Research on audiovisual speech integration has reported high levels of individual variability, especially among young infants. In the present study we tested the hypothesis that this variability results from individual differences in the maturation of audiovisual speech processing during infancy. A developmental shift in selective attention to audiovisual speech has been demonstrated between 6 and 9 months with an increase in the time spent looking to articulating mouths as compared to eyes (Lewkowicz & Hansen-Tift. (2012) Proc. Natl Acad. Sci. USA, 109, 1431–1436; Tomalski et al. (2012) Eur. J. Dev. Psychol., 1–14). In the present study we tested whether these changes in behavioural maturational level are associated with differences in brain responses to audiovisual speech across this age range. We measured high-density event-related potentials (ERPs) in response to videos of audiovisually matching and mismatched syllables /ba/ and /ga/, and subsequently examined visual scanning of the same stimuli with eye-tracking. There were no clear age-specific changes in ERPs, but the amplitude of audiovisual mismatch response (AVMMR) to the combination of visual /ba/ and auditory /ga/ was strongly negatively associated with looking time to the mouth in the same condition. These results have significant implications for our understanding of individual differences in neural signatures of audiovisual speech processing in infants, suggesting that they are not strictly related to chronological age but instead associated with the maturation of looking behaviour, and develop at individual rates in the second half of the first year of life.

In this study, Artificial Neural Networks are applied to multistep long term solar radiation prediction. The networks are trained as one-step-ahead predictors and iterated over time to obtain multi-step longer term predictions. Auto-regressive and Auto-regressive with exogenous inputs solar radiationmodels are compared, considering cloudiness indices as inputs in the latter case. These indices are obtained through pixel classification of ground-to-sky images. The input-output structure of the neural network models is selected using evolutionary computation methods.

In this paper we address the real-time capabilities of P-NET, which is a multi-master fieldbus standard based on a virtual token passing scheme. We show how P-NET’s medium access control (MAC) protocol is able to guarantee a bounded access time to message requests. We then propose a model for implementing fixed prioritybased dispatching mechanisms at each master’s application level. In this way, we diminish the impact of the first-come-first-served (FCFS) policy that P-NET uses at the data link layer. The proposed model rises several issues well known within the real-time systems community: message release jitter; pre-run-time schedulability analysis in non pre-emptive contexts; non-independence of tasks at the application level. We identify these issues in the proposed model and show how results available for priority-based task dispatching can be adapted to encompass priority-based message dispatching in P-NET networks.

Debugging electronic circuits is traditionally done with bench equipment directly connected to the circuit under debug. In the digital domain, the difficulties associated with the direct physical access to circuit nodes led to the inclusion of resources providing support to that activity, first at the printed circuit level, and then at the integrated circuit level. The experience acquired with those solutions led to the emergence of dedicated infrastructures for debugging cores at the system-on-chip level. However, all these developments had a small impact in the analog and mixed-signal domain, where debugging still depends, to a large extent, on direct physical access to circuit nodes. As a consequence, when analog and mixed-signal circuits are integrated as cores inside a system-on-chip, the difficulties associated with debugging increase, which cause the time-to-market and the prototype verification costs to also increase. The present work considers the IEEE1149.4 infrastructure as a means to support the debugging of mixed-signal circuits, namely to access the circuit nodes and also an embedded debug mechanism named mixed-signal condition detector, necessary for watch-/breakpoints and real-time analysis operations. One of the main advantages associated with the proposed solution is the seamless migration to the system-on-chip level, as the access is done through electronic means, thus easing debugging operations at different hierarchical levels.

