Os edifícios escolares são espaços com características específicas, onde a garantia de condições de trabalho saudáveis é fundamental para a aprendizagem e bem-estar dos ocupantes. Contudo, os novos padrões arquitectónicos têm potenciado o aparecimento de edifícios mais herméticos, com reduzidas taxas de ventilação e problemas relacionados com a qualidade do ar interior (QAI). Com a recente legislação, adoptaram-se valores de referência para os parâmetros químicos e microbiológicos. A concentração de CO2 e microrganismos viáveis pode dar uma boa indicação da eficiência da ventilação. Neste âmbito, efectuou-se um estudo que teve como objectivos relacionar as concentrações de CO2 e microrganismos viáveis com os níveis de ocupação e eficiência da ventilação de salas de aulas. A quantificação dos parâmetros ambientais baseou-se na caracterização estrutural e de funcionamento do edifício. As amostragens foram efectuadas na Primavera, em nove salas de aula de quatro jardins-de-infância (JI) com diferentes tipos de construção. Os resultados demonstraram que os JI recentes apresentaram concentrações médias de CO2 superiores às verificadas nos JI do "plano centenário", atingindo valores máximos de 3400 ppm nos períodos de ocupação. Obtiveram-se concentrações elevadas de bactérias gram positivas nos JI de construção recente, que podem estar relacionadas com a sobrelotação dos espaços e ventilação insuficiente. Os valores de velocidade do ar também revelam a constante estagnação do ar nos espaços. O aumento das taxas de ventilação e sensibilização dos ocupantes são medidas chave para a melhoria da QAI. Contudo, é na fase de projecto do edifício que devem surgir preocupações com a QAI.

OBJECTIVE To evaluate if temperature and humidity influenced the etiology of bloodstream infections in a hospital from 2005 to 2010.METHODS The study had a case-referent design. Individual cases of bloodstream infections caused by specific groups or pathogens were compared with several references. In the first analysis, average temperature and humidity values for the seven days preceding collection of blood cultures were compared with an overall “seven-days moving average” for the study period. The second analysis included only patients with bloodstream infections. Several logistic regression models were used to compare different pathogens and groups with respect to the immediate weather parameters, adjusting for demographics, time, and unit of admission.RESULTS Higher temperatures and humidity were related to the recovery of bacteria as a whole (versus fungi) and of gram-negative bacilli. In the multivariable models, temperature was positively associated with the recovery of gram-negative bacilli (OR = 1.14; 95%CI 1.10;1.19) or Acinetobacter baumannii (OR = 1.26; 95%CI 1.16;1.37), even after adjustment for demographic and admission data. An inverse association was identified for humidity.CONCLUSIONS The study documented the impact of temperature and humidity on the incidence and etiology of bloodstream infections. The results correspond with those from ecological studies, indicating a higher incidence of gram-negative bacilli during warm seasons. These findings should guide policies directed at preventing and controlling healthcare-associated infections.

Seventy three children (6-15 years) and 75 adults (18-47 years) with active schistosomiasis mansoni were treated with oltipraz. All cases had at least 100 eggs per gram of feces as determined by the Kato-Katz technique. Children and adults were divided in two groups receiving respectively 25 or 30 mg/kg, as a single oral dose. Clinical examination, laboratories tests (haemogram, urinalysis, hepatic and kidney functions tests, glycemia, cholesterol, triglicerides, lipoprotein HLD and LDL) and ECG were performed before, 3 or 7 days and 1 month after treatment. Parasitological control with 3 daily coprological examinations, was done on the 1st, 3rd j 6th month after drug administration. Giddiness, somnolence, headache, nausea, vomiting and abdominal distress were the most frequent side effects. Pain in the finger tips that need further investigations also occurred. No significant alteration in complementary tests were observed, whereas eosinophilia 1 month after treatment was detected, probably indicating worm death. The cure rate in children was 81.8% and 74.2% with 25 and 30 mg/kg respectively, and in adults 75.0% and 81.2% of the patients. No statistical significant difference was observed between cure rate and side effects at different dosages employed, neither between adults nor children. In all groups the percentage of egg reduction in feces in the non cured patients was higher than 96.0%. Further investigation with this new compound is necessary to accomplish the real value of oltipraz in the schistosomiasis chemotherapy.

A population-based clinical epidemiologic study on schistosomiasis mansoni was carried out in Tuparecê, Minas Gerais. The patients were interviewed for symptoms, water contact, past history and examined for spleen and liver enlargement. From the 830 people registered in the census, 777 (93.6%) had their stools examined (Kato-Katz method) and 696 (83.9%) were clinically evaluated. The overall index of Schistosoma mansoni infection was 43.2%. Significant and increased infection risks could be detected in the young age group (2-14 years old) regarding occupation, time of residence in the area and frequency of water contact. Bloody stools were significantly more prevalent among positives, while diarrhea was significantly more prevalent among those negative. The area was shown to have a low morbidity as well as a low intensity of infection measured by the number of S. mansoni eggs per gram of feces. A close correlation was found between water contact pattern and the age prevalence curve. It has emphasized the importance of habits in determining prevalence rates, besides suggesting that schistosomiasis mansoni in the area is manifested as a light and somewhat harmless infection with little consequence for the population as a whole.

