A novel acceptor substrate for galactosyltransferase was synthesized containing GlcNAcalpha-pyrophosphate, covalently bound to a hydrophobic phenoxyundecyl moiety (GlcNAc alpha-O-PO(3)-PO(3)-(CH(2))(11)-O-Phenyl). The new substrate was used to develop an assay for a galactosyltransferase activity from Escherichia coli strain VW187 that is involved in lipopolysaccharide synthesis and has not been studied by others. We showed that Gal was transferred from UDP-Gal to the novel acceptor substrate. This was a significant improvement over our previous preliminary assays of the enzyme using endogenous substrate, and showed that these synthetic substrates are useful for assaying enzymes that utilize lipid-bound substrates in O-chain synthesis in Gram-negative bacteria.

In this work, we demonstrate that the wbbD gene of the O7 lipopolysaccharide (LPS) biosynthesis cluster in Escherichia coli strain VW187 (O7:K1) encodes a galactosyltransferase involved in the synthesis of the O7-polysaccharide repeating unit. The galactosyltransferase catalyzed the transfer of Gal from UDP-Gal to the GlcNAc residue of a GlcNAc-pyrophosphate-lipid acceptor. A mutant strain with a defective wbbD gene was unable to form O7 LPS and lacked this specific galactosyltransferase activity. The normal phenotype was restored by complementing the mutant with the cloned wbbD gene. To characterize the WbbD galactosyltransferase, we used a novel acceptor substrate containing GlcNAcalpha-pyrophosphate covalently bound to a hydrophobic phenoxyundecyl moiety (GlcNAc alpha-O-PO(3)-PO(3)-(CH(2))(11)-O-phenyl). The WbbD galactosyltransferase had optimal activity at pH 7 in the presence of 2.5 mM MnCl(2). Detergents in the assay did not increase glycosyl transfer. Digestion of enzyme product by highly purified bovine testicular beta-galactosidase demonstrated a beta-linkage. Cleavage of product by pyrophosphatase and phosphatase, followed by HPLC and NMR analyses, revealed a disaccharide with the structure Gal beta1-3GlcNAc. Our results conclusively demonstrate that WbbD is a UDP-Gal: GlcNAcalpha-pyrophosphate-R beta1,3-galactosyltransferase and suggest that the novel synthetic glycolipid acceptor may be generally applicable to characterize other bacterial glycosyltransferases.

Objective To investigate the association between periodontitis and mortality from all causes in a prospective study in a homogenous group of 60- to 70-year-old West European men. Methodology A representative sample of 1400 dentate men, (mean age 63.8, SD 3.0 years), drawn from the population of Northern Ireland, had a comprehensive periodontal examination between 2001 and 2003. Men were divided into thirds on the basis of their mean periodontal attachment loss (PAL). The primary endpoint, death from any cause, was analysed using Kaplan-Meier survival plots and Cox's proportional hazards model. Results In total, 152 (10.9%) of the men died during a mean follow-up of 8.9 (SD 0.7) years; 37 (7.9%) men in the third with the lowest PAL (<1.8 mm) died compared with 73 (15.7%) in the third with the highest PAL (>2.6 mm). The unadjusted hazard ratio (HR) for death in the men with the highest level of PAL compared with those with the lowest PAL was 2.11 (95% CI 1.42-3.14), p < 0.0001. After adjustment for confounding variables (age, smoking, hypertension, BMI, diabetes, cholesterol, education, marital status and previous history of a cardiovascular event) the HR was 1.57 (1.04-2.36), p = 0.03. Conclusion The European men in this prospective cohort study with the most severe loss of periodontal attachment were at an increased risk of death compared with those with the lowest loss of periodontal attachment.

Objective: The incidence of oesophageal adenocarcinoma (EAC) has increased rapidly over the past 40 years and accumulating evidence suggests that obesity, as measured by body mass index (BMI), is a major risk factor. It remains unclear whether abdominal obesity is associated with EAC and gastric adenocarcinoma.
Design: Cox proportional hazards regression was used to examine associations between overall and abdominal obesity with EAC and gastric adenocarcinoma among 218 854 participants in the prospective NIHeAARP cohort.
