The scientific evidence supporting the management of the chronically ill in a positive psychological perspective in opposition to traditional pathological approach is scarce. This study examines issues associated with recovery of health status in heart failure, in particular hope, affection, and happiness. We use a longitudinal study of 128 symptomatic patients who after medical intervention reported improved quality of life and function at 3-month follow-up. We evaluated the contribution of happiness, hope and affection, individually and as a whole, in the quality of life and functionality of individuals with heart failure. Happiness (Subjective Happiness Scale), Hope (HOPE Scale), and affection (PANAS (positive and negative affect schedule)) were determined before medical intervention. Individually, we found that happiness is correlated with the quality of life and functionality, hope to self-efficacy dimension of the quality of life scale, positive affect to functionality and negative affect with symptoms dimension, quality of life dimension, and overall sum of the quality of life scale. Overall, we found that happiness has a unique contribution to the quality of life, except in self-efficacy dimension where hope takes this contribution and positive affect has a unique contribution to the functionality in this short-term follow-up. The results highlight the importance of positive variables to health outcomes for people with heart failure and should be considered in intervention programs for this syndrome.

COPD is a major cause of morbidity and mortality worldwide, representing a major public health problem due to the high health and economic resource consumption. Pulmonary rehabilitation is a standard care recommendation for these patients, in order to control the symptoms and optimize the functional capacity, reducing health care costs associated with exacerbations and activity limitations and participation. However, in patients with severe COPD exercise performance can be difficult, due to extreme dyspnea, decreased muscle strength and fatigue. In addition, hypoxemia and dyspnea during efforts and daily activities may occur, limiting their quality of life. Thus, NIV have been used as adjunct to exercise, in order to improve exercise capacity in these patients. However, there is no consensus for this technique recommendation. Our objective was to verify whether the use of NIV during exercise is effective than exercise without NIV in dyspnea, walked distance, blood gases and health status in COPD patients, through a systematic review and meta-analysis.

OBJECTIVE To analyze gender differences in the incidence and determinants of disability regarding instrumental activities of daily living among older adults. METHODS The data were extracted from the Saúde, Bem-Estar e Envelhecimento (SABE – Health, Wellbeing and Ageing) study. In 2000, 1,034 older adults without difficulty in regarding instrumental activities of daily living were selected. The following characteristics were evaluated at the baseline: sociodemographic and behavioral variables, health status, falls, fractures, hospitalizations, depressive symptoms, cognition, strength, mobility, balance and perception of vision and hearing. Instrumental activities of daily living such as shopping and managing own money and medication, using transportation and using the telephone were reassessed in 2006, with incident cases of disability considered as the outcome. RESULTS The incidence density of disability in instrumental activities of daily living was 44.7/1,000 person/years for women and 25.2/1,000 person/years for men. The incidence rate ratio between women and men was 1.77 (95%CI 1.75;1.80). After controlling for socioeconomic status and clinical conditions, the incidence rate ratio was 1.81 (95%CI 1.77;1.84), demonstrating that women with chronic disease and greater social vulnerability have a greater incidence density of disability in instrumental activities of daily living. The following were determinants of the incidence of disability: age ≥ 80 and worse perception of hearing in both genders; stroke in men; and being aged 70 to 79 in women. Better cognitive performance was a protective factor in both genders and better balance was a protective factor in women. CONCLUSIONS The higher incidence density of disability in older women remained even after controlling for adverse social and clinical conditions. In addition to age, poorer cognitive performance and conditions that adversely affect communication disable both genders. Acute events, such as a stroke, disables elderly men more, whereas early deficits regarding balance disable women more.

