BACKGROUND: Pediatric rheumatic diseases have a significant impact on children's quality of life and family functioning. Disease control and management of the symptoms are important to minimize disability and pain. Specialist clinical nurses play a key role in supporting medical teams, recognizing poor disease control and the need for treatment changes, providing a resource to patients on treatment options and access to additional support and advice, and identifying best practices to achieve optimal outcomes for patients and their families. This highlights the importance of investigating follow-up telenursing (TN) consultations with experienced, specialist clinical nurses in rheumatology to provide this support to children and their families. METHODS/DESIGN: This randomized crossover, experimental longitudinal study will compare the effects of standard care against a novel telenursing consultation on children's and family outcomes. It will examine children below 16 years old, recently diagnosed with inflammatory rheumatic diseases, who attend the pediatric rheumatology outpatient clinic of a tertiary referral hospital in western Switzerland, and one of their parents. The telenursing consultation, at least once a month, by a qualified, experienced, specialist nurse in pediatric rheumatology will consist of providing affective support, health information, and aid to decision-making. Cox's Interaction Model of Client Health Behavior serves as the theoretical framework for this study. The primary outcome measure is satisfaction and this will be assessed using mixed methods (quantitative and qualitative data). Secondary outcome measures include disease activity, quality of life, adherence to treatment, use of the telenursing service, and cost. We plan to enroll 56 children. DISCUSSION: The telenursing consultation is designed to support parents and children/adolescents during the course of the disease with regular follow-up. This project is novel because it is based on a theoretical standardized intervention, yet it allows for individualized care. We expect this trial to confirm the importance of support by a clinical specialist nurse in improving outcomes for children and adolescents with inflammatory rheumatisms. BACKGROUND: Greenstick fractures suffered during growth have a high risk for refracture and posttraumatic deformity, particularly at the forearm diaphysis. The use of a preemptive completion of the fracture by manipulation of the concave cortex is controversial and data supporting this approach are few. AIM: Aim of this study was to determine the factors which predispose to refracture and deformities, and to define therapeutic strategies. METHODS: We prospectively gathered clinical and radiographic data over a period of one year on greenstick fractures of the middle third of the forearm in children as part of a multi-centre study. Endpoint was a follow-up visit at one year. Radiographic deformity, state of consolidation at resumption of physical activities and refracture rate were analysed statistically (ANOVA, Student's t-test and Pearson's chi-square test) with regard to patient age, gender, fracture type, therapy and time in plaster. RESULTS: We collected the data of 103 patients (63 boys, 40 girls), average age 6.6 years (1.3-14.5 years), the vast majority of whom had a combined greenstick fracture of the radius and ulna. 6.7% of the patients sustained a refracture within 49 days (29-76) after plaster removal. They were significantly older (p=0.017) with a significantly higher incidence of manual completion of the fracture with radiographic signs of partial consolidation (p=0.025). Residual deformities were significantly smaller after completion of the fracture compared to reduction without completion (p=0.019) or plaster fixation alone (p<0.005). CONCLUSIONS: Completion of a greenstick fracture does not prevent refracture. Nevertheless, it diminishes the extent of secondary deformities in cases where the primary angulation exceeds the remodelling capacity. Prevention of refracture should include a routine radiographic follow-up 4-6 weeks after injury with continuation of plaster fixation in cases of partial consolidation.

Una de les qüestions que ens preocupen com a psicòlegs i psicòlogues en l'àmbit penitenciari és saber fer front d’una manera vàlida i efectiva a les diverses situacions de crisi o emergència que es produeixen en aquest medi, encara que sigui puntualment, no per això deixen de ser significatives i a vegades traumàtiques, tant per a les persones preses com per al seu context. Tanmateix, som conscients que els professionals que assisteixen les persones o grups en crisi a la presó no són necessàriament psicòlegs, sinó que sovint són els funcionaris de vigilància o els mateixos membres de la direcció del centre els que han d’assumir aquestes funcions. És com a tècnics especialistes en el comportament i la personalitat humana i en la solució dels problemes relacionals que podem col·laborar i assessorar els responsables en la gestió d’aquests conflictes en procediments i estratègies del tractament més efectius a l’hora de negociar i assolir un equilibri adaptatiu entre les diverses parts que conformen els escenaris en crisi. Un dels objectius que ens plantejarem la comunitat de pràctiques en aquest curs 2009 és identificar les diverses situacions de crisi que es poden produir a la presó i elaborar un conjunt de protocols d’actuació psicològica que orientin el professional (el psicòleg inicialment però també d’altres agents del canvi dels comportaments antisocials) sobre quina és la millor manera d’intervenir-hi. No obstant això, es considera que més enllà de l'elaboració de guies o manuals d’intervenció (al cap i a la fi això es pot consultar en la literatura especialitzada sobre aquest tema), es tractava de ser el més pràctics possible i recollir en un tríptic aquella informació pertinent (allò imprescindible que s’ha de saber per actuar de manera efectiva i quins són els errors que cal evitar encara que només sigui per no complicar encara més les coses) que fos fàcil d’utilitzar per part del professional, és a dir, un recordatori esquematitzat però rigorós del pla d’acció que cal seguir per fer front a la crisi. També es pretén que sigui obert a les experiències i coneixements resolutius de qui intervé en aquestes situacions d’emergència i que permeti el feedback amb el grup de la comunitat de pràctiques, qui ho validaria, per tal d’anar incrementat progressivament la validesa i fiabilitat de l'instrument. Aquest curs presentem dos protocols/tríptics d’actuació: 1) notificació de males noticies/acompanyament en el dol; 2) intervenció en el risc de suïcidi o en conductes de autòlisi. La idea és elaborar-ne més en pròximes edicions. A més a més, com a protocol en format audiovisual, es presenta un curtmetratge on es desenvolupa una actuació psicològica per tal de desactivar una conducta violenta/ amenaçant.