Results: 253 incident EAC, 191 gastric cardia adenocarcinomas and 125 gastric non-cardia adenocarcinomas accrued to the cohort. Overall obesity (BMI) was positively associated with EAC and gastric
cardia adenocarcinoma risk (highest ($35 kg/m2) vs referent (18.5e<25 kg/m2); HR 2.11, 95% CI 1.09 to 4.09 and HR 3.67, 95% CI 2.00 to 6.71, respectively). Waist circumference was also positively associated with EAC and gastric cardia adenocarcinoma risk (highest vs referent; HR 2.01, 95% CI 1.35 to 3.00 and HR 2.22, 95% CI 1.43 to 3.47, respectively), whereas waist-to-hip ratio (WHR) was positively associated with EAC risk only (highest vs referent; HR 1.81, 95% CI 1.24 to 2.64) and persisted in patients with normal BMI (18.5e<25 kg/m2). Mutual adjustment of WHR and BMI attenuated
both, but did not eliminate the positive associations for either with risk of EAC. In contrast, the majority of the anthropometric variables were not associated with adenocarcinomas of the gastric non-cardia.
Conclusion Overall obesity was associated with a higher risk of EAC and gastric cardia adenocarcinoma, whereas abdominal obesity was found to be associated with increased EAC risk; even in people with normal BMI

Importance: This article provides, to our knowledge, the first longitudinal population-based data on refractive error (RE) in Chinese persons.
Objective: To study cohort effects and changes associated with aging in REs among Chinese adults.
Design, Setting, and Participants: A 2-year, longitudinal population-based cohort study was conducted in southern China. Participants, identified using cluster random sampling, included residents of Yuexiu District, Guangzhou, China, aged 35 years or older who had undergone no previous eye surgery.
Methods: Participants underwent noncycloplegic automated refraction and keratometry in December 2008 and December 2010; in a random 50% sample of the participants, anterior segment ocular coherence tomography measurement of lens thickness, as well as measurement of axial length and anterior chamber depth by partial coherence laser interferometry, were performed.
Main Outcomes and Measures: Two-year change in spherical equivalent refraction (RE), lens thickness, axial length, and anterior chamber depth in the right eye.
Results: A total of 745 individuals underwent biometric testing in both 2008 and 2010 (2008 mean [SD] age, 52.2 [11.5] years; 53.7% women). Mean RE showed a 2-year hyperopic shift from −0.44 (2.21) to −0.31 (2.26) diopters (D) (difference, +0.13; 95% CI, 0.11 to 0.16). A consistent 2-year hyperopic shift of 0.09 to 0.22 D was observed among participants aged 35 to 64 years when stratifying by decade, suggesting that a substantial change in RE with aging may occur during this 30-year period. Cross-sectionally, RE increased only in the cohort younger than 50 years (0.11 D/y; 95% CI, 0.06 to 0.16). In the cross-sectional data, axial length decreased at −0.06 mm/y (95% CI, −0.09 to −0.04), although the 2-year change in axial length was positive and thus could not explain the cross-sectional difference. These latter results suggest a cohort effect, with greater myopia developing among younger persons.
Conclusions and Relevance: This first Chinese population-based longitudinal study of RE provides evidence for both important longitudinal aging changes and cohort effects, most notably greater myopia prevalence among younger persons.

A recent phase 2 study of metastatic colorectal carcinoma (CRC) patients showed that mismatch repair gene status was predictive of clinical response to PD-1-targeting immune checkpoint blockade. Further examination revealed strong correlation between PD-L1 protein expression and microsatellite instability (MSI) in stage IV CRC, suggesting that the amount of PD-L1 protein expression could identify late stage patients who may benefit from immunotherapy. To assess whether the clinical associations between PD-L1 gene expression and MSI identified in metastatic CRC are also present in stage II/III CRC, we used in silico analysis to elucidate the cell types expressing the PD-L1 gene. We found a significant association of PD-L1 gene expression with MSI in early stage CRC (P < 0.001) and show that unlike in non-CRC tumors, PD-L1 is derived predominantly from the immune infiltrate. We demonstrate that PD-L1 gene expression has positive prognostic value in the adjuvant disease setting (PD-L1low v PD-L1high HR = 9.09; CI, 2.11-39.10). PD-L1 gene expression had predictive value, as patients with high PD-L1 expression appear to be harmed by standard-of-care treatment (HR = 4.95; CI,1.10-22.35). Building on the promising results from the metastatic CRC PD-1-targeting trial, we provide compelling evidence that PD-L1high/MSI/immunehigh stage II/III CRC patients should not receive standard chemotherapy. This conclusion supports the rationale to clinically evaluate this patient subgroup for PD-1 blockade treatment.