OBJECTIVE To analyze the temporal evolution of maternal mortality and its spatial distribution.METHODS Ecological study with a sample made up of 845 maternal deaths in women between 10 and 49 years, registered from 1999 to 2008 in the state of Rio Grande do Sul, Southern Brazil. Data were obtained from Information System on Mortality of Ministry of Health. The maternal mortality ratio and the specific maternal mortality ratio were calculated from records, and analyzed by the Poisson regression model. In the spatial distribution, three maps of the state were built with the rates in the geographical macro-regions, in 1999, 2003, and 2008.RESULTS There was an increase of 2.0% in the period of ten years (95%CI 1.00;1.04; p = 0.01), with no significant change in the magnitude of the maternal mortality ratio. The Serra macro-region presented the highest maternal mortality ratio (1.15, 95%CI 1.08;1.21; p < 0.001). Most deaths in Rio Grande do Sul were of white women over 40 years, with a lower level of education. The time of delivery/abortion and postpartum are times of increased maternal risk, with a greater negative impact of direct causes such as hypertension and bleeding.CONCLUSIONS The lack of improvement in maternal mortality ratio indicates that public policies had no impact on women’s reproductive and maternal health. It is needed to qualify the attention to women’s health, especially in the prenatal period, seeking to identify and prevent risk factors, as a strategy of reducing maternal death.

OBJECTIVE To analyze conditional and unconditional healthy life expectancy among older Brazilian women.METHODS This cross-sectional study used the intercensal technique to estimate, in the absence of longitudinal data, healthy life expectancy that is conditional and unconditional on the individual’s current health status. The data used were obtained from the Pesquisa Nacional por Amostra de Domicílios (National Household Sample Survey) of 1998, 2003, and 2008. This sample comprised 11,171; 13,694; and 16,259 women aged 65 years or more, respectively. Complete mortality tables from the Brazilian Institute of Geography and Statistics for the years 2001 and 2006 were also used. The definition of health status was based on the difficulty in performing activities of daily living.RESULTS The remaining lifetime was strongly dependent on the current health status of the older women. Between 1998 and 2003, the amount of time lived with disability for healthy women at age 65 was 9.8%. This percentage increased to 66.2% when the women already presented some disability at age 65. Temporal analysis showed that the active life expectancy of the women at age 65 increased between 1998-2003 (19.3 years) and 2003-2008 (19.4 years). However, life years gained have been mainly focused on the unhealthy state.CONCLUSIONS Analysis of conditional and unconditional life expectancy indicated that live years gained are a result of the decline of mortality in unhealthy states. This pattern suggests that there has been no reduction in morbidity among older women in Brazil between 1998 and 2008.

OBJECTIVE To estimate the degree of educational inequality in the occurrence of abdominal obesity in a population of non-faculty civil servants at university campi.METHODS In this cross-sectional study, we used data from 3,117 subjects of both genders aged 24 to 65-years old, regarding the baseline ofPró-Saúde Study, 1999-2001. Abdominal obesity was defined according to abdominal circumference thresholds of 88 cm for women and 102 cm for men. A multi-dimensional, self-administered questionnaire was used to evaluate education levels and demographic variables. Slope and relative indices of inequality, and Chi-squared test for linear trend were used in the data analysis. All analyses were stratified by genders, and the indices of inequality were standardized by age.RESULTS Abdominal obesity was the most prevalent among women (43.5%; 95%CI 41.2;45.9), as compared to men (24.3%; 95%CI 22.1;26.7), in all educational strata and age ranges. The association between education levels and abdominal obesity was an inverse one among women (p < 0.001); it was not statistically significant among men (p = 0.436). The educational inequality regarding abdominal obesity in the female population, in absolute terms (slope index of inequality), was 24.0% (95%CI 15.5;32.6). In relative terms (relative index of inequality), it was 2.8 (95%CI 1.9;4.1), after the age adjustment.CONCLUSIONS Gender inequality in the prevalence of abdominal obesity increases with older age and lower education. The slope and relative indices of inequality summarize the strictly monotonous trend between education levels and abdominal obesity, and it described educational inequality regarding abdominal obesity among women. Such indices provide relevant quantitative estimates for monitoring abdominal obesity and dealing with health inequalities.

