Background: The Geneva Prognostic Score (GPS), the Pulmonary Embolism Severity Index (PESI), and its simplified version (sPESI) are well known clinical prognostic scores for pulmonary embolism (PE).Objectives: To compare the prognostic performance of these scores in elderly patients with PE. Patients/Methods: In a multicenter Swiss cohort of elderly patients with venous thromboembolism, we prospectively studied 449 patients aged ≥65 years with symptomatic PE. The outcome was 30-day overall mortality. We dichotomized patients as low- vs. higher-risk in all three scores using the following thresholds: GPS scores ≤2 vs. >2, PESI risk classes I-II vs. III-V, and sPESI scores 0 vs. ≥1. We compared 30-day mortality in low- vs. higher-risk patients and the areas under the receiver operating characteristic curve (ROC). Results: Overall, 3.8% of patients (17/449) died within 30 days. The GPS classified a greater proportion of patients as low risk (92% [413/449]) than the PESI (36.3% [163/449]) and the sPESI (39.6% [178/449]) (P<0.001 for each comparison). Low-risk patients based on the sPESI had a mortality of 0% (95% confidence interval [CI] 0-2.1%) compared to 0.6% (95% CI 0-3.4%) for low-risk patients based on the PESI and 3.4% (95% CI 1.9-5.6%) for low-risk patients based on the GPS. The areas under the ROC curves were 0.77 (95%CI 0.72-0.81), 0.76 (95% CI 0.72-0.80), and 0.71 (95% CI 0.66-0.75), respectively (P=0.47). Conclusions: In this cohort of elderly patients with PE, the GPS identified a higher proportion of patients as low-risk but the PESI and sPESI were more accurate in predicting mortality.

This work investigates novel alternative means of interaction in a virtual environment (VE).We analyze whether humans can remap established body functions to learn to interact with digital information in an environment that is cross-sensory by nature and uses vocal utterances in order to influence (abstract) virtual objects. We thus establish a correlation among learning, control of the interface, and the perceived sense of presence in the VE. The application enables intuitive interaction by mapping actions (the prosodic aspects of the human voice) to a certain response (i.e., visualization). A series of single-user and multiuser studies shows that users can gain control of the intuitive interface and learn to adapt to new and previously unseen tasks in VEs. Despite the abstract nature of the presented environment, presence scores were generally very high.

