Connue pour son rôle dans la cascade de coagulation, la thrombine, une protéase à sérine, peut également agir par l’intermédiaire de PAR1, un récepteur activé par protéase et couplé aux protéines G liant le GTP (GPCR). La thrombine se lie et clive l’extrémité N-terminale du PAR1 entre l’Arg41 et la Ser42, exposant une nouvelle extrémité terminale qui agit elle-même comme un ligand. La thrombine et une séquence peptidique de cinq acides aminés, composée des résidus Ser42 à Arg46, nommée PAR1-AP, déclenchent dans diverses cellules de mammifères une réponse intracellulaire comportant une composante calcique. Dans cette étude, le système d’expression par baculovirus dans les cellules Sf9 d'insecte nous a permis d'exprimer le récepteur PAR1 du rat à la surface de ces cellules et de réaliser son couplage fonctionnel à leur signalisation intracellulaire (modèle rPAR1-Sf9). La composante calcique de celle-ci, en réponse au PAR1-AP, a ensuite été étudiée en détail à l’aide de la sonde fluorescente Fura-2 et de plusieurs inhibiteurs agissant sur les canaux calciques ou d'autres éléments de la cascade de signalisation du calcium intracellulaire. Lorsque le milieu extracellulaire contient du calcium (Ca2+), la thrombine ou PAR1-AP déclenchent un signal calcique qui consiste en une augmentation rapide de [Ca2+]i suivi d’un plateau relativement soutenu, puis d'un retour lent vers le niveau de base initial. En l'absence de Ca2+ dans le milieu extracellulaire, l'augmentation initiale rapide de [Ca2+]i est suivie d'un retour rapide vers le [Ca2+]i de base. À l’aide d’inhibiteurs de canaux calciques, tels que le lanthane, la nifédipine et le D-600, l'entrée du calcium du milieu extracellulaire dans les cellules est inhibée, abolissant le plateau soutenu de [Ca2+]i. L’inhibition de la pompe Ca2+-ATPase par la thapsigargine supprime la réponse au PAR1-AP après épuisement des sites de stockage de Ca2+intracellulaire. Le TMB-8 agit de façon discordante quant à l’inhibition de la libération de Ca2+ des sites de stockage intracellulaires. La réponse à PAR1-AP n’est pas affectée par le D-609, un inhibiteur de la phospholipase β. L’inhibition de la protéine kinase C (PKC) par le bisindolylmaléimide induit des oscillations en présence de Ca2+ extracellulaire et atténue fortement le signal calcique en absence de Ca2+ extracellulaire. En présence de Ca2+ extracellulaire, l’activation de la PKC par le PBDu tronque le flux de [Ca2+]i tandis que la réponse calcique est abolie en absence de Ca2+ dans le milieu extracellulaire. Le H-89, un inhibiteur de la protéine kinase A (PKA), cause une prolongation de la durée du plateau de [Ca2+]i dans un milieu riche en calcium et la suppression de la réponse à PAR1-AP lorsque le milieu extracellulaire est dépourvu de Ca2+. Les résultats obtenus nous permettent de conclure que la PKC et possiblement la PKA jouent un rôle critique dans la mobilisation du Ca2+ induite par le PAR1-AP dans le modèle rPAR1-Sf9.

