Enjeux et contexte La recherche de cette dernière décennie sur les acides gras n-3 PUFA contenus dans l'huile de poisson a montré que ceux-ci, et particulièrement l'ΕΡΑ et le DHA, avaient des propriétés anti¬inflammatoires et anti arythmiques puissantes, potentiellement utiles chez les septiques et « cardiaques ». Les mécanismes sous-jacents sont nombreux, incluant l'incorporation des acides gras dans les membranes de phopholipides, la réduction de la production de médiateurs pro-inflammatoires (prostaglandines, leukotrienes, thromboxane), l'augmentation de la production de résolvines et protectines dérivées du DHA, et la régulation de voies de signalisation cellulaire. Cependant, les doses de n-3 PUFA utilisées dans les études cliniques et chez le sujet sain avant le travail de Yann-Karim Pittet étaient nettement supérieures aux doses nutritionnelles de l'ordre de 5-8 g/j par voie orale ou 1 g/kg par voie intraveineuse. De plus, la voie entérale avait la réputation de nécessiter plusieurs jours à semaines de traitement avant d'aboutir à une incorporation d'acides gras membranaire suffisante pour avoir un impact clinique; quant au temps minimal requis pour obtenir cet effet par voie IV, il était inconnu. Depuis, le développement d'émulsions lipidiques intraveineuses destinées à la nutrition parentérale a permis d'imaginer l'administration de prétraitements IV rapides. Pour les étudier, notre laboratoire a développé un modèle d'endotoxine (LPS d'E.Coli) qui mime les réponses physiologique, endocrinienne et biologique du sepsis chez le sujet sain, utilisant des doses de 2 ng/kg IV. Les réponses sont totalement réversibles en 8 heures. Dans le but de réduire à la fois la dose de lipides et le temps de perfusion, ce travail a étudié l'influence de 3 doses dégressives de n-3 PUFA sur les réponses à l'endotoxine, et sur l'incorporation membranaire de ces acides gras. Méthodes Etude prospective chez 3 groupes consécutifs de sujets sains soumis à un challenge d'endotoxine. Intervention : perfusions d'huile de poisson (0.5 et 0.2 g/kg de n-3 PUFA, Omegaven® 10%) ou placebo, administrées en 3 heures ou en 1 heure, soit le jour avant ou le jour-même du test d'endotoxine. Mesures : variables physiologiques (T°, fc, tension artérielle, calorimétrie indirecte) Laboratoire - prises de sang à T0, 60, 120 et 360 min après l'injection de LPS: TNF-α, hs-CRP, hormones de stress, composition en acides gras des membranes plaquettaires. Statistiques Les résultats ont été rapportés en moyennes et écarts types. Des aires sous la courbe (AUC) ont été calculées avec la méthode des parallélépipèdes pour toutes les variables déterminées de manière répétée. L'effet du temps a été exploré par des two-way ANOVA pour mesures répétées. Les comparaisons post-hoc ont été réalisées avec des tests de Dunnett's ou de Scheffe. Les modifications de composition membranaires ainsi que les AUC ont été analysées par des tests non-paramétriques (Kruskal-Wallis). Résultats Après LPS, la température, les concentrations d'ACTH et TNF-α ont augmenté dans les 3 groupes. Ces réponse ont été significativement atténuées (p<0.0001) par l'huile de poisson comparé à ce que nous avions observé dans le groupe contrôle de Pluess et al (ICM 2007). Les concentrations les plus faibles d'ACTH, de TNF-α, et les AUC les plus basses des températures, ont été observées après une dose unique de 0.2 g/kg de n-3 PUFA administrée 1 heure avant le LPS. Par contre, l'incorporation membranaire d'EPA est dose-dépendante. Conclusions Sachant que la réponse à l'endotoxine est reproductible, cette étude montre que 3 doses différentes d'huile de poisson atténuent de manière différente cette réponse. La perfusion de 0.2 g/kg administrée juste avant l'endotoxine s'est avérée la plus efficace à atténuer la réponse fébrile, les cytokines et les hormones de stress, suggérant une capture de l'endotoxine par l'émulsion lipidique qui se surajoute aux effets systémiques et membranaires.