Resumo - A presente tese explora o contributo de uma abordagem de Promoção da Saúde para o desenvolvimento de uma política integrada relativa ao envelhecimento e às pessoas idosas. Propôs-se, em particular, reunir elementos de apoio à fundamentação de medidas e políticas nacionais promotoras da saúde e bem-estar das pessoas de 65 e mais anos em Portugal. ● Delineia-se o enquadramento conceptual - referencial PromS - que clarifica a perspectiva de Promoção de Saúde adoptada. Um entendimento abrangente, positivo e multideterminado da saúde, a par do ênfase nos valores de equidade e de empowerment são alguns dos traços centrais desta abordagem. ● Conjugam-se dados empíricos quantitativos e qualitativos, concorrendo para o diagnóstico da situação de saúde da população de 65+ anos em Portugal: estudo qualitativo explorando as concepções leigas de saúde de pessoas idosas, discutidas em termos de literacia de saúde e de dimensões, determinantes e modos de acção sobre a saúde valorizados; perfil de saúde da população portuguesa de 65+ anos, caracterizando o seu estado de saúde/ bem-estar e factores (individuais e sociais/ambientais) que o influenciam; recorre a indicadores de diversas fontes, incluindo, quando possível, uma dimensão comparativa com outros grupos etários e outros países europeus; análise do padrão e magnitude de desigualdades sociais em resultados e determinantes de saúde das pessoas idosas em Portugal (dados do ESS3); breve análise de medidas/políticas nacionais relevantes para a saúde deste grupo populacional. ● Sugerem-se objectivos e áreas prioritárias para a actuação, bem como algumas estratégias e aspectos do dispositivo de intervenção a contemplar na formulação e implementação de uma política nacional de saúde dos idosos.----------------------------Abstract - The thesis explores contributions of a health promotion approach to the development of an integrated ageing policy. More specifically, it offers several elements in support of policies/measures promoting the health and well-being of people aged 65+ in Portugal. ● A conceptual framework - PromS - clarifies the health promotion approach adopted, stressing a comprehensive and positive understanding of health and its multiple determinants and emphasising the values of equity and empowerment. ● Quantitative and qualitative data are combined to render an assessment of the health situation of the over-65 population in Portugal, comprising: a qualitative study exploring older people’s lay views on health, discussed in terms of health literacy and favoured health dimensions, determinants and actions; a health profile of the Portuguese population aged 65 and over, covering health status and well-being and several determining factors (individual and social /environmental); it uses indicators from several sources, including, whenever possible, a comparison with other age groups and other European countries; an analysis of pattern and magnitude of social inequalities in health outcomes and in the distribution of some of it’s determinants among elderly people in Portugal (ESS3 data); a brief review of some national policies/measures pertinent to this group’s health. ● Objectives and priority areas for action are suggested, along with possible strategies and guidelines on infrastructure and processes regarding the formulation and implementation of a national health policy for older people.

RESUMO - O aumento da incidência das doenças crónicas representa um desafio enorme para todos os Sistemas de Saúde, pelo que a assistência de Saúde a doentes crónicos se tornou num problema das sociedades ocidentais. Os países mais pobres são os que mais sofrem, embora nos países desenvolvidos também se verifique um aumento notável das doenças crónicas. Estima-se que estas representem mais de 60% do total das doenças mundiais no ano 2020 (WHO, 2001). A adaptação dos actuais modelos de saúde aos doentes crónicos não atingiu os objectivos, o que conduziu a que, de há alguns anos a esta parte, se procure alternativas mais eficazes e eficientes. Uma das pressões do mercado que se fazem sentir será caracterizada por uma maior aposta na Promoção da Saúde e Prevenção da doença. O conceito de “Saúde” migrou de “não existência de doença” para “Bem-estar Físico e Psicológico”. Desta forma, o foco dos cuidados de saúde teve que ser adaptado, o que conduziu a uma situação em que o âmbito da prestação de cuidados de saúde é um contínuo de serviços que vai desde a promoção da