Streptococcus mutans, the predominant bacterial species associated with dental caries, can enter the bloodstream and cause infective endocarditis. The aim of this study was to investigate S. mutans biofilm formation and adherence to endothelial cells induced by human fibrinogen. The putative mechanism by which biofilm formation is induced as well as the impact of fibrinogen on S. mutans resistance to penicillin was also evaluated. Bovine plasma dose dependently induced biofilm formation by S. mutans. Of the various plasma proteins tested, only fibrinogen promoted the formation of biofilm in a dose-dependent manner. Scanning electron microscopy observations revealed the presence of complex aggregates of bacterial cells firmly attached to the polystyrene support. S. mutans in biofilms induced by the presence of fibrinogen was markedly resistant to the bactericidal effect of penicillin. Fibrinogen also significantly increased the adherence of S. mutans to endothelial cells. Neither S. mutans cells nor culture supernatants converted fibrinogen into fibrin. However, fibrinogen is specifically bound to the cell surface of S. mutans and may act as a bridging molecule to mediate biofilm formation. In conclusion, our study identified a new mechanism promoting S. mutans biofilm formation and adherence to endothelial cells which may contribute to infective endocarditis.

The aim of this study was to evaluate the influence of chlorhexidine and Er, Cr:YSGG laser irradiation on the bond strength and external adaptation in mixed healthy and caries-affected class V cavities before and after thermal cycling. Thirty-six cavity preparations were made in mixed class V buccal human molars, half of them being artificially caries-induced. Any remaining affected dentin was removed from the cavity with a round burr at low speed. The teeth were divided into six groups, according to cleaning agent for both healthy and caries-induced dentin: no treatment, chlorhexidine and erbium, chromium-doped: yttrium, scandium, gallium, garnet (Er,Cr:YSGG) laser irradiation. A Filtek P90 (3M ESPE, St Paul, MN, USA) silorane adhesive restorative system was used. The specimens were subjected to 5000 thermal cycles (5-55 degrees C 60 min). Epoxy replicas were obtained to characterize the external adaptation under scanning electron microscopy. The average percentages of non-continuous margins were 5.41% and 6.49% in enamel dentin before thermal cycling and 25% and 33.7% after thermal cycling, respectively. The caries-affected and laser irradiated cavities showed higher non-continuous margins. Thermal cycling was able to raise the percentage of non-continuous margin for all groups. Chlorhexidine did not affect the marginal adaptation results, and the Er,Cr:YSGG laser irradiation showed significantly worse results compared with the control group.

The purpose of this study was to evaluate the accuracy of the depth of carious lesions on bitewing radiographs. Methods Recently extracted primary molars had their proximal surfaces evaluated visually (EC) and classified as healthy surface (0), signs that suggest the presence of carious lesions in enamel (1), signs of a superficial lesion in dentin (2) and carious lesions in deep dentin (3). Results The results were obtained by consensus between the investigators. The gold standard was determined by histological analysis. The values of sensitivity, specificity, accuracy and area under the ROC (Receiver Operating Characteristic) curve were evaluated. There was equilibrium between sensitivity (76.92% EC and 88.46% ER) and specificity (95.83% EC and 95.83% ER). Accuracy was 86.01% (EC) and 88.46% (ER). The Spearman correlation test was used to prove the correlation between clinical and radiographic examinations (0.886), for clinical and histological (0.736) and for radiographic and histological analysis (0.843).

The pattern of caries lesions has been modified over decades. If the presence or absence of a caries lesion is based upon only a visual examination, its diagnosis might become underestimated since dentinal lesions underneath non cavitated or minimally demineralized enamel is highly frequent in dental clinics. The aim of this paper is to report a clinical case of a child with an upper molar with questionable occlusal aspect, presenting asymptomatic pulpal hyperplasia on the distolingual pit. Periapical radiographic examination was essential for diagnosing hidden caries and elaborating the treatment plan. As part of the approach, removal of the decayed tissue and pulpotomy with calcium hydroxide were performed. The association of an accurate visual examination to an adequate oral radiographyis is extremely important in cases which the presence of hidden caries is doubtful. As a result, an early diagnosis and the reduction of more invasive interventions related to the oral health care of pediatric patients can be achieved as well.