La voix est tout sauf un stimulus auditif ordinaire. Pour cause, elle prend son importance de manière très précoce chez l’Homme lorsque, dans l’environnement amniotique, le fœtus entend pour la toute première fois la voix de sa mère. C’est en quelque sorte par l’intermédiaire de cette voix que les premiers contacts avec le monde extérieur, mais également avec l’Autre, s’effectuent. Le statut particulier de la voix humaine perdure au fil du développement, devenant plus tard le principal médium véhiculant le langage oral, si significatif pour l’Homme. En parallèle, et de manière tout aussi adaptative, elle permet la transmission d’informations non langagières renseignant sur l’identité, l’état émotionnel mais également le statut social de chaque individu. C’est ainsi que simplement en entendant la voix d’une personne inconnue, il est généralement possible d’en extrapoler son âge, son genre, mais également d’avoir une idée assez précise de l’état émotionnel dans lequel elle se trouve. Les capacités permettant d’extraire de la voix les divers éléments informationnels qu’elle contient ne seraient toutefois pas stables au fil du temps. Ainsi, le vieillissement normal semble associé à des difficultés de traitement des informations vocales de nature langagière, mais également non langagière. De nombreuses études se sont intéressées au déclin des capacités de traitement du discours avec l’âge. Beaucoup moins de travaux ont cependant considéré les conséquences du vieillissement sur le domaine paralinguistique et, lorsque des travaux s’y sont attardés, c’est essentiellement la sphère affective qui a été investiguée. En raison de ce peu d’études, mais également de leur focus portant spécifiquement sur la sphère émotionnelle, il est extrêmement ardu de généraliser les résultats obtenus au traitement vocal général. La présente thèse s’est donc intéressée aux capacités de traitement de la voix dans le vieillissement normal. La première étude de cette thèse (Article 1) avait pour objectif d’évaluer l’impact du vieillissement normal sur les capacités comportementales de traitement paralinguistique vocal non émotionnel. Pour ce faire, une batterie informatisée composée de quatre tâches a été élaborée : la batterie d’évaluation de la perception vocale (Batterie EPV; tâches de catégorisation de genre, de discrimination de sources sonores, adaptative de discrimination et de mémorisation). Cette batterie permettait de comparer les performances d’adultes jeunes et âgés lors du traitement de stimuli vocaux et non vocaux, mais également lors du traitement de divers stimuli vocaux. Cette première étude met en évidence, pour trois des quatre tâches comportementales, des performances inférieures chez les adultes âgés et ce, malgré le contrôle statistique des contributions du déclin auditif et cognitif. Pour les aînés, le traitement de stimuli vocaux, en comparaison au traitement de stimuli non vocaux, n’était toutefois pas systématiquement inférieur à celui des jeunes adultes. Sans que les performances ne puissent être prédites par la mesure cognitive utilisée comme covariable (performances au MoCA), il appert que les demandes cognitives inhérentes aux tâches participent à ces différences intergroupes. Le second article de ce travail visait quant à lui à explorer à l’aide de l’imagerie par résonnance magnétique fonctionnelle (IRMf), l’influence du vieillissement normal sur les réseaux neuronaux sous-tendant le traitement de l’information vocale, une telle investigation n’ayant jamais été effectuée auparavant. Pour ce faire, une tâche d’écoute passive (permettant le contraste de blocs de sons vocaux et non vocaux) ainsi qu’une tâche adaptative de discrimination ont été utilisées. La tâche adaptative, basée sur une type de protocole psycoacoustique « up-down », assurait l’obtention de niveau de performance équivalent entre les deux groupes, une condition nécessaire pour la comparaison de groupe au niveau neurofonctionnel. La comparaison des adultes jeunes et âgés n’a mis en évidence aucune disparité quant au recrutement des aires répondant préférentiellement à la voix : les aires vocales temporales (AVT). Ce résultat suggère que l’âge n’affecte pas la mobilisation des aires spécialisées dans le traitement de la voix. Néanmoins, à l’extérieur des AVT et chez les aînés, le sous recrutement d’une portion du cortex auditif a été observé, en parallèle au recrutement additionnel de régions pariétale, temporale et frontale (Article 2 – Étude 1). Lors de la réalisation d’une tâche adaptative de discrimination, contrairement à ce qui était attendu, les seuils de discrimination des deux groupes d’âges étaient comparables. Pour effectuer la tâche, les participants âgés ont cependant recruté un réseau neuronal plus étendu que celui des jeunes adultes, et pour les aînés, l’activation additionnelle de régions frontale et temporale sous-tendaient la réalisation de la tâche (Article 2 - Étude 2). Les données comportementales présentées dans cette thèse suggèrent que l’effet délétère que semble avoir le vieillissement normal sur les capacités de traitement paralinguistique vocal affectif est également retrouvé lors du traitement d’informations vocales émotionnellement neutres. En parallèle, la mise en place de phénomènes de plasticité cérébrale est objectivée. Ces derniers ne toucheraient cependant pas les réseaux spécialisés dans le traitement de la voix, qui seraient recrutés de manière comparable par les adultes jeunes et âgés. Néanmoins, la tâche d’écoute passive a mis en évidence la présence, chez les aînés, du recrutement sous-optimal d’une portion du cortex auditif (gyrus temporal transverse). En parallèle, et ce pour les deux tâches, des réseaux neuronaux surnuméraires étaient sollicitées par les adultes âgés, permettant potentiellement d’assurer, chez les ainés, le maintien de performances adéquates.

The aim of this study was to perform voice evaluation in teachers with and without vocal symptoms, identifying etiologic factors of dysphonia, voice symptoms, vocal qualities, and laryngeal lesions. Eighty teachers were divided into two groups: GI (without or sporadic symptoms, 40) and GII (with frequent vocal symptoms, 40). They answered a specific questionnaire, and were subject to a perceptual vocal assessment (maximum phonation time, glottal attack, resonance, coordination of breathing and voicing, pitch, and loudness), GIRBAS scale, and to videolaryngoscopy. Females were predominant in both groups, and the age range was from 36 to 50 years. Elementary teachers predominated, working in classes with 31-40 students. Voice symptoms and alterations in the perceptual vocal analysis and in the GIRBAS scale were more frequent in GII In 46 teachers (GI-16; GII-30), videolaryngoscopy exams were abnormal with the vocal nodules being the most frequent lesions. These results indicate that a teacher's voice is compromised, and requires more attention including control of environmental factors and associated diseases, preventive vocal hygiene, periodic laryngeal examinations, and access to adequate specialist treatment.