saúde, medicina preventiva e medicina curativa aos cuidados continuados e cuidados paliativos. As tecnologias de informação e comunicação desempenharão um importante papel nesta tendência, permitindo estabelecer ligações contínuas entre os consumidores e prestadores de cuidados de saúde. Por outro lado, as potencialidades da Internet, das comunicações móveis, dispositivos portáteis e do instrumental electrónico, tornam-se evidentes no desenvolvimento de serviços de e-Saúde: para monitorização, seguimento e controlo dos doentes extra hospitalar - serviços estes centrados no doente. O objectivo geral do presente estudo consiste no desenho de um projecto de investigação para posterior avaliação da percepção do estado de saúde dos doentes seguidos na consulta de hipocoagulação do Hospital de Santa Marta. Devido à escassez de investigação na temática deste trabalho em Portugal, procedeu-se a um trabalho exploratório, descritivo, de carácter comparativo e enquadrado na abordagem quantitativa. O campo de análise consiste em comparar doentes que fazem anticoagulação oral, seguidos na consulta de cardiologia (consulta convencional), com os doentes seguidos no programa Airmed (através das comunicações móveis). 4 Para avaliação da percepção do estado de saúde foi utilizado o questionário SF-36.----ABSTRACT - The increasing incidence of chronic diseases represents an enormous challenge to the Health Systems and on cause of that, the Health Assistance to chronic patients became a concern of the Occidental society. The Countries with lower economical resources are the ones that suffers the most, but also the Developed countries have a noticeable increase of chronic diseases. It is estimated these will represent over 60% of total diseases world wide in 2020 (WHO,2001). The adaptation of the actual Health Models to chronic patients did not achieved it’s goals, what leaded to look for more effective and efficient alternatives. One of the more sensitive market pressure factor is to look for a better Health Promotion and Disease Prevention. The concept of “Health” merged from “Disease absence” to “Physic and Psychic Wellness”. In this way the Health Care focus had to be adapted, what drove to a status where the scope of the Health care is a continuum of services that goes from the Health Promotion, Preventive and Curative to Continued and Palliative Medical Care. The Information and Communication Technologies will play a crucial role in this trend, allowing to establish continued connections between patients and Health Care providers. In parallel the potential of the Internet, mobile communications, portable devices and electronic instruments became evident to deploy e-Health services: to monitor, follow-up and control of patients outside the Hospital. The overall objective of the present study is an Investigation Project Design to further evaluate the health status perception of the patients followed in the consultation for Hypocoagulation in the “Hospital de Santa Marta”. Due to lack of investigation in this thematic, in Portugal, this study is developed in an exploratory way, descriptive, comparative, within a scope of a quantified approach. An integrated chemical-biological effects monitoring was performed in 2010 and 2012 in two NW Iberian estuaries under different anthropogenic pressure. One is low impacted and the other is contaminated by metals. The aim was to verify the usefulness of a multibiomarker approach, using Carcinus maenas as bioindicator species, to reflect diminishing environmental contamination and improved health status under abiotic variation. Sampling sites were assessed for metal levels in sediments and C. maenas, water abiotic factors and biomarkers (neurotoxicity, energy metabolism, biotransformation, anti-oxidant defences, oxidative damage). High inter-annual and seasonal abiotic variation was observed. Metal levels in sediments and crab tissues were markedly higher in 2010 than in 2012 in the contaminated estuary. Biomarkers indicated differences between the study sites and seasons and an improvement of effects measured in C. maenas from the polluted estuary in 2012. Integrated Biomarker Response (IBR) index depicted sites with higher stress levels whereas Principal Component Analysis (PCA) showed associations between biomarker responses and environmental variables. The multibiomarker approach and integrated assessments proved to be useful to the early diagnosis of remediation measures in impacted sites.