Alors que la prévalence de l’obésité est un problème d’ampleur mondiale, les avenues permettant de mieux utiliser l’exercice (Ex) s’avèrent d’un grand intérêt. L’Ex peut réduire l'appétit et l'apport énergétique, soit l’effet anorexigène de l'Ex. Des études récentes de notre laboratoire ont montré l’importance du moment de la pratique d’Ex, pour diminuer l’apport énergétique. Cependant, aucune ne certifie que le positionnement dans le temps de l'Ex maximise la réduction de masse corporelle en contexte naturel. Le devis croisé de l’étude visait donc à déterminer s’il existe un positionnement idéal de l’Ex, afin de potentialiser la perte de poids corporel et d'adiposité, en comparant l'effet de deux programmes sur l’anthropométrie d’adultes en surpoids. Huit adultes montréalais volontaires (18-45 ans) en surpoids ou obèses ont complété l’étude. Aléatoirement, ils ont effectué deux programmes d'Ex (2 x 15 min. d’Ex par intervalles quotidiennement) de quatre semaines : 1) Ex avant les repas (ExMeal) vs 2) Ex à tout moment, sauf dans l'heure précédent les repas (MealEx). Les consultations hebdomadaires à l'Université de Montréal comprenaient : les mesures anthropométriques, les questionnaires standardisés sur la pratique d’activités physiques et l’alimentation, ainsi que le suivi des entrainements faits en milieu naturel. Les analyses Mann- Whitney U ont révélé des résultats similaires concernant le profil anthropométrique, la pratique d’Ex à l’intérieur et hors du programme et l’ingestion calorique (contenu calorique et % de l’énergie des glucides, protéines et lipides), entre les programmes ExMeal et MealEx (p > 0.05). Cependant, le programme ExMeal a été associé à une ingestion calorique sous forme de protéines de 2,8% plus importante (p= 0.05). D’autres analyses exploratoires, ont fait ressortir que c’est surtout la séquence mensuelle des évènements qui était liée à une réduction du pourcentage de gras et à une assiduité plus importante aux Ex structurés lors du premier mois. Par ailleurs, même si plusieurs études ont vérifié et confirmé l’effet anorexigène de l’Ex aigu dans certaines conditions, il semble que l’effet à plus long terme sur le profil anthropométrique ne soit pas démontré avec cette étude pilote. Enfin, des facteurs comme le statut d’adiposité, la structure du programme, la durée des séances d’Ex et la pratique en milieu naturel peuvent avoir rendu plus difficile l’amélioration du profil anthropométrique. La sclérose latérale amyotrophique est une maladie neurodégénérative fatale caractérisée par la dégénérescence progressive des neurones moteurs centraux et périphériques. L’un des premiers signes de la maladie est la dénervation de la jonction neuromusculaire (JNM). Les diverses unités motrices (UM) ne présentent toutefois pas la même vulnérabilité à la dénervation dans la SLA: les UM rapide fatigables sont en fait les plus vulnérables et les UM lentes sont les plus résistantes. Alors que des études précédentes ont démontré dans plusieurs modèles animaux de la SLA de nombreuses variations synaptiques, les découvertes ont été contradictoires. Par ailleurs, le type d’UM n’a pas été tenu en compte dans ces divers travaux. Nous avons donc émis l’hypothèse que la présence de la mutation SOD1 pourrait affecter différemment la transmission synaptique des UM, en accord avec leur vulnérabilité sélective. En effectuant des enregistrements électrophysiologiques et de l’immunohistochimie, nous avons étudié la transmission synaptique des différents types d’UM du muscle à contraction rapide Extensor Digitorum Longus (EDL; rapide fatigable (FF) MU) et du muscle à contraction lente Soleus (SOL; lente (S) and rapide fatigue-résistante (FR) MU) de la souris SOD1G37R et leur congénères WT. Pour identifier le type d’UM, un marquage par immunohistochimie des chaînes de myosine a été effectué. Un triple marquage de la JNM a également été effectué pour vérifier son intégrité aux différents stades de la maladie. À P160, dans la période asymptomatique de la maladie, alors qu’aucune altération morphologique n’était présente, l’activité évoquée était déjà altérée différemment en fonction des UM. Les JNMs FF mutantes ont démontré une diminution de l’amplitude des potentiels de plaque motrice (PPM) et du contenu quantique, alors que les JNMs lentes démontraient pratiquement le contraire. Les JNMs FR montraient quant à elles une force synaptique semblable au WT. À P380, dans la période présymtomatique, de nombreuses altérations morphologiques ont été observées dans le muscle EDL, incluant la dénervation complète, l’innervation partielle et les extensions du nerf. La transmission synaptique évoquée des UM FF étaient toujours réduites, de même que la fréquence des potentiels de plaque motrice miniatures. À P425, à l’apparition des premiers symptômes, l’activité synaptique des JNMs S était redevenue normale alors que les JNMs FR ont montré à ce moment une diminution du contenu quantique par rapport au contrôle. De manière surprenante, aucun changement du ratio de facilitation n’a été observé malgré les changements flagrants de la force synaptique. Ces résultats révèlent que la fonction de la JNM est modifiée différemment en fonction de la susceptibilité des UM dans l’ALS. Cette étude fournit des pistes pour une meilleure compréhension de la physiologie de la JNM durant la pathologie qui est cruciale au développement d’une thérapie adéquate ciblant la JNM dans la SLA.