TEMA: a voz do professor tem sido foco de estudos nas últimas duas décadas devido à alta ocorrência de alterações vocais nesta classe profissional, assim, reforçou-se a necessidade dos professores participarem de ações para garantir saúde vocal. Poucos são os estudos na literatura que descrevem programas e seus resultados. PROCEDIMENTOS: Descrever um Programa de Saúde Vocal desenvolvido no período 2002 a 2005, para educadores de ensino público (infantil e fundamental) do interior do Estado de São Paulo, composto por grupos básicos de voz oferecendo conhecimento teórico prático de cuidados vocais, com triagem da qualidade da voz dos participantes; grupos avançados buscando reorganização dos processos de fonação e do uso vocal em sala de aula. RESULTADOS: em média 56% dos educadores inscritos frequentaram as ações; 62,9% das vozes apresentavam distúrbios na triagem vocal com maioria em grau discreto; no início dos grupos avançados 100% dos educadores participantes referiram mais de 3 sintomas associados ao uso vocal, e após, somente 45% deles ainda de 4 a 13 sintomas; os dados de autopercepção vocal revelaram baixos escores de impacto da voz nas atividades profissionais. CONCLUSÃO: a descrição revelou a necessidade de ajustes constantes do programa para alcançar seus objetivos. A baixa participação às ações pode estar relacionada à presença de impacto discreto da voz nas atividades profissionais, fato a ser mais bem investigado no futuro, e o benefício constatado objetivamente da participação dos educadores nos grupos avançados de voz foi a diminuição na quantidade de sintomas vocais.

In this work a computational method is presented to simulate the movements of vocal folds in three dimensions. The proposed model consists of a mesh free structure where each vertex is connected its neighbor through a group spring-damper. Forced oscillations were studied by time varying surface forces. The preliminary results using this model are similar with the literature and with the experimental stroboscopic observations of larynx. © 2006 IEEE.

Background: We examined the vocal complaints and evaluated the correlation between the vocal handicap index (VHI) and heart rate variability (HRV) in physical education teachers. We evaluated 46 teachers. Method: The subjects were investigated regarding voice complaint and the VHI was applied. HRV was recorded at seated rest for ten minutes and it was analyzed in the time, frequency domains, geometric indices and fractal exponents. The three domains of the VHI were correlated with the indices of HRV. Results: The physical education teachers presented a VHI score much below the standard of the physiological normality. There was correlation of the organic domain of the VHI with the NN50 and pNN50 and correlation of the functional domain and organic domain of the VHI with the HF index of HRV. Conclusion: The physical education teachers evaluated reported vocal complaints that affected their function and it is suggested to be related with the cardiac autonomic regulation.

The aims of this study were to analyse the validity, sensitivity and specificity of the protocol of oro-facial myofunctional evaluation with scores (OMES) for oro-facial myofunctional disorder (OMD) diagnosis in young and adult subjects. Eighty subjects were examined. The OMES was validated against the Nordic Orofacial Test-Screening (NOT-S) protocol (criterion validity) (Spearman correlation test). The construct validity was tested by analysis of the ability of the OMES (i) to differentiate healthy subjects (n = 22) from temporomandibular disorder (TMD) patients (n = 22), which frequently have OMD (MannWhitney test) and (ii) to measure the changes that occurred in a subgroup with TMD between the period before and after oro-facial myofunctional therapy (T group, n = 15) (Wilcoxon test). Two speech therapists trained with the OMES participated as examiners (E). There was a statistically significant correlation between the OMES and NOT-S protocols, which was negative because the two scales are inverse (r = -0.86, P < 0.01). There was a significant difference between the healthy and TMD subjects regarding the oro-facial myofunctional status (OMES total score, P = 0.003). After therapy, the T group showed improvement in the oro-facial myofunctional status (OMES total score, P = 0.001). Inter- and intra-examiner agreement was moderate, and the reliability coefficients ranged from good to excellent. The OMES protocol presented mean sensitivity and specificity = 0.80, positive predictive value = 0.76 and negative predictive value = 0.84. Conclusion: The OMES protocol is valid and reliable for clinical evaluation of young and adult subjects, among them patients with TMD.