Several aspects beyond the restorative phase itself such as orthodontic movement and periodontal treatment must be considered in cases of closure of diastemas. In such cases, a multidisciplinary approach is essential. As patients during orthodontic treatment may show high risk of developing dental caries and periodontal disease, inflammation of the gingival tissue is a common finding. For this reason, a preliminary basic periodontal treatment is critical to the success of restorative procedure. In addition, postoperative care and instruction in phonetics, oral hygiene and periodic control must be considered by professionals and patients. Thus, this paper demonstrates through a case report, pre and postoperative issues that should be considered during the closure of diastemas using a layering technique with resin composites

Aim: The aim of this study was investigate the effect of photodynamic therapy (PDT) using curcumin (C) as a photosensitizing agent irradiated with an LED (L) in the blue wavelength as a light source on a standard and clinical isolate of Streptococcus mutans (S. mutans) in a planktonic suspension model. Materials and methods: Suspensions of both strains were divided into 4 groups as follows: absence of C and L (control group: C–L–), with C and without L (C group: C+L–), absence of C with L (L group: C–L+) and presence of C and L (PDT group: C+L+). Three different concentrations of curcumin (0.75 mg/ml, 1.5 mg/ml and 3 mg/ml) and three light fluences of studied light source (24, 48 and 72 J cm–2) were tested. Aliquots of each studied group was plated in BHI agar and submitted to colony forming units counting (CFU/ml) and the data transformed into logarithmical scale. Results: A high photoinactivation rate of more than 70% was verified to standard S. mutans strain submitted to PDT whereas the clinical isolate showed a lower sensitivity to all the associations of curcumin and LED. A slight bacterial reduction was verified to C+L– and C–L+, demonstrating no toxic effects to the isolated application of light and photosensitizer to both S. mutans strains tested. Conclusion: Photodynamic therapy using a combination of curcumin and blue LED presented a substantial antimicrobial effect on S. mutans standard strain in a planktonic suspension model with a less pronounced effect on its clinical isolate counterparts due to resistance to this alternative approach. Clinical significance: Alternative antimicrobial approaches, as photodynamic therapy, should be encouraged due to optimal results against cariogenic bacteria aiming to prevent or treat dental caries.

Posterior teeth restorations have changed the contemporary treatment philosophy influenced by the aesthetic demand of patients, progress of adhesive material science and techniques for preservation and rehabilitation of affected teeth by dental caries and trauma. The development of Onlay restorations with semi-direct technique in endodontically teeth treated aims to preserve the remaining surfaces, to reduce the possibility of fracture and polymerization shrinkage. In addition, better restoration adaptation and marginal seal, resistance to wear and dimensional stability are achieved. This case reports the rehabilitation of an endodontically treated permanent maxillary first molar in a 13 years old- patient who attended the Araraquara School of Dentistry, Brazil, using Miris 2 Composite resin with semi-direct technique and obtaining an aesthetic and functional restoration in a single appointment. The fundaments and clinical guidelines of the procedure are detailed, based on the review of the literature that supports this conservative treatment.

This study ev aluated oral health indic ators by determining the pr evalence of dental c aries, periodontal diseases, prosthetic conditions and need in pr egnant women; identifi ed self-perceived oral health status and evaluated association bet ween variables. A clinical examination was performed in fi fty pregnant women assisted in the Preventive Dentistry Clinic-FOAr-UNESP using traditional oral health indicators: DMFT, C ommunity Periodontal Index ( CPI) and pr osthetic c onditions. A semi-struc tured questionnair e was applied to identify oral health perception and to collect sociodemographic variables. For data analysis, pregnant women were allocated in three groups according to their oral health perception (good, fair, poor) and compared according to clinical variables by the Kruskal-Wallis and Dunn tests. DMFT was high, all of them showed periodontal changes, most do not use and do not r equire any type of prosthesis; 36% considered their oral appearance as good. There was signifi cant diff erence between groups for the DMFT index and prosthetic need. It may be concluded that despite dental caries experience of pregnant women was found to be high, the prosthetic need have been detected in most of them and the presence of calculus was observed in all the volunteers, the majority considered their oral health status satisfactory.

To evaluate the effect of chlorhexidine (CHX) on the wettability of sound and caries affected dentin by a simplified adhesive system. Material and Methods: Flat coronal dentin surfaces were produced on 60 sound molars, 30 of which were artificially decayed. The teeth were divided randomly into 3 groups (n = 10) with smear layer (SL), without SL impregnated with water and without SL impregnated with chlorhexidine. The SL removal was performed by phosphoric acid etching for 15 s. 20 uL of distilled water or 2% chlorhexidine digluconate were applied on the demineralized dentin for 60 s. Then, a drop of Single Bond 2 was deposited on each surface. Contact angles between dentin surface and adhesive was measured by means of a goniometer and data were submitted to ANOVA and Tukey tests (α = 0.05). Results: Higher contact angles were obtained on sound versus caries affected dentin (p <0.05), regardeless of the surface treatment. For both substrates, contact angles statistically higher were obtained for dentin covered with SL (P <0.05). The SL removal resulted in significant reduction of the angles (P <0.05) and no difference was found among angles produced on demineralized dentin impregnated with water or chlorhexidine (p> 0.05). Conclusion: Caries affected dentin wettability was higher than sound dentin and that characteristic was not influenced by chlorhexidine application.