Calmodulin (CaM) is a ubiquitous Ca(2+) buffer and second messenger that affects cellular function as diverse as cardiac excitability, synaptic plasticity, and gene transcription. In CA1 pyramidal neurons, CaM regulates two opposing Ca(2+)-dependent processes that underlie memory formation: long-term potentiation (LTP) and long-term depression (LTD). Induction of LTP and LTD require activation of Ca(2+)-CaM-dependent enzymes: Ca(2+)/CaM-dependent kinase II (CaMKII) and calcineurin, respectively. Yet, it remains unclear as to how Ca(2+) and CaM produce these two opposing effects, LTP and LTD. CaM binds 4 Ca(2+) ions: two in its N-terminal lobe and two in its C-terminal lobe. Experimental studies have shown that the N- and C-terminal lobes of CaM have different binding kinetics toward Ca(2+) and its downstream targets. This may suggest that each lobe of CaM differentially responds to Ca(2+) signal patterns. Here, we use a novel event-driven particle-based Monte Carlo simulation and statistical point pattern analysis to explore the spatial and temporal dynamics of lobe-specific Ca(2+)-CaM interaction at the single molecule level. We show that the N-lobe of CaM, but not the C-lobe, exhibits a nano-scale domain of activation that is highly sensitive to the location of Ca(2+) channels, and to the microscopic injection rate of Ca(2+) ions. We also demonstrate that Ca(2+) saturation takes place via two different pathways depending on the Ca(2+) injection rate, one dominated by the N-terminal lobe, and the other one by the C-terminal lobe. Taken together, these results suggest that the two lobes of CaM function as distinct Ca(2+) sensors that can differentially transduce Ca(2+) influx to downstream targets. We discuss a possible role of the N-terminal lobe-specific Ca(2+)-CaM nano-domain in CaMKII activation required for the induction of synaptic plasticity.

The PAT family of lipid droplet proteins includes 5 members in mammals: perilipin, adipose differentiation-related protein (ADRP), tail-interacting protein of 47 kDa (TIP47), S3-12, and OXPAT. Members of this family are also present in evolutionarily distant organisms, including insects, slime molds and fungi. All PAT proteins share sequence similarity and the ability to bind intracellular lipid droplets, either constitutively or in response to metabolic stimuli, such as increased lipid flux into or out of lipid droplets. Positioned at the lipid droplet surface, PAT proteins manage access of other proteins (lipases) to the lipid esters within the lipid droplet core and can interact with cellular machinery important for lipid droplet biogenesis. Genetic variations in the gene for the best-characterized of the mammalian PAT proteins, perilipin, have been associated with metabolic phenotypes, including type 2 diabetes mellitus and obesity. In this review, we discuss how the PAT proteins regulate cellular lipid metabolism both in mammals and in model organisms.

Gammaherpesviruses, including the human pathogens Epstein-Barr virus and Kaposi's sarcoma-associated herpesvirus, are causative agents of lymphomas and other malignancies. The structural characterization of these viruses has been limited due to difficulties in obtaining adequate amount of virion particles. Here we report the first three-dimensional structural characterization of a whole gammaherpesvirus virion by an emerging integrated approach of cryo-electron tomography combined with single-particle cryo-electron microscopy, using murine gammaherpesvirus-68 (MHV-68) as a model system. We found that the MHV-68 virion consists of distinctive envelope and tegument compartments, and a highly conserved nucleocapsid. Two layers of tegument are identified: an inner tegument layer tethered to the underlying capsid and an outer, flexible tegument layer conforming to the overlying, pleomorphic envelope, consistent with the sequential viral tegumentation process inside host cells. Surprisingly, comparison of the MHV-68 virion and capsid reconstructions shows that the interactions between the capsid and inner tegument proteins are completely different from those observed in alpha and betaherpesviruses. These observations support the notion that the inner layer tegument across different subfamilies of herpesviruses has evolved significantly to confer specific characteristics related to viral-host interactions, in contrast to a highly conserved capsid for genome encapsidation and protection.

