The aim of this paper is to analyse the effects of recent regulatory reforms that Spanish Health Authorities have implemented in the pharmaceutical market: the introduction of a reference price system together with the promotion of generic drugs. The main objectives of these two reforms are to increase price competition and, ultimately, reduce pharmaceutical costs. Before the introduction of reference prices, consumers had to pay a fixed copayment of the price of whatever drug purchased. With the introduction of such system, the situation differs in the following way: if (s)he buys the more expensive branded drug, then (s)he pays a sum of two elements: the copayment associated to the reference price plus the difference between the price of this good and the reference price. However, if the consumer decides to buy the generic alternative, with price lower than the reference price, then (s)he has to pay the same copayment as before. We show that the introduction of a reference price system together with the promotion of generic drugs increase price competition and lower pharmaceutical costs only if the reference price is set in a certain interval. Also profits for the duopolists might be reduced. These results are due to the opposing effects that reference prices have on branded and generic producers respectively.

The purpose of this paper is to analyze the effect of multimarket contact on the behavior of pharmaceutical firms controlling for different levels of regulatory constraints using IMS MIDAS database. Theoretically, firms that meet in several markets are expected to be capable of sustaining implicitly more profitable out- comes, even if perfect monitoring is not possible. Firms may find it profitable to redistribute their market power among markets where they are operating. We present evidence for nine OECD countries with different degrees of regulation and show that regulation affects the importance of economic forces on firms' price setting behavior. Furthermore, our results confirms the presence of the predictions of the multimarket theory for more market friendly countries (U.S. and Canada) and less regulated ones (U.K., Germany, Netherlands), in contrast, for highly regulated countries (Japan, France, Italy and Spain) the results are less clear with some countries being

Scandals of selective reporting of clinical trial results by pharmaceutical firms have underlined the need for more transparency in clinical trials. We provide a theoretical framework which reproduces incentives for selective reporting and yields three key implications concerning regulation. First, a compulsory clinical trial registry complemented through a voluntary clinical trial results database can implement full transparency (the existence of all trials as well as their results is known). Second, full transparency comes at a price. It has a deterrence effect on the incentives to conduct clinical trials, as it reduces the firms'gains from trials. Third, in principle, a voluntary clinical trial results database without a compulsory registry is a superior regulatory tool; but we provide some qualified support for additional compulsory registries when medical decision-makers cannot anticipate correctly the drug companies' decisions whether to conduct trials. Treball de recerca realitzat per alumnes d’ensenyament secundari i guardonat amb un Premi CIRIT per fomentar l'esperit científic del Jovent l’any 2008. La pastilla anticonceptiva masculina és un producte químicament possible però encara no llençat al mercat. El motiu de la recerca ha estat investigar per comprovar si el disseny d’una campanya de màrqueting tindria èxit en la promoció d’aquest producte. En primer lloc, s’ha dut a terme una recerca teòrica sobre l’anticoncepció i el màrqueting. En segon lloc, s’ha realitzat un treball de camp en dues parts. D’una banda s’han confeccionat enquestes avaluant l’acceptació del producte en la població. De l’altra s’han realitzat entrevistes a professionals: sexòleg-psicòleg, ginecòleg, publicista i farmacèutica.Posteriorment s’ha dissenyat la campanya de màrqueting, gravat l’espot televisiu i dissenyat una pàgina web.

En 1923, Ramón Plá i Armengol (1880-1958) fundó el Instituto Ravetllat-Pla para la comercialización y producción de dos productos antituberculosos (Hemo-Antitoxina y Suero Ravetllat-Pla) fundamentados en una teoría heterodoxa postulada por el veterinario Joaquim Ravetllat i Estech (1871-1923). A través del instituto creó una gran red internacional científico-comercial principalmente en Latinoamérica. Plá i Armengol fue doctor en medicina y participó activamente en la lucha antituberculosa en Cataluña sin dejar de lado su militancia socialista. A través de estos dos productos, logró crear un mercado que se sustentaba en una teoría heterodoxa que integraban sus principios e ideología.

