however, these polymorphisms might be associated with a greater degree of biliary damage during the course of HCV infection.

A pimenta longa (Piper hispidinervum C. DC.) é uma planta nativa do Estado do Acre. Caracteriza-se pela produção de óleo essencial com alto teor de safrol, usado na produção de perfumes, comésticos e inseticidas. Por ser uma planta ainda em fase de domesticação, há necessidade de pesquisas para maximizar sua produtividade. Um desses estudos refere-se ao melhor mês de corte e ainda se é possível efetuar um ou mais cortes num intervalo de 12 meses. Neste trabalho, objetivou-se, avaliar o melhor mês e freqüência de corte da pimenta longa durante 3 anos. Foram instalados dois experimentos, utilizando-se o delineamento de blocos ao acaso em esquema de parcela subdividida no tempo, sendo os anos as parcelas e os meses as subparcelas. No primeiro, utilizou-se quatro repetições e oito plantas úteis por parcela no esp§amento de 1 x 1 m. Foi realizado somente um corte, num intervalo de 12 meses, em: outubro, novembro, dezembro, janeiro, fevereiro, março e abril. No segundo, utilizou-se nove repetições, efetuando-se dois cortes em 12 meses, com intervalo de quatro meses entre o primeiro e o segundo, sendo estes realizados em outubro/fevereiro, novembro/março e dezembro/abril. Verificou-se em ambos os experimentos que os cortes efetuados mais próximos do final do período chuvoso, março e abril, foram os que apresentaram as maiores produtividades de óleo e que o rendimento (% de óleo essencial) em rel§Ã£o à matéria seca foi maior quando se realizou somente um corte num intervalo de 12 meses.

Neste trabalho, a espécie Piper hispidinervum (pimenta longa), endêmica do estado do Acre, foi cultivada na região do Vale do Itaj­ - SC, e sua adapt§Ã£o foi avaliada em rel§Ã£o à composição química do óleo essencial obtido pelos processos de hidrodestil§Ã£o convencional e por micro-ondas. O safrol foi identificado como o constituinte majoritário do óleo essencial desta espécie, o qual foi empregado como parâmetro de avali§Ã£o da capacidade de adapt§Ã£o desta planta à região sul do Brasil, já que a proposta abrange avaliar a utiliz§Ã£o desta espécie como fonte alternativa de safrol, em substituição a Canela Sassafrás (Ocotea odorifera), espécie muito explorada até a década de 90 nesta região. As amostras da planta foram obtidas de diferentes regiões do Acre e foram cultivadas na est§Ã£o experimental da EPAGRI - Itaj­-SC. O óleo essencial das folhas forneceu um teor médio de safrol entre 76,6% e 89,9%. A análise por CG-DIC e CG-EM do óleo com maior concentr§Ã£o de safrol, apresentou os seguintes constituintes: safrol (89,93%), α-terpineno (0,35%), (E)-β-ocimeno (0,54%), terpinoleno (3,10%), valenceno (0,21%), (Z)-β-bisaboleno (1,70%) e guaiol (0,29%).

Wild boar (Sus scrofa) and red deer (Cervus elaphus) are the main maintenance hosts for bovine tuberculosis (bTB) in continental Europe. Understanding Mycobacterium tuberculosis complex (MTC) excretion routes is crucial to define strategies to control bTB in free-ranging populations, nevertheless available information is scarce. Aiming at filling this gap, four different MTC excretion routes (oronasal, bronchial-alveolar, fecal and urinary) were investigated by molecular methods in naturally infected hunter-harvested wild boar and red deer. In addition MTC concentrations were estimated by the Most Probable Number method. MTC DNA was amplified in all types of excretion routes. MTC DNA was amplified in at least one excretion route from 83.0% (CI95 70.8-90.8) of wild ungulates with bTB-like lesions. Oronasal or bronchial-alveolar shedding were detected with higher frequency than fecal shedding (p < 0.001). The majority of shedders yielded MTC concentrations <10(3) CFU/g or mL. However, from those ungulates from which oronasal, bronchial-alveolar and fecal samples were available, 28.2% of wild boar (CI95 16.6-43.8) and 35.7% of red deer (CI95 16.3-61.2) yielded MTC concentrations >10(3) CFU/g or mL (referred here as super-shedders). Red deer have a significantly higher risk of being super-shedders compared to wild boar (OR = 11.8, CI95 2.3-60.2). The existence of super-shedders among the naturally infected population of wild boar and red deer is thus reported here for the first time and MTC DNA concentrations greater than the minimum infective doses were estimated in excretion samples from both species.

