BACKGROUND: This study aimed to investigate the influence of deep sternal wound infection on long-term survival following cardiac surgery. MATERIAL AND METHODS: In our institutional database we retrospectively evaluated medical records of 4732 adult patients who received open-heart surgery from January 1995 through December 2005. The predictive factors for DSWI were determined using logistic regression analysis. Then, each patient with deep sternal wound infection (DSWI) was matched with 2 controls without DSWI, according to the risk factors identified previously. After checking balance resulting from matching, short-term mortality was compared between groups using a paired test, and long-term survival was compared using Kaplan-Meier analysis and a Cox proportional hazard model. RESULTS: Overall, 4732 records were analyzed. The mean age of the investigated population was 69.3±12.8 years. DSWI occurred in 74 (1.56%) patients. Significant independent predictive factors for deep sternal infections were active smoking (OR 2.19, CI95 1.35-3.53, p=0.001), obesity (OR 1.96, CI95 1.20-3.21, p=0.007), and insulin-dependent diabetes mellitus (OR 2.09, CI95 1.05-10.06, p=0.016). Mean follow-up in the matched set was 125 months, IQR 99-162. After matching, in-hospital mortality was higher in the DSWI group (8.1% vs. 2.7% p=0.03), but DSWI was not an independent predictor of long-term survival (adjusted HR 1.5, CI95 0.7-3.2, p=0.33). CONCLUSIONS: The results presented in this report clearly show that post-sternotomy deep wound infection does not influence long-term survival in an adult general cardio-surgical patient population.

El projecte és l’inici de la creació d’un nou prototip per a poder competir la temporada 2008 a la cursa de vehicles de baix consum Shell Eco-Marathon. El principal objectiu és aconseguir un xassís que redueixi, en la mesura del possible, el pes del prototip a la vegada que asseguri una millor rigidesa i millori l’ergonomia de tot el conjunt. Es dissenyarà tota la part estructural de la carrosseria, que serà sotmesa a càlcul mitjançant la tècnica dels elements finits i posteriorment es realitzarà una guia de producció per tal de guiar els membres de l’equip que en realitzin la producció

L'objectiu principal d'aquest projecte és estudiar les mancances en la interoperabilitat entre els repositoris de material docent i els recol·lectors de metadades, així com incloure una proposta d'arquitectura per superar-ho, basada en ontologies per a cada tipus de material docent o mitjançant crosswalks.

Objective: To outline the treatment by patches of microcurrent at home of patients with disabling knee osteoarthritis to perform their activities of daily life and work. Material and methods: We selected 80 patients in the surgical waiting list for total knee arthroplasty. 40 of whom received active patch and 40 inactive. All were given exercise protocol knee. The variables analyzed were: pain (VAS), joint balance, muscular balance and quality of life (SF-12). Patients were reviewed after two months of treatment. Variables were analyzed with SPSS 17.0 statistical program. Results: Statistically significant results were obtained (p <0.05) for decrease in mean pain according to VAS scale. No significant differences for other variables were found. Conclusions: Microcurrent therapy could be an alternative in the treatment of knee osteoarthritis pain, with no differences with the traditional treatment.

ABSTRACT: BACKGROUND: The prevalence of obesity has increased in societies of all socio-cultural backgrounds. To date, guidelines set forward to prevent obesity have universally emphasized optimal levels of physical activity. However there are few empirical data to support the assertion that low levels of energy expenditure in activity is a causal factor in the current obesity epidemic are very limited. METHODS: The Modeling the Epidemiologic Transition Study (METS) is a cohort study designed to assess the association between physical activity levels and relative weight, weight gain and diabetes and cardiovascular disease risk in five population-based samples at different stages of economic development. Twenty-five hundred young adults, ages 25-45, were enrolled in the study; 500 from sites in Ghana, South Africa, Seychelles, Jamaica and the United States. At baseline, physical activity levels were assessed using accelerometry and a questionnaire in all participants and by doubly labeled water in a subsample of 75 per site. We assessed dietary intake using two separate 24-h recalls, body composition using bioelectrical impedance analysis, and health history, social and economic indicators by questionnaire. Blood pressure was measured and blood samples collected for measurement of lipids, glucose, insulin and adipokines. Full examination including physical activity using accelerometry, anthropometric data and fasting glucose will take place at 12 and 24 months. The distribution of the main variables and the associations between physical activity, independent of energy intake, glucose metabolism and anthropometric measures will be assessed using cross-section and longitudinal analysis within and between sites. DISCUSSION: METS will provide insight on the relative contribution of physical activity and diet to excess weight, age-related weight gain and incident glucose impairment in five populations' samples of young adults at different stages of economic development. These data should be useful for the development of empirically-based public health policy aimed at the prevention of obesity and associated chronic diseases.

It has been long stated that there are profound analogies between fracture experiments and earthquakes; however, few works attempt a complete characterization of the parallelisms between these so separate phenomena. We study the Acoustic Emission events produced during the compression of Vycor (SiO&sub&2&/sub&). The Gutenberg-Richter law, the modified Omori's law, and the law of aftershock productivity hold for a minimum of 5 decades, are independent of the compression rate, and keep stationary for all the duration of the experiments. The waiting-time distribution fulfills a unified scaling law with a power-law exponent close to 2.45 for long times, which is explained in terms of the temporal variations of the activity rate.

Os autores que analisam o conflito trabalho-família têm ressaltado a importância da busca do equilíbrio do tempo na dedicação para o trabalho sem que haja detrimento do tempo dedicado à família. Destacam-se três dimensões em que se podem encontrar conflitos entre o trabalho e a família: tempo, tensão e comportamento. Nesta pesquisa, tem-se como objetivo apresentar os conflitos trabalho-família percebidos por duas empresárias do setor de comércio de material de construção da cidade de Curitiba, bem como as ações dessas empresárias para lidarem com os conflitos identificados. Por meio de pesquisa qualitativa, do tipo estudo de multicasos, foram entrevistados tanto as empresárias quanto seus parceiros. Os achados da pesquisa corroboram os estudos citados na revisão teórica, sendo o fator tempo o mais frequentemente citado como gerador de conflitos trabalho-família e o controle emocional como a ação mais citada como atenuante desses mesmos conflitos.

Com o objetivo de estudar aspectos da ocorrência de acidente do trabalho por material perfurocortante entre trabalhadores de enfermagem de um hospital universitário, foram entrevistados 46 trabalhadores que se acidentaram no período de junho a dezembro de 1994. Os resultados forneceram dados importantes que poderão subsidiar programas de prevenção.

Realizou-se esta investigação, com os objetivos de identificar a ocorrência de acidentes de trabalho com material pérfuro-cortante potencialmente contaminado, entre enfermeiros e auxiliares de enfermagem de um Hospital de ensino geral do interior do Estado de São Paulo; relacionar a ocorrência destes acidentes com a categoria profissional e anos de trabalho na instituição; identificar o tipo de material pérfuro-cortante causador do acidente e identificar as condutas tomadas após o acidente, pelo acidentado. Para a coleta de dados, através de entrevista individual, utilizou-se um formulário estruturado onde registrava-se, a causa atribuída ao acidente, o período de atuação do profissional acidentado, o objeto causador, a conduta tomada após o acidente. Evidenciou-se que as Precauções Padrão ou as antigas Precauções Universais não são utilizadas como deveriam, conferindo portanto, risco à equipe de enfermagem. Considera-se que esta equipe necessita de orientações específicas através de educação continuada sobre o assunto.