Les drogodependències, la infecció pel VIH-VHC i la tuberculosi, entre d’altres patologies, incideixen actualment de manera molt important en els centres penitenciaris, de manera que han canviat radicalment la sanitat en aquestes institucions en les darreres dues dècades. Les necessitats terapèutiques són diferents i la utilització de fàrmacs a les presons s’ha convertit en més complexa. La població penitenciària ha crescut en els darrers cinc anys més d’un 36% i, d’aquesta manera, ha augmentat la demanda assistencial i en conseqüència la prescripció farmacèutica, amb una corresponent pujada de la despesa en medicaments. Segons la legislació penitenciària general, l’Administració Penitenciària és responsable del control de les malalties transmissibles a les presons, en col·laboració amb l’Administració Sanitària. La gestió dels medicaments, per ser eficaç, ha d’incidir en una sèrie d’elements, com ara la selecció dels medicaments, l’adquisició, la distribució, la dispensació i l’administració, entre d’altres. Els medicaments utilitzats per al tractament de patologies del sistema nerviós són els productes més consumits en el CPHB, amb una major quantitat d’unitats dispensades. Alhora, la despesa d’aquests medicaments suposa aproximadament la meitat de la factura total. La diferència de preus entre especialitats farmacèutiques genèriques i especialitats farmacèutiques de marca evidencia de manera teòrica que la utilització de genèrics pot reduir la despesa total farmacèutica. Els estudis de selecció, adquisició i control de medicaments a centres penitenciaris per part de professionals sanitaris són eines fonamentals i necessàries per a la millora en l’ús de fàrmacs.

Aquest projecte s'ha realitzat al Servei de Microscòpia de la Universitat Autònoma de Barcelona, i ha tingut una durada de dos anys (2007-2009). Els objectius concrets d'aquest projecte assolits per a les diferents assignatures ha estat l'anàlisi de característiques citològiques, característiques morfològiques i anatòmiques de les estructures vegetatives dels diferents nivells d’organització, característiques morfològiques i anatòmiques de les estructures reproductores. El material docent obtingut en aquest projecte està sent utilitzat en les assignatures següents: Botànica Farmacèutica, Biodiversitat vegetal marina, Recursos vegetals aquàtics: les algues, Plantes d'ús alimentari, Conserv. i gestió de poblacions i comunitats marines, Usos i cultius de recursos algals marins, B. i diversitat de criptògames, Biologia II, Tècniques Exp. de biologia II, Botànica, Biologia animal i vegetal. Per a la realització de l’estudi, s'ha obtingut una col·lecció d’imatges al MLC, MER i MET per aclarir els criteris sistemàtics. Durant el primer any s’han adquirit totes les imatges necessàries, per tal de poder elaborar el material didàctic en suport digital. Durant el segon any s’ha implementat aquest material a les respectives classes pràctiques i s’ha avaluat la seva importància en la millora de la formació dels alumnes i del suport al professor. L’aplicació del material elaborat a la docència pràctica durant el tot segon any ha permès fer un primer estudi per valorar la seva eficàcia i fer-hi les correccions oportunes. Durant tot el projecte, el treball conjunt de professors i tècnics ha estat bàsic per a l’obtenció de material didàctic adequat a la realitat dels estudis actuals i a la docència pràctica.