BACKGROUND: First hospitalisation for a psychotic episode causes intense distress to patients and families, but offers an opportunity to make a diagnosis and start treatment. However, linkage to outpatient psychiatric care remains a notoriously difficult step for young psychotic patients, who frequently interrupt treatment after hospitalisation. Persistence of symptoms, and untreated psychosis may therefore remain a problem despite hospitalisation and proper diagnosis. With persisting psychotic symptoms, numerous complications may arise: breakdown in relationships, loss of family and social support, loss of employment or study interruption, denial of disease, depression, suicide, substance abuse and violence. Understanding mechanisms that might promote linkage to outpatient psychiatric care is therefore a critical issue, especially in early intervention in psychotic disorders. OBJECTIVE: To study which factors hinder or promote linkage of young psychotic patients to outpatient psychiatric care after a first hospitalisation, in the absence of a vertically integrated program for early psychosis. Method. File audit study of all patients aged 18 to 30 who were admitted for the first time to the psychiatric University Hospital of Lausanne in the year 2000. For statistical analysis, chi2 tests were used for categorical variables and t-test for dimensional variables; p<0.05 was considered as statistically significant. RESULTS: 230 patients aged 18 to 30 were admitted to the Lausanne University psychiatric hospital for the first time during the year 2000, 52 of them with a diagnosis of psychosis (23%). Patients with psychosis were mostly male (83%) when compared with non-psychosis patients (49%). Furthermore, they had (1) 10 days longer mean duration of stay (24 vs 14 days), (2) a higher rate of compulsory admissions (53% vs 22%) and (3) were more often hospitalised by a psychiatrist rather than by a general practitioner (83% vs 53%). Other socio-demographic and clinical features at admission were similar in the two groups. Among the 52 psychotic patients, 10 did not stay in the catchment area for subsequent treatment. Among the 42 psychotic patients who remained in the catchment area after discharge, 20 (48%) did not attend the scheduled or rescheduled outpatient appointment. None of the socio demographic characteristics were associated with attendance to outpatient appointments. On the other hand, voluntary admission and suicidal ideation before admission were significantly related to attending the initial appointment. Moreover, some elements of treatment seemed to be associated with higher likelihood to attend outpatient treatment: (1) provision of information to the patient regarding diagnosis, (2) discussion about the treatment plan between in- and outpatient staff, (3) involvement of outpatient team during hospitalisation, and (4) elaboration of concrete strategies to face basic needs, organise daily activities or education and reach for help in case of need. CONCLUSION: As in other studies, half of the patients admitted for a first psychotic episode failed to link to outpatient psychiatric care. Our study suggests that treatment rather than patient's characteristics play a critical role in this phenomenon. Development of a partnership and involvement of patients in the decision process, provision of good information regarding the illness, clear definition of the treatment plan, development of concrete strategies to cope with the illness and its potential complications, and involvement of the outpatient treating team already during hospitalisation, all came out as critical strategies to facilitate adherence to outpatient care. While the current rate of disengagement after admission is highly concerning, our finding are encouraging since they constitute strategies that can easily be implemented. An open approach to psychosis, the development of partnership with patients and a better coordination between inpatient and outpatient teams should therefore be among the targets of early intervention programs. These observations might help setting up priorities when conceptualising new programs and facilitate the implementation of services that facilitate engagement of patients in treatment during the critical initial phase of psychotic disorders.

Pelvic floor anatomy is complex and its three-dimensional organization is often difficult to understand for both undergrad- uate and postgraduate students. Here, we focused on several critical points that need to be considered when teaching the perineum. We have to deal with a mixed population of students and with a variety of interest. Yet, a perfect knowledge of the pelvic floor is the basis for any gynecologist and for any surgical intervention. Our objectives are several-fold; i) to estab- lish the objectives and the best way of teaching, ii) to identify and localize areas in the female pelvic floor that are suscepti- ble to generate problems in understanding the three-dimensional organization, iii) to create novel approaches by respecting the anatomical surroundings, and iv) prospectively, to identify elements that may create problems during surgery i.e. to have a closer look at nerve trajectories and on compression sites that may cause neuralgia or postoperative pain. A feedback from students concludes that they have difficulties to assimilate this much information, especially the different imaging tech- niques. Eventually, this will lead to a severe selection of what has to be taught and included in lectures or practicals. Another consequence is that more time to study prosected pelves needs to be given.