In der vorliegenden Arbeit, deren erster und zweiter Teil in den vorangegangenen Heften dieser Zeitschrift veröffentlicht wurden, werden die Kosten der “Weichkäserei” am Beispiel der Herstellung von Camembert- und Brie-Käse modellhaft bestimmt. Damit werden die im Jahr 1970 in gleicher Zeitschrift (Heft 5, Band 22) veröffentlichten Analysen der Produktionskosten in Camembertkäsereien hinsichtlich der Funktionsinhalte ausgedehnt und dem neuesten Stand der Technik sowie heutigen Produktionsstrukturen im Weichkäsesektor angepaßt. Gleichzeitig sind die generell für alle Modellabteilungen geltenden methodischen Weiterentwicklungen (2) in den Kalkulationen berücksichtigt worden. In den sechs Unterabteilungen Vorstapelung, Bruchbereitung/Portionierung, Umhorden/Salzen, Reifung, Abpackung und Fertiglager werden aus der Produktgruppe Weichkäse die Sorten Camembert mit 30 und 60 % F.i.Tr. sowie Brie mit 45 % F.i.Tr. in unterschiedlichen Stückgrößen hergestellt und hinsichtlich ihrer Kostenverursachung untersucht. Zur Bestimmung der Modellkosten wurden vier Modelle gebildet, deren Kapazitäten in der Kesselmilchverarbeitung zwischen 8.000 und 30.000 l/h liegen. In Abhängigkeit vom Beschäftigungsgrad, der für Werte zwischen 15 und 100 % simuliert wurde, können so die Kosten für Käsemengen zwischen rd. 700-17.0001 Käse/Jahr bestimmt werden.Nach den vorgegebenen Kapazitäten sind die technischen Voraussetzungen der einzelnen Unterabteilungen modellspezifisch festgelegt worden, wobei die technische Auslegung an eine verringerte Auslastung bei 65 und 33%iger Beschäftigung angepaßt wurde. Die zu tätigenden Investitionen für die Grundversion betragen 25,0 Mio DM im Modell 1 und 54,5 Mio DM im Modell 4. Bezogen auf die jeweilige Outputmenge an Käse ergeben sich hieraus spezifische Investitionen, die sich mit zunehmender Modellgröße von 5.125 auf 3.205 DM/t jährliche Käsemenge erheblich senken. Produktspezifische Investitionen und Faktormengenverbräuche führen zu den Einzelkosten der ausgewählten Weichkäseprodukte, die z.B. für den Camembert mit 60 % F.i.Tr. je nach Modellgrößeund Beschäftigungsgrad zwischen 500,7 Pf/kg und 620,2 Pf/kg Käse liegen. Die Gesamtkosten der Abteilung “Weichkäserei”, die sich aus den Einzelkosten der Produkte und den Einzelkosten der Abteilung zusammensetzen, betragen im größten Modell bei 100%iger Beschäftigung 522,1 Pf/kg Käse, die sich im kleinsten Modell bei nur 15%iger Beschäftigung auf 1.027,6 Pf/kg Käse erhöhen. Bei einem Beschäftigungsgrad von 65 %, dem die Produktionsmenge eines 2-Schichtbetriebes zugeordnet ist, entfallen von den Gesamtkosten der Abteilung je nach Modellgröße 62-72 % auf die Rohstoffkosten, 14-20 % auf die Anlagekosten und 4-7 % auf die Personalkosten, während die übrigen Kostenartengruppen nur von geringerer Bedeutung sind. Betrachtet man die Gesamtkosten (ohne Rohstoffkosten) hinsichtlich ihrer Entstehung in den Unterabteilungen, so ist festzustellen, daß die Unterabteilungen Bruchbereitung/Portionierung sowie Abpackung die höchsten Kosten verursachen. Für einen Beschäftigungsgrad von 65 % betragen die Kosten in den Unterabteilungen Bruchbereitung/Portionierung im Modell 1 85,8 Pf/kg, die mit zunehmender Modellgröße auf 45,9 Pf/kg im Modell 4 zurückgehen. Die Kosten für die Abpackung liegen im Modell 1 bei 76,2 Pf/kg, während sie im Modell 4 nur noch 58,3 Pf/kg betragen. Die geringsten Kosten verursacht die Unterabteilung Fertiglager mit 2,6 Pf/kg im kleinsten und 1,5 Pf/kg im größten Modell bei 65%iger Beschäftigung. Die Ergebnisse der Modellabteilungsrechnung lassen erkennen, daß mit zunehmender Kapazitätsgröße und ansteigendem Beschäftigungsgrad erhebliche Stückkostendegressionen zu erreichen sind, die bei betriebsindividuellen oder branchenbedingten Entscheidungen genutzt werden sollten. So empfiehlt es sich, daß auf eine dem Markt