RESUMO - Enquadramento: O envelhecimento da população ocorre em todas as sociedades desenvolvidas, resultando num aumento da prevalência da dependência funcional, associado recorrentemente à presença de doenças crónicas. Estes novos padrões demográficos, epidemiológicos, implicando populações vulneráveis com necessidades específicas, resultam em desafios incontestáveis. Como resposta a este novo paradigma, em 2006, Portugal implementa a Rede Nacional de Cuidados Continuados Integrados (RNCCI). Finalidade/objectivos: Caracterização da população com base no perfil das necessidades auto-referidas pelas pessoas com ≥65 anos, com algum nível de independência/dependência nas actividades de vida diária e/ou com pelo menos uma doença crónica. Pretende-se, ainda, desenvolver uma metodologia que permita simular cenários que contribuam para o planeamento do número de camas para internamento de carácter permanente em Unidades de Longa Duração e Manutenção (ULDM) da RNCCI. Metodologia: Construção de dois indicadores: índice de independência/dependência e existência ou não de doenças crónicas. Análise estatística e caracterização, individual e conjunta, das variáveis sociodemográficas, socioeconómicas, auto-avaliação do estado de saúde, nível de independência/dependência e/ou existência de pelo menos uma doença crónica. Simulação de cenários com base nas metas definidas pela RNCCI para 2013. Resultados e Conclusões: Da aplicação do índice de independência/dependência, resulta que 78,8% são independentes na realização das actividades de vida diária e 21,2% apresentam algum nível de dependência. À excepção do Centro, todas as regiões apresentam padrões similares. Globalmente, os resultados obtidos vão de encontro aos enunciados na literatura internacional, realçando-se apenas alguns mais pertinentes: Observa-se uma predominância de mulheres idosas. Destaca-se também uma relação directa entre a idade e os níveis de dependência. As variáveis socioeconómicas indicam que a existência de algum nível de dependência tende a ser mais frequente entre os que têm menor escolaridade e rendimento. Em média o estado de saúde é auto-avaliado como mau, piorando com o aumento da idade e níveis de dependência mais acentuados e melhorando com o aumento da escolaridade. Da simulação de cenários destaca-se que, face às 4 camas previstas nas metas de 2013, seria de alocar em média 1,7 camas ou 1 cama ao internamento permanente em ULDM. Trabalhar em rede implica canais de comunicação. A incorporação da distribuição espacial das necessidades e serviços com recurso aos sistemas de informação geográfica torna-se numa mais-valia. Possibilita avaliar hipóteses, análises sustentadas e disseminação de informação e resultados, contribuindo para um planeamento, monitorização e avaliação mais eficaz e eficiente das actividades do sector da saúde. ---------------------------------- ABSTRACT - Background: Population aging occurs in all developed societies resulting in an increased prevalence of functional dependence, frequently associated with the presence of chronic diseases. These new demographic and epidemiological patterns, which include dependency ad vulnerability situations, with specific needs, result in undeniable challenges. In response to this new paradigm, in 2006, Portugal implements the National Network for Integrated Care (RNCCI). Aim/Objectives: Characterize the population based on the self-reported needs of ≥65 year’s people, with some level of independence/dependency in activities of daily living and/or with at least one chronic disease. Also intends to develop a methodological approach that allows scenarios simulation which contributes to the planning of the number of permanent inpatient beds in Long Term Care Units (ULDM) of RNCCI. Methods: Construction of two indicators: independence/dependence index and existence of chronic diseases. Statistical analysis and characterization, individually and jointly, of sociodemographics, socioeconomics, selfassessment of health status, level of independence/dependence and/or existence of at least one chronic disease variables. Scenarios simulation based on RNCCI targets set for 2013. Results and Conclusions: According with independence/dependence index, 78.8% are independent in carrying out the activities of daily living and 21.2% have some level of dependency. With the exception of the Centroregion, all regions have similar patterns. Generally, the results are concordant with international literature, highlighting here only some of the most relevant results: A predominance of older women is observed. A direct relationship between age and levels of dependence is emphasized. Socio-economic variables indicate that the existence of some level of dependency tends to be more frequent among those with lower income and education levels. On average, health status is self-assessed as poor, being even more critical with aging and higher dependency level. On the other hand, high education levels are related with better health status. Scenarios simulations highlights that, based on 4 beds considered in the 2013 planned goals, an average of 1.7 or 1 beds in ULDM should be allocated to permanent inpatient beds. Networking involves communication channels. The incorporation of spatial distribution of needs and services using geographical information systems becomes an added value. It enables hypothesis, evaluation, sustainable analysis and information and results dissemination, contributing to a more effective and efficient planning, monitoring and assessment of the health sector activities.