There is growing support for the theory that an interaction between the immune and reproductive/endocrine systems underlies the pathogenesis of autoimmune rheumatic diseases. Most of the recent evidence derives from studies of sex hormones and pregnancy in women with systemic lupus. Other than an ameliorative effect of pregnancy, little is known about reproductive factors in relation to rheumatoid arthritis. To elucidate the relationship, a population-based retrospective study was undertaken. Included were 378 female residents of Olmsted County, Minnesota diagnosed with rheumatoid arthritis between 1950 and 1982 (cases) and 325 arthritis-free, married female controls matched to the 324 married cases on birth-year, age at first marriage, and duration of Olmsted County residency. Information of reproductive factors was extracted from the medical records system maintained by the Mayo Clinic.^ Cases had lower fertility rates compared with the female population of Minnesota (rate ratio = 0.86, 95% confidence interval (CI)= 0.80-0.92). Fertility was significantly reduced even prior to the onset of rheumatoid factor positive arthritis. Restricting the comparison to married Olmsted County residents did not alter the results. Further adjustments for time not at risk of conception using survival analysis and proportional hazards modeling only intensified the fertility reduction in the married cases compared with controls. Nulligravidity was more common among cases than controls (odds ratio = 3.16, CI = 1.61-6.20). Independent of fertility, pregnancy had a protective effect against rheumatoid arthritis (odds ratio = 0.31, CI = 0.11-0.89), which was dramatically reversed in the 12 months postpartum (odds ratio = 4.67, CI = 1.50-14.47). Cases were younger at menopause than controls (p $<$ 0.01).^ Small but statistically insignificant associations were observed between rheumatoid arthritis and the following factors: increased frequency of complaints to a physician of infertility; increased frequency of spontaneous abortion, premature birth, and congenital malformations following arthritis onset; and increased prevalence of menopause at arthritis onset. Cases did not differ from controls on age at menarche, duration of pregnancy, or birth weight.^ The findings provide further support for the involvement of the reproductive/endocrine systems in the pathogenesis of autoimmune rheumatic disease. The search for biological mechanisms should be intensified. ^

Recent reports have suggested the possible association of status epilepticus and multiple organ system failure. The purpose of this case control study was to investigate this association and to identify factors that predispose individuals with status epilepticus (SE) or aborted status epilepticus (ASE) to develop multiple organ system failure (MOSF) or multiple organ system dysfunction (MODS).^ For the purpose of the study, definitions of SE, ASE, MOSF, and MODS were operationalized as follows: SE was defined as any seizure lasting for a duration of $\ge$30 minutes or intermittent seizures lasting for $\ge$30 minutes from which the patient does not regain consciousness. ASE was defined as any seizure lasting for a duration of $\ge$10 minutes but $<$30 minutes and which was aborted as a result of a medical intervention. MOSF was defined as the failure of $\ge$2 organ systems in the same patient; organ system failure was said to be present whenever standard MOSF criteria were met. MODS was defined as the dysfunction of $\ge$2 organ systems in the same patient; organ system dysfunction was said to be present, whenever the monitor(s) of that organ's function exceeded the normal range for the physiological or laboratory parameters.^ Medical records of 686 individuals between the age of 5 and 44 years, with history of seizures needing hospitalization at the Texas Children's Hospital or Methodist Hospital, Houston, Texas, between 1991-95 were reviewed and 100 individuals with SE/ASE were identified. Of these 100 individuals with SE/ASE, 45 developed MOSF/MODS during their hospitalization and 9 of these individuals died. Using multivariate analyses, it was found that adult individuals who had an "acute" etiology of their seizure disorder (OR = 5.23; 95%CI: 0.41, 66.24) and children who had a "remote" etiology of their seizure disorder (OR = 3.92; 95%CI: 0.53, 29.22), were more likely to develop MOSF/MODS compared with those who had other etiologies of the seizure disorder. Individuals with SE lasting more than one hour were more likely to develop MOSF/MODS compared with individuals with SE lasting less than 1 hour (OR = 6.51; 95%CI: 1.63, 25.92). Individuals who presented with the SE/ASE episode as their first seizure episode were more likely to develop MOSF/MODS compared to those with a previous history of seizure episodes (OR = 1.78; 95%CI: 0.36, 8.82).^ The major limitations of this study includes the relatively small sample size and the study being performed in only two institutions. However, this is the first study of this kind and should therefore be viewed as largely exploratory in nature. Future studies should investigate the relationship of the risk factors identified in this study using a larger number of institutions and patients. ^