Objective: To assess the associations between obesity markers (BMI, waist circumference and %body fat) and inflammatory markers (interleukin-1β (IL-1β); interleukin-6 (IL-6); tumor necrosis factor-α (TNF-α) and high-sensitivity C-reactive protein (hs-CRP)). Methods: Population sample of 2,884 men and 3,201 women aged 35-75 years. Associations were assessed using ridge regression adjusting for age, leisure-time physical activity, and smoking. Results: No differences were found in IL-1β levels between participants with increased obesity markers and healthy counterparts; multivariate regression showed %body fat to be negatively associated with IL-1β. Participants with high %body fat or abdominal obesity had higher IL-6 levels, but no independent association between IL-6 levels and obesity markers was found on multivariate regression. Participants with abdominal obesity had higher TNF-α levels, and positive associations were found between TNF-α levels and waist circumference in men and between TNF-α levels and BMI in women. Obese participants had higher hs-CRP levels, and these differences persisted after multivariate adjustment; similarly, positive associations were found between hs-CRP levels and all obesity markers studied. Conclusion: Obesity markers are differentially associated with cytokine levels. %Body fat is negatively associated with IL-1β; BMI (in women) and waist circumference (in men) are associated with TNF-α; all obesity markers are positively associated with hs-CRP. 1. Few examples of habitat-modelling studies of rare and endangered species exist in the literature, although from a conservation perspective predicting their distribution would prove particularly useful. Paucity of data and lack of valid absences are the probable reasons for this shortcoming. Analytic solutions to accommodate the lack of absence include the ecological niche factor analysis (ENFA) and the use of generalized linear models (GLM) with simulated pseudo-absences. 2. In this study we tested a new approach to generating pseudo-absences, based on a preliminary ENFA habitat suitability (HS) map, for the endangered species Eryngium alpinum. This method of generating pseudo-absences was compared with two others: (i) use of a GLM with pseudo-absences generated totally at random, and (ii) use of an ENFA only. 3. The influence of two different spatial resolutions (i.e. grain) was also assessed for tackling the dilemma of quality (grain) vs. quantity (number of occurrences). Each combination of the three above-mentioned methods with the two grains generated a distinct HS map. 4. Four evaluation measures were used for comparing these HS maps: total deviance explained, best kappa, Gini coefficient and minimal predicted area (MPA). The last is a new evaluation criterion proposed in this study. 5. Results showed that (i) GLM models using ENFA-weighted pseudo-absence provide better results, except for the MPA value, and that (ii) quality (spatial resolution and locational accuracy) of the data appears to be more important than quantity (number of occurrences). Furthermore, the proposed MPA value is suggested as a useful measure of model evaluation when used to complement classical statistical measures. 6. Synthesis and applications. We suggest that the use of ENFA-weighted pseudo-absence is a possible way to enhance the quality of GLM-based potential distribution maps and that data quality (i.e. spatial resolution) prevails over quantity (i.e. number of data). Increased accuracy of potential distribution maps could help to define better suitable areas for species protection and reintroduction.

O objetivo deste trabalho foi simular o aparecimento de folhas em genótipos de arroz cultivados e em biótipos de arroz-vermelho por meio da adaptação do modelo de Wang e Engel, modificado por Streck et al. Dois experimentos foram conduzidos em Santa Maria, RS, em 2004/2005 e 2005/2006. Foram utilizadas as cultivares IRGA 417 e EEA 406, um híbrido e dois biótipos de arroz-vermelho. O modelo de aparecimento de folhas foi ajustado para os genótipos usando-se dados de estágio de Haun (HS), de cinco épocas de semeadura, em 2004/2005, e a avaliação do modelo foi feita com dados de HS medidos em três épocas de semeadura, em 2005/2006. Realizou-se também um experimento numérico utilizando-se dados de temperatura de 2003/2004 e de 2006/2007, e comparou-se o HS simulado dos genótipos cultivados em relação aos biótipos de arroz-vermelho. O modelo apresentou bom desempenho para simular o HS com raiz do quadrado médio do erro geralmente menor do que uma folha. A emissão de folhas na haste principal foi maior nos genótipos modernos - IRGA 417 e híbrido - do que no genótipo tradicional EEA 406. A emissão de folhas é diferente em distintos biótipos de arroz-vermelho.