BACKGROUND: The aim of this study was to evaluate the efficacy and tolerability of fulvestrant, an estrogen receptor antagonist, in postmenopausal women with hormone-responsive tumors progressing after aromatase inhibitor (AI) treatment. PATIENTS AND METHODS: This is a phase II, open, multicenter, noncomparative study. Two patient groups were prospectively considered: group A (n=70) with AI-responsive disease and group B (n=20) with AI-resistant disease. Fulvestrant 250 mg was administered as intramuscular injection every 28 (+/-3) days. RESULTS: All patients were pretreated with AI and 84% also with tamoxifen or toremifene; 67% had bone metastases and 45% liver metastases. Fulvestrant administration was well tolerated and yielded a clinical benefit (CB; defined as objective response or stable disease [SD] for >or=24 weeks) in 28% (90% confidence interval [CI] 19% to 39%) of patients in group A and 37% (90% CI 19% to 58%) of patients in group B. Median time to progression (TTP) was 3.6 (95% CI 3.0 to 4.8) months in group A and 3.4 (95% CI 2.5 to 6.7) months in group B. CONCLUSIONS: Overall, 30% of patients who had progressed following prior AI treatment gained CB with fulvestrant, thereby delaying indication to start chemotherapy. Prior response to an AI did not appear to be predictive for benefit with fulvestrant.

BACKGROUND: Excision and primary midline closure for pilonidal disease (PD) is a simple procedure; however, it is frequently complicated by infection and prolonged healing. The aim of this study was to analyze risk factors for surgical site infection (SSI) in this context. METHODS: All consecutive patients undergoing excision and primary closure for PD from January 2002 through October 2008 were retrospectively assessed. The end points were SSI, as defined by the Center for Disease Control, and time to healing. Univariable and multivariable risk factor analyses were performed. RESULTS: One hundred thirty-one patients were included [97 men (74%), median age = 24 (range 15-66) years]. SSI occurred in 41 (31%) patients. Median time to healing was 20 days (range 12-76) in patients without SSI and 62 days (range 20-176) in patients with SSI (P < 0.0001). In univariable and multivariable analyses, smoking [OR = 2.6 (95% CI 1.02, 6.8), P = 0.046] and lack of antibiotic prophylaxis [OR = 5.6 (95% CI 2.5, 14.3), P = 0.001] were significant predictors for SSI. Adjusted for SSI, age over 25 was a significant predictor of prolonged healing. CONCLUSION: This study suggests that the rate of SSI after excision and primary closure of PD is higher in smokers and could be reduced by antibiotic prophylaxis. SSI significantly prolongs healing time, particularly in patients over 25 years.

The prlmaiy objective of this study was to Identify and describe the physical and psychological stress factors that elementary school teachers experience and how teachers cope with stress. A secondary objective was to offer boards and teachers potential coping strategies counteracting stress and the effects of stress. The sample consisted of 120 elementaiy teachers from southern Ontario. Ten elementaiy schools were randomly chosen. The Teacher Stress Inventory questionnaire (Flmian, 1989) was used. Data were analyzed using a variety of statistics. Test norms and interpretations were performed based on standard results obtained from the author of the questionnaire (Flmian, 1988). Overall, the results indicated that work-related stressors were the main factor for teacher stress. This Included such factors as caseload/class is too big, too much administrative paperwork, and having little time to prepare lessons. Implications for further research and practical suggestions for further reseairch are discussed. Also a variety of recommendations to boards and for individual use are discussed. Some recommendations are having counselling available for teachers, workshops on how to handle stress, and learning how to breathe and using calm visualization.