Two cross-sectional studies on schistosomiasis mansoni were done in Comercinho, Minas Gerais (Brazil), at an interval of 7 years. In 1974 and 1981 feces examinations (KATO-KATZ method) were done in 89 and 90% of the population (about 1,500 inhabitants) and clinical examinations were done in 78 and 92% of the patients who excreted Schistosoma mansoni eggs in the feces, respectively. The rate of infection by S. mansoni did not change (69.9% in 1974 and 70.4% in 1981), but the geometrical mean of eggs per gram of feces (431 ± 4 and 334 ± 4, respectively) and the rate of splenomegaly (11 and 7%, respectively) decreased significantly in 1981, when compared to 1974. This reduction was observed only in the central zones of the town (zones 1-2) where the rate of dwellings with piped water increased from 17 to 44%. In the surroundings (zones 3-4), where the proportion of houses with piped water did not change significantly between 1974 (10%) and 1981 (7%), the geometrical mean of S. mansoni eggs and the rate of splenomegaly did not change either.

A double-blind clinical trial involving 120 patients with chronic schistosomiasis was carried out to compare the tolerability and efficacy of praziquantel and oxamniquine. The patients were randomly allocated into two groups. One was treated with praziquantel, 55 mg/kg of body weight CBWT), and the other one with oxamniquine, 15mg/kg bwt, administered in a single oral dose. The diagnosis and the parasitological follow-up was based on stool examinations by quantitative Kato-Katz method and on rectal biopsies. Side-effects mainly dizziness, sleepness, abdominal distress, headache, nausea and diarrhea were observed in 87% of the cases. Their incidence, intensity and duration were similar for both drugs but abdominal pain was significantly more frequent after praziquantel intake and severe dizziness was more commonly reported after oxamniquine. A significant increase of alanine-aminotransferase and y-glutamyltransferase was found with the latter drug and of total bilirubin with the former one. A total of 48 patients treated with praziquantel and 46 with oxamniquine completed with negative findings the required three post-treatment parasitological controls three slides of each stool sample on the first, third and sixth month. The achieved cure rates were 79.2% and 84.8%, respectively, a difference without statistical significance. The non-cured cases showed a mean reduction in the number of eggs per gram of feces of 93.5% after praziquantel and of 84.1% after oxamniquine. This diference also was not significant. Five patients retreated with praziquantel were cured but only one out of three treated a second time with oxamniquine. These findings show that both drugs despite their different chemical structures, pharmacological properties and mechanisms-of-action induce similar side-effects as well as a comparable therapeutical efficacy, in agreement with the results reported from analogous investigations.

No período de abril de 1979 a julho de 1980 foram estudadas 45 crianças, de ambos os sexos, na faixa etária de 2 meses a 5 anos de idade, selecionadas entre as atendidas no Instituto da Criança "Pedro Alcântara" do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, e acometidas de pneumopatias agudas. Dos 45 pacientes estudados, a maioria (46,7%) encontrava se na faixa de 2-12 meses de idade e, quanto ao estado nutricional, 42,2% eutróficos e 57,8% desnutridos, variando essa desnutrição do grau I ao grau III. Empregando amostras obtidas por aspiração pulmonar e examinando-as pelo método de coloração de Gram e cultura, foi possível identificar o agente etiológico de 26 (57,7%) pneumonias bacterianas nos 45 casos estudados. A identificação direta pelo método de Gram mostrou-se útil como orientação inicial para antibioticoterapia em 44,4% dos casos. Houve nítida predominância do Streptococcus pneumoniae como agente etiológico bacteriano nas pneumopatias agudas da criança nas várias faixas etárias estudadas seguido do Haemophilus influenzae e o Staphylococcus aureus. O estudo comparativo da cultura do aspirado pulmonar e da hemocultura permitiu maior precisão diagnostica (56,7%), revelando-se a hemocultura positiva em apenas 30,0% dos casos. Não foi constatada nenhuma participação de germes anaeróbios nas pneumopatias da amostragem estudada.