RESUMO - Em Portugal, as doenças cardiovasculares (DCV), incluindo o acidente vascular cerebral (AVC) e a doença cardíaca isquémica (DCI), são das principais causas de morbi-mortalidade e invalidez. Sabe-se que o nível socioeconómico (NSE) influencia o estado de saúde, todavia são escassas as evidências sobre as desigualdades socioeconómicas na DCV em Portugal. O objectivo deste estudo foi analisar a distribuição da DCV de acordo com o NSE na população portuguesa. Foi realizado um estudo transversal exploratório-descritivo usando a base de dados do 4º Inquérito Nacional de Saúde, 2005/06. As desigualdades socioeconómicas nas DCV, AVC e DCI, factores de risco [sedentarismo, hipertensão arterial (HTA), diabetes mellitus (DM), tabagismo, obesidade e sofrimento psicológico (Mental Health Inventory ≤ 52)] e número de consultas médicas, foram analisadas através dos odds ratio por NSE (rendimento familiar equivalente, escala modificada da OCDE) com intervalo de confiança de 95% e dos índices e curvas de concentração. Dos 21 807 indivíduos, 53,34% são do sexo feminino, a idade média é de 54±11 e entre 35 e 74 anos. A DCV, a DCI, o AVC, a HTA, a DM e a obesidade estão associados com NSE mais baixos, o tabagismo está associado aos NSE mais elevados, enquanto o sedentarismo, o número de consultas médicas e o sofrimento psicológico não apresentam associação significativa com o NSE. Os resultados revelam a associação entre os estilos de vida, morbilidade e NSE e demonstram que são necessárias políticas de saúde mais abrangentes, de acordo com as características individuais, culturais e socioeconómicas e dirigidas à promoção da saúde e prevenção da doença. -------------------------------------------- ABSTRACT - Cardiovascular diseases (CVD), including stroke and ischemic heart disease (IHD), are the leading causes of morbidity, mortality and disability in Portugal. It is known that socioeconomic status (SES) influences health status; however there is little evidence about socioeconomic inequalities in CVD in Portugal. The aim of this study was to analyze the distribution of CVD according to SES in the Portuguese population. We conducted a cross-sectional descriptive exploratory study using the database of the 4th National Survey of Health, 2005/06. Socioeconomic inequalities in CVD, stroke, IHD, risk factors [physical inactivity, arterial hypertension (AHT), diabetes mellitus (DM), smoking, obesity and psychological distress (Mental Health Inventory ≤ 52)], as well as the number of medical visits, were analyzed by SES (family income using the OECD modified equivalent scale) using odds ratio (confidence interval = 95%), and concentration curves and indices. Of the 21 807 individuals, 53.34% are female, aged between 35 and 74 with mean 54 ± 11 years. CVD, IHD, stroke, AHT, MD and obesity are associated with lower SES, smoking is associated with higher SES, while physical inactivity, number of medical visits and psychological distress showed no significant association with SES. Results suggest an association between lifestyle, morbidity and SES. They also demonstrate the need for comprehensive health strategies, involving health promotion and disease prevention, that incorporate individual, cultural and socioeconomic characteristics.