angepaßte Produktion auch eine auf die geplante Tagesproduktionsmenge angepaßte Ausstattung der Abteilung folgt, da sich, wie in den Modellkostenkurven bei 65 und 33 % Beschäftigung dargestellt, die Kosten sprunghaft senken können. Kostendegressionseffekte werden auch erreicht, wenn durch Spezialisierung der Stückkäse-Produktion die Vielfalt der Formatgrößen eingeschränkt werden kann. Das höchste Kosteneinsparungspotential ist aber durch Strukturveränderungen im Weichkäsesektor zu erwarten, die, wie an zwei Beispielen erläutert, der Branche langfristige Kosteneinsparungen von rd.25 bzw. 95 Mio DM/Jahr ermöglichen können.

This programme of research used a developmental psychopathology approach to investigate females across the adolescent period. A two-sided story is presented; first, a study of neuroendocrine and psychosocial parameters in a group of healthy female adolescents (N = 63), followed by a parallel study of female adolescents with anorexia nervosa (AN) (N = 8). A biopsychosocial, multi-method measurement approach was taken, which utilised self-report, interview and hypothalamic-pituitary-adrenocortical (HPA) axis measures. Saliva samples for the measurement of cortisol and DHEA were collected using the best-recommended methodology: multiple samples over the day, strict reference to time of awakening, and two consecutive sampling weekdays. The research was adolescent-orientated: specifically, by using creative and ageappropriate strategies to ensure participant adherence to protocol, as well as more generally by adopting various procedures to facilitate engagement with the research process. In the healthy females mean (± SD) age 13.9 (± 2.7) years, cortisol and DHEA secretion exhibited typical adult-like diurnal patterns. Developmental markers of chronological age, menarche status and body mass index (BMI) had differential associations with cortisol and DHEA secretory activity. The pattern of the cortisol awakening response (CAR) was sensitive to whether participants had experienced first menses, but not to chronological age or BMI. Those who were post-menarche generally reached their peak point of cortisol secretion at 45 minutes post-awakening, in contrast to the pre-menarche group who were more evenly spread. Subsequent daytime cortisol levels were also higher in post-menarche females, and this effect was also noted for increasing age and BMI. Both morning and evening DHEA were positively associated with developmental markers. None of the situational or self-report psychosocial variables that were measured modulated any of the key findings regarding cortisol and DHEA secretion. The healthy group of girls were within age-appropriate norms for all the self-report measures used, however just under half of this group were insecurely attached (as assessed by interview). Only attachment style was associated with neuroendocrine parameters. In particular, those with an anxious insecure style exhibited a higher awakening sample (levels were 7.16 nmol/l, 10.40 nmol/l and 7.93 nmol/l for secure, anxious and avoidant groups, respectively) and a flatter CAR (mean increases over the awakening period were 6.38 nmol/l, 2.32 nmol/l and 8.61 nmol/l for secure, anxious and avoidant groups, respectively). The afore-mentioned pattern is similar to that consistently associated with psychological disorder in adults, and so this may be a pre-clinical vulnerability factor for subsequent mental health problems. A group of females with AN, mean (± SD) age 15.1 (± 1.6) years, were recruited from a specialist residential clinic and compared to the above group of healthy control (HC) female adolescents. A general picture of cortisol and DHEA hypersecretion was revealed in those with AN. The mean (± SD) change exhibited in cortisol levels over the 30 minute post-awakening period was 7.05 nmol/l (± 