A clinical trial involving 80 patients of both sexes, from ages 15 to 55, with chronic intestinal or hepatointestinal schistosomiasis mansoni, was carried out to evaluate the therapeutical efficacy of different dose regimens of praziquantel. The patients were randomly allocated into four groups with an equal number of cases and were then treated with one of the following dosages: 60 mg/kg for 1 day; 60 mg/kg daily for 2 days; 60 mg/kg daily for 3 days; and 30 mg/kg daily for 6 days. The assessment of parasitological cure was based on the quantitative oogram technique through rectal mucosa biopsies which were undertaken prior to, as well as, 1,2,4 and 6 months post-treatment. Concurrently, stool examinations according to the qualitative Hoffman, Pons & Janer (HPJ) and the quantitative Kato-Katz (K-K) methods were also performed. The best tolerability was observed with 30 mg/kg daily for 6 days whereas the highest incidence of side-effects (mainly dizziness and nausea) was found with 60 mg/kg daily for 3 days. No serious adverse drug reaction has occurred. The achieved cure rates were: 25% with 60 mg/kg for 1 day; 60% with 60 mg/kg daily for 2 days; 89.5% with 60 mg/kg daily for 3 days; and 90% with 30 mg/kg daily for 6 days. At the same time there has been a downfall of 64%, 73%, 87% and 84% respectively, in the median number of viable S. mansoni ova per gram of tissue. Thus, a very clear direct correlation between dose and effect could be seen. The corresponding cure rates according to stool examinations by HPJ were 39%, 80%, 100% and 95%; by K-K 89%, 100%, 100% and 100%. This discrepancy in results amongst the three parasitological methods is certainly due to their unequal accuracy. In fact, when the number of viable eggs per gram of tissue fell below 5,000 the difference in the percentage of false negative findings between HPJ (28%) and K-K (80%) became significative. When this number dropped to less than 2,000 the percentage of false negative results obtained with HPJ (49%) turned significant in relation to the oogram as well. In conclusion, it has been proven that praziquantel is a highly efficacious agent against S. mansoni infections. If administered at a total dose of 180 mg/kg divided into either 3 or 6 days, it yields a 90% cure rate. Possibly, one could reach 100% by increasing the total dose to 240 mg/kg. Furthermore, it was confirmed that the quantitative oogram technique is the most reliable parasitological method when evaluating the efficacy of new drugs in schistosomiasis mansoni.

A randomized clinical trial was carried out to compare the efficacy of a low-dosage combination of oxamniquine (7.5 mg/kg) plus praziquantel (20 mg/kg) against either agent, oxamniquine (15 mg/kg) or praziquantel (40 mg/kg) alone, in the treatment of schistosomiasis mansoni in the Brazilian north-east. The drugs were randomly administered per os to 91 patients. Six and twelve months after treatment 89% of those admitted to the trial were reexamined by Kato-Katz method (ten slides) and MIF technique (one gram of stool) The achieved cure rates, as defined by absence of S. mansoni eggs in the faeces of individual patients at all points during the parasitological follow-up, were 81.8%, 81.2% and 67.6% for praziquantel, oxamniquine and the combination respectively. The reduction of eggs excretion in non cured patients six months after therapy ranged from 93.8-96.8% with praziquantel, 32.5-97% with oxamniquine and 76.9-99.5% with the combination. It is concluded that, at the used dosages, the three therapeutical regimens give similar and satisfactory results in the treatment of uncomplicated S. mansoni infection in Brazil.

The interrelation between schistosomiasis and the retinol blood levels was studied in a double blind method, by comparing the serum vitamin A of the infected and non-infected group of an endemic area of Schistosoma mansoni infection. The infected group was characterized by 106 parasitized persons in the intestinal and hepatointestinal forms, who eliminated less than 500 eggs/gram of feces (Modified Kato's method); the non-infected group was characterized by 112 inhabitants of this endemic area without eggs in the stools and presenting negative intradermal reactions, and absence of previous specific treatment. The blood levels of retinol was determinated using trifluoracetic acid method, regarding the normal levels > 20,0mg/100ml. The results of this study point out the absence of correlation between S. mansoni infection and blood levels of vitamin A.

Foi analisada a flora bacteriana de 99 abscessos causados por picadas de serpentes do gênero Bothrops, correspondendo a 61,1% dos casos que ocorreram em 1030 acidentes ofídicos atendidos no Hospital de Doenças Tropiciais (HDT) de Goiânia, no período de janeiro de 1984 a abril de 1988. O exsudato dos abscessos foi estudado através de bacterioscopia, cultura e testes de sensibilidade para aeróbios. Os bacilos Gram negativos foram isolados em maior frequência, destacando-se a Morganella morganii, Escherichia coli e Providencia sp presentes respectivamente em 44,4%, 20,2% e 13,1% das amostras. Esta flora aeróbica foi semelhante à encontrada na cavidade oral e no veneno das serpentes em outros estudos, nos quais predominaram Morganella morganii. Foi sugerido o uso do cloranfenicol no tratamento dos abscessos que não respondam à simples drenagem, face à grande sensibilidade destes microorganismos demonstrada nos testes "in vitro".