Objective: To investigate practical teaching of nurse residents in a multidisciplinary residency in oncology. Method: A qualitative descriptive study grounded in the problematization methodology and its steps, represented by the Maguerez Arch. Data were analyzed using content analysis. Results: Potentiating and limiting elements of the residency guided the design of a practical teaching protocol from the perspective of residents, structured in three stages: Welcoming and ambience; Nursing care for problem situations; and, Evaluation process. Conclusion: Systematization of practical teaching promoted the autonomy of individuals and the approximation of teaching to reality, making residency less strenuous, stressful and distressing.

Objective: Presenting a Virtual Environment (VE) based on the Protocol of Treatment of Hypertension and Diabetes Mellitus type 2, used in Primary Care for evaluation of dietary habits in nursing consultations. Method: An experimental study applied by two nurses and a nurse manager, in a sample of 30 deaf patients aged between 30 and 60 years. The environment was built in Visual Basic NET and offered eight screens about feeding containing food pictures, videos in Libras (Brazilian sign language) and audio. The analysis of the VE was done through questionnaires applied to patients and professionals by the Poisson statistical test. Results: The VE shows the possible diagnostics in red, yellow, green and blue colors, depending on the degree of patients’ need. Conclusion: The environment obtained excellent acceptance by patients and nurses, allowing great interaction between them, even without an interpreter. The time in consultation was reduced to 15 minutes, with the preservation of patient privacy.

Objective To develop a safety protocol for the management of thirst in the immediate postoperative period. Method Quantitative, methodological, and applied study conducted in April-August 2012. An extensive literature search and expert consultation was carried out to develop the protocol and its operating manual. Theoretical and semantic analyzes were carried out by experts. Results Assessment of level of consciousness, reflexes of protection of the airways (cough and swallowing), and absence of nausea and vomiting were selected as safety criteria. These criteria were grouped and formatted in a graph algorithm, which indicates the need to interrupt the procedure if a security criterion does not reach the expected standard. Conclusion The protocol was elaborated to fill in the gap in the literature of a specific model concerning nursing actions in the safe management of thirst in the immediate postoperative period.

OBJECTIVE To analyze the scientific production about the validity and reliability of the Manchester Triage System (MTS) protocol. METHOD A descriptive study of an integrative literature review. Articles on the validity and reliability of the MTS developed with children and adults published between 1999 and 2013 were included. RESULTS 14 articles were selected from a total of 8438, nine of validity and five of reliability. The reliability of the MTS ranged from moderate to almost perfect, with higher intra-evaluation. Regarding validity, the results seem to point to equivalent and satisfactory sensibility and specificity levels of the MTS. The instrument proved to be a good predictor of the need for hospitalization and of hospital mortality. CONCLUSION The reliability and validity of the MTS obtained in the studies is varied. It is recommended that new studies indicate necessary modifications to the MTS so that it is more safely used by nurses.

Background: Shared decision making (SDM) is a process by which a healthcare choice is made jointly by the healthcare professional and the patient. SDM is the essential element of patient-centered care, a core concept of primary care. However, SDM is seldom translated into primary practice. Continuing professional development (CPD) is the principal means by which healthcare professionals continue to gain, improve, and broaden the knowledge and skills required for patient-centered care. Our international collaboration seeks to improve the knowledge base of CPD that targets translating SDM into the clinical practice of primary care in diverse healthcare systems. Methods: Funded by the Canadian Institutes of Health Research (CIHR), our project is to form an international, interdisciplinary research team composed of health services researchers, physicians, nurses, psychologists, dietitians, CPD decision makers and others who will study how CPD causes SDM to be practiced in primary care. We will perform an environmental scan to create an inventory of CPD programs and related activities for translating SDM into clinical practice. These programs will be critically assessed and compared according to their strengths and limitations. We will use the empirical data that results from the environmental scan and the critical appraisal to identify knowledge gaps and generate a research agenda during a two-day workshop to be held in Quebec City. We will ask CPD stakeholders to validate these knowledge gaps and the research agenda. Discussion: This project will analyse existing CPD programs and related activities for translating SDM into the practice of primary care. Because this international collaboration will develop and identify various factors influencing SDM, the project could shed new light on how SDM is implemented in primary care.