5.99) and 8.33 nmol/l (± 6.41) for HC and AN groups, respectively. The mean (± SD) evening cortisol level for the HC girls was 1.95 nmol/l (± 2.11), in comparison to 6.42 nmol/l (± 11.10) for the AN group. Mean (± SD) morning DHEA concentrations were 1.47 nmol/l (± 0.85) and 2.25 nmol/l (± 0.88) for HC and AN groups, respectively. The HC group’s mean (± SD) concentration of 12 hour DHEA was 0.55 nmol/l (± 0.46) and the AN group’s mean level was 0.89 nmol/l (± 0.90). This adrenal steroid hypersecretion evidenced by the AN group was not associated with BMI or eating disorder symptomatology. Insecure attachment characterised by fearfulness and anger was most apparent; a style which was unparalleled in the healthy group of female adolescents. The causal directions of the AN group findings remain unclear. Examining some of the participants with AN as case studies one year post-discharge from the clinic illustrated that for one participant who was recovered, in terms of returning to ordinary school life and no longer exhibiting clinical levels of eating disorder symptomatology, her CARs were no longer inconsistent over sampling days and her DHEA levels were also now generally comparable to the healthy control group. For another participant who had not recovered from her AN one year later, the profile of her CAR continued to be inconsistent over sampling days and her DHEA concentrations over the diurnal period were significantly higher in comparison to the healthy control group. In its entirety, this work’s unique contribution lies in its consideration of methodological and developmental issues specifically pertaining to adolescents. Findings also contribute to knowledge of AN and understanding of vulnerability factors, and how these may be used to develop interventions dedicated to improving adolescent health.

In this thesis, a feed-forward, back-propagating Artificial Neural Network using the gradient descent algorithm is developed to forecast the directional movement of daily returns for WTI, gold and copper futures. Out-of-sample back-test results vary, with some predictive abilities for copper futures but none for either WTI or gold. The best statistically significant hit rate achieved was 57% for copper with an absolute return Sharpe Ratio of 1.25 and a benchmarked Information Ratio of 2.11.

Introduction:¦L'incidence annuelle du cancer de l'ovaire en Suisse est de 600 cas, il touche principalement les femmes âgées de plus de 60 ans. Le cancer de l'ovaire est aujourd'hui la 1ère cause de mortalité par cancer gynécologique chez la femme. Le but de notre recherche, est de créer une base de données de toutes les¦patientes atteintes d'un cancer de l'ovaire et hospitalisées au CHUV pour la prise en charge de leur maladie.¦Cette étude rétrospective monocentrique nous permettra en premier lieu d'analyser les caractéristiques de¦cette tumeur présentées par les patientes, les traitements instaurés pour traiter ce cancer et les taux de¦récidives et de survie des patientes en fonction de ces variables.¦Méthodologie:¦Analyse rétrospective de 147 patientes diagnostiquées d'un cancer de l'ovaire et hospitalisées au CHUV entre¦septembre 2001 et mars 2010 pour la prise en charge de leur tumeur ovarienne. Utilisation du programme informatique ARCHIMED qui contient les dossiers médicaux de toutes les patientes inclues dans l'étude et création de 2 bases des données. La 1ère base de données regroupe l'ensemble des patientes de l'étude y¦compris les tumeurs type borderline, la 2ème base de données concerne uniquement les patientes ayant récidivé de leur tumeur. Les tumeurs bénignes ont été exclues de l'étude.¦Résultats:¦La probabilitéde survie à 1 an chez les patientes avec un cancer de l'ovaire, tous stades FIGO et prises en¦charge confondus, hospitalisées au CHUV est de 88,04% (95% CI = 0.7981-208;0.9306), à 3 ans la probabilité de survie est de 70,4% (95% CI = 0.5864-208;0.7936), et à plus de 5 ans, elle est de 60% (95% CI = 0.4315-208;0.6859).