The purpose of the study was to investigate the relative contribution of skate blade properties to on-ice skating speed. Thirty-two male ice hockey players (mean age = 19±2.65 yrs.) representing the Ontario Minor Hockey Association (OMHA; Midget AAA and Junior), Canadian Inter University Sport (CIS: Varsity), Ontario hockey league (OHL) and East Coast Hockey League (ECHL), and the playing positions of forwards (n=18) and defense (n=14) were recruited to participate. Skate related equipment worn by the players for the purpose of the research was documented and revealed that 80% of the players wore Bauer skates, Tuuk blade holders and LS2 skate blades. Subjects completed a battery of eight on-ice skating drills used to measure and compare two aspects of skating speed; acceleration [T1(s)] and total time to complete each drill [TT(s)] while skating on three skate blade conditions. The drills represented skills used in the game of hockey, both in isolation (e.g., forward skating, backward skating, stops and starts, and cornering) and in sequence to simulate the combination of skills used in a shift of game play. The three blade conditions consisted of (i) baseline, represented by the blades worn by the player throughout their current season of play; (ii) experimental blades (EB), represented by brand name experimental blades with manufacturers radius of contour and a standardized radius of hollow; and (iii) customized experimental blades (CEB), represented by the same brand name experimental blades sharpened to the players’ preference as identified in the baseline condition. No significant differences were found in acceleration time [T1(s)] or total time to complete [TT(s)] the isolated drills across blade conditions; however significant differences were revealed in both T1(s) and TT(s) measured during the execution of the sequenced drill across blade conditions. A iii Bonferroni post hoc test revealed that players skated significantly faster when skating on the CEB condition compared to the baseline condition (p≤.05). A questionnaire assessing subjects perceived comfort, confidence and effort expended while skating on the experimental blades revealed that players were significantly more comfortable when skating on the CEB versus the EB condition (p≤.05). Outcomes of the study provide evidence to suggest that the experimental skate blades customized with the players preferred blade sharpening characteristics results in faster skating speed in a combination drill representing skills performed in gameplay.

Objectifs. L’objectif de ce mémoire est de parfaire nos connaissances quant à l’effet des actions policières sur les collisions routières au Québec. Ultimement, ce mémoire permettra d’identifier les conditions nécessaires pour que l’action policière influe sur les comportements des automobilistes. Pour se faire, deux études de cas sont employées. Dans un premier temps, nous évaluons l’effet d’un relâchement d’environ 60 % dans l’émission de constats d’infraction par les policiers de la ville de Québec sur les collisions avec blessures. Dans cet article, nous distinguons également les effets respectifs des constats d’infraction, des interceptions policières sans constat et des médias. Dans un second temps, nous évaluons l’impact d’une stratégie de sécurité routière mise en place conjointement par l’Association des directeurs de police du Québec (ADPQ) et la Société de l’assurance automobile du Québec (SAAQ). Dans les deux cas, un changement important est survenu dans l’émission de constats d’infraction émis en vertu du Code de la sécurité routière (CSR). Méthodologie. Afin d’évaluer l’effet de ces deux stratégies, nous avons agrégé les données sur les collisions et infractions au CSR sur une base mensuelle. Ces données proviennent principalement des rapports de collisions et des constats d’infraction remplis par les policiers et transmis à la SAAQ. Dans l’ensemble, nous avons utilisé un devis quasi-expérimental, soit celui des séries chronologiques interrompues. Résultats. Les résultats des deux articles démontrent que les policiers sont des acteurs clés en matière de sécurité routière. Les collisions avec blessures sont affectées par les fluctuations de leurs activités. La première série d’analyses établit qu’un relâchement d’environ 60 % dans le nombre de constats émis par les policiers se traduit par une hausse d’environ 10 % des collisions avec blessures, ce qui correspond à 15 collisions avec blessures supplémentaires par mois sur le territoire du Service de police de la ville de Québec. De plus, nos résultats montrent qu’une interception policière suivie d’un avertissement verbal n’est pas suffisante pour prévenir les collisions. De même, l’effet observé n’est pas attribuable aux médias. La deuxième série d’analyse montre que la stratégie conjointe de l’ADPQ et de la SAAQ, caractérisée par une hausse des constats émis et des campagnes médiatiques, fut suivie de baisses variant entre 14 et 36 % des collisions avec blessures graves. Interprétation. Les résultats démontrent que les actions policières ont une influence sur le bilan routier. Par contre, avant d’influer sur le comportement des automobilistes, certaines conditions doivent être respectées. Premièrement, l’intensité des contrôles policiers doit être suffisamment modifiée par rapport à son niveau initial. Deuxièmement, que ce soit une hausse ou une baisse, ce niveau doit être maintenu sur une période relativement longue (entre 12 et 24 mois environ) pour que les automobilistes soient exposés au message pénal et qu’ils considèrent ce changement dans le niveau de répression comme étant crédible. Troisièmement, l’émission de constats est un élément clé; la simple présence policière n’est pas suffisante pour prévenir les collisions. Enfin, les campagnes de sensibilisation semblent importantes, mais d’autres études sont nécessaires pour mieux apprécier leur rôle.