RESUMO: Objectivo: O exercício tem sido identificado como um factor de promoção da qualidade de vida em mulheres submetidas a cirurgia por cancro de mama. Mas os níveis de actividade física tendem a reduzir após o diagnóstico de cancro de mama e a manterem-se baixos após fim dos tratamentos. O objectivo deste estudo é verificar se um programa de exercício físico supervisionado, associado a estratégias motivacionais em mulheres submetidas a cirurgia por cancro da mama e que mantêm a intervenção usual praticada, é mais efectivo no aumento dos níveis de actividade física, na redução do índice de massa corporal e na melhoria da qualidade de vida, do que o tratamento usualmente praticado. Metodologia: Trata-se de um estudo piloto experimental, aleatorizado e controlado. Cumpriram os critérios de inclusão no estudo 37 utentes submetidas a cirurgia por cancro de mama no Hospital Fernando Fonseca, Amadora, e submetidos à intervenção usual da fisioterapia. Foram aleatoriamente distribuídas pelos grupos experimental (n=19) e de controlo (n=18). O contacto telefónico foi feito por um elemento externo, cego em relação à distribuição dos sujeitos. Alguns elementos desistiram ou não puderam participar no estudo. O número de sujeitos final foi de 11 para o grupo experimental e de 10 para o grupo de controlo, idênticos na média de idades. Todos os sujeitos mantiveram a intervenção usual da fisioterapia. Os sujeitos do grupo experimental foram ainda submetidos ao programa de exercício e promoção da actividade física. Todo o grupo foi avaliado no início do programa, no final do mesmo (12 semanas) e após 6 meses de follow-up, nos seguintes indicadores: nível de actividade física, pelo International Physical Activity Questionnaire (IPAQ), índice de massa corporal e qualidade de vida, pelos questionários: European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) e Supplementary Questionnaire Breast Cancer Module (QLQ-BR23). Resultados: O programa implementado demonstrou-se efectivo no aumento dos níveis de actividade física (p=0,017) e na qualidade de vida associada ao estado funcional (p=0,016) e ao estado de saúde global auto-percepcionado (p=0,030) no final do programa. Foi ainda possível identificar resultados positivos noutras variáveis estudadas, como por exemplo a fadiga auto-reportada, mas que não se demonstraram estatisticamente significativos, facto que pode atribuir-se à reduzida dimensão da amostra. Conclusões: Apesar de algumas limitações, este estudo é de grande a importância para a comunidade científica preocupada com crescente problemática do cancro de mama. Mais uma vez se demonstra que o exercício físico é uma estratégia importante para a melhoria do estado de saúde de pacientes confrontados com doença. Esta comprovação é de grande interesse para os profissionais de saúde e particularmente para os fisioterapeutas dedicados à promoção da saúde nesta população.---------------------ABSTRACT: Purpose: Exercise has been identified as a quality of life promoting factor, amongst women after breast cancer surgery. But the physical activity levels reduce significantly after a diagnosis of breast cancer and remain low after treatment is completed. The aim of this study is to determine whether a supervised group exercise program allied to motivating strategies in women after breast cancer surgery in conjunction with standard treatment, is more effective in improving physical activity levels, reducing body mass index and promoting quality of life, than standard treatment on its own. Methods: This is a pilot randomized controlled trial. Thirty-seven women, submitted to breast cancer surgery in the Hospital Fernando Fonseca, Amadora, and submitted to standard physiotherapy intervention, completed the inclusion criteria. They were randomly allocated into intervention group (n=19) and control group (n=18). The telephone contact was done by an external element, blind to the subjects’ allocation. Some elements declined or could not participate. The final intervention group was n=11 and the control n=10, similar in age. All subjects maintained the standard physiotherapy intervention. The experimental group was submitted to the exercise and health promotion program. The complete group was evaluated in the beginning of the program, at the final (after 12 weeks) and after 6 months follow-up, using as outcomes: physical activity index with the international Physical Activity Questionnaire (IPAQ), the body mass index and quality of life using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and Supplementary Questionnaire Breast Cancer Module (QLQBR23). Results: The studied program demonstrated to be effective in improving the physical activity index (p=0,017) and quality of live related to physical functioning (p=0,016) and to global health status (p=0,030) at the end of the program. It was possible to find positive results in some other outcomes, such as fatigue, although the differences were not statistically significant. We believe that these results can be attributed to the small sample size. Conclusions: Although we can identify some methodological constrains, this is a very important study for scientific community working on the breast cancer subject. Once more, exercise was identified as an important strategy to improve global heath status in breast cancer patients. This represents an important contribution to the health professionals and mostly for physiotherapists working on health promotion subject.