¦Nous avons comparé le taux de survie en fonction du stade FIGO de la tumeur ovarienne et nous avons observé une différence significative de survie entre les stades FIGO précoces et les stades avancés (Pvaleur=¦0.0161).¦En plus d'une intervention chirurgicale, les patientes atteintes d'un cancer de l'ovaire sont normalement traitées par une chimiothérapie. Dans notre étude, 70 patientes ont bénéficié d'une chimiothérapie; un¦traitement adjuvant a été donné dans 78 % des cas (N = 55), un traitement néoadjuvant a été administré chez 22% des patientes (N=15). Le type combiné carboplatine-208;taxane est la chimiothérapie la plus fréquente (75%). Au total sur l'étude, 66 patientes sur les 147 (44%) ont récidivé de leur tumeur. En ce qui¦concerne leur prise en charge, 46% des patientes ont reçu une chimiothérapie unique comme du gemzar, cealyx ou taxotère après leur récidive. Une cytoréduction secondaire a également été effectuée chez 33% de ces patientes ayant une récidive. Nous avons également étudié l'intervalle de temps entre la date de la¦récidive et celle du décès. Parmi les 28 patientes décédées chez les récidives, 10 d'entre-208;elles (36%) ont survécu moins d'un an une fois la récidive diagnostiquée, 8 (28%) patientes ont survécu jusqu'à 2 ans, et¦les 10 (36%) autres patientes ont survécu de 2 à 5 ans. En ce qui concerne le taux de mortalité; 39 patientes sur les 147 étudiées sont décédées pendant la période d'observation, soit 26% des cas. La tumeur¦type borderline, présente une prolifération épithéliale atypique sans invasion dans le stroma et représente¦10 à 20% de toutes les tumeurs ovariennes. Dans notre étude, 41 patientes sont porteuses de cette tumeur¦(28%) et la moyenne d'âge est de 49 ans. En ce qui concerne leur prise en charge, l'intervention chirurgicale¦la plus fréquente, soit 23% des cas, est l'annexectomie unilatérale, qui reste une attitude conservative pour¦ces patientes désirant préserver leur fertilité. 6 patientes présentant ce type de tumeur ont récidivé, soit 14% des cas, avec une progression pelvienne, et 3 de ces 6 patientes sont décédées. Dans notre analyse, on observe que la probabilité de vivre plus longtemps que 1an pour les patientes ayant une tumeur borderline est de 93,8% (95% CI= 0.6323-208; 0.9910), à 3 ans elle est également de 93,8% (95% CI = 0.6323-208;0.9910) et à 5 ans elle est de 78,1% (95% CI = 0.3171-208;0.9483). Nous n'avons pas observé de¦différence de survie dans notre étude entre les patientes présentant une tumeur borderline et le « non208;borderline ». (Pvaleur=0.3301)

BACKGROUND: Lipid-lowering therapy is costly but effective at reducing coronary heart disease (CHD) risk. OBJECTIVE: To assess the cost-effectiveness and public health impact of Adult Treatment Panel III (ATP III) guidelines and compare with a range of risk- and age-based alternative strategies. DESIGN: The CHD Policy Model, a Markov-type cost-effectiveness model. DATA SOURCES: National surveys (1999 to 2004), vital statistics (2000), the Framingham Heart Study (1948 to 2000), other published data, and a direct survey of statin costs (2008). TARGET POPULATION: U.S. population age 35 to 85 years. Time Horizon: 2010 to 2040. PERSPECTIVE: Health care system. INTERVENTION: Lowering of low-density lipoprotein cholesterol with HMG-CoA reductase inhibitors (statins). OUTCOME MEASURE: Incremental cost-effectiveness. RESULTS OF BASE-CASE ANALYSIS: Full adherence to ATP III primary prevention guidelines would require starting (9.7 million) or intensifying (1.4 million) statin therapy for 11.1 million adults and would prevent 20,000 myocardial infarctions and 10,000 CHD deaths per year at an annual net cost of $3.6 