Relatam os autores isolamento de Legionella pneumophila sorogrupo 1, acompanhado de evidências sorológicas de infecção atual, em homem de 40 anos com infecção respiratória grave que evoluiu para insuficiência respiratória aguda. Esta foi caracterizada por hipoxemia severa refratária a altas concentrações de oxigênio e radiograficamente por infiltrados difusos em ambos pulmões. Com introdução de clindamicina, amicacina, ceftriaxone e ventilação à volume com Pressão Expiratória Positiva Final (PEEP) de 14 cm de H(2)0, houve estabilização do quadro e gradual recuperação. Suspeitando-se de legionelose, foi colhido sangue e secreção traqueal para exames específicos. A secreção traqueal foi semeada em meio BCYE com isolamento de bacilo gram-negativo, identificado como Legionella pneumophila sorogrupo 1 por características culturais, bioquímicas e reações de imunofluorescência direta e de aglutinação em lâmina. O estudo sorológico revelou títulos de anticorpos 128, 1024, 4096 e 8192 para amostras coletadas na 1ª, 3ª, 4ª e 6ª semanas após o início do quadro. Os resultados definitivos foram obtidos com o paciente em recuperação. É realçada a comprovação da presença de Legionella sp. como agente patológico em nosso meio; a importância das medidas de suporte na evolução do paciente; a necessidade de se pensar neste agente no diagnóstico diferencial de pneumonias e de se pesquisar mais esta etiologia com metodologia laboratorial específica.

The Peniche section has revealed moderately-to-well preserved calcareous nannofossil assemblages across the Pliensbachian/Toarcian boundary. This good record has allowed the proposition of a refined biostratigraphic scheme. The stage boundary, as defined by ammonites, is comprised within the NJ5b C. impontus (NW Europe; BOWN & COOPER, 1998) or the NJT5b L. sigillatus (Mediterranean Tethys; MATTIOLI & ERBA, 1999) nannofossil subzones. Since in the Lusitanian Basin a mixing of N- and S-Tethyan taxa is observed, both biozonation schemes can be applied. Some nannofossil events (mainly first occurrences) are observed earlier in Portugal than in other Tethyan settings. It is still unclear if these events are real first occurrences. A diversification phase occurred across the Pliensbachian/Toarcian boundary. This phase is well recorded at Peniche, where a change is observed passing from the Pliensbachian, when assemblages are dominated by muroliths, to the Toarcian showing assemblages where placoliths are abundant. A quantification of nannofossils per gram of rock shows that absolute abundances are the highest across the Pliensbachian/Toarcian boundary. Indeed, Peniche exhibits nannofossil abundances very high with respect to correlative levels in other Tethyan settings. The pelagic carbonate fraction (produced by nannofossils) is important in the marly hemi-couplets of Peniche. In some levels, nannofossils account for more than 50% of the total carbonate fraction.

A cross-sectional case-control study designed to evaluate the role of malnutrition in the association between the intensity of Schistosoma mansoni infection and clinical schistosomiasis, was conducted in an area with both low frequency of infection and low morbidity of schistosomiasis in Brazil. Cases (256) were patients with a positive stool examination for S. mansoni; their geometrical mean number of eggs/gram of feces was 90. Controls (256) were a random sample of the negative participants paired to the cases by age, sex and length of residence in the area. The clinical signs and symptoms found to be associated with S. mansoni infection, comparing cases and controls, were blood in stools and presence of a palpable liver. A linear trend in the relative odds of these signs and symptoms with increasing levels of infection was detected. Adjusting by the level of egg excretion, the existence of an interaction between palpable liver and ethnic group (white) was suggested. No differences in the nutritional status of infected and non-infected participants were found.

Antibacterial activity of novel Active Pharmaceutical Ingredient Ionic Liquids (API-ILs) based on ampicillin anion [Amp] have been evaluated. They showed growth inhibition and bactericidal properties on some sensitive bacteria and especially some Gram-negative resistant bacteria when compared to the [Na][Amp] and the initial bromide and chloride salts. For these studies were analysed the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBIC) against sensitive Gram-negative bacteria Escherichia coli ATCC 25922 and Klebsiella pneumonia (clinically isolated), as well as sensitive Gram positive S. Aureus ATCC 25923, Staphylococcus epidermidis and Enterococcus faecalis and completed using clinically isolated resistent strains: E. coli TEM CTX M9, E. coli CTX M2 and E. coli AmpC Mox. From the obtained MIC values of studied APIs-ILs and standard [Na][Amp] were derived RDIC values (relative decrease of inhibitory concentration). High RDIC values of [C16Pyr][Amp] especially against two resistant Gram-negative strains E. coli TEM CTX M9 (RDIC>1000) and E. coli CTX M2 (RDIC>100) point clearly to a potential promising role of APIs-ILs as antimicrobial drugs especially against resistant bacterial strains.