RESUMO - Os sistemas de saúde deparam-se, actualmente, com novos paradigmas, ao nível da qualidade da prestação de cuidados de saúde, bem como no que se refere à necessidade de controlo dos custos com o sector da saúde, obrigando as organizações a adaptarem-se por forma a fornecerem a resposta mais adequada às crescentes necessidades dos indivíduos. O reconhecimento desta realidade tem levado os governos de muitos países a definir políticas orientadas para problemas de saúde específicos e a adoptar estratégias de intervenção que privilegiam uma abordagem integrada com o objectivo de melhorarem progressivamente o nível de saúde das populações, a qualidade dos cuidados prestados e a eficiência na utilização de recursos. Em Portugal, a aplicação dos princípios que estiveram na base dos modelos de gestão da doença, deu origem ao modelo experimental de Gestão Integrada da Doença, que incorpora a gestão clínica da doença, centrada no doente, com especial enfoque na sua autogestão e na clarificação das melhores práticas profissionais, visando a sua uniformização; a reorganização do modelo de prestação de cuidados, com a criação de Centros de Elevada Diferenciação e Centros de Tratamento, com especiais preocupações no que concerne à orientação do doente no sistema para que os cuidados lhe seja ministrados no nível mais adequado; um modelo de financiamento específico, indexado aos resultados, que reflicta a adopção das melhores práticas; um sistema de informação que permita a monitorização e avaliação constante deste processo. O desenvolvimento deste modelo organizacional tem-se revelado como uma estratégia inovadora e como uma ferramenta de elevado potencial para a melhoria da prestação de cuidados de saúde e para a promoção de uma maior efectividade e eficiência, tal como poderá, ainda, constituirse como um veículo, importante e permanente, de informação de apoio à decisão em Saúde. Este modelo visa, no fundo, promover uma acção concertada no sentido da obtenção de uma intervenção precisa, através da mobilização de recursos adequados, que permitam uma melhoria do estado de saúde, da qualidade de vida e do bem-estar global dos doentes. Esta abordagem passa pela colaboração e coordenação dos diferentes níveis de prestação de cuidados, no sentido de oferecerem cuidados integrados de saúde, com níveis de qualidade elevados em termos de prevenção, diagnóstico, tratamento, reabilitação e acompanhamento. ------------------------ --ABSTRACT – The health systems are faced with new paradigms, on one hand in the healthcare services delivered to the populations, and on the other hand, in the need to control costs in the health sector, forcing organizations to adapt and provide the most appropriate response to the individuals growing needs. The magnitude of this problem, in terms of public health, requires the adoption of a directed, targeted, planned and integrated action, based on clear and well defined strategies in order to obtain health gains, improving the quality of care and streamlining the costs. In Portugal, the application of those principles forming the basis of the disease management models, led to the Integrated Disease Management model which, apart from the clinical management of the disease, also incorporates the healthcare delivery structure reorganization, a specific financing model based on an information system that allows the process monitoring and evaluation. The development of Integrated Disease Management models is a central strategy and a tool for improving healthcare delivery, more effectively and efficiently, and can even be an important and permanent vehicle of information for health decision support. Therefore, it is important to promote a concerted action towards achieving a precise intervention, mobilizing the resources, improving the health status, quality of life and the overall patients’ wellbeing. Sertraline is widely prescribed worldwide and frequently detected in aquatic systems. There is, however, a remarkable gap of information on its potential impact on estuarine and coastal invertebrates. This study investigated sertraline accumulation and effects in Carcinus maenas. Crabs from a moderately contaminated (Lima) and a low-impacted (Minho) estuary were exposed to environmental and high levels of sertraline (0.05, 5, 500 μg L−1). A battery of biomarkers related to sertraline mode of action was employed to assess neurotransmission, energy metabolism, biotransformation and oxidative stress pathways. After a seven-day exposure, sertraline accumulation in crabs’ soft tissues was found in Lima (5 μg L−1: 15.3 ng L−1 ww; 500 μg L−1: 1010 ng L−1 ww) and Minho (500 μg L−1: 605 ng L−1 ww) animals. Lima crabs were also more sensitive to sertraline than those from Minho, exhibiting decreased acetylcholinesterase activity, indicative of ventilatory and locomotory dysfunction, inhibition of anti-oxidant enzymes and increased oxidative damage at ≥0.05 μg L−1. The Integrated Biomarker Response (IBR) index indicated their low health status. In addition, Minho crabs showed non-monotonic responses of acetylcholinesterase suggestive of hormesis. The results pointed an influence of the exposure history on differential sensitivity to sertraline and the need to perform evaluations with site-specific ecological receptors to increase relevance of risk estimations when extrapolating from laboratory to field conditions.