Bone loss secondary to inflammatory bowel diseases (IBD) is largely explained by activated T cells producing cytokines that trigger osteoclastogenesis and accelerate bone resorptionwhile inhibiting bone formation. In IBD, elevated expression of interleukin (IL)-15, a T cell growth factor, plays a central role in T cell activation, pro-inflammatory cytokine production and the development of colitis. We previously reported that IL-15 enhances RANKL-induced osteoclastogenesis and that an IL-15 antagonist, CRB-15, prevents weight and bone loss in a mousemodel of dextran sulfate sodium-induced colitis.We hypothesized that inhibition of IL-15 signalingmight prevent bone loss in IL-10 deficient (IL10−/−) mice, that develop spontaneous bowel inflammation associatedwith osteopeniawhen they are no longer raised under germ-free conditions.Mice received anIL-15 antagonist (CRB-15, 5 μg/day, n=5) or IgG2a (5 μg/day, n=4) fromweek 10 to 14 of age. The severity of colitis was assessed by histology and bowel cytokine gene expression by real time PCR. Bone mass and architecturewere evaluated by ex vivo DXA on femur and micro-computed tomography on femur and vertebra. Bodyweight gainwas similar in the two groups. After 4 weeks, colonwas 29% shorter in CRB-15 treatedmice (p<0.006), a sign of reduced inflammation. Histological analysis indicated a transmural infiltration of inflammatory cells, lymphoepithelial lesions and increased size of villi (histological score=4/6) in IgG2a treated mice, whereas colon from CRB-15 treated mice exhibited mild infiltration of inflammatory cells of the lamina propria, no mucosal damages and a minimal increased size of villi (histological score=1.6/6). Levels of TNFα, IL-17 and IL-6 mRNA in the colon were significantly reduced in CRB-15 treated mice (p<0.04 vs IgG2), indicating a decrease in colon inflammation. CRB-15 improved femur BMD (+10.6% vs IgG2a, p<0.002), vertebral trabecular bone volume fraction (BV/TV, +19.7% vs IgG2a, p<0.05) and thickness (+11.6% vs IgG2a, p<0.02). A modest but not significant increase in trabecular BV/TV was observed at the distal femur. Cortical thicknesswas also higher at themidshaft femur in CRB-15 treatedmice (+8.3% vs IgG2a, p<0.02). In conclusion, we confirm and extend our results about the effects of CRB-15 in colitis. Antagonizing IL-15 may exert favorable effects on intestinal inflammation and prevent bone loss and microarchitecture alterations induced by colitis. Thioredoxins comprise a conserved family of redox regulators involved in many biological processes, including stress resistance and aging. We report that the C. elegans thioredoxin TRX-1 acts in ASJ head sensory neurons as a novel modulator of the insulin-like neuropeptide DAF-28 during dauer formation. We show that increased formation of stress-resistant, long-lived dauer larvae in mutants for the gene encoding the insulin-like neuropeptide DAF-28 requires TRX-1 acting in ASJ neurons, upstream of the insulin-like receptor DAF-2. Genetic rescue experiments demonstrate that redox-independent functions of TRX-1 specifically in ASJ neurons are needed for the dauer formation constitutive (Daf-c) phenotype of daf-28 mutants. GFP reporters of trx-1 and daf-28 show opposing expression patterns in dauers (i.e. trx-1 is up-regulated and daf-28 is down-regulated), an effect that is not observed in growing L2/L3 larvae. In addition, functional TRX-1 is required for the down-regulation of a GFP reporter of daf-28 during dauer formation, a process that is likely subject to DAF-28-mediated feedback regulation. Our findings demonstrate that TRX-1 modulates DAF-28 signaling by contributing to the down-regulation of daf-28 expression during dauer formation. We propose that TRX-1 acts as a fluctuating neuronal signaling modulator within ASJ neurons to monitor the adjustment of neuropeptide expression, including insulin-like proteins, during dauer formation in response to adverse environmental conditions.