Lithium-ion battery cathodes have been fabricated by screen-printing through the development of CLiFePO4 inks. It is shown that shear thinning polymer solutions in N-methyl-2-pyrrolidone (NMP) with Newtonian viscosity above 0.4 Pa s are the best binders for formulating a cathode paste with satisfactory film forming properties. The paste shows an elasticity of the order of 500 Pa and, after shear yielding, shows an apparent viscosity of the order of 3 Pa s for shear rates corresponding to those used during screen-printing. The screen-printed cathode produced with a thickness of 26 mm shows a homogeneous distribution of the active material, conductive additive and polymer binder. The total resistance and diffusion coefficient of the cathode are 450 V and 2.5 10 16cm2 s 1, respectively. The developed cathodes show an initial discharge capacity of 48.2 mAh g 1 at 5C and a discharge value of 39.8 mAh g 1 after 50 cycles. The capacity retention of 83% represents 23% of the theoretical value (charge and/or discharge process in twenty minutes), demonstrating the good performance of the battery. Thus, the developed C-LiFePO4 based inks allow to fabricate screen-printed cathodes suitable for printed lithium-ion batteries

OBJETIVO: Descrever os achados ao ecocardiograma transesofágico e evolução clínica de portadores de flail mitral valve. MÉTODOS: No período de janeiro/93 a março/97, 1675 pacientes foram submetidos, em nossa instituição, a ecocardiograma transesofágico, sendo que em 35 casos foi feito o diagnóstico de flail mitral valve e, posteriormente, obtida sua evolução clínica. RESULTADOS: A idade dos pacientes variou 12 a 87 anos (média 65±15) e 27 (77%) eram do sexo masculino. O folheto posterior foi o mais acometido (25 pacientes, 71%). O mecanismo do flail foi ruptura de cordoalha tendínea em todos os casos, exceto um, que apresentava importante alongamento e redundância de cordoalha. A etiologia foi prolapso e/ou degener§Ã£o mixomatosa em 15 pacientes, degenerativa em 9, isquêmica em 5, reumática em 4 e endocardite em 3. Regurgit§Ã£o mitral de grau importante ocorreu em 25 (71%) pacientes e moderada em 10 (29%). O tempo médio de acompanhamento foi de 375±395 dias (1 a 1380). Foram submetidos a tratamento clínico 19 pacientes e a tratamento cirúrgico 16, sendo que em todos foi confirmado o diagnóstico transesofágico. O número total de óbitos (hospitalar e pós-hospitalar) foi alto (34%), tanto em pacientes submetidos a tratamento clínico quanto cirúrgico. Entre os sobreviventes, 17 estão em classe funcional (CF) I e 6 em CF II da NYHA. CONCLUSÃO: O diagnóstico de flail mitral valve ao ecocardiograma transesofágico é acurado, permitindo a definição de sua etiologia e mecanismo. A alta mortalidade à época do diagnóstico, provavelmente, se relaciona à gravidade da doença subjacente. Embora os pacientes não operados estejam evoluindo bem, a baixa CF observada nestes pacientes pode ser atribuída ao curto período de seguimento.

OBJECTIVE: To compare blood pressure response to dynamic exercise in hypertensive patients taking trandolapril or captopril. METHODS: We carried out a prospective, randomized, blinded study with 40 patients with primary hypertension and no other associated disease. The patients were divided into 2 groups (n=20), paired by age, sex, race, and body mass index, and underwent 2 symptom-limited exercise tests on a treadmill before and after 30 days of treatment with captopril (75 to 150 mg/day) or trandolapril (2 to 4 mg/day). RESULTS: The groups were similar prior to treatment (p<0.05), and both drugs reduced blood pressure at rest (p<0.001). During treatment, trandolapril caused a greater increase in functional capacity (+31%) than captopril (+17%; p=0.01) did, and provided better blood pressure control during exercise, observed as a reduction in the variation of systolic blood pressure/MET (trandolapril: 10.7±1.9 mmHg/U vs 7.4±1.2 mmHg/U, p=0.02; captopril: 9.1±1.4 mmHg/U vs 11.4±2.5 mmHg/U, p=0.35), a reduction in peak diastolic blood pressure (trandolapril: 116.8±3.1 mmHg vs 108.1±2.5 mmHg, p=0.003; captopril: 118.2±3.1 mmHg vs 115.8±3.3 mmHg, p=0.35), and a reduction in the interruption of the tests due to excessive elevation in blood pressure (trandolapril: 50% vs 15%, p=0.009; captopril: 50% vs 45%, p=0.32). CONCLUSION: Monotherapy with trandolapril is more effective than that with captopril to control blood pressure during exercise in hypertensive patients.