O objetivo deste trabalho foi avaliar o padrão espacial da variabilidade genética entre plantas, dentro de três populações naturais de barueiro (Dipteryx alata Vogel), pela genotipagem por RAPD e técnicas de autocorrelação espacial. Os cinco iniciadores RAPD permitiram a codificação de 45 locos, utilizados nas análises de diversidade, estrutura e distribuição espacial da variabilidade genética entre populações. As populações apresentaram diversidade genética (Hs) com valor médio 0,314. Verificou-se que 12% da variação total se encontra entre as populações, o que indica que estas mantêm um considerável nível de variabilidade genética. Foi observada tendência de autocorrelação espacial positiva nas primeiras classes de distâncias, nas três populações, o que indica a formação de grupos de vizinhança com estruturação familiar, dentro das populações de barueiro. Entretanto, o tamanho desses grupos de vizinhança varia entre as populações; isso mostra que outros processos ecológicos influenciaram a distribuição espacial da variabilidade genética. As populações naturais de barueiro apresentam consideráveis níveis de diversidade genética, com base nos 45 locos RAPD avaliados.

BACKGROUND: Particulate air pollution is associated with increased risk of cardiovascular disease and stroke. Although the precise mechanisms underlying this association are still unclear, the induction of systemic inflammation following particle inhalation represents a plausible mechanistic pathway.¦METHODS: We used baseline data from the CoLaus Study including 6183 adult participants residing in Lausanne, Switzerland. We analyzed the association of short-term exposure to PM10 (on the day of examination visit) with continuous circulating serum levels of high-sensitive C-reactive protein (hs-CRP), interleukin 1-beta (IL-1β), interleukin 6 (IL-6), and tumor-necrosis-factor alpha (TNF-α) by robust linear regressions, controlling for potential confounding factors and assessing effect modification.¦RESULTS: In adjusted analyses, for every 10 μg/m3 elevation in PM10, IL-1ß increased by 0.034 (95 % confidence interval, 0.007-0.060) pg/mL, IL-6 by 0.036 (0.015-0.057) pg/mL, and TNF-α by 0.024 (0.013-0.035) pg/mL, whereas no significant association was found with hs-CRP levels.¦CONCLUSIONS: Short-term exposure to PM10 was positively associated with higher levels of circulating IL-1ß, IL-6 and TNF-α in the adult general population. This positive association suggests a link between air pollution and cardiovascular risk, although further studies are needed to clarify the mechanistic pathway linking PM10 to cardiovascular risk.

Polyclonal intravenous immunoglobulin (IVIg) treatment reduces crossmatch positivity and increases rates of transplantation in highly sensitised patients (HS). We quantified the panel reactive antibody (PRA) by microlymphocytotoxicity (MLCC), and we analysed anti-HLA class I and class II IgG specific antibody repertoire by Luminex before and after IVIg infusion alone in HS patients awaiting kidney transplantation. Five patients received three monthly infusions of 1 g/kg of IVIg. Serum samples collected pre and post IVIg treatment were submitted for PRA analysis by MLCC. Anti-class I and anti-class II antibody specificities were then tested by Luminex. We focused on the anti-HLA class I and class II antibodies directed against HLA expressed by a previous graft. We also analysed the anti-HLA antibody repertoire in three patients who had not received IVIg infusion. The PRA level determined by MLCC decreased significantly in one of the five patients, dropping from 40% to 17%. The Luminex assay showed fluctuations of the anti-HLA antibody levels over time, but no significant longterm modifications of the anti-HLA antibody repertoire were observed, even in the patient with a strong and prolonged reduction of the PRA determined by MLCC. Our results show that IVIg at 1 g/kg is not sufficient to reduce PRA and does not modify the repertoire of specific anti-HLA antibody determined by Luminex.

Head space gas chromatography with flame-ionization detection (HS-GC-FID), ancl purge and trap gas chromatography-mass spectrometry (P&T-GC-MS) have been used to determine methyl-tert-butyl ether (MTBE) and benzene, toluene, and the ylenes (BTEX) in groundwater. In the work discussed in this paper measures of quality, e.g. recovery (94-111%), precision (4.6 - 12.2%), limits of detection (0.3 - 5.7 I~g L 1 for HS and 0.001 I~g L 1 for PT), and robust-ness, for both methods were compared. In addition, for purposes of comparison, groundwater samples from areas suffering from odor problems because of fuel spillage and tank leakage were analyzed by use of both techniques. For high concentration levels there was good correlation between results from both methods.