Background. Research investigating symptom management in patients with chronic obstructive pulmonary disease (COPD) largely has been undertaken assuming the homeostatic construct, without regard to potential roles of circadian rhythms. Temporal relations among dyspnea, fatigue, peak expiratory flow rate (PEFR) and objective measures of activity/rest have not been reported in COPD. ^ Objectives. The specific aims of this study were to (1) explore the 24-hour patterns of dyspnea, fatigue, and PEFR in subjects with COPD; (2) examine the relations among dyspnea, fatigue, and PEFR in COPD; and (3) examine the relations among objective measures of activity/rest and dyspnea, fatigue, and PEFR in COPD. ^ Methods. The repeated-measures design involved 10 subjects with COPD who self-assessed dyspnea and fatigue by 100 mm visual analog scales, and PEFR by peak flow meter in their home 5 times a day for 8 days. Activity/rest was measured by wrist actigraphy. Single and population mean cosinor analyses and correlations were computed for dyspnea, fatigue, and PEFR; correlations were done among these variables and activity/rest. ^ Results. Circadian rhythms were documented by single cosinor analysis in 40% of the subjects for dyspnea, 60% for fatigue, and 60% for PEFR. The population cosinor analysis of PEFR yielded a significant rhythm (p < .05). The 8-day 24-hour means of dyspnea and fatigue was moderately correlated (r = .48, p < .01). Dyspnea and PEFR, and fatigue and PEFR, were weakly correlated in a negative way (r = −.11, p < .05 and r = −.15, p < .01 respectively). Weak to moderate correlations (r = .12–.34, p < .05) were demonstrated between PEFR and mean activity level measured up to 4 hours before PEFR measurement. ^ Conclusions. The findings suggest that (1) the dyspnea and fatigue experienced by COPD patients are moderately related, (2) there is a weak to modest positive relation between PEFR and activity levels, and (3) temporal variation in lung function may not affect the dyspnea and fatigue experienced by patients with COPD. Further research, examining the relations among dyspnea, fatigue, PEFR, and activity/rest is needed. Replication of this study is suggested with a larger sample size. ^

Mycobacterium avium complex (MAC) is a ubiquitous organism responsible for most pulmonary and disseminated disease caused by non-tuberculosis (NTM) mycobacteria. Though MAC lung disease without predisposing factors is uncommon, in recent years it has been increasingly described in middle-aged and elderly women. Recognition and correct diagnosis, is often delayed due to the indolent nature of the disease. It is unclear if these women have significant clinical disease as or if their airways are simply colonized by the bacterium. This study describes the clinical presentation, identifies risk factors, and describes the clinical significance of MAC lung disease in HIV-negative women aged 50 or greater. ^ A hybrid study design utilizing both cross-sectional and case-control methodologies was used. A comparison population was selected from previously identified tuberculosis suspects found throughout Harris County. The study population had at least one acid fast bacillus pulmonary culture performed between 1/1/1998 and 12/31/2000 from a pulmonary source. Clinical presentation and symptoms were analyzed using a cross-sectional design. Past medical history and other risk factors were evaluated using a traditional case-control study design. Differences in categorical variables were estimated with the Chi Square or Fisher's Exact test as appropriate. Odds ratios and 95% confidence intervals were utilized to evaluate associations. Multivariate logistic regression was used to identify predictive factors for MAC. All statistical tests were two-sided and P-values <0.05 were considered statistically significant. ^ Culture confirmed MAC pulmonary cases were more likely to be white, have bronchiectasis, scoliosis, evidence of cavitation and pleural changes on chest radiography and granulomas on histopathologic examination than women whose pulmonary cultures were AFB negative. After controlling for selected risk factors, white race continued to be significantly associated with MAC lung disease (OR = 4.6, 95% CI = 2.3, 9.2). In addition, asthma history, smoking history and alcohol use were less likely to be evident among MAC cases in a multivariate analysis. Right upper and right middle lobe disease was further noted among clinically significant cases. Based on population data, MAC lung disease appears to represent a significant clinical syndrome in HIV-negative women thus supporting the theory of the Lady Windermere Syndrome. ^

Protection against Mycobacterium tuberculosis requires development and maintenance of granulomatous lesions, a feature considered to be the pathological hallmark of Tuberculosis (TB) disease. Upon encountering Mtb or mycobacterial antigens, specifically trehalose 6,6'-dimycolate (TDM), a strong local pro-inflammatory response is initiated. Systemic production of anti-inflammatory glucocorticoids (GCs) is also induced. Emergence of these antagonists at the inflammatory foci is counterproductive to development of the granulomatous structure and detrimental to host protection against TB. Therefore, it was hypothesized that local enzymatic regulation of GCs occurs locally at the site of granulomatous inflammation. The experiments described here strongly suggest that 11β-hydroxysteroid dehydrogenases (11βHSDs) shuttle GCs between active and inert forms during the acute granulomatous response, supporting the net reduction of corticosterone. The patterns of GC and 11βHSD regulation were specific to the lung (the site of inflammation) and were not observed in other tissues. Furthermore, 11βHSD2, which decreases corticosterone concentrations, was not expressed in models of dysregulated granulomatous inflammation. These findings suggest that cellular exposure to local active GC concentrations is restricted via 11βHSDs as a mechanism to initiate and maintain granuloma formation. The information derived from the experiments outlined in this dissertation provides a better understanding of the events required for establishment and maintenance of the protective granulomatous response. As a practical consequence, exploiting 11βHSD2 modulation of GCs at the site of Mtb infection may lead to improvement of Tuberculosis treatment strategies.^