BACKGROUND Adherence to interferon β-1b (INFβ-1b) therapy is essential to maximize the beneficial effects of treatment in multiple sclerosis (MS). For that reason, the main objectives of this study are to assess adherence to INFβ-1b in patients suffering from MS in Spain, and to identify the factors responsible for adherence in routine clinical practice. METHODOLOGY/PRINCIPAL FINDINGS This was an observational, retrospective, cross-sectional study including 120 Spanish patients with MS under INFβ-1b treatment. Therapeutic adherence was assessed with Morisky-Green test and with the percentage of doses received. The proportion of adherent patients assessed by Morisky-Green test was 68.3%, being indicative of poor adherence. Nevertheless, the percentage of doses received, which was based on the number of injected medication, was 94.3%. The main reason for missing INFβ-1b injections was forgetting some of the administrations (64%). Therefore, interventions that diminish forgetfulness might have a positive effect in the proportion of adherent patients and in the percentage of doses received. In addition, age and comorbidities had a significant effect in the number of doses injected per month, and should be considered in the management of adherence in MS patients. CONCLUSION/SIGNIFICANCE Among all the available methods for assessing adherence, the overall consumption of the intended dose has to be considered when addressing adherence.

La qualitat d’un producte elaborat és un factor important, tant pels consumidors, com pels òrgans reguladors que en defineixen normatives cada cop més estrictes. Iniciatives com la del PAT (Process Analytical Technology) en el sector farmacèutic, responen a aquestes necessitats. El PAT afavoreix la implantació de noves tècniques analítiques que facilitin el monitoratge i el control de paràmetres clau in-/on-line durant els processos de producció. En aquest sentit, el NIR-CI (Near Infrarred-Chemical Imaging) podria ser una eina molt útil en la millora de la qualitat de la indústria farmacèutica, ja que aprofita les avantatges del NIR com a tècnica analítica (ràpid, no invasiu, no destructiu) i les aplica a tota la superfície espacial de la mostra. És una tècnica capaç de proporcionar una gran quantitat d’informació, tant espectral com espacial, en una sola imatge. L’objectiu d’aquest treball és avaluar la capacitat de la tècnica NIR-CI, com a eina pel control de paràmetres de qualitat de comprimits farmacèutics. Concretament, s’han analitzat quantitativament la concentració i la distribució dels components (principi actiu i excipients) d’un comprimit farmacèutic amb i sense recobriment. A més, també s’ha determinat el gruix de la pel·lícula de laca de recobriment i la seva distribució a la superfície del comprimit. Per obtenir aquesta informació, es parteix d’imatges NIR-CI hiperespectrals dels comprimits. Per a l’extracció de les dades d’interès s’ha usat l’algoritme PLS en les diferents versions dels softwares Isys 5.0 i Unscrambler 9.8. La versió de l’Isys permet determinar la contribució de cada component pur a cada punt de la imatge, emprant únicament l’espectre del component en estudi. Amb la de l’Unscrambler, en canvi, es construeix un model de calibratge que, a partir d’unes mostres de referència, prediu la distribució del gruix de recobriment sobre la superfície del comprimit.