Aim: The obesity epidemic has increased the number of obese patients admitted to the ICU. In vitro studies suggest that adipose tissue response to inflammation is enhanced: in vivo data are not conclusive yet. The aim of this study was to test the physiologic response of healthy obese subjects to a standardized intravenous LPS challenge.Methods: Prospective single-blind, randomized, cross-over study in eight subjects (four men, four women), aged 34 +/- 7 years, BMI 34.7 +/- 4.2, without glucose intolerance and lipid abnormalities, testing the impact of intravenous LPS (2 ng kg(-1) of actual body weight) versus placebo.Results: Temperature, hemodynamic variables, indirect calorimetry and blood samples (TNF-alpha, IL-6, stress hormones, hs-CRP) were collected. After LPS temperature, heart rate. TNF-alpha and IL-6 concentrations and stress hormones (cortisol and glucagon) increased significantly, with maximal responses between 120 and 240 min after the injection. The pattern, the timing and the magnitude of change were similar to those observed in lean subjects.Conclusion: This study shows that healthy obese subjects have a similar response pattern to intravenous LPS as described in lean subjects.

O objetivo deste trabalho foi avaliar o desempenho de métodos de estimativa da evapotranspiração de referência (ETo), em diferentes condições de nebulosidade, no Município de Seropédica, RJ. As estimativas de ETo, entre 1/6/2006 e 31/7/2007, foram comparadas com medidas diárias realizadas em lisímetro de pesagem, com agrupamento total dos dados e com a discretização pela variação do índice de claridade diário K T em quatro classes: K T<0,35, nublado (Nub); 0,35HS) apresentaram coeficientes de desempenho de 84,05 e 79,52%, respectivamente, enquanto os métodos Jensen & Haise, Linacre e Makking apresentaram desempenho inferior a 60%. A variação das condições de nebulosidade influenciaram o desempenho dos métodos de estimativa de ETo. Os melhores resultados foram obtidos pelos métodos da radiação solar (86,1%) e de Camargo (81,8%), nas condições Ab e Nub. Os métodos PMF e HS podem ser empregados de forma complementar, pois apresentaram desempenhos de 78,4% (Nub e PNAd) e de 77,6% (PNAb e Ab), respectivamente.

OBJECTIVE: To assess the association between socioeconomic status (SES) and inflammatory markers using two different European population samples. METHODS: We used data from the CoLaus (N=6412, Lausanne, Switzerland) and EPIPorto (N=1205, Porto, Portugal) studies. Education and occupational position were used as indicators of socioeconomic status (SES). High-sensitivity C-reactive protein (hs-CRP) was available for both cohorts. Interleukin-6 (IL-6) and tumour necrosis factor-α (TNF-α) were available in CoLaus; leukocyte count and fibrinogen in EPIPorto. RESULTS: We showed that low SES was significantly associated with high inflammation in both studies. We also showed that behavioural factors contributed the most to SES differences in inflammation. In both studies the larger difference between the lowest and the highest SES was observed for hs-CRP. In the Swiss sample, a linear association between education and hs-CRP persisted after adjustment for all mediating factors and confounders considered (p for linear trend <0.001). CONCLUSION: Large social differences exist in inflammatory activity, in part independently from demographic and behavioural factors, chronic conditions and medication use. SES differences in inflammation are also similar in countries with different underlying socioeconomic conditions.