billion ($42,000/QALY) if low-intensity statins cost $2.11 per pill. The ATP III guidelines would be preferred over alternative strategies if society is willing to pay $50,000/QALY and statins cost $1.54 to $2.21 per pill. At higher statin costs, ATP III is not cost-effective; at lower costs, more liberal statin-prescribing strategies would be preferred; and at costs less than $0.10 per pill, treating all persons with low-density lipoprotein cholesterol levels greater than 3.4 mmol/L (>130 mg/dL) would yield net cost savings. RESULTS OF SENSITIVITY ANALYSIS: Results are sensitive to the assumptions that LDL cholesterol becomes less important as a risk factor with increasing age and that little disutility results from taking a pill every day. LIMITATION: Randomized trial evidence for statin effectiveness is not available for all subgroups. CONCLUSION: The ATP III guidelines are relatively cost-effective and would have a large public health impact if implemented fully in the United States. Alternate strategies may be preferred, however, depending on the cost of statins and how much society is willing to pay for better health outcomes. Les aléas du développement de la temporalité chez l'enfant passent souvent inaperçus, masqués par divers troubles psychopathologiques. Le temps constitue toutefois une dimension essentielle de l'adaptation scolaire, familiale ou sociale. En première partie, cet article retrace la psychogenèse de la temporalité et plus particulièrement le développement du temps notionnel et le développement des notions cinématiques. La seconde partie est consacrée à la présentation du questionnaire temporel pour l'enfant (QTE), étalonné sur un échantillon de 153 enfants de 6 à 13 ans. Cet instrument d'évaluation du temps notionnel offre au clinicien un outil de screening permettant d'identifier les difficultés temporelles chez les enfants.

BACKGROUND/OBJECTIVES: This study aims to assess whether patent foramen ovale (PFO) closure is superior to medical therapy in preventing recurrence of cryptogenic ischemic stroke or transient ischemic attack (TIA). METHODS: We searched PubMed for randomized trials which compared PFO closure with medical therapy in cryptogenic stroke/TIA using the items: "stroke or cerebrovascular accident or TIA" and "patent foramen ovale or paradoxical embolism" and "trial or study". RESULTS: Among 650 potentially eligible articles, 3 were included including 2303 patients. There was no statistically significant difference between PFO-closure and medical therapy in ischemic stroke recurrence (1.91% vs. 2.94% respectively, OR: 0.64, 95%CI: 0.37-1.10), TIA (2.08% vs. 2.42% respectively, OR: 0.87, 95%CI: 0.50-1.51) and death (0.60% vs. 0.86% respectively, OR: 0.71, 95%CI: 0.28-1.82). In subgroup analysis, there was significant reduction of ischemic strokes in the AMPLATZER PFO Occluder arm vs. medical therapy (1.4% vs. 3.04% respectively, OR: 0.46, 95%CI: 0.21-0.98, relative-risk-reduction: 53.2%, absolute-risk-reduction: 1.6%, number-needed-to-treat: 61.8) but not in the STARFlex device (2.7% vs. 2.8% with medical therapy, OR: 0.93, 95%CI: 0.45-2.11). Compared to medical therapy, the number of patients with new-onset atrial fibrillation (AF) was similar in the AMPLATZER PFO Occluder arm (0.72% vs. 1.28% respectively, OR: 1.81, 95%CI: 0.60-5.42) but higher in the STARFlex device (0.64% vs. 5.14% respectively, OR: 8.30, 95%CI: 2.47-27.84). CONCLUSIONS: This meta-analysis does not support PFO closure for secondary prevention with unselected devices in cryptogenic stroke/TIA. In subgroup analysis, selected closure devices may be superior to medical therapy without increasing the risk of new-onset AF, however. This observation should be confirmed in further trials using inclusion criteria for patients with high likelihood of PFO-related stroke recurrence.