Aims: To investigate the long-term efficacy and safety of denosumab (DMAb) for the treatment of postmenopausal women with osteoporosis in an open-label extension to the 3-year FREEDOM study.1Methods: All women who completed the FREEDOM study were eligible to enter a long-term open-label extension (up to 10 years). After providing informed consent, participants received 6-monthly subcutaneous injections of DMAb (60 mg). Here we report data from the first year of followup. For women randomized to DMAb in the FREEDOM study ('long-term group'), this represents up to 48 months of DMAb exposure (eight 6-monthly injections). For those randomized to placebo ('de novo group') the data are from up to 12 months of exposure (two injections). All participants continued to take calcium (1 g) and vitamin D (≥400 IU) supplements daily. Changes in bone mineral density (BMD) and bone turnover markers (BTM) are reported for subjects enrolled in the extension. No formal statistical testing was planned for this interim report. P-values are descriptive.Results: Overall, 4,550 eligible women (70.2%) who completed the FREEDOM study entered the open-label extension study (long-term, n=2,343; de novo, n=2,207). During the first year of the extension, lumbar spine (LS) BMD in the long-term group further increased by 2.0% (12.1% increase vs. FREEDOM baseline at 48 months), and total hip (TH) BMD further increased by 0.8% (6.5% increase at 48 months) (p<0.0001 for both BMD gains during year 4; Fig. 1). During the first year of the extension, LS and TH BMD increased by 5.4% and 3.0%, respectively in the de novo group (both p<0.0001). After DMAb initiation, serum C-telopeptide (CTX) in the de novo group decreased rapidly and similarly to the long-term group (Fig. 2). Reductions in BTMs continue to attenuate at the end of the dosing interval as previously reported. Adverse event (AE) rates were similar (70.4% of women in the longterm group and 67.9% in the de novo group). Serious Aes were also similar (9.8% and 11.2% of women, respectively). During year 4, osteoporotic nonvertebral fractures were reported in 31 women in the long-term group and 51 in the denovo group.Fig. 1. Neste artigo, pretende-se analisar experiências de desenvolvimento de competências e reconhecimento profissional da pessoa com deficiência já inserida no contexto organizacional, considerando os principais atores envolvidos nesse processo. Trata-se de um estudo de caso qualitativo, realizado em uma multinacional farmacêutica, sediada no estado de São Paulo, em que se teve por objetivo descrever e discutir as práticas formais de desenvolvimento profissional da pessoa com deficiência (PcD) adotadas por essa organização, bem como as experiências informais pelas quais passaram esses indivíduos na empresa em questão. Além disso, buscou-se analisar a articulação entre atores externos que estavam imbricados no processo de inclusão, a saber: o sindicato das indústrias farmacêuticas e uma organização não governamental (ONG) especializada em PcDs. Os resultados problematizam a iniciativa de promover um processo de construção e desenvolvimento de competências por meio de experiências formais e informais, e mesmo de autodesenvolvimento, específicas para a PcD. Nesse sentido, a experiência em estudo revelou que nessa iniciativa prevaleceram as premissas acerca das limitações decorrentes das características biológicas dos indivíduos, já que as práticas de desenvolvimento nem sempre trabalharam sobre o potencial desses profissionais, que pouco avançaram, considerando-se as possibilidades que a organização oferece. Outro resultado relevante é que, embora a articulação entre empresa e agentes externos ocorra, não há um diálogo nem uma parceria efetiva que permita fazer avançar o processo de inclusão e de desenvolvimento desse grupo de profissionais. Ao final, procurou-se apresentar ainda elementos que permitam a reflexão sobre a condução de programas organizacionais voltados à PcD e suas implicações, que podem estar a serviço tanto da promoção como da manutenção do status quo da PcD na corporação.

Behçet's disease (BD) is universally recognized as a multisystemic inflammatory disease of unknown etiology with chronic course and unpredictable exacerbations: its clinical spectrum varies from pure vasculitic manifestations with thrombotic complications to protean inflammatory involvement of multiple organs and tissues. Treatment has been revolutionized by the progressed knowledge in the pathogenetic mechanisms of BD, involving dysfunction and oversecretion of multiple proinflammatory molecules, chiefly tumor necrosis factor- (TNF-) α, interleukin- (IL-) 1β, and IL-6. However, although biological treatment with anti-TNF-α agents has been largely demonstrated to be effective in BD, not all patients are definite responders, and this beneficial response might drop off over time. Therefore, additional therapies for a subset of refractory patients with BD are inevitably needed. Different agents targeting various cytokines and their receptors or cell surface molecules have been studied: the IL-1 receptor has been targeted by anakinra, the IL-1 by canakinumab and gevokizumab, the IL-6 receptor by tocilizumab, the IL12/23 receptor by ustekinumab, and the B-lymphocyte antigen CD-20 by rituximab. The aim of this review is to summarize all current experiences and the most recent evidence regarding these novel approaches with biological drugs other than TNF-α blockers in BD, providing a valuable addition to the actually available therapeutic armamentarium.