Introduction: Recent data have suggested that a population of CD4+ CD25high T cells, phenotypically characterized by the expression of CD45RO and CD127, is significantly expanded in stable liver and kidney transplant recipients and represents alloreactive T cells. Induction therapies may have an impact on this alloreactive T cell population. In this study, we prospectively analyzed CD4+ CD25high CD45RO+ CD127high T cells after induction with either thymoglobulin or basiliximab. Patients & methods: A total of twenty-seven kidney transplant recipients were prospectively enrolled; 14 received thymoglobulin induction followed by a 4-day course of steroids with tacrolimus and mycophenolate mofetil ("thymo group"), and 13 received basiliximab induction followed by standard triple immunosuppression (tacrolimus, mycophenolate mofetil and prednisone) ("BSX group"). Phenotypical analysis by flow cytometry of the expression of CD25, CD45RO and CD127 on peripheral CD4+ T cells was performed at 0, 3 and 6 months after transplantation. Twenty-four healthy subjects (HS) were studied as controls. Results: There were no differences in baseline characteristics between the groups; at 6 months, patient survival (100%), graft survival (100%), serum creatinine (thymo versus BSX group: 129 versus 125 μmol/l) and acute rejection (2/14 versus 2/13) were not significantly different. Thymo induction produced a strong CD4 T cell depletion. As compared to pre-transplantation values, an expansion of the alloreactive T cell population was observed at 3 months in both thymo (mean: from 6.38% to 14.72%) and BSX (from 8.01% to 18.42%) groups. At 6 months, the alloreactive T cell population remained significantly expanded in the thymo group (16.92 ± 2.87%) whereas it tended to decrease in the BSX group (10.22 ± 1.38%). Conclusion: Overall, our results indicate that the expansion of alloreactive T cells occurs rapidly after transplantation in patients receiving either thymo or BSX induction. Whether differences at later timepoints or whether different IS regimens may modify this alloreactive population remains to be studied.

OBJECTIVE: To evaluate the public health impact of statin prescribing strategies based on the Justification for the Use of Statins in Primary Prevention: An Intervention Trial Evaluating Rosuvastatin Study (JUPITER). METHODS: We studied 2268 adults aged 35-75 without cardiovascular disease in a population-based study in Switzerland in 2003-2006. We assessed the eligibility for statins according to the Adult Treatment Panel III (ATPIII) guidelines, and by adding "strict" (hs-CRP≥2.0mg/L and LDL-cholesterol <3.4mmol/L), and "extended" (hs-CRP≥2.0mg/L alone) JUPITER-like criteria. We estimated the proportion of CHD deaths potentially prevented over 10years in the Swiss population. RESULTS: Fifteen % were already taking statins, 42% were eligible by ATPIII guidelines, 53% by adding "strict", and 62% by adding "extended" criteria, with a total of 19% newly eligible. The number needed to treat with statins to avoid one CHD death over 10years was 38 for ATPIII, 84 for "strict" and 92 for "extended" JUPITER-like criteria. ATPIII would prevent 17% of CHD deaths, compared with 20% for ATPIII+"strict" and 23% for ATPIII + "extended" criteria (+6%). CONCLUSION: Implementing JUPITER-like strategies would make statin prescribing for primary prevention more common and less efficient than it is with current guidelines.

The aim of our study was to provide an innovative HS-GC/MS method applicable to the routine determination of butane concentration in forensic toxicology laboratories. The main drawback of the GC/MS methods discussed in literature concerning butane measurement was the absence of a specific butane internal standard necessary to perform quantification. Because no stable isotope of butane is commercially available, it is essential to develop a new approach by an in situ generation of standards. To avoid the manipulation of a stable isotope-labelled gas, we have chosen to generate in situ an internal labelled standard gas (C(4)H(9)D) following the basis of the stoichiometric formation of butane by the reaction of deuterated water (D(2)O) with Grignard reagent butylmagnesium chloride (C(4)H(9)MgCl). This method allows a precise measurement of butane concentration and therefore, a full validation by accuracy profile was presented.

We review some of the most influential papers from 2012 in the different aspects of emergency medicine, such as prehospital medicine, resuscitation, early diagnosis and timely ED discharge and treatment. In particular, intramuscular benzodiazepines have been shown to be efficient in prehospital status epilepticus, epinephrines usefulness in cardiopulmonary resuscitation has been challenged, colloids have been shown to be deleterious in the treatment of severe sepsis and septic shock, the time window for thrombolysis in acute stroke will probably be extended, acute pyelonephritis treatment duration can be decreased, new D-dimers thresholds for older patients may prevent further diagnosis tests, and hs-Troponin may allow earlier discharge of low coronary risk patients.