The need for an early neurorehabilitation pathway was identified in an acute university hospital. A team was formed to draw up and implement it. A neuro-sensorial, interdisciplinary and coordinated therapy program was developed, focused on tracheostomised patients as soon as they were admitted to the intermediate care in neurology and neurosurgery. The impact of this care plan was evaluated by comparing the results obtained with that pertaining to patients treated previously in the same services. The comparison showed a reduction of 48% of the mean duration of tracheostomy, of 39% in the time to inscription in a neurorehabilitation centre and of 20% in the length of stay in the intermediate care. An early neurorehabilitation care program, with an interdisciplinary and coordinated team, reduces complications and lengths of stay.

Aujourd'hui, la construction sociale de la ménopause intéresse de nombreux chercheurs en sciences sociales. Cependant, la façon dont les femmes vivent ou se représentent cet événement reste peu documentée. L'objectif de cette thèse est donc d'approfondir la compréhension de l'expérience de la ménopause en accordant une place primordiale aux discours et aux pratiques des femmes elles-mêmes. En s'appuyant sur une recherche ethnographique en Suisse romande et au Centre-Cameroun, cette étude parcourt différentes dimensions de l'expérience de la ménopause (représentations, vécus, pratiques de gestion) qui, corrélées les unes aux autres, forment un tout. Au-delà des divergences que l'on peut observer au niveau culturel, de la situation économique et sociale des femmes, des systèmes de santé et du statut occupé par la ménopause dans les deux pays, les résultats de ce travail montrent que les expériences de la ménopause chez les Suissesses et les Camerounaises interviewées ne peuvent pas simplement être classifiées de manière binaire ou dichotomique. Dans chacun des contextes, ces expériences sont plurielles et dépendent de plusieurs facteurs. D'une part, elles découlent d'une observation empirique de la part de ces femmes elles-mêmes et de leurs interactions avec différents acteurs dont les médias (particulièrement en Suisse), les pairs, les proches et les professionnels de la santé. D'autre part, elles sont influencées par de nombreuses variables parmi lesquelles le contexte économique, socioculturel, familial et conjugal, le statut professionnel, la prévalence des troubles ressentis et le statut ménopausique de ces femmes. Mais, ces facteurs ne sont pas hiérarchisés puisqu'ils agissent différemment pour chacune d'entre elles. Dès lors, s'il apparaît que les expériences ménopausiques n'échappent point aux déterminations sociales, il n'en demeure pas moins qu'elles relèvent aussi des capacités réflexives des femmes, les conduites sociales n'étant pas réductibles à des applications des codes intériorisés. Au fil du travail, la médicalisation de la ménopause, bien qu'existant à des degrés variables entre la Suisse et le Cameroun, a émergé comme une problématique transversale. Interrogeant les logiques qui la sous-tendent, cette étude se propose d'analyser le rôle que jouent les femmes elles-mêmes dans ce processus. - Today, the social construction of the menopause is of great interest for many researchers in social sciences. Neverthless, the way of living or of representing this event is still little documented. The aim of this thesis is to study thoroughly the understanding of menopausal experience through the discourses and practices of women themselves. Based on an ethnograph ic research, in French-speaking Switzerland and in Centre-Cameroon, this study looks at different dimensions of menopausal experience (representations, real-life experiences, pratices) that, connected to each other, form a whole. Inspite of the cultural, economic, social, health systems and menopausal status differences between these two countries, the results of this thesis show that menopausal experiences among the Swiss and the Cameroonians interviewee can not simply be classified in two dichotomous groups. In each context, those experiences are plural and depend on several factors. On the one hand, they arise from women's own empirical observations and from their interactions with several actors like the media (especially in Switzerland), the peers, the people closest to them and health professionals. On the other hand, they are influenced by many elements such as the economical, sociocultural, family and marital context, the professional status, the prevalence of felt disorders and the menopausal status of those women. But, these factors are not hierarchical beacause they operate differently for each person. Accordingly, if menopausal experiences don't escape from social determinism, nonetheless they also depend on the reflexive capacities of women beacause social conducts can not be reduced to the application of interiorised codes. Through this work, the médicalisation of the menopause, even though varying between Switzerland and Cameroon, comes to light as a cross-cutting problematic. Questioning its underlying logic, this study proposes to analyse the role that women themselves play in this process.

AbstractOBJECTIVETo report the nurse's experience of inclusion in interdisciplinary clinical study about technological innovation, involving people with spinal cord injury.METHODDescriptive experience report. The empirical support was based on notes about perspectives and practice of clinical research, with a multi-professional nursing, physical education, physiotherapy and engineering staff.RESULTThe qualification includes the elaboration of the document for the Ethics Committee, familiarization among the members of staff and with the studied topic, and also an immersion into English. The nurse's knowledge gave support to the uptake of participants and time adequacy for data collection, preparation and assistance of the participants during the intervention and after collection. Nursing theories and processes have contributed to reveal risky diagnoses and the plan of care. It was the nurse's role to monitor the risk of overlapping methodological strictness to the human aspect. The skills for the clinical research must be the object of learning, including students in multidisciplinary researches.CONCLUSIONTo qualify the nurse for clinical research and to potentialize its caregiver essence, some changes are needed in the educational system, professional behavior, attitude and educational assistance.