The possible trade-off between the roles of glucocorticoids as facilitators of energy substrate mobilization and neural inhibitors of sexual behavior during breeding season is under debate. We studied the relationship between calling and territorial behavior with plasma levels of corticosterone (CORT) and plasma levels of testosterone (T) across the breeding season of Hypsiboas faber, a large and territorial Neotropical treefrog. We investigated these relationships through focal observations of males calling naturally, followed by blood sampling for hormonal radioimmunoassay. We additionally used an experimental approach, which consisted of broadcasting recorded advertisement calls for 10 min to simulate an invasion in the territory of the focal subjects, followed by behavioral observation and blood sampling for hormonal radioimmunoassay. Results showed a pattern of co-variation between CORT and T across the breeding season. Furthermore, individual variation in CORT and T was related to different aspects of behavior: individuals with higher CORT showed higher calling rates, and individuals with higher steroid levels, mainly T, showed higher responsivity to social stimulation by other males in the chorus. Experimental simulation of territorial intrusion by using playback of advertisement calls of this species did not elicit consistent changes in agonistic behavior and CORT, but decreased T in focal males. Objective: Parameters to distinguish normal from deviant voices in early childhood have not been established. The current study sought to auditorily and acoustically characterize voices of children, and to study the relationship between vocal behavior reported by teachers and the presence of vocal aberrations. Methods: One hundred children between four and 6 years and 11 months, who attended early childhood educational institutions, were included. The sample comprised 50 children with normal voices (NVG) and 50 with deviant voices (DVG) matched by gender and age. All participants were submitted to auditory and acoustic analysis of vocal quality and had their vocal behaviors assessed by teachers through a specific protocol. Results: DVG had a higher incidence of breathiness (p < 0.001) and roughness (p < 0.001), but not vocal strain (p = 0.546), which was similar in both groups. The average F-0 was lower in the DVG and a higher noise component was observed in this group as well. Regarding the protocol used "Aspects Related to Phonotrauma - Children's Protocol", higher means were observed for children from DVG in all analyzed aspects and also on the overall means (DVG = 2.15; NVG = 1.12, p < 0.001). In NVG, a higher incidence of vocal behavior without alterations or with discrete alterations was observed, whereas a higher incidence of moderate, severe or extreme alterations of vocal behavior was observed in DVG. Conclusions: Perceptual assessment of voice, vocal acoustic parameters (F-0, noise and GNE), and aspects related to vocal trauma and vocal behavior differentiated the groups of children with normal voice and deviant voice. Die vorliegende Arbeit über den Komponisten Johannes Driessler besteht aus einer Biographie, Werkanalysen und einem Werkverzeichnis inklusive Verlagsangaben. Johannes Driessler - geboren am 26. Januar 1921 in Friedrichsthal, gestorben am 4. Mai 1998 in Detmold - entfaltet, neben seiner pädagogischen Tätigkeit als Kompositionslehrer an der Nordwestdeutschen Musikakademie in Detmold, zwischen 1946 und 1971 ein reiches kompositorisches Schaffen. Sein Werk umfaßt geistliche und weltliche Chormusik - A-cappella-Werke, Kantaten, Oratorien, Opern, eine Messe - Liedkompositionen, Kammermusik, Klavier- und Orgelmusik, Orchesterwerke und Symphonien. Johannes Driesslers Werk ist in der geistlichen Musik verwurzelt, er entwickelt eine eigene Tonsprache: Die Gestaltung von Werken aus einer Grundidee, der Bogen, der Ostinato, das Kontrapunktische im Kanon, in der Fuge, in der Passacaglia und eine ungebundene Harmonik sind Elemente seines intellektuellen Kompositionsstils. Johannes Driesslers ureigenes Feld liegt im Vokalbereich. Hier gibt es hervorragende Werke wie zum Beispiel das erste Oratorium Dein Reich komme. Die zyklischen geistlichen Werke durch das Kirchenjahr sind wichtige Bausteine in den Gattungen Orgelmusik und Evangelienspruch im 20. Jahrhundert. Die frühe Kammermusik und die Opern des Komponisten sollten neu entdeckt werden. Auch die didaktisch wertvolle Musik in den Lehrwerken für Schüler und Studierende hat Bestand.
